The object of this study was to evaluate the single oral dose toxicity of platycodin D, a saponin from the root of Platycodon grandiflorum in male and female mice. Platycodin D was administered to female and male mice as an oral dose of 2000, 1000, 500, 250 and 125 mg/kg (body wt.). Animals were monitored for the mortality and changes in body weight, clinical signs and gross observation during 14 days after treatment, upon necropsy, organ weight and histopathology of 14 principle organs were examined. As the results, no platycodin D treatment related mortalities, clinical signs, changes on the body and organ weights, gross and histopathological observations against 14 principle organs were detected up to 2000 mg/kg in both female and male mice. Therefore, $LD_{50}$ (50% lethal dose) and approximate LD of playtcodin D after single oral treatment in female and male mice were considered over 2000 mg/kg - the limited dosages recommended by KFDA Guidelines [2009-116, 2009], respectively.
To quantitatively evaluate how setup errors in conjunction with dose gradients contribute to the error in IMRT dose quality assurance (DQA) measurements. The control group consisted of 5 DQA plans of which all individual field dose differences were less than ${\pm}5%$. On the contrary, the examination group was composed of 16 DQA plans where any individual field dose difference was larger than ${\pm}10%$ even though their total dose differences were less than ${\pm}5%$. The difference in 3D dose gradients between the two groups was estimated in a cube of $6{\times}6{\times}6\;mm^3$ centered at the verification point. Under the assumption that setup errors existed during the DQA measurements of the examination group, a three dimensional offset point inside the cube was sought out, where the individual field dose difference was minimized. The average dose gradients of the control group along the x, y, and z axes were 0.21, 0.20, and 0.15 $cGy{\cdot}mm^{-1}$, respectively, while those of the examination group were 0.64, 0.48, and 0.28 $cGy{\cdot}mm^{-1}$, respectively. All 16 plans of the examination group had their own 3D offset points in the cube. The individual field dose differences recalculated at the offset points were mostly diminished and thus the average values of total and individual field dose differences were reduced from 3.1% to 2.2% and 15.4% to 2.2%, respectively. The offset distribution turned out to be random in the 3D coordinate. This study provided the quantitative data that support the large individual field dose difference mainly stems from possible geometric errors (e.g., random setup errors) under the influence of steep dose gradients of IMRT field.
목 적: 환자 체표면과 고정기구 사이에 발생하는 공기층 두께에 따른 선량 변화를 치료 계획을 통해 알아보고자 한다. 대상 및 방법: 팬텀과 열가소성 고정기구 사이에 5 mm 두께의 Bolus를 0, 1, 2, 3장을 놓아 공기층의 두께를 조절하였고 고정기구를 씌워 총 4가지 조건으로 전산화 모의단층촬영을 시행하였다. 430 cGy (Relative Biological Effectiveness,RBE)씩 6번이 조사 되도록 계획하였으며, 임상표적체적의 95% 부피에 전달된 선량이 2580 cGy (RBE)가 되도록 치료 계획을 수립하였다. 임상표적체적의 선량은 Lateral dose profile의 반치폭값으로 평가하였고 피부 선량은 선량 체적 곡선으로 평가하였다. 결 과: 임상표적체적에서 Lateral dose profile 반치폭 값은 4.89, 4.86, 5.10, 5.10 cm로 나타났다. 피부에서 4가지 조건의 선량의 평균값은 D95%3.25±1.7 cGy (RBE), D30%1193.5±10.2 cGy (RBE)의 차이를 보였으며 처방 선량 1%에서의 피부 부피 값 평균은 83.22±4.8% 이내의 차이를 확인하였다. 공기층 두께 변화에 따른 임상표적체적과 피부에서의 선량 모두 큰 변화를 보이지는 않았다. 결론 : 탄소입자 치료를 위해 Solid 형태의 고정기구 제작 시 약간의 공기층은 CTV의 선량 적용 범위를 벗어나지 않는다.

Objective: Three-dimensional rotational angiography (3D-RA) is increasingly used for the evaluation of intracranial aneurysms (IAs); however, radiation exposure to the lens is a concern. We investigated the effect of head off-centering by adjusting table height on the lens dose during 3D-RA and its feasibility in patient examination. Materials and Methods: The effect of head off-centering during 3D-RA on the lens radiation dose at various table heights was investigated using a RANDO head phantom (Alderson Research Labs). We prospectively enrolled 20 patients (58.0 ± 9.4 years) with IAs who were scheduled to undergo bilateral 3D-RA. In all patients' 3D-RA, the lens dose-reduction protocol involving elevation of the examination table was applied to one internal carotid artery, and the conventional protocol was applied to the other. The lens dose was measured using photoluminescent glass dosimeters (GD-352M, AGC Techno Glass Co., LTD), and radiation dose metrics were compared between the two protocols. Image quality was quantitatively analyzed using source images for image noise, signal-to-noise ratio, and contrast-to-noise ratio. Additionally, three reviewers qualitatively assessed the image quality using a five-point Likert scale. Results: The phantom study showed that the lens dose was reduced by an average of 38% per 1 cm increase in table height. In the patient study, the dose-reduction protocol (elevating the table height by an average of 2.3 cm) led to an 83% reduction in the median dose from 4.65 mGy to 0.79 mGy (P < 0.001). There were no significant differences between dose-reduction and conventional protocols in the kerma area product (7.34 vs. 7.40 Gy·cm2, P = 0.892), air kerma (75.7 vs. 75.1 mGy, P = 0.872), and image quality. Conclusion: The lens radiation dose was significantly affected by table height adjustment during 3D-RA. Intentional head off-centering by elevation of the table is a simple and effective way to reduce the lens dose in clinical practice.
Purpose : Three dimensional conformal radiotherapy planning is being used widely for the treatment of patients with brain tumor. However, it takes much time to develop an optimal treatment plan, therefore, it is difficult to apply this technique to all patients. To increase the efficiency of this technique, we need to develop standard radiotherapy plant for each site of the brain. Therefore we developed several 3 dimensional conformal radiotherapy plans (3D plans) for tumors at each site of brain, compared them with each other, and with 2 dimensional radiotherapy plans. Finally model plans for each site of the brain were decide. Materials and Methods : Imaginary tumors, with sizes commonly observed in the clinic, were designed for each site of the brain and drawn on CT images. The planning target volumes (PTVs) were as follows; temporal $tumor-5.7\times8.2\times7.6\;cm$, suprasellar $tumor-3\times4\times4.1\;cm$, thalamic $tumor-3.1\times5.9\times3.7\;cm$, frontoparietal $tumor-5.5\times7\times5.5\;cm$, and occipitoparietal $tumor-5\times5.5\times5\;cm$. Plans using paralled opposed 2 portals and/or 3 portals including fronto-vertex and 2 lateral fields were developed manually as the conventional 2D plans, and 3D noncoplanar conformal plans were developed using beam's eye view and the automatic block drawing tool. Total tumor dose was 54 Gy for a suprasellar tumor, 59.4 Gy and 72 Gy for the other tumors. All dose plans (including 2D plans) were calculated using 3D plan software. Developed plans were compared with each other using dose-volume histograms (DVH), normal tissue complication probabilities (NTCP) and variable dose statistic values (minimum, maximum and mean dose, D5, V83, V85 and V95). Finally a best radiotherapy plan for each site of brain was selected. Results : 1) Temporal tumor; NTCPs and DVHs of the normal tissue of all 3D plans were superior to 2D plans and this trend was more definite when total dose was escalated to 72 Gy (NTCPs of normal brain 2D $plans:27\%,\;8\%\rightarrow\;3D\;plans:1\%,\;1\%$). Various dose statistic values did not show any consistent trend. A 3D plan using 3 noncoplanar portals was selected as a model radiotherapy plan. 2) Suprasellar tumor; NTCPs of all 3D plans and 2D plans did not show significant difference because the total dose of this tumor was only 54 Gy. DVHs of normal brain and brainstem were significantly different for different plans. D5, V85, V95 and mean values showed some consistent trend that was compatible with DVH. All 3D plans were superior to 2D plans even when 3 portals (fronto-vertex and 2 lateral fields) were used for 2D plans. A 3D plan using 7 portals was worse than plans using fewer portals. A 3D plan using 5 noncoplanar portals was selected as a model plan. 3) Thalamic tumor; NTCPs of all 3D plans were lower than the 2D plans when the total dose was elevated to 72 Gy. DVHs of normal tissues showed similar results. V83, V85, V95 showed some consistent differences between plans but not between 3D plans. 3D plans using 5 noncoplanar portals were selected as a model plan. 4) Parietal (fronto- and occipito-) tumors; all NTCPs of the normal brain in 3D plans were lower than in 2D plans. DVH also showed the same results. V83, V85, V95 showed consistent trends with NTCP and DVH. 3D plans using 5 portals for frontoparietal tumor and 6 portals for occipitoparietal tumor were selected as model plans. Conclusion : NTCP and DVH showed reasonable differences between plans and were through to be useful for comparing plans. All 3D plans were superior to 2D plans. Best 3D plans were selected for tumors in each site of brain using NTCP, DVH and finally by the planner's decision.
To examine the possibility of ear mushroom (EM) as a source of natural vitamin D, the UVB (ultraviolet B) was treated according to sample drying status, drying methods before UVB treatment and harvest time. And then, vitamin D2 and ergosterol contents were investigated. According to the sample drying status, the vitamin D2 contents of fresh and freeze-dried EM (whole) increased to 4,634.4~4,780.9 ㎍/100 g D.W. (dry weight) under UVB dose 52.5~70.0 kJ/㎡ and above 18,693.1 ㎍/100 g D.W. under above 105 kJ/㎡, respectively. By drying methods before UVB treatment, vitamin D2 contents of EM powder (below 500 ㎛) that dried in the vinyl house and freeze-dryer increased to 4,886.2~5,132.9 ㎍/100 g D.W. under above 105 kJ/㎡ and 17,103.7 ㎍/100 g D.W. under 70 kJ/㎡, respectively. Ergosterol content decreased with increasing UVB dose in all experiments. According to the harvest time, vitamin D2 content under UVB dose 210 kJ/㎡ showed marked difference and in order of June, July, August, October and April. As for the results, the optimum harvest time, drying method before UVB treatment, sample size, UVB dose for the EM contained high vitamin D2 content were June, freeze-drying, whole, and 105 kJ/㎡, respectively.
목 적: 좌측 유방암 환자 치료 시 4D-CT를 이용하여 기존에 호흡이 고려되지 않았던 치료와 비교하여 심장의 체적 변화와 호흡에 따른 심장의 선량 차이를 알아보고자 한다. 대상 및 방법: 2012년 3월부터 5월까지 본원에 내원한 2문 접선조사로 치료받은 좌측 유방암 환자 10명을 대상으로 하였다. 좌측 유방이 충분히 포함될 수 있도록 FOV (Field-of-view)를 설정하고 자유 호흡 상태의 일반 CT (Free-Breathing CT)와 4D-CT를 통해 영상을 획득하였다. 선량은 50.4 Gy (1.8 Gy/28 fraction)로 필요에 따라 추가적으로 계획용표적체적(PTV: Planning target volume)에 전자선 1문 조사 9 Gy (1.8 Gy/5 Fraction)를 처방하였고 선량체적히스토그람(Dose Volume Histogram)를 이용하여 심장에 받는 선량과 체적을 비교 분석하였다. 결 과: Free-Breathing CT 영상과 4D-CT 영상의 심장에 받는 선량과 체적을 비교 분석한 결과 4D-CT 영상에서 심장의 체적이 최대 40.5% 크게 나타났으며 또한 심장의 총 흡수선량도 최대, 최소, 평균선량이 최대 8.8%, 27.9%, 37.4% 높게 나타났다. 결 론: 4D-CT를 촬영하여 호흡을 고려한 치료계획이 Free-Breathing CT 영상으로 치료 계획한 것 보다 환자의 심장선량이 높게 조사된 것으로 나타났다. 따라서 심장과 근접한 좌측 유방 방사선치료 시 심장박동에 의한 심장의 체적변화를 고려한 치료계획을 세워야 할 것으로 사료된다.
토모테라피를 이용한 3D-CRT의 임상적 적용 사례를 Helical mode와 Direct mode로 구분하여 치료목적에 따라 주 치료목적 및 보조적 치료 목적으로 적용된 사례에 대해 선량학적 평가를 시행하였고 기존의 라이낙을 이용한 기법과 종양의 발생 부위별 선량학적 분포의 차이를 비교, 분석하였다. 선량 분석은 육종, 뇌척수조사, 유방, 뇌, 췌장, 척추 전이, 상대정맥 증후군, 식도에 토모테라피를 이용한 3차원 입체조형 방사선치료로 치료받는 환자 8명을 대상으로 Helical mode와 Direct mode, 라이낙 기반 치료 기법 사이의 치료 계획 간 비교하였으며, 선량분석은 선량 체적 히스토그램과 등선량 곡선을 이용했다. Direct mode로 치료한 육종, 뇌척수 조사 환자의 경우 체적 길이에 대한 영향을 받지 않아 Junction effect 없는 선량적 결과를 보여주었고, 유방 환자는 D 99%와 D 95%의 선량에서 $49.2{\pm}0.4$ Gy와 $49.9{\pm}0.4$ Gy, V 105%와 V 110%에서 0%로 나타났다. Helical mode로 치료한 뇌, 췌장, 척추 전이 환자에서 dosimetric block을 이용해 정상조직의 선량을 효과적으로 줄일 수 있었고, 상대정맥 증후군 환자는 선량 제한(dose limitation)을 통해 방사선 치료를 재 실시해도 척수선량을 38 Gy이하로 효과적으로 제한할 수 있었다. 식도 환자에서는 기존에 다른 기법과 함께 추가조사 목적으로 사용하여 총 척수선량을 45 Gy 이하로 하고 폐의 V 20%을 20% 이하로 제한할 수 있었다. 토모테라피를 이용한 3차원 입체조형치료는 기존 선형가속기를 이용한 3차원 입체조형 방사선치료에 비해 선량적으로 제한되는 부분을 개선해주고 임상적으로 적절한 선량분포를 구현하는데 효과적인 방법 중의 하나라고 사료된다.
In pediatric thoracic CT, respiratory motion is generally treated as a motion artifact degrading the image quality. Conversely, respiratory motion in the thorax can be used to answer important clinical questions, that cannot be assessed adequately via conventional static thoracic CT, by utilizing four-dimensional (4D) CT. However, clinical experiences of 4D thoracic CT are quite limited. In order to use 4D thoracic CT properly, imagers should understand imaging techniques, radiation dose optimization methods, and normal as well as typical abnormal imaging appearances. In this article, the imaging techniques of pediatric thoracic 4D CT are reviewed with an emphasis on radiation dose. In addition, several clinical applications of pediatric 4D thoracic CT are addressed in various thoracic functional abnormalities, including upper airway obstruction, tracheobronchomalacia, pulmonary air trapping, abnormal diaphragmatic motion, and tumor invasion. One may further explore the clinical usefulness of 4D thoracic CT in free-breathing children, which can enrich one's clinical practice.
최근 시행되고 있는 호흡동조 방사선치료는 환자의 호흡의 주기를 이용하여 일정 주기에만 방사선을 조사하는 최신 방사선치료기술로 4D Computed Tomography와 RPM (Real-time Position Management) 시스템과 같은 호흡 모니터링 시스템의 개발로 환자들에게 시행이 되고 있다. 그러나 이러한 호흡동조 방사선치료에 대한 정도 관리는 아직 체계적으로 수행되고 있지 않으며 특히 환자에게 계획된 방사선치료선량이 환자의 호흡에 따라서 치료계획된 대로 조사되는지에 대한 정도관리에 대한 필요성이 요구되고 있다. 따라서 본 기관에서는 환자의 호흡신호를 사용하여 환자의 움직임을 동일하게 모사할 수 있는 팬텀을 제작하여 호흡동조 방사선치료의 2차원적 선량 분포를 평가할 수 있는 시스템을 구축하였고 특정환자의 호흡신호와 방사선치료계획을 이용하여 검증하였다. 환자의 호흡신호는 LabVIEW 7.0을 이용하여 모사하였고, 자체 제작한 팬텀 및 Kodak EDR2 필름을 사용하여 방사선을 조사한 뒤 gamma index를 사용하여 2차원 선량 분포를 비교 분석하였다. 본 연구에서 개발된 4D 정도관리 시스템을 좀 더 보완하여 호흡동조 방사선치료 과정에 있어서 조사선량의 적정성을 평가할 수 있는 정도관리 시스템으로 사용할 수 있을 것이라 생각된다.

- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
