목적: 대퇴골 전치환술은 사지재건술의 극단적 술식 중 하나이며 증례가 드물어 이에 대한 연구는 주로 술식 후의 합병증에 대한 분석이며 본 술식의 적응증에 대한 분석은 미미하다. 저자들은 대퇴골 전치환술 36예의 1) 종양적 문제로 치환술을 받은 환자의 생존에 관련된 예후인자, 2) 치환물 및 하지의 생존율, 3) 치환물을 3년 이상 추시한 예의 합병증, 최종 하지 상태 및 기능적 결과를 알아보고자 하였다. 대상 및 방법: 대퇴골 전치환술을 받은 이유에 따라 원발성 종양에 의한 경우(15예, 1군), 오인 수술 및 국소재발로 인한 경우(16예, 2군), 재건물 실패로 인한 경우(5예, 3군)의 세 군으로 분류하였다. 환자 생존인자 분석 항목은 나이, 성별, 전치환술 원인, 항암요법 전후 종양 부피 변화, 절제연, 국소재발이었다. 결과: 전체 36예의 5년 환자 생존율은 31.5%±16.2%였다. 종양과 관련된 적응증으로 치환술을 받은 31예의 5년 생존율은 21.1%±15.6%였다. 항암요법 전후 종양의 크기가 같거나 감소한 10예의 5년 생존율은 50.0%±31.0%로 크기가 증가한 13예의 0.0%±0.0%보다 유의하게 높았다(p=0.02). 광범위 절제연을 얻은 12예의 5년 생존율은 41.7%±27.9%로 변연부 절제연의 0.0%±0.0% 보다 유의하게 높았다(p=0.03). 성별, 나이, 대퇴골 전치환술을 시행받은 원인, 치환술 후 국소 재발여부와 생존율과의 유의한 차이는 없었다. 치환물 36예에서 5년 생존율은 16.6%±18.2%였다. 하지의 10년 생존율은 85.9%±14.1%였다. 최종 추시상 종양인 공관절을 유지한 것이 12예, 회전성형술은 2예, 슬관절 상부 절단술 1예, 관절고정술 1예였다. 대퇴골 전치환술 후 3년 이상 추시가 가능했던 16예 중 수술이 필요했던 주 합병증이 14예, 보존적 치료만 했던 저등급 감염이 2예였다. 수술한 환자 14예 중 감염이 10예(71.4%)예, 국소재발이 2예, 고관절 탈구, bushing 파손, 대퇴동맥 폐색이 각각 1예였다. 결론: 종양과 관련된 적응증으로 대퇴골 전치환술을 시행한 환자 중 항암요법 후 종양의 크기가 커지거나 절제연이 불량한 경우, 환자가 조기에 사망하는 경우가 많았다. 따라서 전치환술의 시행에 신중해야 할 것으로 생각된다. 장기 생존 환자에서도 감염률이 높고 기능적 결과도 만족스럽지 않으나 현재 적응증을 고려했을 때 대퇴골 전치환술은 사지보존을 위한 최선의 방법으로 보인다.
목적 :다형성아교모세포종 환자들에서 cyclooxygenase-2 (COX-2) 단백의 발현 정도와 생존율에 미치는 영향을 조사하고자 한다. 대상 및 방법: 1997년부터 2006년까지 다형성아교모세포종으로 수술 및 방사선치료를 받은 환자들 중에서, 의식 상태의 악화로 40 Gy 전에 방사선치료가 중단된 3명을 제외한 30명을 대상으로 하였다. 조직에서의 COX-2의 발현은 면역조직화학염색으로 검사하였다. 생존 분석과 성별, 나이, 활동도, 수술 정도, 방사선량, COX-2 발현 정도 등이 생존율에 미치는 영향을 Kaplan Meier 법과 log rank test로 분석 및 검증하였다. 결과: 중앙추적관찰기간은 13.3개월이었다($6{\sim}83$). 전체 환자들에서 COX-2의 발현이 관찰되었고, 종양 세포의 75%이상에서 COX-2가 양성이었던 환자가 24명이었다. 종양 세포의 25%미만, 3명(10.0%); $25{\sim}50%$, 1명(3.3%); $50{\sim}75%$, 2명(6.7%); $75{\sim}100%$, 24명(80.0%). 중앙생존기간이 13.5개월이었고, 2년 생존율은 17.5%없다. 수술 정도(50% 이상 종양 제거)와 방사선량(59 Gy 이상 조사)이 생존율에 유의하게 영향을 주었다(p<0.05). 종양 세포의 75% 미만에서 COX-2가 발현되었던 환자군과 75% 이상에서 발현되었던 환자군에서 중앙생존기간은 각각 15.5개월과 13.0개월이었고(p>0.05), 2년 생존율은 각각 33.3%와 13.3%없다(p>0.05). 결론: 다형성아교모세포종에서의 COX-2 양성도는 높았지만, 다형성아교모세포종 환자들에서 COX-2 발현의 정도와 생존율 간에는 통계적인 유의성이 없었으므로, 향후 보다 많은 환자들을 대상으로 COX-2 발현 정도가 생존율에 미치는 영향에 대한 연구가 필요하다.

Purpose: To explore the role of salvage radiotherapy (RT) for recurrent thymoma as an alternative to surgery. Materials and Methods: Between 2007 and 2015, 47 patients who received salvage RT for recurrent thymoma at Yonsei Cancer Center were included in this study. Recurrent sites included initial tumor bed (n = 4), pleura (n = 19), lung parenchyma (n = 10), distant (n = 9), and multiple regions (n = 5). Three-dimensional conformal and intensity-modulated RT were used in 29 and 18 patients, respectively. Median prescribed dose to gross tumor was 52 Gy (range, 30 to 70 Gy), with equivalent doses in 2-Gy fractions (EQD2). We investigated overall survival (OS), progression-free survival (PFS), and patterns of failure. Local failure after salvage RT was defined as recurrence at the target volume receiving >50% of the prescription dose. Results: Median follow-up time was 83 months (range, 8 to 299 months). Five-year OS and PFS were 70% and 22%, respectively. The overall response rate was 97.9%; complete response, 34%; partial response, 44.7%; and stable disease, 19.1%. In multivariate analysis, histologic type and salvage RT dose (≥52 Gy, EQD2) were significantly associated with OS. The high dose group (≥52 Gy, EQD2) had significantly better outcomes than the low dose group (5-year OS: 80% vs. 59%, p = 0.046; 5-year PFS: 30% vs. 14%, p=0.002). Treatment failure occurred in 34 patients; out-of-field failure was dominant (intra-thoracic recurrence 35.3%; extrathoracic recurrence 11.8%), while local failure rate was 5.8%. Conclusion: Salvage RT for recurrent thymoma using high doses and advanced precision techniques produced favorable outcomes, providing evidence that recurrent thymoma is radiosensitive.
목적 : 천막상부 약성 신경교종의 치료로서 수술 후 방사선 치료의 역할을 생존율을 중심으로 평가하며 그 예후 인자를 분석한다. 대상 및 방법 : 1985년 9월부터 1997년 3월까지 수술 후 방사선 치료를 받은 69례의 환자를 대상으로 분석하였고 나이의 범위는 7세에서 66세이며 중앙값은 47세였다. 42례(61$\%$)는 다형성 교모세포종 이었고 27례(39$\%$)는 악성 성상세포종이었다. 20례(29$\%$)는 Karnofsky 역할수행능력 점수가 80점 이상이었다. 43례(62$\%$)는 수술 전 3개월 이내에 증상을 호소하였다. 24례 (35$\%$)는 육안 상 완전 절제를, 40례(58$\%$)는 부분 절제를, 나머지 5례(7$\%$)에서는 뇌정위 조직검사만 시행하였다. 총 방사선 조사량은 50.4-61.2Gy(중앙값, 55.8; 최빈값, 59.4)로 하루 1.8-2.0 Gy 씩 33-83일 동안(중앙값, 48) 치료하였다. 단 3례는 수술 후 전신 상태의 불량으로 하루 3.0Gy 씩 각각 33, 36, 39Gy 까지 치료하였다. 추적 관찰율은 93$\%$였고 중앙간은 14개월이었다. 결과 : 전체 환자의 2년 및 3년 생존율과 중앙 생존기간은 각각 38$\%$, 20$\%$, 16개월이었다. 악성 성상세포종과 다형성 교모세포종의 중앙생존기간은 각각 34, 14개월이었다(p=0.0001). 완전절제, 부분절제, 뇌정위 조직검사군에서의 3년 생존율은 각각 38$\%$, 11$\%$, 0$\%$였다(p=0.02). 연령이 40세 미만, 40-59, 60세 이상 군의 3년 생존율은 각각 52$\%$, 8$\%$, 0$\%$였다(p=0.0007). 역할수행 능력점수 80점 이상과 80점 미만군의 3년 생존율은 각각 53$\%$, 9$\%$였파(p=0.008). 상기와 같이 절제 정도에 따른 3개의 수술 군과 3개의 연령 군을 포함한 다요인 분석에서 병리학적등급, 수술 정도, 연령이 유의한 인자였다. 그러나 2개의 수술 군(완전절제와 비완전 절제)과 2개의 연령 군(50세 미만과 50세 이상)을 공변량으로한 다요인 분석에서 연령대신 병리학적 등급, 수술 정도, 역할수행 능력이 유의한 인자였고, 이들 세 가지 요인을 갖춘 5례는 치료와 관련한 심각한 부작용 없이 3년 누적생존율이 100$\%$로 생존하고 있다. 결론 : 악성 신경교종의 치료에서 수술 후 방사선치료를 추가함으로써 기존의 수술 단독의 생존율을 향상시키는 역할을 확인할 수 있었다. 악성 성상세포종 환자 중, 특히 양호한 역할수행 능력과 완전 절제를 받은 환자군의 생존율이 가장 양호하였다. 다형성 교모세포종 환자에서는 특히 젊은 나이인 경우 적절한 신경학적 기능보존과 함께 최대한의 종양 절제가 선행되어야 할 것으로 생각된다. 그러나 양호한 예후 인자를 갖지 못한 대부분의 다형성 교모세포종 환자군의 생존율은 여전히 좋지 않아 이 환자 군에서는 수술 후 통상적인 방사선 치료보다는 다른 적극적인 치료가 추가 또는 시도되어야 할 것으로 생각된다.
Purpose: Unresectable rectal cancer has a grave prognosis. regardless of the therapy used and median survival is less than 1 rear. Also, it is reported by many authors that $50-80\%$ of unresectable lesions were rendered resectable by radiation therapy and the median survival time for the completely resected patients were better than that of the unresected patients. So we analyzed retrospectively our data for the better treatment outcome in these patients. Materials and MEthods:From 1980 to 1992, 45 patients with initially unresectable tumors in the rectum were treated with radiation therapy with/without surgery in Department of Radiation Oncology, Yonsei Cancer Center 10 MV radiation and multiple field technique (box or AP/PA) were used. The total dose was 28-70 Gy and median dose was 48 Gy. We evaluated the lesion status at 45-50 Gy for operability. If the lesions appeared to be resectable, the Patients were operated on 4-6 weeks after radiation therapy. But if the lesions were still fixed, the radiation dose was increased to 60-65 Gy. Results: For all patients, the 2-year actuarial survival was $13.3\%$ and median survival was 9.5 months. Of 6 patients who had received less than 45 Gy, only $17\%$ of patients responded, but in the patients who had received more than 45 Gy, $60\%$ of response rate was achieved Six of the 24 patients$(25\%)$ underwent surgical resections following RT. For patients undergoing curative resection. the two-rear survival was $50\%,$ but that of the patients without resection was $9.5\%$ (p<0.01). Survival of patients with complete response following RT was $50\%$ at 2 years. Survival of patients with partial response, stable disease and progressive disease after RT was $13.4\%,\;15.4\%,\;0\%$ respectively (P<0.05). Conclision: Our data suffests that the efforts which can increase the response rate and aggressive surgical approach are needed to achieve the better local control and survival in unresectable rectal cancers.
Background: Data regarding childhood and adolescent non Hodgkin lymphomas in Iran are limited. The aim of this study was to assess the epidemiological and histomorphological features and survival of affected patients in our center. Materials and Methods: The clinicopathologic features and outcome of 44 children and adolescents with non Hodgkin lymphoma diagnosed during 2004-2012, were investigated retrospectively. The influence of potential prognostic parameters in overall survival was investigated by log-rank test and Cox regression analysis. Results: The mean age at presentation was $13.8{\pm}6.16$ years with a male predilection (M: F=3:1). Malignant lymphoma, not otherwise specified, diffuse large cell lymphoma and Burkitt lymphoma were the three most common histological types observed. The tumors were 36.4% intermediate grade, 27.3% high grade and 34.1% belonged to the malignant lymphoma not otherwise specified group. Immunohistochemistry findings were available in 39 cases. Out of these cases 33 (84.6%) had B cell lineage, 4 (10.25%) T cell lineage and 2 (5.12%) of the cases belonged to miscellaneous group. 3 year and 5 year survivals were 48% and 30% respectively and median survival was 36 months (95%CI=21.7-50.3 months). Overall survival in patients with high grade tumors was 19.5 months, in the intermediate group,79 months, and for malignant lymphomas not otherwise specified it was 33.6 months (p value=0.000). Conclusions: The survival rate for children and adolescents with non Hodgkin lymphomas at our center during 2004-2012 was at a low level.
Objective: To explore the clinical application of recombinant human endostatin (Endostar) in the treatment of patients with non-small cell lung cancer (NSCLC) in Chinese mainland. Materials and Methods: A total of 75 patients diagnosed as NSCLC were randomly divided into control group (37 cases) and treatment group (38 cases). Control group was treated with postoperative complementary chemotherapy containing two-agent platinum protocol on postoperative d21, 3 weeks as a cycle, for totally 4~6 cycles. On this basis, treatment group was added with Endostar $7.5mg/m^2$ on postoperative d8~9, 3~4 h/time, qd, 14 weeks as a cycle, for totally 4 cycles. The interval between every two cycles was 7 d. The 5-year progression-free survival (PFS), 5-year survival time and complications in both groups were observed. Results: Compared with control group, the average PFS increased evidently in treatment group by 9.8 months (41.6 months vs. 31.8 months), and there was significant difference (P<0.05). And the median PFS was 42.5 months in treatment group, obviously longer than that in control group (33.7 months) by 8.8 months (P<0.05). Additionally, the 5-year overall survival rate (OS), average survival time and median survival time (MST) were 47.4%, 50.1 months and 59.3 months in treatment group, significantly higher than the 29.7%, 42.1 months and 43.5 months in control group (P<0.05). Only 1 patient showed poor healing of surgical wound in treatment group, but no surgery-associated complication was found in control group. Moreover, the postoperative complementary therapy-connected complication rates were 63.2% (24/38) and 59.5% (22/37) in treatment group and control group respectively, but there was no significant difference (P>0.05). Conclusions: The application of Endostar combined with sensitive platinum-contained chemotherapeutic agents in the postoperative complementary chemotherapy can be widely used in clinic because it can significantly prolong the long-term survival time of patients with NSCLC.

Objectives: The surgical guide is a static computer-assisted device used for implant surgery planning and guidance. By taking an impression and referring to the patients' three-dimensional computed tomography scan of the desired implant site, a surgical guide can be created. During surgery, the surgical guide aids in achieving the designed implant placement position and direction. We examined and evaluated the long-term clinical outcomes of implant surgery using surgical guides. Materials and Methods: This study investigated a total of 15 patients with 32 implants that were placed using surgical guides from 2009 to 2011 with a mean follow-up period extended beyond 10 years. Patient demographics and implant survival rates were recorded. We analyzed marginal bone loss (MBL) by assessing the radiographs acquired at installation, three months after installation, and one month, one, two, and five years after prosthesis delivery. Results: The mean patient age was 57.33 years at implant placement. Of the 32 implants, five implants were placed in the anterior region and 27 implants were in the posterior region. Six implants failed and three of them were replaced, resulting in an 81.25% survival rate. The mean follow-up period was 10 years and nine months. Mean MBL compared to post-installation was significantly higher than at three months after installation, and one month, one, two, and five years after prosthesis delivery. Mean MBL at three months after installation, and one month, one year, and two years were significantly higher compared to the previous visit (P<0.05). However, MBL at five years after prosthesis delivery did not differ significantly compared to at two years. Conclusion: In this study, implant rehabilitation assisted by surgical guides exhibited favorable survival rates. With the limitation of the sample amount in this study, further research and more samples are required to evaluate the long-term clinical effectiveness of surgical guides.
Purpose: The authors evaluated the clinical results and prognosis after amputating the lower extremity due to diabetic foot. Materials and Methods: From 1991 to 2003, the patients who had suffered amputation of his lower extremity due to diabetic foot ulcer were evaluated retrospectively. 79 patients were male and 6 patients were female. The author evaluated the patient who had the ipsilateral additional surgery, contralateral amputation, level of blood sugar, combined disease and mortality rate within 5 years from medical record. Statistical analysis was done by Chi-square test and Kaplan-Meier survival test. Results: Mean age of patients who had first experienced amputation was 63.4 years old. The mean duration of diabetes until amputation was $14.5{\pm}7.5$ years. Major amputations were 50 cases and minor amputations 35 cases. 20 patients (23.5%) were suffered ipsilateral secondary surgery including revised stump. Overall 5-year mortality rate was 18.8% (16 cases). Death rate within 1 year was 8.2% (7 cases), mortality rate within 3 years was 14.1% (12 cases). 5-year mortality rate after major amputation was 20% (10 cases) and after minor amputation was 17.1% (6 cases). It was statistically significant (p<0.05). Patient who underwent more than 2 combined vascular related disease had higher mortality rate than diabetic amputee without combined disease (p<0.05). Conclusion: Mortality rate after major amputation was significant higher than amputation after minor amputation in diabetic patients from our data.
Purpose: To evaluate the treatment outcome and prognostic factor of postoperative radiotherapy for extremity soft tissue sarcoma (STS). Materials and Methods: Forty three patients with extremity STS were treated with conservative surgery and postoperative radiotherapy from January 1981 to December 2010 at Korea University Medical Center. Median total 60 Gy (range, 50 to 74.4 Gy) of radiation was delivered and 7 patients were treated with chemotherapy. Results: The median follow-up period was 70 months (range, 5 to 302 months). Twelve patients (27.9%) sustained relapse of their disease. Local recurrence occurred in 3 patients (7.0%) and distant metastases developed in 10 patients (23.3%). The 5-year overall survival (OS) was 69.2% and disease free survival was 67.9%. The 5-year local relapse-free survival was 90.7% and distant relapse-free survival was 73.3%. On univariate analysis, no significant prognostic factors were associated with development of local recurrence. Histologic grade (p = 0.005) and stage (p = 0.02) influenced the development of distant metastases. Histologic grade was unique significant prognostic factor for the OS on univariate and multivariate analysis. Severe acute treatment-related complications, Common Terminology Criteria for Adverse Events (CTCAE) grade 3 or 4, developed in 6 patients (14.0%) and severe late complications in 2 patients (4.7%). Conclusion: Conservative surgery with postoperative radiotherapy achieved a satisfactory rate of local control with acceptable complication rate in extremity STS. Most failures were distant metastases that correlate with tumor grade and stage. The majority of local recurrences developed within the field. Selective dose escalation of radiotherapy or development of effective systemic treatment might be considered.

허위 또는 타인의 정보를 등록할 경우 일체의 권리를 주장할 수 없습니다.

ID의 비밀번호 관리소홀, 부정사용에 의하여 발생하는 모든 결과에 대한 책임은 회원에게 있습니다.

제 4 장 서비스의 이용
제 12 조 (서비스 이용 시간)
① 서비스 이용은 당 사이트의 업무상 또는 기술상 특별한 지장이 없는 한 연중무휴, 1일 24시간 운영을
원칙으로 합니다. 단, 당 사이트는 시스템 정기점검, 증설 및 교체를 위해 당 사이트가 정한 날이나 시간에
서비스를 일시 중단할 수 있으며, 예정되어 있는 작업으로 인한 서비스 일시중단은 당 사이트 홈페이지를
통해 사전에 공지합니다.
② 당 사이트는 서비스를 특정범위로 분할하여 각 범위별로 이용가능시간을 별도로 지정할 수 있습니다. 다만
이 경우 그 내용을 공지합니다.
제 13 조 (홈페이지 저작권)
① NDSL에서 제공하는 모든 저작물의 저작권은 원저작자에게 있으며, KISTI는 복제/배포/전송권을 확보하고
있습니다.
② NDSL에서 제공하는 콘텐츠를 상업적 및 기타 영리목적으로 복제/배포/전송할 경우 사전에 KISTI의 허락을
받아야 합니다.
③ NDSL에서 제공하는 콘텐츠를 보도, 비평, 교육, 연구 등을 위하여 정당한 범위 안에서 공정한 관행에
합치되게 인용할 수 있습니다.
④ NDSL에서 제공하는 콘텐츠를 무단 복제, 전송, 배포 기타 저작권법에 위반되는 방법으로 이용할 경우
저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에
당 사이트 홈페이지를 통하여 회원에게 공지합니다.
② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
