Introduction of a New Staging System of Breast Cancer for Radiologists: An Emphasis on the Prognostic Stage

  • In 2017, the American Joint Committee on Cancer announced the 8th edition of its cancer staging system. For breast cancer, the most significant change in the staging system is the incorporation of biomarkers into the anatomic staging to create prognostic stages. Different prognostic stages are assigned to tumors with the same anatomic stages according to the tumor grade, hormone receptor (estrogen receptor; progesterone receptor) status, and HER2 status. A Clinical Prognostic Stage is assigned to all patients regardless of the type of therapy used; in contrast, a Pathologic Prognosis Stage is assigned to patients in whom surgery is the initial treatment. In a few situations, low Oncotype DX recurrence scores can change the prognostic stage. The radiologists need to understand the importance of the biologic factors that can influence cancer staging.

Occlusal scheme in a group of Thais

  • PURPOSE. To study the distribution of various occlusal schemes as well as associated static occlusal relationship among Thais. MATERIALS AND METHODS. The subjects'occlusal schemes on the left and right sides were classified as canine protected occlusion, group function, or unclassified according to the definition from Glossary of Prosthodontic Terms ($8^{th}$ edition). In addition, the presence of occlusal interferences during mandibular excursions were recorded. The measurement of the horizontal overlap and vertical overlap was also performed. Chi-square, One-way ANAVA and Mann-Whitney U statistical tests were used with level of significance set at P=.05. RESULTS. Total of 104 subjects were included in this study. The ratio for male to female was 1.8 to 1. Average age of the population was $25.01{\pm}6.87$ years old. The mean vertical overlap and horizontal overlap were $1.94{\pm}1.20$ and $2.41{\pm}1.32$ mm respectively. The majority of the populations (68.3%) possessed group function occlusal scheme. For the remaining, 17.3% possess canine protected occlusion and 12.5% possess combination of both occlusal schemes. We also found that occlusal interference was presented in 20.2% of the population. The most common was protrusive interference (57.14%), the second was balancing interference (38.1%) and the third was working interference (4.1%). CONCLUSION. Among Thais, the most common occlusal scheme was group function, however there were no significant occlusal factors related to any particular occlusal scheme.

봉화군 청옥산에 분포하는 대한민국약전 수재 약용식물의 분포 특성 (Distribution of Medicinal Plants included in the Korean Pharmacopoeia at Cheongoksan Bonghwagun in Korea)

  • This text was analyzed and investigated the distribution of medicinal plants in Cheongoksan Bonghwagun Korea, in order to search the medicinal resources that are used in modern medicine. Medicinal plants of the Korean Pharmacopoeia (10th edition) distributed in Cheongoksan Bonghwagun were consisted of 93 taxa ; 82 species, 10 varieties, 1 forma of 79 genus, 50 families. In medicinal plants of the Korean Pharmacopoeia, rate of native species and exotic species was 89.2% (83 taxa) and 10.8% (10 taxa) respectively. Family classification was the most of compositae of 8 taxa, and life form classification was most of herb of hemicryptophyte species. The classification by using parts were 34 taxa of root use and the classification of efficacy utilization was 24 taxa of Cheongyeolyak (heat-clearing drug) use.

항공기(航空機) 사고조사제도(事故調査制度)에 관한 연구(硏究) (A Study on the System of Aircraft Investigation)

  • The main purpose of the investigation of an accident caused by aircraft is to be prevented the sudden and casual accidents caused by wilful misconduct and fault from pilots, air traffic controllers, hijack, trouble of engine and machinery of aircraft, turbulence during the bad weather, collision between birds and aircraft, near miss flight by aircrafts etc. It is not the purpose of this activity to apportion blame or liability for offender of aircraft accidents. Accidents to aircraft, especially those involving the general public and their property, are a matter of great concern to the aviation community. The system of international regulation exists to improve safety and minimize, as far as possible, the risk of accidents but when they do occur there is a web of systems and procedures to investigate and respond to them. I would like to trace the general line of regulation from an international source in the Chicago Convention of 1944. Article 26 of the Convention lays down the basic principle for the investigation of the aircraft accident. Where there has been an accident to an aircraft of a contracting state which occurs in the territory of another contracting state and which involves death or serious injury or indicates serious technical defect in the aircraft or air navigation facilities, the state in which the accident occurs must institute an inquiry into the circumstances of the accident. That inquiry will be in accordance, in so far as its law permits, with the procedure which may be recommended from time to time by the International Civil Aviation Organization ICAO). There are very general provisions but they state two essential principles: first, in certain circumstances there must be an investigation, and second, who is to be responsible for undertaking that investigation. The latter is an important point to establish otherwise there could be at least two states claiming jurisdiction on the inquiry. The Chicago Convention also provides that the state where the aircraft is registered is to be given the opportunity to appoint observers to be present at the inquiry and the state holding the inquiry must communicate the report and findings in the matter to that other state. It is worth noting that the Chicago Convention (Article 25) also makes provision for assisting aircraft in distress. Each contracting state undertakes to provide such measures of assistance to aircraft in distress in its territory as it may find practicable and to permit (subject to control by its own authorities) the owner of the aircraft or authorities of the state in which the aircraft is registered, to provide such measures of assistance as may be necessitated by circumstances. Significantly, the undertaking can only be given by contracting state but the duty to provide assistance is not limited to aircraft registered in another contracting state, but presumably any aircraft in distress in the territory of the contracting state. Finally, the Convention envisages further regulations (normally to be produced under the auspices of ICAO). In this case the Convention provides that each contracting state, when undertaking a search for missing aircraft, will collaborate in co-ordinated measures which may be recommended from time to time pursuant to the Convention. Since 1944 further international regulations relating to safety and investigation of accidents have been made, both pursuant to Chicago Convention and, in particular, through the vehicle of the ICAO which has, for example, set up an accident and reporting system. By requiring the reporting of certain accidents and incidents it is building up an information service for the benefit of member states. However, Chicago Convention provides that each contracting state undertakes collaborate in securing the highest practicable degree of uniformity in regulations, standards, procedures and organization in relation to aircraft, personnel, airways and auxiliary services in all matters in which such uniformity will facilitate and improve air navigation. To this end, ICAO is to adopt and amend from time to time, as may be necessary, international standards and recommended practices and procedures dealing with, among other things, aircraft in distress and investigation of accidents. Standards and Recommended Practices for Aircraft Accident Injuries were first adopted by the ICAO Council on 11 April 1951 pursuant to Article 37 of the Chicago Convention on International Civil Aviation and were designated as Annex 13 to the Convention. The Standards Recommended Practices were based on Recommendations of the Accident Investigation Division at its first Session in February 1946 which were further developed at the Second Session of the Division in February 1947. The 2nd Edition (1966), 3rd Edition, (1973), 4th Edition (1976), 5th Edition (1979), 6th Edition (1981), 7th Edition (1988), 8th Edition (1992) of the Annex 13 (Aircraft Accident and Incident Investigation) of the Chicago Convention was amended eight times by the ICAO Council since 1966. Annex 13 sets out in detail the international standards and recommended practices to be adopted by contracting states in dealing with a serious accident to an aircraft of a contracting state occurring in the territory of another contracting state, known as the state of occurrence. It provides, principally, that the state in which the aircraft is registered is to be given the opportunity to appoint an accredited representative to be present at the inquiry conducted by the state in which the serious aircraft accident occurs. Article 26 of the Chicago Convention does not indicate what the accredited representative is to do but Annex 13 amplifies his rights and duties. In particular, the accredited representative participates in the inquiry by visiting the scene of the accident, examining the wreckage, questioning witnesses, having full access to all relevant evidence, receiving copies of all pertinent documents and making submissions in respect of the various elements of the inquiry. The main shortcomings of the present system for aircraft accident investigation are that some contracting sates are not applying Annex 13 within its express terms, although they are contracting states. Further, and much more important in practice, there are many countries which apply the letter of Annex 13 in such a way as to sterilise its spirit. This appears to be due to a number of causes often found in combination. Firstly, the requirements of the local law and of the local procedures are interpreted and applied so as preclude a more efficient investigation under Annex 13 in favour of a legalistic and sterile interpretation of its terms. Sometimes this results from a distrust of the motives of persons and bodies wishing to participate or from commercial or related to matters of liability and bodies. These may be political, commercial or related to matters of liability and insurance. Secondly, there is said to be a conscious desire to conduct the investigation in some contracting states in such a way as to absolve from any possibility of blame the authorities or nationals, whether manufacturers, operators or air traffic controllers, of the country in which the inquiry is held. The EEC has also had an input into accidents and investigations. In particular, a directive was issued in December 1980 encouraging the uniformity of standards within the EEC by means of joint co-operation of accident investigation. The sharing of and assisting with technical facilities and information was considered an important means of achieving these goals. It has since been proposed that a European accident investigation committee should be set up by the EEC (Council Directive 80/1266 of 1 December 1980). After I would like to introduce the summary of the legislation examples and system for aircraft accidents investigation of the United States, the United Kingdom, Canada, Germany, The Netherlands, Sweden, Swiss, New Zealand and Japan, and I am going to mention the present system, regulations and aviation act for the aircraft accident investigation in Korea. Furthermore I would like to point out the shortcomings of the present system and regulations and aviation act for the aircraft accident investigation and then I will suggest my personal opinion on the new and dramatic innovation on the system for aircraft accident investigation in Korea. I propose that it is necessary and desirable for us to make a new legislation or to revise the existing aviation act in order to establish the standing and independent Committee of Aircraft Accident Investigation under the Korean Government.

우리나라 일부 여대생의 영양섭취실태에 관한 연구 -지방 및 단백질섭취의 질적 평가를 중심으로- (Investigations on Nutrient Intakes Among Korean Female College Students -Quality Evaluations for Fat and Protein Consumption-)

  • This study was performed to investigate the adequacy of dietary fat and protein intakes among female college students. Daily intakes of energy, fat, protein, major amino acids and other nutrients were measured in 52 female college students. Daily energy intake was 75.8% of the recommended intake. Fat and protein consist 19.2% and 16.7% of the total calorie, respectively. The average protein consumption per day was 105% of the recommended intake. Essential amino acids intakes were more than the recommended amounts which appears in the 6th edition of Recommended Dietary Allowances for Koreans. However, when the intake of each essential amino acid was compared to the recommended amino acid requirement pattern, these subjects did not meet the estimated requirements. There was a highly significant correlation between daily protein intake and lipid intake implying the major sources of protein in the diet were also major sources of fat. Daily intakes of dietary fiber, vitamin C, iron, and phosphorous were above the recommended levels of intake. However, blood hemoglobin concentration was marginal indicating dietary iron consumption is not a good marker for iron status. Also, calcium intake was only 63.5% of the recommended intake. Therefore, these results imply that main problems for these subjects are low energy consumption, low calcium intake, and the quality of protein. However, as opposed to the hypothesis, the main energy sources were not the food items high in saturated fats such as instant foods, which should be emphasized further.

Bloom의 교육목표 분류에 기반한 치위생학 학습목표 분석 (Analysis of dental hygiene learning objectives based on Bloom's taxanomy)

  • Objectives: We evaluated the learning objectives of dental hygiene courses based on Bloom's learning objectives, and analyze the degree of match with the dental hygienist's job for each detailed subject. Methods: The 5th edition of 'Dental hygiene and learning objectives' was analyzed by subject based on Bloom's cognitive domain classification from March 10 to April. In addition, the degree of match between the contents of the secondary job analysis of the dental hygienist and the learning objectives for each detailed subject were analyzed. Results: The total number of dental hygiene learning objectives was 2,975 (2,762 theory, 52 practice). Among the cognitive domains, the comprehension domain was the most common (79.8%), and the skill domain was very low (4.9%). In the job for each detailed subject of dental hygiene, the most frequently performed was 'dental prophylaxis and practice' with 103 subjects. Conclusions: Overall, dental hygiene learning objectives are mostly theory-oriented, so it is necessary to expand and improve in the direction related to the jobs that clinical dental hygienists perform in the field. In addition, it is necessary to continuously develop timely learning goals, and prepare active strategies for developing high-quality items.

일본에 있어서의 파종공에 의한 법면녹화와 자연회복녹화 (Slope Revegetation by Seeding works and Resent Nature Restoration in Japan)

  • The historical changes of seeding work in Japan, the typical slope revegetation method, is possible to divide into five periods as follows; 1) The early period after creation(1927-1948) 2) The spreading period of modern revegetation work with manpower(1947-1958) 3) The spreading period of rapid revegetation technique using exotic grasses with machineries(1959-1985${\dots}$) 4) The spreading period of rapid reforestation technique by fast growing species mainly using leguminous shrub species(1986-1995${\dots}$) 5)The developing period of nature restoration technique using endemic arborous species(1996-) The evaluation basis of vegetated condition after slope seeding has been described to a official guide book, "The Standards of Slope Stabilizing Works" It was mainly revised three times after first edition in 1972, and also the point of evaluation basis changed from the vegetation coverage to the density of woody plant through the 3rd period and the 4th period Recently main purpose of slope seeding has been developing from the erosion protection to the nature restoration in the current of the 5th period, and "The tentative guidelines of slope nature restoration" was announced by The Slope Revegetation Section of Japanese Society of Revegetation Technology in 2004. It was proposed the planning techniques of using suitable seeds/plants based on the preservation level at the real each construction sites Consequently, at present the importance of advanced monitoring methods for vegetation maintenance and plant sociologic survey to evaluate the plant succession is increasing.

Refining prognostic stratification of human papillomavirus-related oropharyngeal squamous cell carcinoma: different prognosis between T1 and T2

  • Purpose: To validate the 8th edition of the American Joint Committee on Cancer/Union for International Cancer Control (AJCC/UICC) TNM staging system for human papillomavirus (HPV)-related oropharyngeal squamous cell carcinoma (OPSCC) and investigate whether a modified classification better reflects the prognosis. Materials and Methods: Medical records of patients diagnosed with non-metastatic HPV-related OPSCC between 2010 and 2016 at a single institution were retrospectively reviewed. HPV status was determined by immunohistochemical analysis of p16 and/or HPV DNA polymerase chain reaction (PCR). We reclassified TNM stage T0-1 and N0-1 as group A, T2-3 or N2 as B, and T4 or N3 as C. Survival analysis according to 8th AJCC/UICC TNM staging and the modified classification was performed. Results: Of 383 OPSCC patients, 211 were positive for HPV DNA PCR or p16. After exclusion, 184 patients were included in this analysis. Median age was 56 years (range, 31 to 81 years). Most primary tumors were in the palatine tonsil (148 tumors, 80%). The eighth AJCC/UICC TNM classification could not differentiate between stage I and II (p = 0.470) or II and III (p = 0.209). Applying modified grouping, the 3-year overall survival rate of group A was significantly higher than that of group B and C (98% vs. 91%, p = 0.039 and 98% vs. 78%, p < 0.001, respectively). Differentiation between group B and C was marginally significant (p = 0.053). Conclusion: The 8th AJCC/UICC TNM staging system did not clearly distinguish the prognosis of stage II from that of other stages. Including the T2N0-1 group in stage II may improve prognostic stratification.

향약구급방(鄕藥救急方)에 대(對)한 고증(考證) (A research on Hyang-Yack-Ku-Keup-Bang(鄕藥救急方) (Restoration and Medico-Historic Investigation))

  • Hyang-Yack-Ku-Keup-Bang(鄕藥救急方) is our own, medical work written about the middle of the time of Korea Dynasty. I restored and researched this book because it needed to be illuminated about its medico-historic value and then I came to some conclusions as follows. 1. Hyang-Yack-Ku-Keup-Bang was published in Dae-jang-do-kam(大藏都監) of Kanghaw island(江華島) about the middle of Korea Dynasty. Choi Ja-ha(崔自河) republished it on original publication ground in Euiheung(義興) of Kyungsang-Province(慶尙道) in July, Taejong's(太宗) 17th year of Chosen Dynasty (A.D.1417) and this book was published again in Chungcheng Province(忠淸道) in Sejong's(世宗) 9th year(A.D.1427). The book published in Taejong's days was in the possession of books department of Kung-nae-cheng(宮內廳) in Japan and was the oldest medical book of existing ones. 2. Bang-Jung-Hyang-Yack-Mock-Cho-Bu(方中鄕藥目草部) of this book was originally intended to be adjusted in each division with the title of Bang-Jung-Hyang-Yack-Mock(方中鄕藥目). But Herb part(草部) only followed editing progress of Jeung-Lew-Bon-Cho(證類本草), the rest is not divided into each part and is together arranged at the below of Herb part with the title of Bang-Jung-Hyang-Yack-Mock-Cho-Bu. The Korean inscriptions on some drugstuffs in this book are different between Native Name(鄕名) of three volumes of provisions and general-spoken(俗云) of Bang-Jung-Hyang-Yack-Mock-Cho-Bu. In this, it is estimated that the publishing time and editor of tile volume of provisions and Bang-Jung-Hyang-Yack-Mock-Cho-Bu are different. I think Choi Ja-ha compiled this behind three volumes of provisions when he published. 3. This book picked some prescriptions which consisted of obtainable drugs with ease in Korea in the books of Chell-Keum-Yo-Bang(千金要方), Oi-Dae-Bi-Yo(外臺秘要), Tae-Peong-Sung-Hye-Bang(太平聖惠方), Ju-Hu-Bang(?後方), Kyung-Hum-Yang- Bang(經驗良方) Bo-Je-Bon-Sa-Bang(普濟本事方) Bi-Ye-Baik-Yo-Bang(備預百要方) and so on and got together our own prescriptions. On the whole Bi-Ye-Baik-Yo-Bang was a chief referrence book, On this, other books referred to and corrected. 4. In provisions quoted from Hyang-Yack-Jip-Sung-Bang(鄕藥集成方), there are seven provisions; leg-paralysis part, coughing part, headache part, obstetrics part, etc. don't show in this book. This is why Choi Ja-ha published only certain texts on Dae-jang-do-kam edition his own posession. So we can think the existing edition has a little misses compared with original edition. 5. This book recorded only names of drugstuffs in animal drug department like fowls, crab, goldbug, earthworm, etc. and didn't tell us ways of taking those. This is effect of Buddhist culture on medicine. This is efforts to practice 'Don't murder';one of Five Prohibition of Buddhism. 6. Beacause this book was published at the time, when our originative medicine would be set forth. This followed the Chinese ways in Theory, Treatment, Prescription and used 'Hyang Yack' in Medication out of theory of Korean medicine, which was a transitional form. So this is all important material which tell us aspects of development of 'Hyang Yack' the middle of Korea Dynasty.and this is also the beginning of originative, medical works like Dong-Eui-Bo-Kam(東醫寶鑑), Dong-Eui-Su-Bo-Won(東醫壽世保元). 7. There are few contents based on 'Byen-Jeung-Lon-Chi(辨證論治)'in this book. So we can see this book is not for doctors who study medical thoughts but for general public who suffer from diseases resulted from war. Because this book was written for a first-aid treatmeant, this is an index of medical service for the people those days. And this is also an useful datum for first-aid medicine or military medicine in these modern days. 8. Nowadays, parts of learned world of Korean medicine disregard essential theories and want to explain Korean medicine only by the theories or the methods of Western medicine. Moreover they don't adopt Chinese and Japanese theorys & thoughts about Oriental medicine in our own style and just view in there level. What was worse, there is a growing tendency for them to indulge in a trimming policy of scholarship and to take others' ideas. I think these trends to ignore our own medical thoughts involving growth of 'Hyang Yack' in the middle of Korea Dynasty, Dong-Eui-Bo-Kam and Dong-Eui-Su-Se-Bo-Won. So we, as researchers of Korean medicine, must get out of this tendency, and take over brilliant tradition and try to develop originative Korean medicine.

영추(靈樞).골도(骨度)에 대(對)한 연구(硏究) (A study on the Gal Do(骨度) of the Young Chu(靈樞))

  • According to the bone-standard measuring, the length between the elbow and the wrist has the span of 1.25 cheoks whereas it has the span of 1.2 cheoks or 1.2 cheoks today, the breadth of the chest has the span of 9.5 chons whereas it has the span of 8 chons today, and the length from celestial pivot to transverse bone has the span of 6.5 chons whereas it has the span of 5 chons, and the length from free rib region to thigh pivot has the span of 6 chons whereas it has the span of 9 chons today. It is said that all of the acupuncture points are correctly prescribed by the bone-standard measuring irrespective of men and women of all ages, fat and lean, and whether large or small in height. This lies at the root of the selecting acupuncture points. The bone standard in Spiritual Pivot and that in common use at present have a little difference and the bone standard of today is based upon A-B Classic. Spiritual Pivot named as Acupuncture Classic was in good preservation until the mid-eleventh century, but the book lost a lot in contents was the incomplete edition. In the 8th year of the king Cheoljong's reign of the North Song Dynasty in 1093, though the nine-volume Spiritual Pivot drafted from the Goryo Dynasty was published, the book was wanting in consistency. While on the other, I think that A-B Classic which has been in a state of perfection had a great influence on the healers of the day.
