Myringotomy and ventilation tube insertion are widely performed in pediatric patients with chronic otitis media. This procedure is performed under general anesthesia or sedation with local anesthesia infiltration in pediatric patients. In this case report, we report a case of transient facial paralysis in a pediatric patient who underwent myringotomy and ventilation tube insertion using sevoflurane inhalation and four-quadrant blocks with lidocaine.

Sinonasal undifferentiated carcinoma (SNUC) is an extremely rare, highly aggressive malignancy of the nasal cavity and paranasal sinuses. Patients with SNUC usually being asymptomatic until the tumor has extensively progressed therefore frequently present with invasion of the orbit or cranial vault. Most case series of SNUC report very poor prognosis despite aggressive multimodality therapy. We recently experienced a 78-year-old male patient diagnosed as SNUC occurred from the left sphenoid sinus with invasion to the cavernous sinus and orbital content, which was treated intranasal endoscopic debulking surgery combined with curative radiation therapy successfully, and report this case with a review of literature.

Objectives To report the clinical effectiveness of the Traditional Korean medical treatment on a patient with recurrent endometriosis after hormone therapy. Methods The patient in this case was diagnosed with endometriosis and had been on hormone therapy for one year. We ascertained the recurrent of endometriosis by ultrasonography. The main symptom of dysmenorrhea has been worsened. The patient was treated by the Traditional Korean medical treatment. The symptom of the patient complained was improved. Results After the Traditional Korean medical treatment, dysmenorrhea of endometriosis was improved. The treatment was terminated and after 3 months the patient was maintained an improved state. Conclusions This case report shows that the Traditional Korean medical treatment was effective for treating endometriosis.

Primary intrapulmonary thymomas (PITs) are defined as thymomas arising in intrapulmonary locations, without an associated mediastinal component. They are rare lesions, the diagnosis of which can be very difficult. We present a case of PIT in an asymptomatic 74-year-old woman in whom pulmonary nodules were found on pulmonary angiography performed for an episode of pulmonary embolism. She underwent wedge resection and the pathology report revealed a PIT. We also summarize this patient's clinicopathological features and discuss the diagnosis, pathogenesis, and treatment of PIT.

Background: The aim of this study was to determine the clinical effects of complex Korean medicine therapy for tinnitus associated with ear fullness, depression, and anxiety. Case Report: A 52-year-old female patient suffering from tinnitus with ear fullness, depression, and anxiety was treated with acupuncture, pharmacopuncture, and herbal medicine. We used the Tinnitus Handicap Inventory (THI), Beck Depression Inventory (BDI-II), Beck Anxiety Inventory (BAI), and a Verbal Numerical Rating Score (VNRS) to assess the patient's symptoms. The administration of the acupuncture, pharmacopuncture, and herbal medicine improved the ear fullness, depression and anxiety symptoms. No side effects were observed during the treatment. Conclusion: The study findings suggest that complex Korean medicine therapy, such as acupuncture, pharmacopuncture, and herbal medicine, may be effective for the treatment of tinnitus associated with ear fullness, depression, and anxiety.

Barlow's disease with mitral annular calcification encompassing the subvalvular apparatus, including the valve leaflet and chordae, is extremely rare, and mitral valve repair in such cases is challenging. We report a case of a 60-year-old woman with mitral valve regurgitation that was successfully controlled by resecting the rough zone of P2 and calcifications on the excess leaflet regions and subvalvular apparatus, while retaining the calcification of P3 and implanting artificial chordae and an annuloplasty ring. Mitral valve repair for such cases requires an individualized and compounded surgical strategy for the technique to treat Barlow's disease and manage calcification to control mitral regurgitation.

Local anesthetics are an essential part of pain control during dental treatment. Despite its effectiveness and safety, patients should constantly be aware of potential adverse effects, including allergic reactions. Allergic reactions to amide-type local anesthetics (LAs), such as lidocaine and mepivacaine, are rare compared to those to ester-type LAs. Herein, we report the case of a patient with a history of allergy to lidocaine and mepivacaine, with symptoms of itching, diffuse erythema of the wrists and hands, dizziness, and pectoralgia. This case report emphasizes the importance of collecting medical and dental histories of patients is necessary, and how an allergy test in the allergy and clinical immunology department helps select safe LAs for patients.

결장에 영향을 미치는 악성 종양은 주로 선암종이며 결장의 평활근육종은 드물다. 저자들은 73세 남자 환자에서 복부 컴퓨터단층촬영상 구불결장-직장 장중첩증과 함께 발견된 구불결장의 평활근육종의 증례를 보고하고자 한다. 결장의 평활근육종은 드물고 장중첩증과 관련되어 보고된 바는 더욱 드물기에, 저자들은 증례 보고와 함께 영상의학적 특징을 기술하였다.

Objective: The aim of this report is to report the effects of Danggwisaeyeok-tang (當歸四逆湯) on Raynaud phenomenon in a patient. Methods: A 57-year-old male diagnosed in 2016 with Raynaud's phenomenon complained of pain, digital ulceration, and discoloration of his fingers. He underwent acupuncture and electroacupuncture treatment, but the symptoms gradually worsened, so we administered herbal medicine, Danggwisaeyeok-tang (當歸四逆湯). Visual observation of the affected part, changes in subjective symptoms, and a verbal numerical rating scale (VNAS) were used to assess the clinical response. Results: After treatment with Danggwisaeyeok-tang, the patient reported improvement in his symptoms of cold sensation, ulceration, and sclerosis of his digits. Furthermore, the patient exhibited no general adverse effects. Conclusions: Danggwisaeyeok-tang (當歸四逆湯) significantly improved the patient's clinical symptoms. This case suggests that Danggwisaeyeok-tang (當歸四逆湯) may represent a good option for the treatment of Raynaud's phenomenon.

Fractures and fracture-dislocations of the ankle are caused by a variety of mechanisms. In addition to fractures, injuries of soft tissue, such as ligaments, tendons, nerves, and muscles may also occur. Among these, a tibialis posterior tendon injury is difficult to be identified due to swelling and pain at the fracture site. It is difficult to observe tibialis posterior tendon injury on a simple radiograph; it is usually found during surgery by accident. There are some studies regarding irreducible ankle fracture-dislocations due to interposition of the tibialis posterior tendon; however, to the best of our knowledge, there has not been any report about interposition of injured tibialis posterior tendon. Herein, we report a case of an irreducible fracture-dislocation of the ankle due to injured tibialis posterior tendon interposition that was observed intraoperatively, interrupting the reduction of ankle fracture-dislocation. We obtained satisfactory clinical result after reduction of the trapped tendon, fracture reduction, and internal fixation; therefore, we are willing to report this case with the consent of the patient. This study was conducted with an approval from the local Institutional Ethics Review Board.
