주의력결핍 과잉행동장애 한국형 치료 권고안(II) - 진단 및 평가 - (The Korean Practice Parameter for the Treatment of Attention - Deficit Hyperactivity Disorder(II) - Diagnosis and Assessment -)

  • Probably the three most important components to a comprehensive evaluation of patients with attention-deficit hyperactivity disorder (ADHD) are the clinical interview, the medical examination, and the completion and scoring or behavior rating scales. Teachers and other school personnel are often the first to recognize that a child or adolescent might have ADHD, and often play an important role in the help-seeking/referral process. A diagnostic evaluation for ADHD should include questions about ADHD symptoms, other problems including alcohol and drug use, family history of ADHD, prior evaluation and treatment for ADHD. Screening interview or rating scales as well as interviews should be used. When it is feasible, clinicians may wish to supplement these components of the evaluation with objective assessments of the ADHD symptoms, such as psychological tests. These tests are not essential to reaching a diagnosis, however, or to treatment planning, but they may yield further information about the presence and severity of cognitive impairments that could be associated with some cases of ADHD. Screening for intellectual ability and academic achievement skills is also important in determining the presence of comorbid developmental delay or loaming disabilities. The number and type of symptoms required for a diagnosis of ADHD vary depending on the specific subtype. To receive a diagnosis of ADHD, the person must be experiencing significant distress or impairment in daily functioning, and must not meet criteria for other mental disorders which might better account for the observed symptoms such as mental retardation, autism or other pervasive developmental disorders, mood disorders, anxiety disorders. This report aims to suggest a practice guideline of assessment and diagnosis for children and adolescents with ADHD in Korea.

시각 주의력과 청각 주의력의 차이를 보이는 주의력 결핍.과잉활동장애 아동의 인지기능과 정서 및 행동 문제 (The Cognitive Performance, Emotional and Behavioral Problems of the Children with ADHD Showing the Difference between Visual and Auditory Attention)

  • 목 적: 시각 주의력과 청각 주의력에서 차이를 보이는 주의력결핍 과잉활동장애(ADHD) 아동의 인지 기능 및 정서, 행동 문제에 대해 연구하고자 한다. 방 법: 동반 질환이 없는 ADHD 아동 98명을 대상으로 '주의력장애 진단시스템(ADS)' 을 실시하여 시각 주의력과 청각 주의력을 측정한 뒤, ADS의 누락 오류와 오경보 오류를 이용하여 이들을 1) 청각 누락 오류 및 오경보 오류가 시각 누락 오류 및 오경보 오류보다 높은 군(AA군) 2) 시각 누락 오류 및 오경보 오류가 청각 누락 오류 및 오경보 오류보다 높은 군(VV군) 3) 1), 2)에 해당하지 않는 군(M군)의 3개 군으로 분류하였다. 세 군 간에 '한국판 웩슬러 아동용 지능 검사(KEDI-WISC)' 의 소검사 결과와 '한국판 아동-청소년 행동평가척도(K-CBCL)' 의 여러 하위 소척도의 점수를 비교하였다. 또한, 전체 아동을 대상으로 시각 누락 오류, 청각 누락 오류, 시각 오경보 오류, 청각 오경보 오류와 KEDI-WISC의 소검사 점수, K-CBCL의 하위 영역 점수 간의 상관관계를 조사하였다. 결 과: 다음의 결과를 얻었다. 1) AA군은 10명, M군은 32명, VV군은 56명이었다. 2) KEDI-WISC의 소검사 점수 비교에서는 VV군의 점수가 나머지 두 군보다 높았다. 특히 언어성 지능(p=.039)에서는 VV군이 AA군보다 유의하게 높았고 토막짜기(p=.015), Kaufman의 요인 2(p=.045), 동작성 지능(p=.004)에서는 VV군이 M군보다 유의하게 높았으며 전체 지능(p=.004)에서는 VV군이 M군 및 AA군보다 유의하게 높았다. 3) K-CBCL의 하위 소척도 점수 비교에서는 신체증상 소척도 점수를 제외한 전 영역에서 VV군의 점수가 나머지 두 군보다 높았다. 특히 사회성 소척도(p=.041)에서는 VV군이 AA군보다 유의하게 높았으며 위축 소척도(p=.021)에서는 VV군이 AA군보다 유의하게 낮았다. 4) 시각 누락 오류, 시각 오경보 오류와 KEDI-WISC의 소검사 점수 간에는 유의한 상관관계가 존재하지 않았다. 한편 청각 누락 오류, 청각 오경보 오류와 KEDI-WISC의 소검사 점수 간에는 유의한 상관관계를 보이는 영역이 매우 많았다. 5) 시각 누락 오류, 시각 오경보 오류와 K-CBCL의 소검사 점수 간에는 시각 누락 오류와 사고의 문제 소척도(r=.205, p=.043) 간에만 유의한 상관관계가 존재하였다. 청각 누락 오류와는 사회성 소척도(r=-.319, p=.001), 사회적 미성숙 소척도(r=.206, p=.042), 사고의 문제 소척도(r=.235, p=.021) 간에 유의한 상관관계가 있었다. 청각 오경보 오류와는 사회성 소척도(r=-.241, p=.017) 및 주의집중문제 소척도(r=.235, p=.020) 간에 유의한 상관관계가 있었다. 결 론: 청각 주의력이 상대적으로 더 나은 ADHD 아동이 다른 아동에 비해 인지 기능이 더 좋고 정서 및 행동 문제가 적었다. 향후 시각 주의력과 청각 주의력에 차이를 보이는 ADHD 아동에 대해 보다 체계적인 연구가 필요할 것이다.

주의력결핍과잉행동장애 아동의 집중력 향상과 충동성 조절을 위한 바이올린 교습 중심의 음악치료 사례 연구 (Improvement of Attention Span and Impulsivity of Children with Attention Deficit Hyperactivity Disorder through Structured Violin Learning)

  • 본 연구의 목적은 바이올린 교습 중심의 음악치료 활동이 주의력결핍과잉행동장애 아동들의 집중력과 충동성 조절에 변화를 가져오는지 연구하고 그러한 변화가 일상생활에 전이가 되는지 보고자 하는 것이다. 본 연구는 연령대가 비슷하고 종합병원에서 ADHD라고 진단을 받은 초등학교 2학년, 3학년, 4학년을 대상으로 선정하여, 본 연구자가 8주에 걸쳐 14번의 개별 세션과 마지막 회기의 발표회 연주로 총 15회기를 실시하였다. ADHD 아동의 집중력 향상과 충동성 조절의 변화를 살펴보기 위하여 15회 음악치료 활동 전 후에 실시한 대상 아동의 문제행동측정리스트(K-CBCL) 검사도구 중 주의 집중문제 항목, 단축형 Conners 부모용 평정척도(ACRS), 가정환경 설문지 개정판(CHSQ-R), 개정판 Conners 교사 평정척도(CCTRS-28) 검사 도구의 점수를 비교 분석하였다. 또한, 세션 안에서 보이는 '주의산만행동'과 '학습 외 행동'은 빈도수 기록으로 나타내었으며, '학습 외 행동' 중 가만히 있지 못하고 손발을 움직이는 '손발 행동'은 등간 기록법, '주의집중시간'은 지속 시간 기록법으로 표시하였다. 위의 척도들이 가지는 양적인 데이터의 단점을 보완하기 위하여 아동들이 음악활동에서 보여 준 집중력과 충동 조절에 관한 반응을 질적으로 분석하였다. 그 결과 바이올린 교습 중심의 음악치료 실시 후 ADHD 아동의 집중력과 충동성 조절이 향상되었으며 가정이나 학급에서 발생하는 문제 행동 감소에 효과가 있음이 확인되었다. 본 연구는 선행 연구가 빈약한 분야인 바이올린 교습을 치료적으로 적용하여 실제 ADHD 아동을 대상으로 음악치료 활동의 효과를 입증한 점에서 의의가 있다. 즉 기존의 약물치료와 인지행동치료의 제한점으로 인해 모든 ADHD 아동들에게 그러한 치료를 적용할 수 없다는 한계와 현실적인 한국 상황 내 치료 관행을 고려해 볼 때, 바이올린 교습 중심의 음악치료적 접근이 ADHD 아동에 대한 치료적인 대안이 될 수 있음을 시사한다.

An Update on Mental Health Problems and Cognitive Behavioral Therapy in Pediatric Obesity

  • Prevalence of pediatric obesity has increased worldwide in the last 20 years. Obese children suffer not only physical complications but also mental health problems such as depression, attention deficit hyperactivity disorder (ADHD), and eating disorders, as well as psychosocial impairments, such as school adjustment problems, bullying, and low self-esteem. Recently, there have been some studies on the association of mental health problems and pediatric obesity. In the treatment of pediatric obesity, many previous studies suggest multidisciplinary treatment. However, cognitive behavioral therapy (CBT) has attracted attention because obese children are accompanied by body image distortion, emotion dysregulation, and difficulties in stimulus control. This review is a narrative summary of the recent studies on mental health problems and CBT in pediatric obesity. The relationship between depression/anxiety and pediatric obesity is still inconsistent but recent studies have revealed a bidirectional relation between depression and obesity. Additionally, some studies suggest that obese children may have eating disorder symptoms, like loss of control eating, and require therapeutic intervention for pediatric obesity treatment. Furthermore, impulsivity and inattention of ADHD symptom is thought to increase the risk of obesity. It has also been suggested that CBT can be very effective for mental health problems such as depression, impulsivity, and body image distortion, that may coexist with pediatric obesity, and use of multimedia and application can be useful in CBT.

  • Attention-deficit hyperactivity disorder (ADHD) is a highly prevalent, impairing, and comorbid disorder that persists into adulthood. ADHD should be diagnosed in the same manner as other common adult mental health disorders. The three most important components in the comprehensive evaluation of patients with ADHD are the clinical interview, medical examination, and completion and scoring of behavior rating scales. The diagnostic evaluation of ADHD should include questions about the symptoms, family history, prior evaluation and treatment of ADHD, as well as other problems including alcohol and drug use. Screening interviews or rating scales, as well as interviews, should be used. When it is feasible, clinicians may wish to supplement these components of the evaluation with the objective assessments of the ADHD symptoms, such as through psychological tests. These tests are not essential to reaching a diagnosis, however, or to treatment planning, but may yield further information about the presence and severity of cognitive impairments that could be associated with some cases of ADHD. As comorbidity is the rule rather than the exception, clinicians should carefully screen for comorbid disorders as part of a comprehensive assessment of ADHD. 주의력결핍 과잉행동장애와 스마트폰 중독, 우울, 불안, 자존감 (Smart-Phone Addiction, Depression/Anxiety, and Self-Esteem with Attention-Deficit Hyperactivity Disorder in Korean Children)

  • Objectives : The current study investigated the risk of smartphone addiction among children and adolescents with or without attention-deficit hyperactivity disorder (ADHD), risk of depression, anxiety, and self-esteem using the Smartphone Addiction Scale Proneness, Kovac's Children's Depression Inventory, State-Trait Anxiety Inventory, and Rosenberg Self-Esteem Scale, commonly used in clinical medicine. Methods : Ninety five students with ADHD who visited psychiatry outpatient clinics completed the questionnaire. At the same time, 592 middle and high school students living in a similar area regardless of ADHD diagnosis, completed the questionnaire as control subjects. Results : Overall, 40.0% of 95 ADHD and 12.8% of 592 control subjects were classified as the smartphone addiction proneness group, 26.3% of the ADHD subjects and 8.3% of the control group were classified as the depression group, and 32.6% of the ADHD subjects and 16.2% of the control group were classified as the anxiety group. Significant differences were observed between the two groups. Conclusion : The results of this study suggest that ADHD subjects are more prone to smartphone addiction, becoming depressed or anxious than those in the control group. From this study, we could suggest that students with ADHD are more easily affected by smartphone addiction than normal control subjects. In addition, we might understand how some psychiatric problems like depression, anxiety, and low self-esteem are related to ADHD and smartphone addiction.

게임을 이용한 행동적 집단상담이 ADHD 아동의 행동변화에 미치는 효과 (Effectiveness of Behavior Group Counseling Utilizing the Games of Children with Attention Deficit Hyperactivity Disorder)

  • The effectiveness of behavior group counseling using the games of children with Attention Deficit Hyperactivity Disorder(ADHD) was studied with 24 primary school children (12 each in the experimental and in the control groups). The subjects were selected on the basis of the Abbreviated Conners Teacher Rating Scale-Revised. The counseling program consisted of 12 sessions over a 6-week period. Effectiveness of the training was evaluated with the Conners Teacher Rating Scale-Revised, the ADHD teacher's rating scale, and the Korea-Child Behavior Checklist. Findings were that the group receiving counseling exhibited improved attention, social relationships, and self control. They also exhibited decreased carelessness-passivity, hyperactivity, hostility and fewer behavior problems.

주의력결핍 과잉행동장애에서 정신자극제를 이용한 약물치료 (Stimulants Medication of Attention-Deficit Hyperactivity Disorder)

  • Attention-deficit hyperactivity disorder (ADHD) is characterized by inattention, hyperactivity, impulsiveness and problems in other higher cognitive processes such as executive function deficits. Currently, there are many treatment modalities, of which pharmacotherapy is the most strongly supported by scientific and clinical evidence. Stimulants, which are first choice in the pharmacological treatment of ADHD, block dopamine reuptake by binding the dopamine transporter and so increasing the concentration of dopamine in synaptic clefts. Stimulants are effective in improving core ADHD symptoms, as well as the nonspecific symptoms, such as aggressiveness and oppositional behavior. Frequently reported short-term adverse effects are decreased appetite, sleep disturbance, headache, dizziness and irritability. Although questions have been raised about the long-term side effects of stimulants, including growth suppression, cardiovascular events, and abuse potential, there is no clear evidence to support these concerns.

Comparison of Adults with Attention-Deficit Hyperactivity Disorder Depending on the Age of Being Diagnosed in Childhood and Adulthood: Based on Retrospective Review in One University Hospital

  • Objectives: The study aimed to identify the characteristics of attention-deficit hyperactivity disorder (ADHD) that was not diagnosed in childhood or adolescence, but only in adulthood. Methods: The characteristics of patients diagnosed with ADHD in adulthood were compared with those of patients diagnosed in childhood were assessed via a retrospective review of the medical records at one university hospital from 2005 to 2013. If the age at which they were confirmed as having ADHD was less than 19 years old, they were grouped as childhood-diagnosed group (CD); if they were 19 years old or more, they were grouped as adulthood-diagnosed group (AD). Results: The CD and AD included 50 (46.3%) and 58 (53.7%) patients, respectively. Inattention was the most common symptom in both groups. Behavioral and emotional problems were the second most frequent symptoms in the CD and AD, respectively. The intelligent quotient was significantly higher in the AD than in the CD. The most common comorbidity was depression in the CD and personality disorder in the AD. The most common reason for visiting the hospital was referral by acquaintances in the CD and media coverage in the AD. Conclusion: Clinicians should put ADHD on the index of suspicion when they examine adults with various psychiatric symptoms, because the diagnosis of ADHD might have been missed in childhood and the symptoms of ADHD might have changed as they grew up.

주의력결핍과잉행동장애 아동에 대한 초등교사의 지도실태와 지식 (Teaching Status and Knowledge of Elementary School Teachers of Children with Attention Deficit Hyperactivity Disorder)

  • Purpose: The purpose of this study was to analyze teaching status and knowledge about children with Attention Deficit Hyperactivity Disorder (ADHD) in elementary school teachers. Methods: The participants of this study were 204 elementary school teachers in Busan. From July to September 2010, the researchers collected data by using self-report questionnaires consisted of questions about their teachers' characteristics and teaching status and KADDS (36 items) developed by Sciutto, Terjesen and Frank. Frequency, percentage, arithmetic mean, standard deviation, t-test and ANOVA were used to analyze the data using SPSS WIN 16.0 program. Results: Less than 50% of the participants had previous education on ADHD. According to this experience, there were significant differences in participants' knowledge about children with ADHD (t=4.703, p<.001). For the 36 items, participants had an average of $19.19{\pm}4.58$ points out of a possible 36. Moreover, lack of knowledge about teaching children with ADHD and communication problems with the parents of these children were considered to be the most disturbing factors. Conclusion: These results suggest that networks among teachers, parents and medical care professionals should be established. The results also indicate that educational programs for elementary school teachers should be developed in order to identify and teach children with ADHD appropriately.
