ROC Analysis of Visual Assessments Made in Gated Blood Pool Scans of Patients with Coronary Artery Disease (관상동맥질환에서 심장풀 스캔의 육안적 평가에 대한 ROC 분석)

  • Visual assessment of regional wall motion abnormality (RWMA) by gated blood pol scan (GBPS) serves as an useful parameter in the diagnosis, functional evaluation, and follow up in various clinical settings, but are still subject to some inherent limitations. On important problem may be the interobserver as well as intraobsever variation that may well be present due to the subjective nature of the interpretations. This study was carried out to determine the reliability and reproducibility of visual assessments made in GBPSs, and to observe the degree to which the results would be influenced by observer variation. Fifty two patients with coronary heart disease had resting GBPS and contrast ventriculography within 4 days appall. Contrast ventriculography-showed normal wall motion in 6 patients and the remaining 46 had RWMA in one or more segments. The anterior and left anterolateral views of all 52 GBPSs were analyzed by three independent observers, who selected from 5 scales, their level of confidence that there was RWMA in that segment. Reciever operating characteristic (ROC) curves for each analysis was plotted and the area under the curve $(\theta)$ was used as a parameter representing each observer's performance in his interpretations. The findings of contrast ventriculographies were used as the standard for RWMA. The apical and inferoapical segments showed the best correlation with contrast ventriculography ($\theta=0.90-0.94$, 0.81-0.94, respectively), and the inferior wall showed the poorest correlation $(\theta=0.70-0.74)$. The interpretations of the inferior, septal, apical, and posteroinferior, segments showed no difference between the observers, but there was significantly better performance in assessment by observer A compared to that by B or C for the anterolateral segments ($\theta=0.87$, 0.78, 0.76, respectively. p<0.01 for A vs B, p<0.05 for A vs C), as well as when all segments were considered altogether ($\theta=0.88$, 0.83, 0.82, respectively. both p<0.05). This was also true for the infero-apical segment between A and C ($\theta=0.09$, 0.81, p<0.05). The intraobserver variation, however, did not appear significant, with only the inferior segment for observer B showing any significant difference when observer A and B repeated the analysis 10 days latter. There was no difference in assessing dyskinesia, with all observers showing a high performance ($\theta=0.98$, 0.87, 0.97, respectively). The visual assessment of left ventricular ejection fraction by all three observers correlated well with the calculated value from a semiautomated method (Spearman's r = 0.91, 0.83, 0.83. p<0.01, p<0.05, p < 0.05). The assessment of LV and RV size also correlated well between the three observers (Kendall's w = 0.80, 0.51, p<0.01 for both left and right ventricles). The above findings suggest that RWMA visually assessed by GBPS correlates well with that done by contrast ventriculography. And although the observer's experience or skill may influence the results in certain segments, visual analysis of GBPS may serve as a reliable and reproducible means for evaluating ventricular function.

Assessment and Comparison of SUVs of Three Different PET/CT Scanners (장비에 따른 SUV의 차이와 이에 관한 고찰)

  • Purpose: The SUV is a widely used semi-quantitative index in PET for the estimation of radio-tracer accumulation in VOI. In this study, SUVs from three different PET/CT scanners were assessed, and differences between SUVs were evaluated. Materials and Methods: The PET/CT scanners which were assessed in this study were GEMINI, GEMINI TF 64 (Philips) and Biograph True Point True V 40 (Siemens). The NEMA PET phantom (Data Spectrum Corp., USA) was used to evaluate SUVs. The NEMA PET phantom has6.8 kg weight and three hot inserts. Two different activity distributions for the background and inserts were tested. The activity ratio were 3.7:3.7:7.4:11.1 MBq (1:1:2:3) and 1.85:7.4:9.25:11.1MBq (1:4:5:6) for each of background, insert 1, insert 2 and insert 3. Acquisition time was 2 minutes per bed position and NEMA PET phantom could be covered by two bed positions for all PET/CT scanners. The SUVs from each PET/CT scanner were compared with calculated true value. Results: For both activity ratios, all scanners showed similar results. The differences between each scanner were insignificant. Each scanner showed 91.2%, 85.9% and 87.2% of true SUV for GEMINI, GEMINI TF 64, Biograph True Point TrueV, respectively. Conclusion: For all scanners, SUVs were slightly lower than true value. However, the difference between scanners was insignificant. The SUVs from these scanners would be clinically meaningful if their consistent underestimation is kept in mind.

Quality of Life among Family Caregivers of Terminal Cancer Patient (말기 암 환자 가족 간병인의 삶의 질)

  • Purpose: Terminal cancer influences on patients as well as their family members. This research was performed to evaluate the quality of lift of primary family caregivers and to investigate the influencing factors. Methods: The results of survey were collected from 81 family caregivers who were taking care of hospitalized terminal cancer patients at an oncology department of university hospital in Daejeon from March 2005 to January 2006 with questionnaires. The questionnaires were consisted of the general characteristics of the subjects, and 36-items short-form Health Survey (SE-36) Korean version to evaluate the quality of life of family caregivers, the characteristics of patients and family caregivers' caring trait. Results: Family caregivers' mean (${\pm}SD$) SF-36 score was 47.9 (${\pm}20.7$). Influential factors on family caregivers' life quality were daily raring hours, economic burden, type of treatment; only supportive care, caring duration, sex, the numbers of ADLs (activity of daily livings) items that patients needed help in order by stepwised multiple logistic regression analysis (overall $R^2=0.639$, P=0.044). Conclusion: Daily raring hours and economic burden were two influential modifiable factors on family caregivers' quality of life. Therefore, social supportive systems are required to reduce family caregivers' daily caring hour and economic burden.

Vertical and horizontal distribution of squids in relation to oceanographic structure in the North Pacific Ocean (북태평양 오징어류의 연직 및 수평분포)

  • Vertical distribution of squids in relation to oceanographic structure was analyzed on the basis of experimental squid hand jigging fishing by R/V Pusan 851 in the area of 34°∼47° N, 150° E∼170°W in the North Pacific in summer from 1987 through 1993 with exception of 1991. The 6 species of squids showed different patterns of vertical and horizontal distribution as following; Boreopacific gonate squid (Gonatopsis borealis) were mainly caught in the layer of 71-80m fishing depth of the Subarctic Domain with water temperature of 6∼11℃ and salinity of 32.2∼33.6‰ and distributed in the latitudes of 41°∼43° N. Boreal clubhook squid (Onychoteuthis borealijaponica) were mainly caught in the layer of 11∼20m fishing depth of the Subarctic Domain with water temperature of 10∼12℃ and salinity of 32.9∼33.6‰ and distributed in the latitudes of 41°∼42N°. Tapanese flying squid (Todarodes pacificus) were mainly caught in the layer of 11∼20m fishing depth of the Transition Zone and the Subtropical Domain with water temperature of 15∼18℃ and salinity of 33.6∼34.0‰ and distributed in the latitudes of 40°∼42°N. Neon flying squid (Ommastrephes bartrami) were mainly caught in the layer of surface∼10m fishing depth of the Subarctic Convergence Zone and the Transition Zone with water temperature of 16∼17℃ and salinity of 33.7∼34.4‰ and distributed in the latitudes of 39 °∼41°N. Luminous flying squid (Symplectoteuthis luminosa) were mainly caught in the layer of 11 20m fishing depth of the Transition Zone and the Subtropical Domain with water temperature of 18∼20℃ and salinity of 33.8∼34.6‰ and distributed in the latitudes of 37°∼39°N. Purpleback flying squid (Symplectoteuthis oualaniensis) were mainly caught in the layer of surface∼10m fishing depth of the Subtropical Domain with water temperature of 24∼25℃ and salinity of 34.2∼34.4‰ and distributed in the latitude of 36°∼37°N.

Clarithromycin Therapy for Scrub Typus (쯔쯔가무시(Tsutusgamushi)병에서 Clarithromycin의 치료 효과)

  • Purpose : Scrub typhus(tsutsugamushi disease) is a febrile disease characterized by fever, rash, eschar, lymphadenopathy. Therapy with tetracycline(doxycycline) or chloramphenicol is currently recommended for the treatment for scrub typhus. But there are limitations in usage a tetracycline(doxycycline) for scrub typhus in the children. Recently, there was a report that azithromycin, a macrolide antibiotic was used for scrub typhus in pregnant woman successfully. So we evaluated the effectiveness of the Clarithromycin, other a macrolide antibiotic, for scrub typhus. Methods : Seven patients with scrub typhus at department of internal medicine and three patients with scrub typhus at department of pediatrics Masan Fatima Hospital were involved for this study. A serologic diagnosis for scrub typhus were performed by use of passive hemagglutination test. Clarithromycin(Abbott Laboratories, North Chicago, IL, USA) was administrated orally in a daily dose of 500 mg for adult patients and 15 mg/kg/bid/day for pediatric patients. Results : There were 7 cases of adult patients, varying from 28 to 76 years of age and 3 cases of pediatirc patients, varying from 4 to 7 years of age with scrub typhus. All of cases had fever, myalgia, headache, rash, eschar. Seven cases had positive passive hemagglutination test and eight cases had abnormal liver function. Mean duration for the removal of fever after medication was 1.3 day(1~2 days) and all cases were recovered without complications. Conclusion : Our results suggest that Clarithromycin therapy may be effective for scrub typhus.

Effects of Organic Iron Supplementation on Productivity and Egg Composition in Laying Hens (유기철의 첨가가 산란계의 생산성 및 난 성분에 미치는 영향)

  • This study was conducted to evaluate the effect of iron reinforcing agent on the performance in laying hens for seven weeks. One hundred ninety eight 30-wk-old Brown Tetran layers were assigned to 9 treatments: control containing 45 mg Fe from iron sulfate per kg diet (FE-45), FE-90 (control multiplied two times), FE-180 (control multiplied four times), YM-45 containing 45mg Fe from yeast mutant, YM-90 (YM-45 multiplied two times), YM-180 (YM-45 multiplied four times), YF-45 containing 45mg Fe from ferritin containing yeast, YF-90 (YF-45 multiplied two times), YF-180 (YF-45 multiplied four times) in the experiment. Each treatment had four replications of 6 birds each. In the average egg production, the YF-180 was highest and the FE-45 (the control) was lowest; the YM-180, YF-90, and YF-180 were significantly (P<0.05) higher than the control. In the comparisons of egg weight and egg mass, the YF-180 was significantly higher (P<0.05) than the FE-90 or the control. In the FCR, the YM-90 was highest and the YF-45 was lowest. There was no significant difference between the control and YM-90 although the YF-45 was significantly (P<0.05) lower in FCR than the control. In the eggshell thickness, the YF-180 was the thickest and the FE-90 was the thinnest; the YF-45 or the YF-180 was significantly thicker (P<0.05) than the control. In the eggshell strength, the YF-45 was the strongest and the YM-45 was the weakest; the YM-90, YF-45, and YF-180 were significantly stronger (P<0.05) than the control. In the cholesterol level of egg yolk, the control (FE-45) was the highest and the YF-45 was the lowest; there was a significantly different (P<0.05) between these two treatments. In conclusion, in case of adding organic iron to feed for layers, it gives improvement on performance of layers and develop chemical composition of eggs.

  • This study was conducted to evaluate the effect of organic iron supplemented to feed for broilers. One hundred forty four Ross broiler chicks were assigned to 6 treatments: control containing 80mg Fe from iron sulfate per kg diet(FE-80), FE-160 (control multiplied two times), YM-80 containing 80mg Fe from yeast mutant, YM-160 (YM-80 multiplied two times), YF-80 containing 80mg Fe from ferritin containing yeast, YF-160 (YF-80 multiplied two times) in the experiment. Each treatment had four replications of 6 birds each. The weight gain of the YM-160 was significantly higher (P<0.05) than that of the control (FE-80). The YM and YF in the serum cholesterol level were significantly higher (P<0.05) than the control. In the cholesterol level of carcass, although the control (FE-80) was highest and the YM and YF were very low; however, there were no significant differences among treatments. In the iron level of carcass, the control (FE-80) showed the lowest level among treatments; the YM and YF were significantly higher (P<0.05) than the control. In conclusion, the supplementation with organic iron to broiler chicks improve productivity. We also expect the possibility on chicken meat with reinforcing iron.

Development of a Small Gamma Camera Using NaI(T1)-Position Sensitive Photomultiplier Tube for Breast Imaging (NaI (T1) 섬광결정과 위치민감형 광전자증배관을 이용한 유방암 진단용 소형 감마카메라 개발)

  • Purpose: The conventional gamma camera is not ideal for scintimammography because of its large detector size (${\sim}500mm$ in width) causing high cost and low image quality. We are developing a small gamma camera dedicated for breast imaging. Materials and Methods: The small gamma camera system consists of a NaI (T1) crystal ($60 mm{\times}60 mm{\times}6 mm$) coupled with a Hamamatsu R3941 Position Sensitive Photomultiplier Tube (PSPMT), a resister chain circuit, preamplifiers, nuclear instrument modules, an analog to digital converter and a personal computer for control and display. The PSPMT was read out using a standard resistive charge division which multiplexes the 34 cross wire anode channels into 4 signals ($X^+,\;X^-,\;Y^+,\;Y^-$). Those signals were individually amplified by four preamplifiers and then, shaped and amplified by amplifiers. The signals were discriminated ana digitized via triggering signal and used to localize the position of an event by applying the Anger logic. Results: The intrinsic sensitivity of the system was approximately 8,000 counts/sec/${\mu}Ci$. High quality flood and hole mask images were obtained. Breast phantom containing $2{\sim}7 mm$ diameter spheres was successfully imaged with a parallel hole collimator The image displayed accurate size and activity distribution over the imaging field of view Conclusion: We have succesfully developed a small gamma camera using NaI(T1)-PSPMT and nuclear Instrument modules. The small gamma camera developed in this study might improve the diagnostic accuracy of scintimammography by optimally imaging the breast.

Parametric Images of Standardized Uptake Values using P-18-FDG Attenuation Corrected Whole Body PET (F-18-FDG감쇠보정 전신 PET을 이용한 표준섭취계수 추정과 매개변수 영상의 구성)

  • Background and Purpose : Standardized uptake value(SUV) has been used as a quantitative index for differentiating benign and malignant tumors with F-18-FDG PET In this study, we produced whole body parametric images of SUV(WBPIS) by body weight normalization, and validated the values by comparison with SUV's calculated with regional scans. Subjects and Methods : Whole body scans were followed by regional scans sequentially on 23 patients. In whole body study, transmission and emission scans were acquired for 2 minutes and 6 minutes for each bed position, respectively. In regional study, transmission and emission scans were acquired for 20 minutes. Measured and segmented/ smoothed attenuation correction were applied using these 2 min transmission scans in whole body studies. The effects of attenuation correction on SUVs were evaluated quantitatively using F-18 filled cylindrical phantom. The mean and peak SUVs obtained from WBPIS were compared with SUVs of the regional scans. Results : In phantom studies, with any method of attenuation correction using regional or whole body studies of phantom, SUVs were nearly consistent. In whole body scan, SUV obtained using measured attenuation correction method was a little higher than SUV of regional scan. SUV obtained using segmented/smoothed attenuation correction method was a little lower. In patient studies, WBPIS using segmented/smoothed attenuation correction method was much smoother and more readable. SUVs of WBPIS obtained with both methods of attenuation correction were well correlated with SUVs of regional scans(r=0.9). SUVs of WBPIS with measured attenuation correction method were 5% lower than SUVs of regional scans. SUVs of WBPIS with segmented/smoothed attenuation correction method were 10% lower than SUVs of regional scans. The differences of SUVs of WBPIS by the two attenuation correction methods were relatively small compared with the possible differences derived from biological characteristics of tumors. Conclusion : We concluded that WBPIS could be useful in the quantification of tumor as well as in localization of whole body lesions, which were often outside the field of view in regional scan. WBPIS made using segmented/smoothed attenuation correction method could be used in clinical routines and SUVs from attenuation corrected F-18-FDG PET could be used interchangeably with SUVs of regional studies.

Perfusion Impairment in Infantile Autism on Brain SPECT Using Tc-99m ECD : Comparison with MR Findings (유아 자폐증 환아에서의 Tc-99m ECD를 이용한 뇌 단일 광전자 방출 전산화 단층 촬영술상의 관류 저하: 자기 공명 영상과의 비교 분석)

  • Neuroanatomic substrate of autism has been the subjects of continuing investigation. Because previous studies had not demonstrated consistent and specific neuroimaging findings in autism and most studies comprised adults and school-aged children, we performed a retrospective review in search of common functional and anatomical abnormalities with brain SPECT using Tc-99m ECD and correlative MRI The patient population was composed of 18 children aged 28 to 89 months(mean age : 55 months) who met the diagnostic criteria of autism as defined in the DSM-IV and CARS. Brain SPECT was performed after intravenous injection of 185-370MBq of Tc-99m ECD using brain dedicated annular crystal gamma camera. MRI was performed in all patients including T1, T2 axial and T1 sagittal sequences. SPECT data were visually assessed. Thirteen patients had abnormal SPECT scan revealing focal areas of decreased perfusion. Decreased perfusion of cerebellar vermis(12/18), cerebellar hemisphere(11/18), thalami(13/18), basal ganglia(4/18), posterior parietal(7/18), and temporal(4/18) area were noted on brain SPECT. Whereas, only 3 patients had abnormal MR findings which were subtle volume loss of parieto-occipital white matter in 3 and mild thinning of posterior body of corpus callosum in 2 and slightly decreased volume of cerebellar vermis in 1. Comparison of the numbers of abnormal findings revealed that regional cerebral blood flow (rCBF) abnormalities seen on SPECT were more numerous than anatomical abnormalities seen on MRI. In conclusion, extensive perfusion impairment involving cerebellum, thalami and parietal lobe were found in this study. SPECT may be more sensitive in reflecting pathophysiology of autism than MRI. However, further studies are mandatory to determine the significance of thalamic and parietal perfusion impairment in autism.
