에폭시레진계 봉함제와 상아질접착제로 근관충전시 미세누출에 관한 연구 (Sealing Ability of Root Canals Obturated with Gutta-percha, Epoxy Resin-based Sealer, and Dentin Adhesives)

  • 본 연구는 에폭시레진계 봉함제 (AH26)과 두 가지의 상아질 접착제와 함께 근관충전을 시 행하였을 때와 에폭시레진계 봉함제 단독으로 사용하였을 때의 미세누출을 혐기성 세균모델을 이용하여 평가하였다. 52개의 단근치를 이용하여 .04, .06 taper Profile (Dentsply-Maillefer, Ballaigues, Swiss)을 사용하여 변형된 crown-down pressureless법으로 근관형성 하였다. 형성된 치근을 12개씩 무작위로 나누어 4개의 실험군으로 하였으며, 나머지 치아 중 2개를 양성대조군에, 2개는 음성대조군으로 사용하였다. 제1군은 All Bond 2(Bisco, Itasca, IL, USA)를 적용하고 거타퍼쳐와 AH26 (Dentsply, Konstanz, Germany)을 이용하여 continuous wave of condensation technique으로 근관충전 하였으며, 제 2군은Prime & Bond NT (Dentsply, Konstanz, Germany)를 적용 후 거타퍼쳐와 AH26을 이용하여 충전하였으며, 제3군은 17% EDTA를 적용하여 도말층을 제거한 후 거터퍼쳐 와 AH26을 사용하여 충전하였다. 제4군은 17% EDTA를 적용하지 않고 거터퍼쳐와 AH26을 사용하여 충전하였다. Fusobacterium nucleatum (VPI 10197)을 추적자로 이용한 혐기성세균모델을 사용하여 혐기성배양기에서 배양시키면서 60일 동안 각군에 대한 미세누출 여부를 관찰하였다. 매일 배양액의 혼탁도와 색상변화를 관찰하여 기록하였다. 제4군에서 통계학적으로 유의할만한 미세누출을 가장 많이 보였으며(p<0.0005) 나머지 3개의 실험군에서는 서로간의 통계학적으로 유의할 만한 차이를 보이지 않았다. 주사전자현미경 관찰 시 제 1군과 제2군의 상아질 접착제가 상아세관으로 침투한 소견을 관찰 할 수 있었다.

근관 충전용 MTA의 밀폐 효과와 작업 시간 평가 (Evaluation of Sealing Effect and Working Time of Root Canal Filling MTA Materials)

  • 본 연구의 목적은 근관 충전재로서 MTA(Endoseal, Endoseal MTA)의 밀폐 효과와 효율을 평가하는 것이다. 총 106개의 발거된 단근치가 사용되었으며, 충전된 실러에 따라 AH군(AH-26), PR군(ProRoot MTA), ES군(Endoseal), EM군(Endoseal MTA)으로 나누어졌고, 모든 충전시 소요 시간이 측정되었다. 그 군들은 다시 A 하위군, B 하위군으로 나누어 근관벽과의 밀폐성과 상아세관내 실러의 침투도를 각각 평가하였다. 결과는 근관 충전용 MTA는 AH-26에 비해 근관벽과의 밀폐성과 상아세관 침투도는 떨어졌으며, ProRoot MTA와 비교시 근관벽과의 밀폐성은 차이가 없었지만, 상아세관 침투도는 높았다. 작업 시간은 근관 충전용 MTA가 ProRoot MTA와 AH-26에 비해 짧았다. 결론적으로 근관 충전용 MTA는 레진계 실러보다 근관 밀폐 효과는 떨어지지만 MTA가 필요한 근관 충전시 효과적인 대안이 될 수 있다.

  • 흰느타리버섯(Pleurotus cornucopiae(Per.) Rolland)중의 단백질을 선택적으로 분리정제하기 위하여 AH-Sepharose 4B를 출발 물질로 하여 benzoyl-AH-Sepharose 4B를 합성하여 친화성 크로마토그래피하였다. 1) AH-Sepharose 4B 중의 amino기의 capacity는 겔 Iml 당 $9.5{\mu}mol$ 이었으며 합성겔의 리간드인 benzoyl기의 capacity는 겔 1ml당 $9.3{\mu}mol4이었다. 2) 친화성이 있는 단백질의 총 겉보기 분자량은 255,000 돌톤 이었으며, 이는 29,500, 31,500, 34,000, 71,000 및 89,000돌톤 단백질의 복합체였다. 3) 친화성 단백질 중의 아미노산 함량은 비극성 아미노산이 45.68%, 극성 아미노산이 26.93%, 양성 아미노산이 11.81%, 그리고 음성 아미노산이15.58%였다. 4) 소수성 benzoyl기에 친화성이 있는 단백질은 비극성인 것이 선택적으로 분리되었다.

생체외 실험을 이용한 근관충전용 Sealer의 세포독성에 관한 연구 (A STUDY OF THE CYTOTOXICITY OF ROOT CANAL SEALER IN VITRO)

  • The purpose of this study was to evaluate the cytotoxicity of four root canal sealers(Tubliseal, AH26, Apatite Root Canal Sealer I, Apatite Root Canal Sealer II) in Vitro. The root canal sealers were mixed and filled in molds which were $14{\times}1.25mm$ in diameter, in height to use for cell counting and agar overlary method, and $7{\times}1.25mm$ for millipore filter method and set for 7 days to use for experiment. Silicone and copper plate were used for negative and positive control respectively. Using the culture of L929 fibroblast, total cell number and vital cell number were counted and the ratio of vital cell number to total cell number was calculated on 2 nd, 4 th, 6 th experimental day, and the change of cell membrane permeability was tested by agar overlay method, and the succinate dehydrogenase activity was tested by millipore filter method. The obtained results were as follows. 1. In ail experimental groups, the mitotic activity of fibroblast was reduced when compared with that of negative control group, so ail experimental groups showed cytotoxicity. Apatite Root Canal Sealer I group exhibited mild cytotoxicity, and Tubliseal, AH26, Apatite Root Cenal Sealer II groups exhibited severe cytotoxicity. 2. In the test of the change of cell membrane permeability by agar overlay method, all experimental groups showed cytotoxicity. AH26 group exhibited mild cytotoxicity, and Apatite Root Canal Sealer I group exhibited moderate cytotoxicity, and Tubliseal and Apatite Root Canal Sealer II group exhibited severe cytotoxicity. 3. In the test of SDH activity by millipore filter method, there was no cytotoxicity in Apatite Root Canal Sealer I and Apatite Root Canal Sealer II group, but Tubliseal and AH26 group showed mild cytotoxicity.

근관충전용 실러의 물리적 성질에 관한 연구 (A STUDY ON THE PHYSICAL PROPERTIES OF ROOT CANAL SEALERS)

  • The purpose of this study was to compare and estimate the physical properties of five root canal sealers classified Calciobiotic root canals sealer as calcium hydroxide based sealer, Apatite root sealer type II as calcium phosphate based sealer, AH-26 as resin based sealer, Canals and Pulpdent root canals sealer as zinc oxide eugenol based sealer. The author investigated dimensional change and flow rate of canal sealers, diametral tensile strength and shear bond strength of sealers to dentin to evaluate the physical properties on affect of complete obturation of root canal and performed the total 100 specimens of each 25 sealers under the condition of root temperature according to manufacturer's instructions. All specimens were stored at $37{\pm}1^{\circ}C$ in 100 % relative humidity. A microscope for measurement of micro distance is used for the dimensional change test and evacuation methods using vaccum were used for the flow rate test. The result differed by the storage time measured on the tests of diametral tensile strength and shear bond strength to dentin. The following results were obtained ; 1 On the test of dimensional change, Canals and Pulpdent expanded slightly, AH-26 and Apatite showed the severe shrinkage after 48 hours. 2. AH-26 and Apatite were the excellent with each 24.59mm, 31.19mm after 3 minutes in the aspect of flow property. 3. On the diametral tensile strength, Calciobiotic root canals sealer showed the highest strength with 27.13kg/$cm^2$ after 48 hours, Apatite root sealer type II showed highest strength with 84.57kg/$cm^2$ after 120 hours. 4. On the shear bond strength to dentin, AH-26 was most excellent with 55.73kgf/$cm^2$ after 24 hours and with 134.71kgf/$cm^2$ after 120 hours.

근관충전용 글라스 아이오노머 시멘트의 근단 폐쇄성과 치질 적합성에 관한 연구 (EVALUATION OF APICAL MICROLEAKAGE AND ADAPTATION TO THE DENTINAL WALL OF THE GLASS-IONOMER ROOT CANAL CEMENT)

  • The purpose of this study was to compare the apical leakage and adaptation to the dentinal wall of a glass ionomer root canal cement(Ketac-Endo) with those of AH-26 and Tubli-Seal. 102 single-rooted teeth were insrumented with step-back technique and randomly divided into four groups according to kinds of root canal cements: Ketac-Endo, AH-26, Tubli-Seal, and no root canal cement. Four experimental groups were as follows; Group 1 : Filling with gutta-percha and Ketac-Endo Group 2 : Filling with gutta-percha and AH-26 Group 3 : Filling with gutta-percha and Tubli-Seal Group 4 : Filling with gutta-percha without root canal cement All the specimens were obturated with lateral condensation technique, and stored in 100% humidity for 4 days. 20 teeth in each group were placed in centrifuge tube filled with India ink, and then centrifuged for 20 minutes at 3,000 rpm. And they were evaluated for linear dye leakage using a stereoscope and analyzed by one-way ANOVA. 22 teeth were split longitudinally and evaluated the adaptation to the dentinal wall using scanning electron microscope(4 teeth in each group) and stereoscope(2 teeth in 1, 2, 3 group). The obtained results were as follows ; 1. In case of not used root canal cement(Group 4), there was showed much degree of dye penetration comparerd with used root canal cements (Group 1, 2, 3)(P<0.01). 2. In the case of used root canal cements(Group 1, 2, 3), there was no significant difference in linear leakage in groups(P>0.01). 3. In the case of used root canal cements(Group 1, 2, 3), there was showed intimate adaptation to the dentinal wall independent on kinds of root canal cements. 4. Fractured surfaces indicated failure of cohesion in Ketac-Endo, and failure of adhesion to dentine in AH-26 and Tubli-Seal. 5. According to similar apical leakage and adaptability to the dentinal wall to another root canal cements, it is possible to be used glass-ionomer cement as root canal cement.

수종(數種) 근관충진재(根管充眞材)의 근관폐쇄성(根管閉鎻性)에 관(關)한 실험적(實驗的) 연구(硏究) (AN EXPERIMENTAL STUDY ON SEALING ABILITY OF VARIOUS ROOT CANAL SEALERS)

  • The purpose of this study was to evaluate and compare the sealing ability of ZOE, FR and AH-26. Seventy two upper and lower anterior and premolor teeth were randomly selected and instrumented in a conventional method with K-file. After instrumentation and dry the canal, the teeth were divided into 3 groups and twenty four teeth in each group were filled with ZOE, FR, and AH-26 respectively. All the specimens were immersed into Indian Ink and decalcified in nitric acid and cleared in methyl salicylate. The apical leakage was evaluated by measuring the degree of ink penetration with caliper at the intervals of 1 day, 7 days and 14 days. The results were as follows; 1. In AH-26 and FR groups, there was increase in penetration related to increased time of immersion in the ink. 2. FR group showed the least penetration in 3 groups. 3. Statistics showed that there was no significant difference among the each sealer groups.

새로운 레진 계통의 근관봉함재의 독성과 항균 작용에 대한 연구 (Cytotoxicity and Antibacterial property of New Resin-based Sealer)

  • 이 연구는 기존의 레진 근관봉함재를 보완하여 개발한 근관봉함재(Adseal; 새로운 레진 계통의 근관봉함재)를 이미 상품화된 레진 계통의 근관봉함재(AH 26, AH Plus), 산화 아연 유지놀 계통의 근관봉함재(TubliSeal EWT, Pulpcanal sealer EWT), 수산화 칼슘 계통의 근관봉함재(Sealapex)와 비교하여 세포독성과 항균작용을 평가하고자 한다. 세포독성 실험은 L929 쥐의 섬유아세포를 사용하여 세포의 viable ratio를 계산한 후, Giemsa stain으로 염색하여 세포의 양상을 관찰하였고, 항균작용 실험은 Enterococcus faecalis Porphyromonas endodontalis, Porphyomonas gingivalis, Prevotella intermedia, Fusobacterium nucleatum 와 Fusobacterium necrophorum를 사용하여 agar diffusion test로 평가한다. Adseal은 다른 근관봉함재에 비해 훨씬 낮은 세포독성을 보였고, AH Plus, AH 26, TubliSeal EWT, Sealapex, Pulpcanal sealer EWT의 순으로 세포독성의 정도가 높아짐을 알 수 있었다. 또한 Adseal은 Enterococcus faecalis 에서는 낮은 항균작용을 보이지만, Black-pigmented bacteria 에서는 높은 항균작용을 보이는데, 모든 근관봉함재는 서로 다른 종에 따라 어느 정도의 항균효과를 가지고 있음을 알 수 있었다.

적응 요인에 따른 보조부화술 (Assited Hatching, AH)의 효과 (The Effects of Assisted Hatching (AH) According to the Indications)

  • Implantation rates remain low following human in vitro fertilization (IVF). Suboptimal culture conditions may limit the ability of embryos to hatch as blastocysts, and artificial opening of the zona pellucida has been proposed as a means to promote subsequent hatching (assisted hatching). In this study, assisted hatching (AH) by zona drilling using acidic Tyrode's solution was performed in 320 patients, due to their age of more than 38 years (group A), the thick zona pellucida (group Z; $ZP\geq0.18{\mu}m$), and failures in implantation more than 3 times in previous IVF-ET trial (group P). This study was designed firstly, to study the effects of AH on the outcomes of IVF-ET according to the indications and secondly, to verify the appropriate application of AH. The results were as follows; 1. There was no difference in pregnancy rate between AH group (26.6%) and non-AH group (26.5%). 2. Assisted hatching (AH) showed significantly higher pregnancy rate of the patients with thick zona pellucid a than those of the patients with age factor and with the history of repeated implantation failure. But in the patients with age factor only, AH resulted in higher pregnancy rate. 3. Interestingly, the patients with complex factors including zona factor (Z: 33.9%; ZA: 30.4%; ZP: 31.6%; ZAP: 21.4%) showed higher pregnancy rates than other complex factors excluding zona factor (A: 24.4%, P: 0%; AP: 10.8%). From these results, AH is more helpful to the patients with thick zona pellucida rather than patients with older age and/or previous repeated implantation failure.

Detection and Characterization of 45 kDa Platelet Activating Factor Acetylhydrolase in Cerebrospinal Fluid of Children with Meningitis

  • Platelet activating factor acetylhydrolase (PAF-AH) activity has been identified in cerebrospinal fluid (CSF) samples taken from children with meningitis. We reported that PAF-AH activity is significantly increased, by about 3 fold, in patients with meningitis compared to control subjects. Because of limited knowledge about this enzyme in CSF, we examined the biochemical properties of CSF PAF-AH. PAF-AH of CSF was calcium independent, showed a broad pH spectrum and was relatively heat stable. In addition, this enzyme activity was strongly inhibited by phenylmethanesulfonyl fluoride (PMSF), partially inhibited by p-bromophenacylbromide (p-BPB), uninhibited by iodoacetamide, and moderately stimulated by dithiothreitol (DTT). PAF-AH of CSF did not degrade phospholipid with a long chain fatty acyl group at sn-2 position. This enzyme hydrolyzed PAF and oxidatively modified phosphatidylcholine. Furthermore, we identified a monomeric polypeptide with a molecular weight of approximately 45 kDa by Western blot using human plasma PAF-AH antibody. These results suggested that plasma type PAF-AH activity exist in CSF taken from children with meningitis.
