Cohort Study on Age at Menopause and Mortality - Kangwha Cohort Study - (폐경 연령과 사망력과의 관계에 대한 코호트 연구 - 강화 코호트 연구 -)

  • Objective : To examine the association between age at menopause and mortality in a population-based sample of women in Kangwha, Korea. Methods : From the Kangwha Cohort, followed-up from 1985 to 1999, the data of the over 55 year old female group(n=3,596) was used in this study to examine the association between age at menopause and mortality. We calculated the all causes mortality risk ratio and the cancer mortality risk ratio by age at menopause grouping using the Cox Proportional Hazards Model with adjustments for age, BMI, smoking, education, chronic disease, self-rated health status, alcohol consumption and age at first birth. Result and conclusion : Compared to women who had menopause at 45-49 years, the all causes mortality risk ratio was 1.24 for women with menopause at less than 40 years(95% CI=1.01-1.53) and 1.05 for women with menopause at over 50 years(95% CI=0.92-1.20). Also, compared to women who had menopause at 45-49 years, the cancer mortality risk ratio was 1.53 for women with menopause at less than 40 years(95% CI=0.78-2.98) and 1.17 for women with menopause at over 50 years(95% CI=0.77-1.80).

Trends in incidences of newly notified tuberculosis in Jeju Province, Korea, 2017-2021

  • In 2021,the incidence of newly notified tuberculosis in Jeju Province decreased by 23.0% compared to that in the previous year. This was the largest decline among the 18 municipalities studied. This study aimed to examine trends in the incidence of tuberculosis among different age and sex groups in Jeju Province between 2017 and 2021. This study observed the number of new cases in the annual report from 2017 to 2021, published by the Jeju Center for Infectious Diseases Control and Prevention. The average annual percentage change (AAPC; %) was calculated to determine the trend in crude incidence rates over the past 5 years by sex and age. While a P-value of <0.1 was considered marginally significant, aP-value of <0.05 was considered statistically significant. The AAPC of all age groups in Jeju Province had negative values. In men,there was a statistically significant decrease in ages 30-34, 40-44, 45-49, and 50-54 years and a marginally significant decrease in ages 35-59, 55-59, 60-64, 65-69, 75-79, and ≥80 years. In women, a statistically significant decrease was observed among patients aged 30-34, 35-39, and ≥80 years, and a marginally significant decrease was observed among patients aged 45-49, 50-54, 60-64, 65-69, and 75-79 years. A statistically significant decrease in tuberculosis was observed among patients in their 30s and 40s, and elderly patients aged ≥80 years comprised the group vulnerable to tuberculosis. Based on these findings, tuberculosis prevention and screening programs conducted in Jeju Province were effective.

Prevalence of Osteopenia/Osteoporosis and Related Risk Factors of Men Aged 50 Years and Older: Korea National Health and Nutrition Examination Survey 2010~2011 Data (우리나라 50세 이상 남성의 골감소증·골다공증 유병률과 관련 요인: 2010~2011 국민건강영양조사 자료)

  • Osteoporosis is a major health problem that can lead to mortality. This study was conducted to estimate the prevalence of osteopenia and osteoporosis separately and to assess the risk factors associated with osteopenia/osteoporosis in Korean men aged 50 years and over. A total of 1,136 subjects were analyzed among the participants of the Korea National Health and Nutrition Examination Survey (KNHANES) 2010~2011 by using SPSS statistics complex samples (windows ver. 23.0). The prevalence rates of osteopenia and osteoporosis were 46.3% and 7.3%, respectively, and the mean ages of both osteopenia and osteoporosis risk groups were significantly higher than that of the normal group. The mean values for lifetime tallest height and bone mineral density in whole body, total femur, femoral neck, and lumbar spine were significantly lower in the risk group(osteopenia/osteoporosis) than in the normal group, whereas the mean values for height, body weight, BMI, waist circumference, alkaline phosphatase, parathyroid hormone, fasting blood glucose, total cholesterol, HDL-cholesterol, LDL-cholesterol, and triglycerides of the two groups showed no significant differences. The intakes of energy and carbohydrates were significantly higher in the risk group than in the normal group. The results of multiple logistic regression showed that being underweight and having hypercholesterolemia were significantly related with the prevalence of osteopenia/osteoporosis, whereas health habits such as smoking and exercise, chronic diseases such as obesity and hypertension, and nutrient intakes were not. These findings suggest the need for further studies to examine osteopenia/osteoporosis risk factors and outcomes specificly focused on Korean men.

  • Current domestic research is to demonstrate the effectiveness and efficiencies of flood prevention measures through one-dimensional numerical analysis and this study's object is to help water managers to make the efficient decisions by applying the two-dimensional urban run off model XP-SWMM model in the flooded area and comparing with the flood prevention measures. Statistics were analyzed, based on the data collected from Cheongju Weather Service from 1967 to 2011 for 45 years. 50 years Flood frequency simulations of water flow capacity analysis of the target area for flooded areas $539,548m^2$, inundation depth 1.0 m, was analyzed by inundation time of 48 minutes. When comparing with the constructions of bypass road and underground storage facilities to increase the water flow capacity of A1 small drainage areas as flood protection, if you install a batching target underground detention basin with a capacity of $13,500m^3$, it is expected that the flood by rainfall with frequency of 50 years will be resolved completely. In preparation for extreme weather in the future flood mitigation measures, underground storage tank installation is considered a better efficient way.

An Analysis of Usefulness of Information Literacy Education for the over 50-years Older Adults in Daegu, Korea (대구지역 장노년층의 정보화교육 유용성 분석)

  • The aim of this study was to analyse the usefulness of information literacy education for the over 50-years older adults in Daegu, Korea. To achieve this objective, data were collected through the questionnaire survey and interview. Of the 311 valid data were categorized into two groups in accordance with their experience of information literacy education, and analysed using AMOS for the structural equation modeling. As the results, both groups showed that the information literacy education influences positively among in 'information convenience', in 'information usefulness' and in 'life satisfaction'. Also, regardless of their experience of information literacy education, the group revealed that 'information convenience' positively impacts on the 'life satisfaction', however, 'information usefulness' did not show any significant relationship with the 'life satisfaction' in this study.

Association of Body Mass Index with Oral Cancer Risk

  • Purpose: Although obesity is a well-established risk factor for many cancers, the effect of body mass index (BMI) on oral cancer risk remains controversial. We therefore investigated the effect of BMI on oral cancer risk in a case-control study in Korea. Methods: Overall, 364 patients with oral cancer and 439 community controls were enrolled. Odds ratios (OR) and 95% confidence intervals (CI) were estimated using logistic regression models, adjusted for age, smoking status and alcohol consumption. Results: We found no overall significant evidence of an association between oral cancer risk and BMI in either gender. However, when the relationship between BMI and oral cancer risk was examined according to female age groups (<50 and ${\geq}$50 years), there was a significant association between oral cancer risk and high BMI in female subjects younger than 50 years of age (OR=3.92, 95% CI 1.03~14.9, $P$ for trend=0.04), but not in older (${\geq}$50 years) female subjects (OR=1.11, 95% CI 0.55~2.24, $P$ for trend=0.76). There was no significant relationship between BMI and oral cancer risk in any of the male age subgroups. Conclusion: Our study provides the first epidemiological evidence supporting an association between obesity and an increased risk of oral cancer.

Association between Metabolic Syndrome and Macronutrient Intakes in Adults aged 50 years and older based on the 2013-2017 Korean National Health and Nutrition Examination Survey Data (2013-2017년 국민건강영양조사 자료에 근거한 50세 이상 성인의 대사증후군과 다량영양소 섭취의 관련성)

  • The purpose of this study was to investigate the association between dietary macronutrient intakes and prevalence of metabolic syndrome (MetS) in Korean adults. Data were obtained from the Korean National Health and Nutrition Examination Survey (2013-2017), and a total of 11,600 Korean adults (4,918 men, 6,682 women) aged 50 years and older were analyzed. The daily intakes and percentages of energy from carbohydrates or fat in men or women with MetS were significantly lower than in normal subjects, respectively. High carbohydrate or fat percentages were negatively associated with MetS based on adjusted odds ratios (OR) of 0.804 in men (p=0.034) and 0.820 in women (p=0.045), respectively. A high percentage of energy from carbohydrates was positively associated with reduced waist circumference, diastolic blood pressure, triglyceride and high-density lipoprotein (HDL) cholesterol in men. On the other hand, a high percentage of energy from fat was positively related with elevated HDL cholesterol and reduced triglycerides in women. In conclusion, our study indicates that high carbohydrate or fat intake is associated with risk of MetS in Korean men or women aged 50 years and older, respectively. Further prospective studies are necessary to elucidate the association between macronutrient intakes and MetS among Korean adults according to age.

Influences of changes in natural environments by natural hazards and human activities in ancient times in Korea on collapse of the Unified Silla Dynasty (자연재해와 인위적 환경변화가 통일신라 붕괴에 미친 영향)

  • After the unification of Three Kingdoms, Silla Dynasty had been on the prosperity with continuous population growth during approximately 120 years. However, Unified Silla Dynasty experienced 11 times of famine and bad year by frequent droughts during 50 years since the late 8th century, and these caused to political, economic and social changes. It is evident that Silla Dynasty slowly declined by the droughts and famines, although ancient records described as still on glory days. Since then, national power of Unified Silla Dynasty was abruptly weakened by migration of residents, and 9 times of rebellion or revolt occurred during 50 years in the late 9th century. As a result, political and social agitations reached the maximum. These led to the collapse of the Unified Silla Dynasty, and King Gyeongsoon handed over the dynasty to King Wanggeon, a founder of Goryeo Dynasty. These resulted from the 500-year-cycle droughts and human activities that seriously devastated the vegetations by using trees as an energy source for hundreds of thousands people during several centuries. The devastation of vegetation destroyed farm lands and led to large changes in river and groundwater systems, and eventually, increased damages by droughts.

Health-Related Behavioral Factors Associated with Nutritional Risks in Korean Aged 50 years and Over (50세 이상 중년 및 노인의 건강행위 요인에 따른 영양위험 연구)

  • Undernutrition could be a significant deterrent to healthy aging and could negatively affect health outcomes in elderly. This study aimed to assess health-related factors which are associated with nutritional risks in middle-aged and elderly individuals by a cross-sectional study. Interviews were conducted with 2660 subjects (847 males, 1813 females), aged 50 years and over, in 15 cities in Korea. Data on food intake were obtained through a validated semi-quantitative food frequency questionnaires. Nutritional status were analyzed according to health-related factors including cigarette smoking, alcohol drinking, exercise, stress and depression level. Less regular exercise was associated with a higher likelihood of a poor nutrition [odds ratio (OR) 1.94; 95% confidence intervals (CI) 1.43-2.65] of middle-aged and elderly male subjects. Cigarette smoking (OR 1.84; 95% CI 1.24-2.71), less exercise (OR 2.58; 95% CI 2.07-3.21), stress (OR 1.73; 95% CI 1.36-2.22), and depression (OR 1.34, 95% CI 1.08-1.67) of middle-aged and elderly female subjects was associated with a higher likelihood of a poor nutrition. The results of the multiple regression analysis showed that less exercise proved to be the strongest predictors for the poor nutrition, followed by stress, smoking, and depression (model $R^2= 9.0%$). It suggests that guidance to promote regular exercise, to quit smoking, to minimize stress and depression level might help to improve nutritional status of middle-aged and elderly in Korea. These findings also suggest that having recommendable health behaviors are beneficial to the good nutrition of subjects aged 50 years and over.

Fertilization of N and Si to Sustain Grain Yield and Growth Characteristics of Rice after Winter Greenhouse Water-melon Cropping

  • In Korea, silicate fertilization (SF) is being practiced every four years to enhance rice production. However, the relationship between nitrogen (N) and SF in view of growth characteristics and grain yield of rice has not been examined after watermelon cropping in plastic film house. This study was carried out to identify useful critical N and Si fertilizer levels to sustain grain yield and to improve N use efficiency for rice. The watermelon-rice cropping system has maintained for three seasons in each year from 1998 to 2001 by farmer before this experiment. Experiments on N and Si fertilization levels were evaluated with Hwayoungbyeo (Oryza sativa L.) in 2002 and 2003 at Uiryeong, Korea. The goal of this experiment was to find out the optimum N and Si levels to sustain rice yield by reducing excessive N fertilizer in watermelon-rice cropping system. Nitrogen fertilization (NF) levels were three ($0,\;57,\;114kg\;ha^{-1};0,\;50,\;100%$ of conventional NF amount) and five (0, 25, 50, 75, 100%) in 2002 and 2003, respectively, and combined with three SF levels ($70,\;130,\;180mg\;kg^{-1};100,\;150,\;200%$ which were adjusted with Si fertilizer in soil) were evaluated for the improvement of N and Si fertilization level in both years. Rice yielded 3.98-5.95 and 2.84-4.02 t/ha in 2002 and 2003, respectively. Our results showed the combinations of 50% and 100% of N with 200% level of Si produced the highest grain yield in both years, respectably. The grain yield was greatly improved in plot of N25% level when compared to conventional NF (Nl00%) in 2003. In conclusion, NF amount could be reduced about 50% compared to recommended level by specific fertilization of N and Si combination levels for rice growing and grain yield after cultivation watermelon in paddy field.
