Purpose: The purpose of this study was to identify changes in knee muscle strength after reconstruction of the anterior cruciate ligament (ACL) and the posterior cruciate ligament (PCL). Methods: Thirteen subjects (males) with anterior ligament injury and ten subjects (males) with posterior ligament injury voluntarily participated in this study. Both groups were evaluated at the pre-and post-reconstruction stages using an isokinetic dynamometer. Peak torque, total work, and the hamstrings to quadriceps (H/Q) peak torque ratio were calculated at angular velocities of 60°/sec and 180°/sec. Statistical analysis was conducted on SPSS 18.0 for Windows using t-tests to compare mean differences. Results: At an angular velocity of 60°/sec, both the ACL and PCL groups showed a significant increase in muscle strength in the flexors and extensors. Muscle strength in the extensors was significantly increased in the PCL group compared to the ACL group. At an angular velocity of 180°/sec, the ACL group showed a significant increase in muscle endurance in the flexors and extensors, and the PCL group showed a significant increase in muscle endurance in the flexors. At angular velocities of 60°/sec and 180°/sec, the H/Q peak torque ratio increased in the ACL group but decreased in the PCL group. Consequently, the H/Q peak torque ratio was significantly different for the two groups. Conclusion: The results suggest that the patients with ACL injury should focus on strengthening the knee extensors and that the patients with PCL injury need to strengthen the knee flexors.
Purpose: The aim of this study was to evaluate short-term clinical results and second-look arthroscopic findings after double-bundle anterior cruciate ligament (DB ACL) reconstruction. Materials and Methods: Forty-nine patients, who were followed up for at least 24 months after DB ACL reconstruction, were included. Clinical results, such as, Lysholm knee and Tegner activity scores, and manual laxity and instrumented anterior laxity test results were evaluated. In fifteen patients (15 knees), second-look arthroscopy with staple removal was performed. At second-look arthroscopy, the authors assessed about reconstructed ACL rupture, subjective graft tension and extent of synovial coverage. Results: Lysholm knee scores significantly improved from 67.4 preoperatively to 96.1 at last follow-up (p<0.01). Tegner activity scale improved from 2.0 to 6.1. The Lachman test, at last follow-up, showed normal laxity in 39 (of 49) patients, and the pivot-shift test showed normal laxity in 36 (of 49) patients. Mean side-to-side differences improved significantly from 10.8 mm to 3.3 mm (p<0.01). Second-look arthroscopic findings showed that all patients had a normal or a near normal anteromedial bundle. However, 8 patients (53.3%) were found to have partial or complete posterolateral bundle rupture. Conclusion: Even though double-bundle ACL reconstruction was clinically effective means of restoring knee rotational and anteroposterior stabilities, there were some ruptured posterolateral bundles observed in cases under arthroscopy after double-bundle ACL reconstruction.
Arthroscopic anterior cruciate ligament(ACL) reconstruction using four-strand hamstring tendon with looping around transfixing screw in femoral tunnel requires osteointegration between the grafted tendon and bone for stability of the knee. Authors have experienced a histologic finding of osteointegration between the grafted autogenous hamstring tendon and bone in femoral tunnel after arthroscopic ACL reconstruction. A patient received arthroscopic ACL reconstruction with autogenous four strand hamstring tendon for the ACL injury. Traumatic re-rupture of mid-substance of ACL graft was developed at thirteenth week after operation. During the procedures of arthroscopic revision at fifteenth week after initial ACL reconstruction, biopsy was performed at the site of interface between grafted tendon and bone in femoral tunnel. Integration between the grafted tendon and bone was evident by demonstrating the continuity of collagen fiber between bond and tendon. This histologic finding and the low incidence of early graft failure suggest that free tendon autograft attached to bone by looping around a transfixing screw in femoral tunnel undergoes adequate osteointegration between 12 and 15 weeks after surgery and authors thought that insertion of bone chip into the femoral tunnel would accelerate osteointegration procedure.

Objectives : The aim of this study was to report the effect of Korean medical therapy on pain and dysfunction after anterior cruciate ligament (ACL) reconstruction. Methods : A 25-year-old man experienced severe pain after right ACL reconstruction surgery. He received Korean medical treatments such as acupuncture, herbal medicine, and physiotherapy from July 10, 2014 to August 2, 2014. Results : After the treatments, his visual analogue scale scores generally decreased and the range of motion of the right knee improved from $0^{\circ}$ to $90^{\circ}$. Furthermore, the Knee Infury and Osteoarthritis Outcome Score increased from 99 to 142. Conclusion : The findings suggest that Korean medical treatments might be effectively used to treat pain and dysfunction after soft-tissue surgeries such as ACL reconstruction. Nevertheless, further research is warranted because of the limited sample size of this study.
Purpose: To investigate the causes of failure on ACL reconstructions and evaluate the effectiveness of one stage revision ACL reconstructions. Materials and Methods: From November 2004 to July 2008, thirty three patients who had got revision ACL recontstructions after reruptures of ACL were evaluated. The causes of failure of ACL reruptures were 22 vertical femoral tunnels, 7 neglected PLRI, 3 severe traumas and 1 deep infection after ACL reconstruction. The femoral tunnels were aimed at the 10 or 2 o'clock position and the tibial tunnels were used with previous tunnels. Previous femoral screws from the improper femoral tunnels were removed and filled with the new allograft bones. Results: The average periods of follow up were 22.2 months (12~52 months). There was improvement on an average Lysholm knee score from $61.5{\pm}16.8$ to $86.3{\pm}11.5$, IKDC score from $63.9{\pm}15.1$ to $81.3{\pm}14.3$. Mean side to side difference was decreased from $6.0{\pm}2.2\;mm$ to $1.6{\pm}1.4\;mm$ using KT-2000 arthrometer. Conclusion: One stage revision ACL reconstruction can be a useful method with good clinical results.

Purpose: This study was to determine the effects of before and after application of silicone sleeve on balance and muscle strength in anterior cruciate ligament (ACL) reconstruction patients. Methods: 13 subjects who had one or more months after ACL surgery were involved. Dynamic balance, timed up and go (TUG), stair step, vertical jump, proprioception and isokinetic knee strength were measured while subjects under taped, untapped and silicone sleeve conditions. Results: For 30 seconds one-leg standing, there was a significant improvement under silicone sleeve on operated side with eyes open and both taping and silicone sleeve revealed similar effects with eyes closed (p<0.01). Application of silicone sleeve showed significant effects in proprioceptive function on the operated side compared to both taping and none (p<0.05). For stair step test, TUG and vertical jump was a tendency to improve after application of silicone sleeve, but no significant different. Muscle strength on operated side of quadriceps and hamstring was significantly improved compared with none or taping(p<0.05). Conclusion: Silicone sleeve application for ACL reconstruction patients was effective immediately on improving strength and balance. Therefore, depending on the intended use and the disease is considered appropriate use of silicone sleeve will be able to help prevention and functional movement.

The knee joint is most commonly affected. Until now, there were several reports of this condition, but this report seems to be the first one reporting localized Pigmented Villonodular Synovitis (PVNS) after anterior cruciate ligament(ACL) reconstruction. The authors experienced localized PVNS after ACL reconstruction with bone-patellar tendon-bone at follow-up 4 years and confirmed the diagnosis by pathologic examination.
The goal of rehabilitation after ACL reconstruction are return the patient to a reinjury level of activity with stable joint, removing pivot shift phenomenon, preservation of meniscus, restoration of range of motion, and minimize patello-femoral complication. The ACL reconstruction should avoid immediate surgery. The preoperative phase emphasizes two important factors. (1) The patient should have a resolution of knee swelling, a return of full ROM, and a normal gait. (2) The patient should be mentally prepared for the operation and subsequent rehabilitation. The postoperative rehabilitation program emphasizes extension, closed kinetic chain function exercises. The regular follow-up is important.

Pretibial cyst formation is a rare complication after anterior cruciate ligament (ACL) reconstruction and there are only few cases reported. Pretibial cyst can be caused by variable reasons. Foreign body reaction due to breakdown of bio-absorbable screw for fixation, graft necrosis at tibial site, joint fluid leakage to tunnel and incomplete incorporation of graft to bony tunnel. The authors experienced one case of massive pretibial cyst after arthroscopic ACL reconstruction using bio-absorbable interference screw in 38-year-old male patient. Thus, authors report this rare case with literature view.
