  • Purpose : In contrast with traditional time-cycled, pressure-limited ventilation, during volume-controlled ventilation, a nearly constant tidal volume is delivered with reducing volutrauma and the episodes of hypoxemia. The aim of this study was to compare the efficacy of pressure-regulated, volume controlled ventilation (PRVC) to Synchronized intermittent mandatory ventilation (SIMV) in VLBW infants with respiratory distress syndrome (RDS).Methods : 34 very low birth weight (VLBW) infants who had RDS were randomized to receive either PRVC or SIMV with surfactant administration : PRVC group (n=14) and SIMV group (n=20). We compared peak inspiratory pressure (PIP), duration of mechanical ventilation, and complications associated with ventilation, respectively with medical records. Results : There were no statistical differences in clinical characteristics between the groups. After surfactant administration, PIP was significantly lower during PRVC ventilation for 48hrs and accumulatevive value of decreased PIP was higher during PRVC ventilation for 24hrs (P<0.05). Duration of ventilation and incidence of complications was no significant difference. Conclusion : PRVC is the mode in which the smallest level of PIP required to deliver the preset tidal volume in VLBW infants with RDS, adaptively responding to compliance change in lung after surfactant replacement.

  • Background: Nitric oxide, a vasodilator synthesized from L-arginine by vascular endothelial cells, accounts for the biological activity of endothelium derived relaxing factor. Previous studies demonstrated that nitric oxide inhibitor, $N^{\omega}$-Nitro-L-Arginine(NNA) diminished the hyperdynamic splanchnic and systemic circulation in portal hypertensive rats The present study was done to determine the role of nitric oxide in the development of hyperdynamic circulations in the prehepatic portal hypertensive rat model produced by partial portal vein ligation. Methods: The portal hypertensive rats were divided into water ingestion group and NNA ingestion group. After partial portal vein ligation, NNA ingestion group and water ingestion group received NNA 1mg/kg/day and plain water through the mouth for 14 days, respectively. Cardiac output, mean arterial pressure, organ blood flow and porto-systemic shunting were measured by radioisotope labeled microsphere methods. Vascular resistances were calculated by standard equation. Results: There were significant decreases in mean arterial pressure, increases in cardiac output and cardiac index, and decreases in total systemic and splanchnic vascular resistance in portal hypertensive rats compared to normal control group (p<0.01). Compared to the water ingestion group, significantly increased mean arterial pressure with decreased cardiac output and cardiac index were developed in the NNA ingestion group. Total systemic and splanchnic vascular resistance were significantly increased in the NNA ingestion group compared to water ingestion group (p<0.05). But, there was no significant difference in portal pressure between the two groups. Conclusion: The hemodynamic results of this study indicate that hyperdynamic circulation in prehepatic portal hypertensive rat mode1 was attenuated by ingestion of NNA. Nitric oxide may play an important role in the development of hyperdynamic circulation with splanchnic vasodilation in chronic portal hypertension.

  • The Kyemyeongsan area of Chungju in the NE part of the Ogcheon metamorphic zone, Koera, consists mainly of the Ogcheon Supergroup(Taehyangsan Quartzite, Hyangsanri Dolomite and Kyemyeogsan Formation) and the MeSozoic Chungju granite. The Kyemyeongsan Formation is composed mainly of metamorphic rocks of various grades derived from conglomeratic, basic, acidic, pelitic and psammitic rocks. The basic and acidic rocks show alternated or interfingered appearence, indicating that they were derived form bimodal type of magmatism in rift environment. Conglomeratic rocks overlie acidic volcanic rocks in geneal, but are underlain by both acidic plutonic and volcanic rocks. This indicaties that the acidic magmatism before the formation of conglomeratic rocks was different from that during or after the formation of conglomeratic rocks in its occurrence mode. The geological structure of the Ogcheon metamorphic zone in the Kyemyeongsan area, Chungju was formed at least by three phases of deormation. The first phase deformation(D1) formed a regional-scale sheath-type fold(F1) closed into the east. Its axial phane(S1) strikes NNW to NW and dips WSW to SW. The stetching lineation(L1), related to the sheath-type fold, plunges westward. The second phase deformation (D2) formed asymmetric fold(F2) of ESE-to SE-vergence with NNE to NE striking axial plane(S2) and $20~45^{\circ}/210~230^{\circ}$ plunging axis(L2). The F2 fold reoriented the original westward plunging L1 into northwestward plunging L1 in its lower limb(overturned limb). The third phase of deformation(D3) was recognized as chevron-type fold(F3) with $45^{\circ}/265$^{\circ}$ plunging axis. The F3 fold was formed by the compression of N-S direction, resulting in the reorientation of the original $20-45^{\circ}/210~230^{\circ}$ plunging L2 into mainly $35~45^{\circ}/260~280^{\circ}$ and subsidiarily $30~45^{\circ}/135~165^{\circ}$ plunging L2. After this deformation, open fold with NS striking and steeply E or W dipping axial plane is formed by the compression of E-W direction.

  • Abundant spinel lherzolite xenoliths showing distinctively different textural types such as protogranular, porphyroclastic, and mylonitic texture are trapped in the basaltic rocks from southeastern part of Jeju Island. These xenoliths show the textural spectrum from coarse-grained protogranular through porphyroclastic with bimodal grain size to fine-grained and foliated mylonitic texture. They tend to decrease in grain sizes and show more linear grain boundaries and more frequent triple junctions from protogranular through porphyroclastic to mylonitic. Spinel has different occurrence mode according the textural type. Spinel is always associated with orthopyroxene in protogranular texture, whereas it is scattered and independent of orthopyroxene in mylonitic texture. Additionally, porphyroblast from porphyroclastic and mylonitic textures has internal deformation features such as kink band, undulatory extinction and curved lamella, whereas neoblast is strain-free. These textural features indicate increasing degree of static/dynamic recrystallization from protogranular through porphyroclastic to mylonitic texture. The mg#[$=100{\times}Mg/(Mg+Fe_t)$] of olivine, orthopyroxene and clinopyroxene is relatively constant (ol: 88-91; opx: 89-92; cpx: 89-92) regardless of textural differences. The mg# of constituent minerals, NiO content (0.3~0.4 wt%) and MnO content (0.1~0.2 wt%) of olivine are similar to those of mantle xenoliths worldwide, also indicating that studied spinel lherzolite xenoliths were mantle residues having experienced 20~25% partial melting. The geochemical and textural characteristics have close relations showing that LREE and incompatible trace elements content of orthopyroxene and clinopyroxene increases from protogranular through porphyroclastic to mylonitic. These observations suggest that the studied mantle xenoliths experienced metasomatism by LREE enriched melt or fluid after partial melting, indicating a close relation between deformation and metasomatism. The metasomatism was possibly confined to narrow shear zones from where porphyroclastic and mylonitic textured xenoliths originated. These shear zones might favorably drive the percolation of LREE-enriched melts/fluids responsible for the metasomatism in the lithospheric mantle below the Jeju Island.

  • To study the vertical variations of major elements, trace elements and rare earth elements(REEs) contents in deep-sea sediments, six cores from Korea Deep-sea Environmental Study area(KODES) were analyzed. Topmost sediment layers of KODES area are divided into two Units; brown-colored and peneliquid Unit I and pale brown-colored and relatively solidified Unit II. Contents of major elements, REEs, Cu, Sr and Rb in each Unit are almost same, while contents of Mn, Ni and Co in Unit I are two or three times higher than those in Unit II. R-mode factor analysis represents that surface sediments are composed of alumino-silicate phase (AI-Ti-K-Mg-Fe-Rb-Ce), apatite phase (Ca-P-Cu-Sr-Trivalent Rare Earth Elements) and Mn-oxide phase(Mn-Ni-Co). Factor scores in silicate and apatite phases in each Unit are nearly same, whereas those in Mn-oxide phase in Unit I is higher than those in Unit II. While NilCu ratio in Unit I is two times higher than that in Unit II. We interprete the geochemical fractionation of Ni and Cu as a result that Ni can be remobilized in oxygen-depleted micro-environment in Units I and II and then easily reprecipitated in Unit I, while most of Cu supplied together with organic material is decomposed mostly in Unit I and sorbed into apatite.

  • Recovery from potentially lethal damage (PLDR) after irradiation was studied in plateau-phase culture of Vero cells in vitro. Unfed plateau-phase cells were irradiated with dose of 1 to 9Gy using Cs-137 irradiator. Cells then were incubated again and left in situ for 0, 1, 2, 3, 4, 5, 6, and 24 hours and then were trypsinized explanted, and subcultured in fresh RPMI-1640 media containing $0.33\%$ agar. Cell survival was measured by colony forming ability. An adequate number of heavily irradiated Vero cells were added as feeder cells to make the total cell number constant in every culture dish. As the postirradiation in situ incubation time increased, surviving fraction increased by PLDR. The rate of PLDR was so rapid that increased surviving fraction reached saturation level at 2 to 4 hours after in situ incubation. As the radiation dose increased, the rate of PLDR fastened and the magnitude of increased surviving fraction at saturation level by PLOR also increased. In analysis of cell survival curve fitted to the linear-quadratic model, the linear inactivation coefficient $(\alpha)$ decreased largely and reached nearly to zero but the quadratic inactivation coefficient $(\beta)$ increased minimally by increment of postirradiation in situ incubation time. So PLDR mainly affected the damage expressed as $\alpha$, In the multitarget model, significant change was not obtained in $D_0\;but\;in D_q$. Therefore, shoulder region in cell survival curve was mainly affected by PLDR and terminal slope was not influenced at all. And dose-modifying factor by PLDR was relatively higher in shoulder region, that is, in low dose area below 3 Gy.

  • This report was presented to explain the relationships between various soil pH based on the present land use, nodes of depositions, and pH measurement methods ($H_2O$ and KCl extract). The samples were collected from 160 soil series in Korea. The results were summarized as follows. 1. The average pH ($H_2O$) of surface soil were $5.3{\pm}0.6$ for paddy soils, $5.5{\pm}0.9$ for upland, $5.4{\pm}0.5$ for forest soils, $5.3{\pm}0.6$ for grassland and $5.4{\pm}0.7$ for country average. 2. The average pH (KCl) of surface soil were $4.2{\pm}0.6$ for representative soils. Paddy soils had $4.2{\pm}0.6$; upland $4.2{\pm}0.8$; forest soils, $4.0{\pm}0.6$; and grassland, $4.3{\pm}0.6$. 3. The soil pH in B and C horizons were generally higher than that of A horizons. 4. The soil pH in field were correlated with lab. soil pH ($H_2O$) and pH (KCl). Field soil pH measured by pH kit could be highly accepted in accuracy compared with lab. pH of upland, grassland, forest and paddy soils. 5. Soil pH ($H_2O$) of surface based on mode of depositions was generally higher in residuum of mountainous and hilly land than those of Fluvio-marine deposits and old alluvium, however soil pH (KCl) was higher in fiuvio-marine deposits than those of mountainous and hilly land. It was shown that soil pH (KCl) was more reasonable than that of soil pH ($H_2O$) in practical use.

  • Fatigue or overload can result in mechanical problems of implant components. The mechanical strength in the implant system is dependent on several factors, such as screw and fixture diameters, material, and design of the fixture-abutment connection and abutment. In these factors, the last rules the strength and stability of the fixture-abutment assembly. There have been some previous reports on the mechanical strength of the fixture-abutment assembly with the compressive bending test or short-term cyclic loading test. However, it is restrictive to predict the long-term stability of the implant system with them. The purpose of this study was to evaluate the influence of the design of the fixture-abutment connection and abutment on the mechanical strength and failure mode by conducting the endurance limit test as well as the compressive bending strength test. Tests were performed according to a specified test(ISO/FDIS 14801) in 4 fixture-abutment assemblies of the Osstem implant system: an external butt joint with Cemented abutment (group BJT), an external butt joint with Safe abutment (group BJS), an internal conical joint with Solid abutment (group CJO), and an internal conical joint with ComOcta abutment (group CJT). The following conclusions were drawn within the limitation of this study. Compressive bending strengths were decreased in order of group BJS(1392.0N), group CJO(1261.8N), group BJT(1153.2N), and group CJT(1110.2N). There were no significant differences in compressive bending strengths between group BJT and group CJT(P>.05). Endurance limits were decreased in order of group CJO(600N), group CJT(453N), group BJS(360N), and group BJT(300N). 3. Compressive bending strengths were influenced by the connection and abutment design of the implant system, however endurance limits were affected more considerably by the connection design.

  • Purpose: To compare the efficacy, predictability, stability and safety of excimer laser photorefracive keratectomy(PRK) for myopia and photoastigmatic refractive keratectomy(PARK) for compound myopic astigmatism. Methods: Two-hundred and three eyes(l18 eyes < -7D spherical equivalent, 85 eyes ${\geq}$ -7D spherical equivalent) received excimer laser correction for compound myopic astigmatism and 152 eyes(116 eyes < -7D, 36 eyes ${\geq}$ -7D) for simple myopia. A VISX 20/20B $VisionKey^{TM}$ excimer laser was used to perform either PARK or PRK. Visual acuity with and without correction, refraction, IOP, corneal haze, and topography were evaluated at 1, 3, 6, and 12 months postoperatively. All patients were followed up for more than 12 months. Results: Postoperative refraction were generally stable after 3 months without significant early overcorrection. At 12 months, 110(94.8%) eyes that underwent PRK and 104(88.1%) eyes that underwent PARK achieved UCVA of 20/30 or better in the group who had lower than -7D correction. For eyes treated with -7D or more, these figures were 31(86.1%) eyes after PRK and 57(67.1%) eyes after PARK. The incidences of within 1D of plano refraction at 1 year follow-up were 97.4% after PRK and 93.2% after PARK in the group who had lower than -7D correction. For eyes treated with -7D or more, these figures were 80.6% after PRK and 70.6% after PARK. Conclusions Myopia with or without astigmatism was successfully treated in most of the eyes using PRK or PARK with VISX 20/20B $VisionKey^{TM}$ excimer laser. The predictability and stability of the postoperative refraction during the first 12 months seem to be quite reliable. Further improvement of excimer laser system and software should increase the clinical outcomes and safety of refractive procedures.
