Application of array comparative genomic hybridization in Korean children under 6 years old with global developmental delay

  • Purpose: Recent advancements in molecular techniques have greatly contributed to the discovery of genetic causes of unexplained developmental delay. Here, we describe the results of array comparative genomic hybridization (CGH) and the clinical features of 27 patients with global developmental delay. Methods: We included 27 children who fulfilled the following criteria: Korean children under 6 years with global developmental delay; children who had at least one or more physical or neurological problem other than global developmental delay; and patients in whom both array CGH and G-banded karyotyping tests were performed. Results: Fifteen male and 12 female patients with a mean age of $29.3{\pm}17.6months$ were included. The most common physical and neurological abnormalities were facial dysmorphism (n=16), epilepsy (n=7), and hypotonia (n=7). Pathogenic copy number variation results were observed in 4 patients (14.8%): 18.73 Mb dup(2)(p24.2p25.3) and 1.62 Mb del(20p13) (patient 1); 22.31 Mb dup(2) (p22.3p25.1) and 4.01 Mb dup(2)(p21p22.1) (patient 2); 12.08 Mb del(4)(q22.1q24) (patient 3); and 1.19 Mb del(1)(q21.1) (patient 4). One patient (3.7%) displayed a variant of uncertain significance. Four patients (14.8%) displayed discordance between G-banded karyotyping and array CGH results. Among patients with normal array CGH results, 4 (16%) revealed brain anomalies such as schizencephaly and hydranencephaly. One patient was diagnosed with Rett syndrome and one with $M{\ddot{o}}bius$ syndrome. Conclusion: As chromosomal microarray can elucidate the cause of previously unexplained developmental delay, it should be considered as a first-tier cytogenetic diagnostic test for children with unexplained developmental delay.

  • A systematic study of Korean Paramuriceidae was done with specimens collected from nine localities in the South Sea of Korea for the period of 1965 through 1978. The identifited Paramuriceidae was turned out to be 14 species and seven genera. They were all new to the Korean gorgonarian fauna: Bebryce indica; B. thomsoni; B. brocki; Calicogorgia granulosa; Filigella mitsukuri; Muricella abnormalis; Paracis pustulata; P. ijimai; Plecauroides reticulata; P. praelonga; P. rigida; P. complexa; Villogorgia alterans; V. antillarum.

Advanced Treatment Efficiency of Biological String Media and Oenanthe javanica for Effluent Quality Improvement in the Sedimentation Model Plant (하수의 최종 유출수 수질 개선을 위한 침전조 모형실험에 적용한 끈상 미생물접촉재와 미나리의 고도처리효율에 관한 연구)

  • This study was aimed at the improvement of the effluent quality of municipal wastewater treatment plant by Oenanthe javanica and Bio-string media. The results were compared with conventional sedimentation tank and pilot sedimentation plant. In this study BOD as organic pollutants, SS, Nitrogen and Phosphorus were tested to evaluate the removal efficiencies for the advanced treatment. The result were as follow: in the Pilot Plant experiment, Tank 2 was fairly high removal efficiency as compared with Tank 1. The removal efficiency of BOD and SS in Tank 2 were as a range of $7.9%{\sim}38.8%,\;14.3%{\sim}60.0%$ and the removal efficiency of T-N, T-P were a range of $9.6%{\sim}31.5%,\;33.0%{\sim}80.0%$ respectively. The removal efficiency of T-P and SS were much higher than BOB and T-N.

Corrosive Stricture Model Induced-Esophageal Burn : Animal Pilot Data (NaOH 용액을 이용한 부식성 식도 협착 동물모델 형성에 관한 연구)

  • The purpose of this study was to access the possibility and ideal model for corrosive-induced tissue hyperplasia in the rat esophagus. Twenty rat were divided into two group: a healthy group, corrosive group. corrosive burn in esophagus were produced using 30% NaOH on the distal esophagus. After surgical procedure, behavioral and weight changes were monitored on a weekly. At 3 weeks after surgical procedure, fluoroscopic esophagogram was performed and then all rats sacrificed for histological analysis by administering inhalable pure carbon dioxide. Technical surgery for corrosive stricture were 100%. A total of 2 rats died in corrosive group from a corrosive burn related to dysphagia within 14 days. The esophageal stenosis ratio was significantly higher in the corrosive group than in the healthy group (40.1 ± 9.2 % and 1.4 ± 7.2%, respectively; p = 0.001). The tissue hyperplasia ratio was also significantly higher in the Corrosive group (62.5 ± 9% and 22.08 ± 6%, respectively; p = 0.001). Infusion of 30% NaOH may suggest alternative option to evaluation tool for preclinical study in a rat corrosive model.

An Action Mechanism of Substance P on the Tracheal Smooth Muscle Contraction in Rabbits (토끼 기관의 평활근 수축에 미치는 substance P 의 작용기전)

  • Substance P[SP] has been known to be a peptide which may be plays a role as a neurotransmitter in central nervous system as well as peripheral autonomic nervous system. It has been reported that SP was widely distributed in the nerve of the tracheal smooth muscle and induced the muscle contraction. However, definite action mechanism of SP in the tracheal smooth muscle was not clear, yet. Thus, present experiment was performed to elucidate an effect of substance P and an action mechanism on contraction of the smooth muscle in rabbits. In order to find a neural mechanism to the effect of SP on the tracheal smooth muscle contraction, atropine sulfate, tetrodotoxin, propranol and phentolamine were administered at 10 min before the addition of SP. Otherwise,to find effect of SP antagonists on the action of SP, [D-Pro2, D-Try7,9]SP, [D-Arg1, D-Pro2, D-Trp7,9, Leu11]SP and [D-Pro4, D-Trp7,9]SP were administered as a same fashion. These following results were obtained. 1] SP induced contraction of the tracheal smooth muscle under resting condition and the contraction was increased dose-dependently. 2] Cholinergic blocker[atropine], neural blocker[tetrodotoxin] and adrenergic blocker[propranol and phentolamine] didn`t have an effect on the contractile response. 3] Three SP antagonists inhibited the contractile response. 4] Isoproterenol relaxed the contraction induced by SP. The above results suggested that SP induced contraction of the tracheal smooth muscle directly act to the smooth muscle in rabbits. The autonomic nervous system did not seem to participate in the SP action.

Can Capecitabine be used Instead of Concurrent Bolus 5-FU in Postoperative Chemoradiotherapy for Gastric Adenocarcinoma?

  • Background: 5-fluoro-uracil (FU) is a common agent in postoperative chemoradiation in gastric adenocarcinoma. However, FU is not well tolerated in a significant proportion of patients. Capecitabine (CA) is an orally administered fluoropyrimidine carbamate which is preferentially converted to active 5-FU and is one of the agents used instead of FU in such cases. We compared the toxicity, local and distant control and survival rates with FU or oral CA during the course of concurrent radiotherapy to assess the role of CA used instead of FU. Materials and Methods: We conducted an analysis of survival, disease control and toxicity data in 46 patients treated with postoperative chemoradiation following total or subtotal gastrectomy for gastric adenocarcinoma with either FU or CA between January 2008 and December 2012. Results: Median follow-up was 19 months (range: 3-59), median survival time was 23 ({\pm}6.08) months and 1-3 years overall survival (OS) rates were 64.9-39% for all patients. Compared with the CA regimen, the incidence of treatment interruption was higher with FU (p=0.023), but no significant differences were seen in local control (p=0.510), distant recurrences (p=0.721) and survival rates (p=0.866) among patients. Conclusions: Concurrent CA with radiotherapy seems to be a more tolerable and an equally effective regimen for the postoperative treatment of gastric adenocarcinoma when compared to FU.

A Study on the Temporomandibular Joint Disorder and School Life Stress of High School Student by Department (계열별 남자고등학생의 학교생활스트레스와 측두하악장애에 관한 연구)

  • The purpose of this study targeted on high school student in the department of liberal arts, industry in Daegu metropolitan city, is to get basic data necessary for the development of dental educational program, to discern prevention and treatment of temporomandibular joint disorder by observing the situation temporomandibular joint disorder and contribution element, of relationship of school life stress The results are as follows.: 1. The percentage of occurring temporomandibular joint disorder in the high school resulted in a joint noise at 61.8% and joint dislocation 6.9%, sharp pain 47.5% at time of chewing. 29.8% at the time of the non-chewing, lockjaw 11.3%, a headache appeared at 40.4%.2. In the contribution factor of occurring temporomandibular joint disorder, the cause of joint noise was the clench one's teeth, lip and cheek clench, For the pain at the time of chewing clench one's teeth, one side chewing, over-chewing, lip clench, sideways sleeping showed the difference. (P < 0.01) For the pain at the time of non-chewing, clench one's teeth, bruxism, one side chewing, lip and cheek clench were similar, and for the lockjaw, clench one's teeth, bruxism, sideways sleeping showed the difference. The plum evil thing period at time of the fault writing that statistically showed the difference. For the headache, the contribution factors were the all bad habits mentioned above excluding one side sleeping.(P < 0.01, P < 0.05). 3. The rate of experiencing temporomandibular joint disorder by oral and maxillofacia was 13.4% in industrial department, and 19.6% in liberal arts. And for the factor of wound was that exercise 26.8%, others 24.4%, fall-down 19.5%. And for the industrial, exercise 44.4%, fall-down 22.2%, others 14.9%. The treatment experience appeared at 5.0% in industrial department, 2.9% in liberal arts. And for the medical institutions, liberal arts were dental clinic 50%, orthopedics 50%, and the industrial department orthopedics 40%, oriental medicine clinic 30%, dental clinic 30%. 4. In case of temporomandibular joint disorder, there were no difference by grades or educational background. And at the time of chewing or non-chewing showed similar difference.(P < 0.01). 5. Compared to stress in the high school, it generally showed higher in liberal arts than in industrial department due to school record. Its scope was $3.75{\pm}1.14$ in liberal arts, $3.01{\pm}1.23$ in industrial department. 6. The school record, school life, stress problems by teachers, chewing/non-chewing pain of temporomandibular joint disorder, joint noise had a similar correlation.(P < 0.01, < 0.05).

Temperature-dependent Longevity and Fecundity of Propylea japonica Thunberg (Coleoptera: Coccinellidae) and Its Predation Amount on Two Aphid Species (꼬마남생이무당벌레(Propylea japonica Thunberg)의 온도별 성충 수명, 산란수 및 두 종 진딧물에 대한 포식량)

  • This study was conducted to investigate the developmental characteristics of Propylea japonica Thunberg (Coleoptera: Coccinellidae) and its consumption of Myzus persicae nymphs at 3 constant temperatures (20.0, 25.0 and $30.0^{\circ}C$; $60{\pm}5%$ relative humidity; 14 h light : 10 h dark). The longevity of adult female P. japonica under 20, 25, and $30^{\circ}C$ was 134.0, 101.0 and 55.2 days, respectively. The total fecundity was 508.6, 875.6, and 383.4 eggs during its life span, respectively. The longevity of adult male P. japonica under 20, 25, and $30^{\circ}C$ was 128.8, 97.8, and 46.5 days, respectively. Average daily consumption of adult M. persicae by 1st, 2nd, 3rd, and 4th instar P. japonica at $25^{\circ}C$ was 2.2, 7.3, 14.5, and 29.1, respectively. The average daily number of M. persicae consumed by male and female P. japonica over their lifetimes was 35.0 and 42.9, respectively. Average daily consumption of adult Aphis gossypii by 1st-4th instar P. japonica at $25^{\circ}C$ was 2.2, 7.5, 13.9, and 29.5, respectively. The average daily number of A. gossypii consumed by male and female P. japonica over their lifetimes was 37.0 and 40.8, respectively.

Evidence-based customized nutritional intervention improves body composition and nutritional factors for highly-adherent children and adolescents with moderate to severe obesity

  • BACKGROUND/OBJECTIVES: Evidence-based customized nutritional interventions are required for effective treatment of moderate to severe obese children and adolescents. SUBJECTS/METHODS: Sixty six (64.1% of 103) of the eligible participants who joined the usual care or physical activity group in the clinic were involved in 16-week intervention. Customized nutritional intervention was implemented for each participant based on a nutrition care process (NCP) model. Sociodemographic assessment, anthropometrics data, health- and dietary-related behaviors, and dietary intake of the study subjects were assessed at baseline and follow-up. All participants engaged in 30-minute nutritional sessions on a monthly basis. RESULTS: After 16 weeks, there were significant improvements in body composition [BMI (-0.8 ± 0.9, P < 0.05), BMI z-score (-0.3 ± 0.2, P < 0.001), body fat (kg) (-1.3 ± 2.1, P < 0.05), and body fat (%)(-1.5 ± 1.9, P < 0.05)] as well as macronutrient intake [total energy intake (kcal) (-563.7 ± 656.8, P < 0.05), energy (%) (-26.5 ± 30.0, P < 0.05) and fat (g) (-28.3 ± 40.6, P < 0.05)] in the adherent group than the non-adherent group. The SOC was higher in both groups after the intervention (P < 0.001). CONCLUSIONS: Our results highlight the positive effects of an evidence-based approach as a multidisciplinary intervention for people-centered nutritional care and weight management.

Prognostic Value of Subcarinal Lymph Node Metastasis in Patients with Esophageal Squamous Cell Carcinoma

  • Purpose: The 7th edition of the American Joint Committee on Cancer Staging Manual for esophageal cancer (EC) categorizes N stage according to the number of metastatic lymph nodes (LNs), irrespective of the site. The aim of this study was to determine the prognostic value of subcarinal LN metastasis in patients undergoing esophagectomy for esophageal squamous cell carcinoma (ESCC). Methods: A retrospective analysis of 507 consecutive patients with ESCC was conducted. Potential clinicopathological factors that could influence subcarinal LN metastasis were statistically analyzed. Univariate and multivariate analyses were also performed to evaluate the prognostic parameters for survival. Results: The frequency of subcarinal LN metastasis was 22.9% (116/507). Logistic regression analysis showed that tumor length (>3cm vs ${\leq}3cm$; P=0.027), tumor location (lower vs upper/middle; P=0.009), vessel involvement (Yes vs No; P=0.001) and depth of invasion (T3-4a vs T1-2; P=0.012) were associated with 2.085-, 1.810-, 2.535- and 2.201- fold increases, respectively, for risk of subcarinal LN metastasis. Multivariate analyses showed that differentiation (poor vs well/moderate; P=0.001), subcarinal LN metastasis (yes vs no; P=0.033), depth of invasion (T3-4a vs T1-2; P=0.014) and N staging (N1-3 vs N0; P=0.001) were independent prognostic factors. In addition, patients with subcarinal LN metastasis had a significantly lower 5-year cumulative survival rate than those without (26.7% vs 60.9%; P<0.001). Conclusions: Subcarinal LN metastasis is a predictive factor for long-term survival in patients with ESCC.
