Unexpected Seizure Attack in a Patient with Spinal Metastasis Diagnosed as Posterior Reversible Encephalopathy Syndrome

  • Seizure is a foreseeable risk in patients with brain lesion. However, seizure during treating non-brain lesion is not a familiar situation to neurosurgeon. Posterior reversible encephalopathy syndrome (PRES) is a relatively common situation after systemic chemotherapy. The aim of this study is to make neurosurgeons aware of this potential medical problem. A 52-year-old woman with advanced gastric cancer, presented with low back pain due to spinal metastasis at the 4th lumbar vertebra. Ten cycles of chemotherapy with FOLFOX (5-Fluoruracil/Oxaliplatin) had been completed 23 days ago. Two days before the planned operation, a generalized tonic clonic seizure occurred. She did not have a history of hypertension or seizure. The seizure was stopped with lorazepam 4mg. The brain magnetic resonance (MR) imaging showed high signal changes in both parieto-occipital lobes on the T2-weighted images, and these were partially enhanced, suggesting PRES. The surgery was preceded by treatment with an antiepileptic drug. The MR images, taken 1.5 months after the seizure, showed that the lesion was no longer present. At 3 month follow-up, no additional seizure attack occurred without any seizure medication. The possibility of a seizure attack should be considered if the patient has a history of chemotherapy.

Turrets Exostosis with Swan Neck Deformity: An Unusual Occurrence

  • Turrets exostosis are extremely rare to occur. Benign bony outgrowths from the dorsum of the phalanges following trauma are termed Turrets exostosis. We report an unusual case of Turrets exostosis of the fourth finger with no preceding trauma which has never been reported before. A 50-year-old female patient presented to us with a bony swelling on the dorsum of the fourth finger of her right hand in one year. The swelling was dormant initially and started to progress in size since three months leading to pain, restriction of movements and deformity of the finger. Turrets exostosis are very rare to occur. A preceding history of trauma is not necessary for it to occur as described in the literature. Surgical excision yields good results and is indicated when the bony mass causes a progressive deformity of the finger and restriction of movements of the finger.

Assessment on Forecasting Study of Traditional Chinese Medicine(1990${\sim}$2010) (중국 중의약 미래 예측 과제(1990${\sim}$ 2010)평가 연구)

  • Objectives : This study was to assess the Traditional chinese medicine forecast subjects that had been expected to be accomplished over 20 year (1990-2010). The result will help Korea medical society to compare the status of Korean Medicine with that of Traditional Chinese Medicine and to plan for polices and studies on Korean Traditional Medicine. Methods : Assessed targets were the subjects selected by the China Academy of Chinese Medical Sciences, which are classified into 6 fields. These were assigned by the quantity of related theses. Reference source is CAJ(China academic Journal) of CNKI (China National Knowledge infrastructure). Results : 1) Forecast subject ratio by field was basic theory 31% / clinical research 17% / Chinese herbal drug 17% / acupuncture and moxa 17% / Tui-na(推拿) and Qi-gong(氣功) 9% / medical information, literature history 6%. 2) Accomplishment percentage (full accomplishment) by field was medical information, literature, history 60% / basic theory 50% / acupuncture and moxa 46% Tui-na(推拿) and Qi-gong(氣功) 38% / chinese herbal drug 25% / clinical research 23%. Conclusions : 78% of all forecast subjects were accomplished or partially accomplished. According to 'accomplishment percentage by field', while those in the medical information, literature, history field were most realized of all, those in the clinical research field were least realized.

Level of Agreement and Factors Associated With Discrepancies Between Nationwide Medical History Questionnaires and Hospital Claims Data

  • Objectives: The objectives of this study were to investigate the agreement between medical history questionnaire data and claims data and to identify the factors that were associated with discrepancies between these data types. Methods: Data from self-reported questionnaires that assessed an individual's history of hypertension, diabetes mellitus, dyslipidemia, stroke, heart disease, and pulmonary tuberculosis were collected from a general health screening database for 2014. Data for these diseases were collected from a healthcare utilization claims database between 2009 and 2014. Overall agreement, sensitivity, specificity, and kappa values were calculated. Multiple logistic regression analysis was performed to identify factors associated with discrepancies and was adjusted for age, gender, insurance type, insurance contribution, residential area, and comorbidities. Results: Agreement was highest between questionnaire data and claims data based on primary codes up to 1 year before the completion of self-reported questionnaires and was lowest for claims data based on primary and secondary codes up to 5 years before the completion of self-reported questionnaires. When comparing data based on primary codes up to 1 year before the completion of selfreported questionnaires, the overall agreement, sensitivity, specificity, and kappa values ranged from 93.2 to 98.8%, 26.2 to 84.3%, 95.7 to 99.6%, and 0.09 to 0.78, respectively. Agreement was excellent for hypertension and diabetes, fair to good for stroke and heart disease, and poor for pulmonary tuberculosis and dyslipidemia. Women, younger individuals, and employed individuals were most likely to under-report disease. Conclusions: Detailed patient characteristics that had an impact on information bias were identified through the differing levels of agreement.

State of Practice of Performance-Based Seismic Design in Indonesia

  • The current 2002 Indonesian Seismic Code consists of prescriptive criteria that are intended to result in buildings capable of providing certain levels of performance. However, the actual performance capability of buildings is not assessed as part of the code procedures. Several analysis procedures are allowed, and the state of practice is to use the RSA with six-zone seismic map developed for 475-year earthquake. This code is being revised and will adopt many of the ASCE7-10 provisions and 2475-year earthquake for MCE. The growth of tall buildings compels engineers to look for more optimal lateral system. The use of RC core wall as single system has been adopted by very few engineering firms, which is allowed in the current code but will no longer be the case if the new one is in effect. Other innovative structural system such as core wall and outrigger is not addressed in the proposed new code. Engineers must then resort to NLRHA. Currently, one 50-story building under construction using RC core wall and outrigger has been designed with RSA and employing capacity design principles, then evaluated using NLRHA per TBI Guidelines. Based on the evaluation, the performance of the 50-story building generally still meets the criteria of the TBI Guidelines. The result of the case study is presented in this paper.

Foraminal Synovial Cyst Associated with Ankylosing Spondylitis

  • Ankylosing spondylitis (AS) is frequently associated with inflammatory lesions of the spine and continuous fatigue stress fractures; however, an association with an intraspinal synovial cyst has not been previously reported. A 55-year-old man with a five year history of AS who presented with back pain and a right radiculopathy was admitted to the hospital. Five years previously, he underwent a percutaneous vertebroplasty for an osteoporotic L1 compression fracture, and was diagnosed with AS at that time. Plain radiographs showed aggravated kyphosis and a stress fracture through the ossified posterior element, below the prior vertebroplasty. Magnetic resonance images revealed a right foraminal cystic lesion at the L2-L3 level with effacement of the nerve root. A 1.6 cm cystic lesion that appeared to arise from the L2-L3 facet joint without direct communication was excised from the L2-L3 foramen. Pathological examination confirmed synovial cyst. The patient's symptoms resolved immediately after surgery except for a mild dysesthesia of the right leg. We report herein a rare case of foraminal synovial cyst associated with AS accompanying posterior element fracture with a review of literature.

The 50-year history of the Korean Society of Plant Taxonomists: Professional manpower training and research activity (한국식물분류학회 50년사: 인력양성과 연구활동)

  • Since the establishment of the Korean Society of Plant Taxonomists (KSPT) on Dec. 13, 1968, the professional manpower training and research activities have been analyzed. The survey method was based on the homepage of the KSPT and the Korean J. Pl. Taxon, and on data provided by each university about professional manpower. Over the past 50 years, a total of 680 specialists in plant taxonomy have been trained, consisting of 537 master's degree holders (274 males, 263 females) from 30 universities and 143 PhDs (97 males, 46 females) from 26 universities, and the number has increased significantly since 1998. With regard to changes in the field of research over the last ten years, revision papers were the most common in the period of 1988-1997 (72%), but this rate has decreased to 51% over the last ten years, while the number of unrecorded papers has increased to 28%. In the 629 revision papers on taxa, 49% of the taxa belong to Asteraceae, Ranunculaceae, Cyperaceae, Liliaceae, Rosaceae, Fabaceae, Apiaceae, Lamiaceae, Orchidaceae, Oleaceae, Euphorbiaceae, Polygonaceae, and Amaryllidaceae. With regard to changes in research methods, the number of morphological papers increased from 6% to 51%, while pollen papers have decreased from a rate of 20% to only 2%. Chromosome studies account for 3-4%, chemotaxonomic studies 2%, and DNA studies remain low at 3-16%. The percentage of papers in English now stands at 43%, mainly due to the increased number of papers on unrecorded species.

Life History of the Endangered Endemic Species, Rhynchocypris semotilus (Pisces: Cyprinidae) in the Songhyeoncheon (Stream), Korea (송현천에 서식하는 멸종위기어류 버들가지 Rhynchocypris semotilus (Pisces: Cyprinidae)의 생활사)

  • Life history of the endangered endemic species, Rhynchocypris semotilus, was investigated to obtain baseline data for its ecological characteristics in Songhyeoncheon, Hyeonnae-myeon, Goseong-gun, Gangwon-do, Korea from April to October, 2022. A total of 10 families and 18 species of fishes were collected from 4 survey stations, and R. semotilus was a subdominant species in the survey period (seven times). The main habitat of R. semotilus was the middle-upper stream of rock and boulder bottoms with aquatic plants and slow rapids and pools. The age groups for R. semotilus estimated by the frequency distribution of total length in spawning season (May) indicated the 4~12 mm is 0-year old, 32~49 mm is 1-year old, the 50~65 mm group is 2-years old, the 66~91 mm is 3-years old, and the 92~116 mm is more than 4-years old, and showed rapid growth from May to September. Total length range of female and male were the relatively similar, and sex ratio (♂/♀) was 0.78. The spawning period is from May to July (water temperature is 17 to 21℃), and gonad was developed more than three years old (more than TL 66 mm). In May, the gonadosomatic index (GSI) was 10.6±2.99% for females and 8.2±2.34% for males, fecundity was 6,830±1,943, and mature eggs size was 1.34±0.16 mm. Finally, life history characteristics of R. semotilus was discussed between the genus Rhynchocypris and Phoxinus species.

Evalution of Earthquake Resistance capacity of Semi-rigid Mid/Low-rise Steel Frame using Composite Panel (복합소재패널을 이용한 반강접 중저층 강골조의 내진성능평가)

  • This paper presented regarding an parametric study to investigate seismic capacity evaluation of semi-rigid steel frame infilled with composit panel. In order to propose the optimum retrofit of the steel frame, we analysed the various pattern of retrofitted steel frame subjected to weak/medium earthquake. Steel frame with composit panel was analysed by Time history analyses analysis. The model were analysed using the suites of ground motion developed by NEHRP project on steel moment resisting frame. These earthquakes consist of 20 horizontal ground acceleration record each, i.e., a 10%, 50% probability of accidence in a 50 year period. We considered the semi-rigid connection which are commonly used in field, and modeled the nonlinear connection element (GAP) between panel and frame. It was shown that how is the steel frame with composit panel effected. We also examined the response of retrofitted frame.

Internal Drainage of an Esophageal Perforation in a Patient with a High Surgical Risk

  • A 71-year-old man presented with a productive cough and fever, and he was diagnosed as having an esophageal perforation and a mediastinal abscess. He had a history of traumatic hemothorax and pleural drainage for empyema in the right chest and was considered unable to tolerate thoracic surgery because of sepsis and progressive aspiration pneumonia. In order to aggressively drain the mediastinal contamination, we performed internal drainage by placing a Levin tube into the mediastinum through the perforation site. This procedure, in conjunction with controlling sepsis and providing sufficient postpyloric nutrition, allowed the esophageal injury to completely heal.
