A Novel Method to Measure Superior Migration of the Humeral Head: Step-off of the C-line

  • Background: Superior migration of humeral head has been conventionally determined by measuring the acromiohumeral distance (AHD), We sought to devise a novel measurement system more reliably and accurately than AHD. We described a structural landmark called 'C-line'. In this study, we investigated the clinical usefulness of 'step-off of the C-line (SOC)' compared to that of AHD. Methods: The C-line formed from the medial margin of the proximal humeral head continuing up to the inferior margin of the articular glenoid and then to the lateral border of the scapula. The superior migration of the humeral head triggered by a rotator cuff tear introduces a discontinuity in this C-line. We measured the distance of this discontinuity. We enrolled 144 patients who underwent a rotator cuff repair. We selected 58 controls who didn't have any cuff lesions apparent on magnetic resonance imaging. Using radiographs derived from standardized true anteroposterior views of the shoulder, we measured the SOC and the AHD. We used t-tests for statistical analyses. Results: A rotator cuff tear was associated with an increase in SOC and a decrease in AHD. In control group, the mean SOC was $1.29{\pm}1.71mm$ and AHD was $9.71{\pm}2.65mm$. In cuff tear group, the mean SOC was $3.15{\pm}3.41mm$ and AHD was $8.28{\pm}1.76mm$. The mean SOCs of the patient group in relation to the mean SOC of the control group according to tear size, the SOCs of medium tear and lager groups showed statistically significant increase (p<0.05). Conclusions: The SOC may be a similarly effective to diagnose cuff tears of medium size and larger compared with AHD.

  • Das Forschungsziel dieser Untersuchung ist es, den linguistischen Begriff der Analogie zu definieren, und die auftretenden analogischen Ausgleichsprozesse vom Mittelhochdeutschen zum Neuhochdeutschen und auch im Neuhochdeutschen selbst insbesondere im System der starken Verben vorzuzeigen und zu $erkl\"{a}ren$. Und diese Forschungsergebnisse sind wie folgt. $Au{\ss}er$ dem Phonemwandel, der immer das Phonemsystem tangiert, gibt es M$Lautver\"{a}nderungen$, die auf assoziativer Ubertragung von Lautungen aus konkurrierenden sprachlichen Formen beruhen, die sog. Analogiebildungen. $N\"{a}mlich$ in der sprachlichen Entwicklung vom Mittelhochdeutschen zum Neuhochdeutschen zeigt sich eine sehr starke innere Tendenz der Sprache, diese durch $gesetzm\"{a}{\ss}igen$ Lautwandel entstandene Formenvielfalt innerhalb eines Paradigmas zu vereinheitlichen. Effects of Mobilization with Movement Combined with Exercise(EMWM) on ADH, ROM and Functional Performance in Patients with Impingement Syndrome of the Shoulder (운동과 MWM을 혼합한 중재(EMWM)가 어깨관절 충돌증후군 환자의 AHD, ROM, 기능수행정도에 미치는 효과)

  • Purpose : This study was to identify the effects of Mobilization with Movement combined with exercise (EMWM) on acromio-humeral distance (AHD), range of motion (ROM), pain intensity, and functional performance in patients with impingement syndrome of the shoulder. Methods : The subjects were 40 patients diagnosed with impingement syndrome of the shoulder. Twenty subjects are randomly assigned to each 2 different groups; Group 1. (exercise group), Group 2. (EMWM group). Three weekly interventions were given for 4 weeks. The main outcome measures were ultrasound, goniometer, visual analogue scale (VAS), and Korean Constant shoulder score (K-CSS). The ultrasound (AHD), ROM (flexion, abduction), pain intensity (for shoulder flexion) and functional performance (K-CSS) were compared between the groups. Results : The AHD was significantly increased in Group 2 compared to Groups 1. No significant difference was observed between the groups in the range of abduction of the shoulder, but the range of flexion was significantly increased in Group 2 compared to Groups 1. Pain intensity was significantly lower in Group 2 than in Group 1, and functional performance was significantly increased in Group 2 compared to Group 1. Conclusion : An intervention that combined mobilization with movement with exercise was more effective than exercise alone for rapid recovery from shoulder injury and improvement in functional performance.

Immediate Effects of Soft Tissue Massage on Posterior Shoulder Muscle Tightness: A Preliminary Study

  • Posterior shoulder muscle tightness is frequently observed in shoulder impingement syndrome because tightness in the posterior portion of the shoulder muscles can cause anterior and superior translation of the humeral head in relation to the glenoid fossa. The purpose of this study was to determine the immediate effects of soft tissue massage on acromiohumeral distance (AHD), anterior translation of the humeral head, and glenohumeral (GH) range of motion (ROM) in subjects with posterior shoulder muscle tightness. Twenty-seven subjects with greater than $10^{\circ}$ difference in the range of GH horizontal adduction between right and left sides were recruited. The range of GH horizontal adduction and internal rotation were measured by a digital inclinometer. The AHD and anterior translation of the humeral head were measured using ultrasonography. A paired t-test was used to compare AHD, anterior translation of the humeral head, and the range of GH horizontal adduction and internal rotation before and after soft tissue massage. The results showed that AHD increased significantly (p<.05) and the anterior translation of humeral head decreased slightly, but not significantly (p=.40) after the soft tissue massage. Furthermore, the ROM of horizontal adduction and internal rotation in the GH joint increased significantly after the soft tissue massage (p<.05). These findings indicate that soft tissue massage on posterior shoulder muscle tightness is an effective method to increase AHD and ROM in the horizontal adduction and internal rotation of the GH joint.

The Effect of Novel Stretching on the Range of Motion and Acromio-Humeral Distance in Patients with Glenohumeral Internal Rotation Deficits with Posterior Shoulder Tightness (노벨 스트레칭과 관절가동술이 어깨 후방 뻣뻣함으로 인한 어깨관절 안쪽돌림 결핍 환자의 관절가동범위와 봉우리-위팔뼈 거리에 미치는 영향)

  • Background: The cross-body and the sleeper stretches have been used to improve posterior shoulder tightness (PST). However, such stretching techniques may not always help achieve scapular stability and may even aggravate patient's symptoms. Therefore, a new stretching technique (passive glenohumeral internal rotation with bridging) was developed as a more effective method that may allow for greater scapular stability without aggravating the symptoms. Thus, this study aimed to examine and compare this novel stretching technique to determine its effect on the range of motion (ROM) and acromio-humeral distance (AHD) in patients with glenohumeral internal rotation deficits (GIRD) with posterior shoulder tightness. Methods: A total of 30 symptomatic patients with asymmetrical GIRD were randomly assigned to two groups: the novel stretching group (n=15) and the joint mobilization group (n=15). The intervention was conducted twice a week for a total of four weeks. The ROM of shoulder internal rotation was measured by a goniometer and the AHD was evaluated by an X-ray before and after the intervention. Results: Both the treatments improved ROM and AHD in patients with GIRD (p<.05). However, the improvements in internal rotation ROM and AHD in the novel stretching group were significantly greater than that of the subjects in the mobilization group. There was a significant difference between the two groups (p<.05). Conclusion: These results show that both novel stretching and joint mobilization improved ROM and AHD in patients with GIRD with PST. However, novel stretching was more effective than joint mobilization.

  • This paper implemented 2.14GHz band Adaptive Hybrid Doherty (AHD) amplifier, as well as, wished to improve the high power characteristics and efficiency by composing bias adjustment circuit with LDMOS. Finally, through CAE, confirmed that AHD amplifier have superior performance than hybrid balanced amplifier. Superior characteristics of AHD amplifier is expected to affect immensely in amplifier field hereafter.

  • The sexual dimorphism and intra sexual dimorphism in males of Rhinogobius brunneus were investigated at Jiam stream, a tributary of the Bukhan river from May, 2005. The frequency distributions in standard length were similar for the females and maleLs, but maleSs were smaller than the females and maleLs. The ratio of MaleL (N=79) to males (N=79) was 1 : 0.46. The comparison between female (N=79) and maleL (N=79) gave significant differences (P<0.05) in 18 morphometric characteristics (SNL, AHD, PHD, ML, DL, MW, AHW, IFW, PHW, ED, HL, FDFL, SDFB, CFL, AFB, VFL, PFL, PFB) and 12 characteristics (SNL, AHD, ML, DL, MW, AHW, IFW, PHW, HL, FDFI, CFL, PFB) (P<0.001). Eight morphometric characteristics (SNL, ML, DL, MW, HL, CFL, VFL, PFL) in female (N=36) and males (N=36) had significant differences (P<0.05) and three characteristics (ML, DL, HL) had very significant differences (P<0.001). Eleven characteristics (SNL, AHD, PHD, ML, DL, MW, AHW, IFW, PHW, HL, FDFL) were significantly different in maleL (N=36) and males (N=36) (P<0.05) and six (ML, DL, MW, AHW, HL, FDFL) were very significantly different (P<0.001).

Xuefuzhuyu Decoction for Essential Hypertension: Meta-Analysis and Systemic Review (혈부축어탕의 본태성 고혈압에 대한 치료 효과 관한 메타 분석 및 체계적 고찰)

  • Objective: The purpose of this study was to investigate the effect reducing blood pressure (BP) using Xuefuzhuyu Decoction (XFZYD) in adults with essential hypertension (EH). Method: Search engine, such as PubMed, EMBASE, Cochrane library, Web of Science, J-STAGE, CiNii, CNKI, OASIS, NDSL, KISS, RISS, and DBpia, were used. The search period was from the beginning of the search engine to June 30, 2018 and there were no limits regarding languages. The selection and extraction of literatures were performed independently by two authors. Meta-analysis was done on the total effective rate (TER), Systolic BP (SBP) and Diastolic BP (DBP). Cochrane's risk of bias (ROB) was used as the methodological quality assessment scale. Results: Twenty studies were finally selected. We observed that a combination treatment using XFZYD and an antihypertensive drug (AHD) was 5.1 times more effective in lowering BP than using AHD alone on TER. The mean differences in SBP and DBP were -10.65 mmHg (95% Confidential Interval (CI) -13.55 mmHg, -7.74 mmHg, P<0.00001), -5.92 mmHg (95% CI -7.14, -4.38, P<0.00001), respectively. Conclusion: A combination treatment using XFZYD and AHD may be more effective in reducing BP than using AHD alone. Because of the poor methodological quality of the studies conducted thus far, high-quality clinical trials will be required in the future.

Physiotherapy Approaches for Chronic Subacromial Impingement Syndrome : Comparison of Effects of Eccentric Training and General Exercise (만성 봉우리 밑 충돌증후군을 위한 물리치료적 접근법 : 편심성 훈련과 일반적 운동의 효과 비교)

  • Purpose : This study was to verify the effect of eccentric training and suggest a specific application method by comparing the effects of acromohumeral distance (AHD), supraspinatus tendon thicknees (STT), pain intensity and functional performance after MWM and eccentric training (MWM-ET) or general exercise (MWM-GE) in chronic subacromial impingement syndrome (SAIS) patients. Methods : A total of 55 participants were randomly assigned to each group, and according to the intervention method, "MWM-ET group (n=28)" vs. "MWM-GE group (n=27)" was divided into two groups. AHD, STT, pain intensity, and functional performance were measured before intervention, and both groups were re-measured 3 times a week after 6 weeks of intervention in the same way. Results : The AHD was significantly increased in MWM-ET group compared to MWM-GE group. No significant difference was observed between the groups in the STT, but Pain intensity was significantly lower in MWM-ET group than in MWM-GE group, and functional performance was significantly increased in MWM-ET group compared to MWM-GE group. Conclusion : As a result of MWM-ET intervention that further increases AHD compared to MGE, it can be clinically presented as a more effective intervention method for faster recovery from injury due to pain reduction and smooth return to daily life due to improved functional performance.

Physical Therapy Program for Patients with Partial Rotator Cuff (Supraspinatus) Tears with Moderate to Severe Shoulder Pain : Comparison of the effects of eccentric training and concentric training (중등도 이상의 어깨통증을 동반한 근육둘레띠(가시위근) 부분파열 환자를 위한 물리치료 프로그램 : 편심성 훈련과 동심성 훈련의 효과 비교)

  • Purpose : This study was to verify the effectiveness of eccentric training and to make clinical recommendations on detailed application methods by comparing the effects of changes in acromiohumeral distance (AHD), pain intensity, muscle strength, and functional performance after applying a 12-week eccentric training program (ET group) or concentric training program (CT group) for patients with partial rotator cuff (supraspinatus) tears with moderate to severe shoulder pain. Methods : A total of 29 subjects were assigned to either the "ET group (n=15)" or the "CT group (n=14)" through simple randomization and were measured in the same way at baseline before intervention, 4 weeks and 12 weeks after intervention. All subjects received a physical therapy program 12 sessions 3 times a week for the first 4 weeks, and physical therapy program, 12 sessions a week, from 4 weeks to 12 weeks, for a total 20 sessions. Ultrasound machine was used for AHD, visual analog scale (VAS) was used for pain intensity, electronic hand held dynamometer was used for muscle strength, Korean Constant shoulder score (K-CSS) used for functional performance. Results : AHD, pain intensity, and muscle strength did not show significant differences at 4 weeks, but improved numerically, and showed significant differences at 12 weeks (p<.05), showing superior results in the eccentric training group compared to the concentric training group. Functional performance showed significant changes at both 4 and 12 weeks (p<.05), and excellent results were also found in the eccentric training group. Conclusion : In patients with partial rotator cuff tears with moderate to severe shoulder pain, an eccentric training program increased acromiohumeral distance (AHD), reduced pain, and increased muscle strength. Therefore, it can be clinically presented as an intervention method that can quickly and effectively improve functional performance, which is the ultimate goal of physical therapy.
