  • 본 논문에서는 근골격계 질환 중에서 가장 흔히 나타나는 어깨의 질환에 대하여 살펴보도록 한다. 특히 어깨를 옆으로 반드시 폈다가 안으로 오므리는 수평 벌림과 수평 모음에 대한 측정 방법을 살펴본다. 여기서 일정한 임계값보다 크게 나타나는 값인 통증 유발에는 반드시 도수 치료가 필요하다고 본다.

Effects of Vowel Differences on Laryngeal DDK (모음에 따른 후두 교호운동 특성)

  • This study investigated the vowel effect on laryngeal DDK (L-DDK) in terms of rate, regularity, and range. Thirteen normal speakers participated in this experiment. Speakers were asked to repeat the vowels /a, e, i, o, u/ for vocal fold adduction DDK, and /ha, he, hi, ho, hul for vocal fold abduction DDK. Acoustic data was analyzed via Motor Speech Profile. There were 6 parameters: DDKavp and DDKavr for rate of L-DDK, DDKcvp and DDKjit for regulariry of L-DDK, and DDKavi and DDKcvi for range of L-DDK. Results of MANOVA and Fredman analysis showed no significant vowel effect on rate and regularity of L-DDK. MANOVA revealed significant effects of vowels and vocal fold ab/adduction on range of L-DDK. DDK peak intensity (DDKavi) in vowel /i/ production was lower than in vowels /a, e, o, u/. Variation of DDK peak intensity (DDKcvi) was significantly greater for /ha/ than for /a/ production. The implication of these findings on voice and speech pathology is discussed.

Generation of 3-dimensional isocomfort workspace using the robot kinematics (로보트 기구학을 이용한 3차원 등편의 작업영역의 생성)

  • The purpose of this study is to obtain 3-dimensional isocomfort workspace using the robot kinematics, which is based on perceived discomfort in varying postures for manipulating four types of controls. Fifteen healthy male subjects participated in the experiment where their perceived discomfort in the given postures was measured, in which L32 orthogonal array was adopted. The shoulder flexion and adduction-abduction, elbow flexion, types of controls, and right/left hands were selected as experimental variables. The results showed that the shoulder flexion and adduction-abduction, elbow flexion, and types of controls significantly affected the perceived discomfort at .alpha. =0.01. Depending upon the types of control used, regression equations predicting perceived dis- comfort and three dimensional isocomfort workspace were suggested based on the experiemntal cata. Using the equations, driver's isocomfort workspace in his/her cabin for pushing operation was illustrated, in which the robot kinematics was employed to describe the translational relationships between the upper arm and the lower arm/hand. It was ecpected that isocomfort workspace could be used as a valuable guideline to design workplaces ergonomically.

  • For an effective acupuncture treatment, the location of muscles around Sutaeumkyongkun and Suyangmyongkyongkun were researched and they were made contracted. The conclusion is as follows; 1. The contraction of muscles around Sutaeumkyongkun gives appearance of the postures ; free movement of thumb, abduction of extension of wrist, flexion and pronation of elbow, depression and abduction of girdle of superior limb, flexion, internal rotation and horizontal flexion of shoulder joint These postures all together consequently produces the action 'holding something in arms'. 2. The contraction of muscles around Suyangmyongkyongkun gives appearance of the postures; extension of metacarpophalangeal and interphalangeal joint of index finger extension and abduction of thumb, extension of wrist, extension and supination of elbow, adduction, elevation and upward rotation of girdle of superior limb, extension, abduction, adduction, internal rotation, external rotation, horizontal extension of shoulder joint, flexion and opposite rotation of neck. These postures all together consequently produces the action 'raising arms'.

  • The upshoot and downshoot in Duane's retraction syndrome is believed to be related to a leash effect from the lateral rectus muscle. When the eye is rotated into the adducted position, the lateral rectus muscle slips over the globe, producing the up-and downshoot on adduction. The splitting of the ends of the lateral rectus into a Y configuration prevents the rotation of the globe up or down by stabilizing the muscle's position on the eye. Three patients with Duane's retraction syndrome demonstrated abnormal vertical movement on adduction and underwent a Y-splitting on the lateral rectus. In all patients, marked decrease in the up-and downshoot is noted after surgery.

  • Laryngeal framework surgery (LFS) is a unique phonosurgical concept that enables us to influence the laryngeal biomechanics by changing the shape/position of the laryngeal cartilages. LFS procedures can be favorably combined with one another but also with other phonosurgical methods, and they are usually reversible and correctable. Type I thyroplasty and arytenoid adduction are still useful in spite of the recent popularity of injection laryngoplasty. Basic surgical principles have seldom been changed since Isshiki's development, but a number of modifications have been tried and are still going on. These delicate surgeries require exhaustive training, but the reward is great to both the surgeon and the patient.

  • Brown syndrome is known as limited elevation of the affected eye during adduction. It is caused by a disorder of the superior oblique tendon, which makes it difficult for the eyeball to look upward, especially during adduction. It is classified into congenital true sheath Brown syndrome and acquired simulated Brown syndrome. Acquired simulated Brown syndrome can be caused by trauma, infection, or inflammatory conditions. The surgical restoration of blowout fractures can also lead to limitations of ocular motility, including Brown syndrome. We report on three patients with acquired simulated Brown syndrome, who complained of diplopia and limitation of ocular motility after operations to treat blowout fractures.

  • 본 연구의 목적은 소방용 coverall의 디자인이 소방수들의 동작기능성에 어떤 영 향을 주는지에 관하여 고찰하여, 보다 기능적 인 coverall을 개발, 궁극적으로 작업자의 노동력을 향상시키고, 보다 안락한 환경 속에서 작업할 수 있도록 도와주는데에 있다. 미국 캔사스 지역의 전문 소방수를 대상으로, interview 및 video analysis를 통하여, 그들이 현재 사용하고 있는 current coverall의 동작기능면에서의 문제점을 조사, 분석한 후, 이를 바탕으로 prototype coverall을 개발하였다 prototype coverall의 동작기능성을 객관적으로 측정하기 위하여 10가지 실험동작(shoulder adduction/ abduction, shoulder flexion/extension, trunk flexion/extension(standing), hip flexion/extension(knee), hip flexion/extension(trunk), upper leg flexion, trunk lateral flexion, hip adduction, trunk flexion/ extension(sitting), upper leg flexion)을 선택하여 이의 관절각도(range-of-motion)를 Leighton flexometer, goniometer, computer-aided method를 이용하여 측정하였으며, 착용자의 주관적 평가를 위하여서는 23 항목의 wearer acceptability scale를 사용하였다. ANOVA(Analysis of Variance)와 LSD(Least Significant Difference)를 이용한 통계처리 결과, prototype coverall이 current coverall에 비해 동작기능성 (특히 crotch, torso, hip movement)에 있어서 우수함을 보이고 있다.

Principles and Complications of Laryngeal Framework Surgery (후두골격수술의 원칙 및 합병증)

  • Laryngeal framework surgery comprises medialization laryngoplasty and arytenoid adduction. Since their introduction in the 1970s, these procedures have become standard treatments for vocal fold paralysis and glottal incompetence. However, frequency of laryngeal framework surgery is conjectured to relatively decrease along with the introduction of injection laryngoplasty. In this manuscript, indications for laryngeal framework surgery were highlighted in contrast to those of injection laryngoplasty. The authors introduced the basic concepts and principles as well as surgical techniques of laryngeal framework surgery. Even though the incidence of major and/or minor complications after laryngeal framework surgery is not high, surgeons should be well aware of its possible complications and they should be familiar with tips and know-how to avoid or cope with complications.
