  • The pediatric condylar fracture occurs very frequently in the mandible, but this injury is occasionally ignored due to difficulty of diagnosis and no cooperation of patient. The adequate initial diagnosis and active treatment must be performed because delayed and improper treatment lead to possible severe complication such as TMJ ankylosis and reta rdation of mandibular development. Most pediatric condylar fracture is mainly performed by the conservative or functional treatment, but it may be required open reduction according to cases. In this study, activator is applied for functional treatment of pediatric condylar fracture and fair recovery of jaw function is acquired.

Investigation of Radiation Effects on the Signal and Noise Characteristics in Digital Radiography (디지털 래디오그라피의 신호 및 잡음 특성에 대한 방사선 영향에 관한 연구)

  • For the combination of phosphor screens having various thicknesses and a photodiode array manufactured by complementary metal-oxide-semiconductor (CMOS) process, we report the observation of image-quality degradation under the irradiation of 45-kVp spectrum x rays. The image quality was assessed in terms of dark pixel signal, dynamic range, modulation-transfer function (MTF), noise-power spectrum (NPS), and detective quantum efficiency (DQE). For the accumulation of the absorbed dose, the radiation-induced increase both in dark signal and noise resulted in the gradual reduction in dynamic range. While the MTF was only slightly affected by the total ionizing dose, the noise power in the case of $Min-R^{TM}$ screen, which is the thinnest one among the considered screens in this study, became larger as the total dose was increased. This is caused by incomplete correction of the dark current fixed-pattern noise. In addition, the increase tendency in NPS was independent of the spatial frequency. For the cascaded model analysis, the additional noise source is from direct absorption of x-ray photons. The change in NPS with respect to the total dose degrades the DQE. However, with carefully updated and applied correction, we can overcome the detrimental effects of increased dark current on NPS and DQE. This study gives an initial motivation that the periodic monitoring of the image-quality degradation is an important issue for the long-term and healthy use of digital x-ray imaging detectors.

  • Eagle's syndrome is the term given to symtomatic elogation of the styloid process or mineralization of the stylohyoid or stylomandibular ligament. Since ossification of stylohyoid ligament at cadeva was fist described by De Manchetis in 1652 and Weinlecher described clinical symptom which produced by elongated styloid process and osteotomy of styloid process in 1872, Clinical symtom which include sensation of a foreign body on the pharynx, dysphagai, dysphonia, referred pain, and mouth opening disturbance was termed by Eagle as Eagle's syndrome. then, case reports of Eagle's syndrome are presented. in these cases, the patient's chief complaints included periauricular radiating pain, mouth opening disturbance, foreign body sensation, dysphagia, tenderness on the neck. Through vairous X-ray examination and palpation of tonsillar fossa, elongated styloid process were confirmed. Under the general anesthesia there were successfully removed out via transoral approach technique, described by Eagle. after resented styloid process, they were freed from the symptoms without further complication. Therefor we reported these cases treated by surgical resection of styloid process with good results.

Nerve Injury from Overfilled Calcium Hydroxide Root Canal Filling Paste for Maxillary Lateral Incisor Endodontic Treatment (상악 측절치 근관치료 중 수산화칼슘 호제근충제 과충전으로 인하여 발생한 신경손상의 치험례)

  • Calcium hydroxide root canal filing paste (vitapex) is widely used as canal filling paste for infected canal. However, chemical burn is possible because of the high alkali base of calcium hydroxide. A 57-year old woman was admitted to our clinic for consistent dull pain and paresthesia in the left upper lip, zygoma and buccal cheek area, which developed during an endodontic treatment of the left lateral incisor. Radiographic finding showed radiopaque material, which exits from the left incisor root apex, and was within the left canine and first premolar buccal soft tissue. The overfilled Vitapex extended to the soft tissue was surgically curetted. The result of the surgical curettage was favorable. Though slight hypoesthesia on the upper lip was still remained, paresthesia on zygomatic and buccal cheek area was completely recovered. As far as we know, this is the first case report of infraorbital nerve damage from overfilled Vitapex material.

The evaluation of clinical outcomes on various procedures using subepithelial connective tissue graft for coverage of gingival recession (치은 퇴축 화복을 위한 상피하 결합조직 이식을 동반하는 다양한 치근피개술에 대한 임상적 평가)

  • Purpose: The subepithelial connective tissue graft(SCTG) has been proven to be a highly predictable treatment modality for coverage of gingival recession. This case report was performed to evaluate the effect of various root coverage procedures using SCTG on gingival recession. Materials and Methods: Three patients presents with Miller's class I recession defect on the maxillary canine. Each other SCTG(coronally advanced flap, Bruno's Tech., envelope Tech.) were performed for root coverage. Clinical parameters assessed included recession depth, recession width, and keratinized gingival width. Measurements were taken at baseline and 2 months and follow up end. Results: The average of root coverage was 4 mm(100% of the pre-operative recession depth) at the 2, 5 months examination. The average increase of keratinized tissue between the baseline and the 2 months amounted to 3.2mm. Conclusion: Within the above results, various root coverage using SCTG is an effective procedure to Miller's class I recession defect and patient could be satisfied aesthetic requirement.

  • Infective endocarditis is a life-threatening disease, although it is relatively uncommon. Substantial morbidity and mortality result from this infection. Therefore, primary prevention of endocarditis whenever possible is very important. The American Heart Association updated recommendations for the prevention of infective endocarditis in individuals at risk for this disease in 1997. But, utilization of antibiotic prophylaxis for patients at risk does not provide absolute immunity from infection. This report presents the case of infective endocarditis that occurred in spite of appropriate antibiotic prophylaxis in the ventricular septal defect(VSD) patient.

  • There are various local and systemic factors which alter the rate of tooth eruption movement. Thyroid hormone has been reported to have proportional relationship with the rate of tooth eruption. The main function of thyroid hormone is the regulation of basal metabolism, but it also affects the rate of tooth eruption. In this report, we will present a case of an 11-year-old girl, who showed sudden increase in orthodontic tooth movement of the impacted canine at certain points, which coincided with the hyperthyroid period. It shows possible relationship between the serum level of thyroid hormone and the rate of orthodontic tooth movement.

  • Neurofibromatosis type 1, an autosomal dominant disorder with prevalence of 1 per 3000 people, has clinical features of cafe au lait spots in skin, multiple neurofibroma and dysplasia in skeletal, endocrinal, and blood vessel systems. Actual intraoral neurofibromas are known to occur in 25% of patients. A 9 year-old girl diagnosed with type 1 neurofibromatosis visited our hospital with chief complaint of gingival swelling. Gingival enlargement in lower anterior region existed and cafe au lait spots were confirmed in patient' skin. Enlarged gingival tissue were excised under local anesthesia. Neurofibroma was confirmed with biopsy. Clinical examination after months showed fine recovery without any evidence of recurrence. Due to its possibility of recurrence, periodic follow-up will be needed.

  • Anterior diastemas are common esthetic problems. One of the challenges in clinical esthetic dentistry is closing anterior diastemas without creating "black triangles" between the teeth. The success of a restorative treatment in anterior teeth depends on the esthetic integration between soft tissues and hard tissues. This report describes the successfully accomplished diastema closure case by producing the emergence profile with natural contours at the gingival-tooth interface and then generating of gingival recontouring process.

  • The patient who has severely absorbed residual ridges, treatments are challenging to satisfy many factors: support, retention, stability, etc. The neutral zone or monoplane occlusion with non-anatomical tooth would be helpful to get additional retention and stability. The monoplane occlusion has been used long time because it can eliminate horizontal forces and many other advantages. The lingualized occlusion was introduced to improve chewing efficiency and esthetics. But from a stability aspect, it seems controversy between monoplane occlusion and lingualized occlusion. This case report shows the results of the treat two flat residual ridge patients using functional impression; piezography, and made 2 other dentures with monoplane and lingualized occlusion that patient can select denture.
