Epidemiologic study of injury and poisoning occurrence in a rural area (일부 농촌지역 손상 및 중독 발생에 관한 역학적 연구)

  • The mortality and morbidity caused by injury and poisoning has been major public health problem in Korea. This study was carried out to get information indispensable in developing prevention srategies peculiar to korean rural area. In this study 1.499 people of 496 households. who are living in five rural villages of Chooncheon City. Kang-Won Province. were interviewed in accordance with structured questionaire in 1996. The results obtained are summarized as follows: 1. The age-standardized annual incidence rates of injury and poisoning per 100 persons surveyed were 4.2 in males and 1.9 in females. The difference between sex was statistically significant. 2. The places where most of injuries and poisonings occurred were road 66.7% among males. road 31.6% among females. 3. The occurrence of the injuries and poisonings were clustered in intensive farming season in both males and females. 4. The laceration and stab wound were the most frequent nature of injuries in males and the fracture was the most frequent nature of injuries in females. The fracture. sprain were more frequent among males but sprain and contusion were more frequent among females. 5. Incidence rates of injuries and poisoning by cause were higher in traffic accidents among males. though falls among females. The cause specific incidence rate by age was high in traffic accident for 30-59 year age group. and for the aged people older than 60 years. 6. In the management pattern, 78.8% of the injuries and poisoning were received medical care in hospitals and clinics, and the duration of the treatment over 4 weeks in 37.0%. The results obtained indicate that organized community effort is urgently required to prevent injuries and poisonings in rural area.

Factors Influencing on Learning Flow to Nursing Students Self-Leadership and Academic Self-Efficacy in both Online and Offline Lectures (온·오프라인 수업을 병행한 간호대학생의 셀프리더십과 학업적 자기효능감이 학습몰입에 영향을 미치는 요인)

  • The purpose of this study was to identify the degree of self-leadership, academic self-efficacy, and learning flow of nursing college students to took both online and offline lectures, and to identify the factors that influence learning flow. Data were collected by using questionnaires from 152 students who were in 2nd year of the nursing college in G city, from September 7 to September 29, 2020. Data were analyzed by t-test, ANOVA, Pearson's correlation, and stepwise multiple regression using SPSS WIN 25.0 program. As a result of the study, Learning flow according to general characteristics showed significant differences in perceived health status, interpersonal relationship, major satisfaction, and grades. Statistically, learning flow showed a significantly positive correlation with self-leadership and academic self-efficacy. Influencing factors on learning flow were academic self-efficacy(β=.44, p<.001), self-leadership(β=.35, p<.001), and major satisfaction(β=-.14, p=.031) accounting for 57% of the total change. Based on this study, strategies to enhance academic self-efficacy and self-leadership are required to improve the learning flow of nursing students.

The Effects of Metacognition of Nursing Students on Proactivity of Problem Solving in Simulation Integrated with Problem-based Learning: Mediating Effects of Self-leadership (문제중심학습 통합 시뮬레이션교육에서 간호대학생의 메타인지가 문제해결 적극성에 미치는 영향: 셀프리더십의 매개효과를 중심으로)

  • This study is a descriptive research study to investigate the mediating effect of self-leadership in the relationship between metacognition and proactivity of problem solving and the effect of metacognition on proactivity of problem solving of nursing students. 194 fourth-year nursing students who experienced in-school simulation practice at a university were selected as subjects for the study. Data were collected from September to December 2021 using a structured questionnaire. The collected data were analyzed by descriptive statistics, correlations and parametric regression using the SPSS 22.0 program. It was found that metacognition and problem-solving proactiveness and self-leadership had a quantitative correlation, and it was analyzed that metacognition and self-leadership had a significant effect on problem-solving proactiveness. Also, it was found that self-leadership is partially mediated in the relationship between metacognition and problem-solving proactiveness. According to the results of this study, it is necessary to develop a program that can improve the elements of metacognition and self-leadership in order to increase the problem-solving activity of nursing students through problem-centered learning integrated simulation education.

Breast Cancer Trend in Iran from 2000 to 2009 and Prediction till 2020 using a Trend Analysis Method

  • Background: Breast cancer is the most common cancer in women worldwide with a rising incidence rate in most countries. Considering the increase in life expectancy and change in lifestyle of Iranian women, this study investigated the age-adjusted trend of breast cancer incidence during 2000-2009 and predicted its incidence to 2020. Materials and Methods: The 1997 and 2006 census results were used for the projection of female population by age through the cohort-component method over the studied years. Data from the Iranian cancer registration system were used to calculate the annual incidence rate of breast cancer. The age-adjusted incidence rate was then calculated using the WHO standard population distribution. The five-year-age-specific incidence rates were also obtained for each year and future incidence was determined using the trend analysis method. Annual percentage change (APC) was calculated through the joinpoint regression method. Results: The bias adjusted incidence rate of breast cancer increased from 16.7 per 100,000 women in 2000 to 33.6 per 100,000 women in 2009. The incidence of breast cancer had a growing trend in almost all age groups above 30 years over the studied years. In this period, the age groups of 45-65 years had the highest incidence. Investigation into the joinpoint curve showed that the curve had a steep slope with an APC of 23.4% before the first joinpoint, but became milder after this. From 2005 to 2009, the APC was calculated as 2.7%, through which the incidence of breast cancer in 2020 was predicted as 63.0 per 100,000 women. Conclusions: The age-adjusted incidence rate of breast cancer continues to increas in Iranian women. It is predicted that this trend will continue until 2020. Therefore, it seems necessary to prioritize the prevention, control and care for breast cancer in Iran.

Development and Effectiveness of a Smoking Preventive Program for Elementary Students (초등학생을 위한 흡연예방 프로그램의 개발 및 효과에 관한 연구)

  • The purpose of this study were to develop a smoking preventive education program for elementary students and evaluate it's effectiveness. This study was a quasi experimental study under the nonequivalent control group with pretest-posttest design. The subjects of this study were 62 who are attending elementary school(31 for each group), 2 different district elementary school. The subjects were matched by grade, similar in anti-smoking educational background of smoking, as well as their residence and income level of their families. The instruments used in this study was 18 criterion referenced test items modeled by Dick & Carey that were developed by researchers for evaluating the subjects' knowledge and attitude about smoking. A pretest was administered a week before treatment The program given to the experimental group is composed of the texts explaining the poisonous substances in tobacco, social and cultural harmfulness of smoking to the body and psychology, indirect smoking, smoking of pregnant women, motives of smoking, refusal skills of smoking; and for the subjects' understanding and the better results of study - pictures, role play, discussion, text through computer based multi-media, puzzle searching for hidden pictures, cross-word puzzle, and finally compensation. The data were collected for 50 days form mid- September to the end of October in the year of 2000, composed of formative evaluation, pre-test and summative evaluation via 2 sessions. Accordingly, the collected data were analysed by t-test, paired t-test, repeated measure ANOVA by the SAS program. This research summarize the findings as follows; 1. There was a significant difference in knowledge between the experimental group(after 1 wks t=10.4680, p=.0001; after 4 wks t= 9.310, p=.0001) and control group(after 1 wks t=0.0420, p= .9669; after 4 wks t= -0.378 p=.7079) in between the results of 1 and 4 week after education in summative evaluation (F=27.45, P=.0001). 2. There was non statistical significant difference in attitude between the experimental group (after 1 wks t=1.2292, p=0.2286 ; after 4 wks t=1.330, p=0.1935) and control group (after 1 wks t=0.1819, p=0.8569 ; after 4 wks t=0.2970, p=0.7685) in between the results of 1 and 4 week after education in summative evaluation(F=0.71, P=0.494). To sum up, the statistics of conclusive analysis evaluative for the children under school age of the 'knowledge acquisition' about smoking harmfulness. On the other hand, as there was already sound attitude about smoking, the evaluation of attitude was non significant difference between control group and experimental group, just there was partially significant difference.

Prevention and Overcoming Strategies for Taeoom in the Nursing Workplace: Based on the P-S-O-R Framework (간호업무 현장에서의 태움 예방 및 극복방안: P-S-O-R 프레임워크를 기반으로)

  • Recently, the high turnover rate of nursing staff and the problems caused by increased job stress have been highlighted as social issues, and the problem of 'Taeoom' in nursing organizations has received increasing attention. Therefore, the purpose of this study is to propose a solution to the Taeoom problem, including bullying in the nursing work environment, as there is an urgent need to find a solution to prevent and overcome this problem. For this purpose, based on the S-O-R framework and previous studies, job stress and turnover intention were derived as outcome variables of Taeoom and communication competence as an antecedent factor, and a research model was constructed with the expectation that mindfulness and social support would serve as moderating variables to help overcome this problem. Data were collected through a survey of 300 nurses who had experienced Taeoom within the past year, and the hypotheses were tested using a structural equation model. The results revealed that the higher the communication competence of nurses, the less they perceived the damage of Taeoom, and that the damage caused by Taeoom leads to turnover intention through high job stress. In addition, mindfulness and social support significantly attenuated the positive effects of burnout on job stress and job stress on turnover intention, respectively. The significance of this study is that it proposed an extended P-S-O-R framework by adding a prevention stage to the existing S-O-R framework, and further tested the moderating effects of mindfulness and social support variables. It is expected that the findings of this study will provide concrete guidelines to prevent and overcome the Taeoom problem that can be applied in practice.

Burden of Disease Attributable to Water-related Diseases in Korea (수질관련 질환에 의한 한국인의 질병부담)

  • Objectives: This study was aimed at re-assessing the environmental burden of disease attributable to waterrelated diseases using available local data from Korea. Methods: The general methods and the operational definitions for water, sanitation and hygiene applied to the study were based on an environmental burden of disease study conducted by WHO. Eleven water-related diseases were selected. The attributable fraction for diarrhea was calculated by assessing local exposure levels to drinking water, sanitation and hygiene according the scenario-based approach. The attributable fractions for the other ten diseases were derived from the results of the environmental burden of diseases study. The attributable DALYs were measured by using the attributable fractions and local health statistics. Results: The total environmental burden of disease attributable to water, sanitation and hygiene for Korea was 0.9210 DALY per 1000 capitals. Of the total burden of disease, the attributable burden of diarrhea was 0.8863 (96.1%), the attributable burden of malaria and malnutrition was 0.0236 and 0.0063 DALY per 1000 capitals, respectively. There was little burden of disease measured for other diseases. Conclusions: This study is meaningful in re-assessing the environmental burden of disease using available local exposure data and health statistics. Quantitative analysis of the environmental risk factors and a health impact assessment would be helpful to prioritize health policies or interventions in the future.

Follow-up of thyroid ultrasonography in patients with hemodialysis (혈액투석 환자에서 갑상선 초음파의 추적)

  • Background: Chronic kidney disease is considered a risk factor for thyroid nodules as well as thyroid dysfunction such as hypothyroidism. Among patients on hemodialysis, we assessed the size of thyroid nodule and goiter at baseline and 1 year later with ultrasonography. Methods: We prospectively selected 47 patients with hemodialysis at January 2012 and reviewed their medical records. We checked goiter and thyroid nodules at January 2012 and December 2012. Results: In the hemodialysis patients (n=47), 24 patients (51.1%) had thyroid nodules and 33 patients (70.2%) had goiter at baseline. Parathyroid hormone (PTH) was higher in patients with thyroid nodules ($204.4{\pm}102.9$ vs. $129.9{\pm}93.6pg/mL$, p=0.01). Thyroid ultrasonography was conducted in 29 patients after 1 year. The thickness of the thyroid isthmus increased ($2.8{\pm}1.6$ vs. $3.2{\pm}1.9mm$, p=0.003), but the number of nodules did not change ($1.2{\pm}1.9$ vs. $1.4{\pm}2.0$, p=0.109). PTH was associated with the enlargement of thyroid nodules significantly through logistic regression analysis. Conclusion: Thyroid goiter and nodules in hemodialysis patients were more prevalent than in the general population. PTH influenced the production of thyroid nodules in hemodialysis patients. Regular examination with thyroid ultrasonography and thyroid function test should be considered in hemodialysis patients.

A Study on the Occurrences and Causes of Accidents in Lower Grade Elementary School Children (초등학교 저학년 아동들의 안전사고 발생 실태 및 관련요인 분석)

  • Accidents involving children are an important cause of death and disability. They also have enomorous financial implications. In order to prevent childhood accidents, research and education for safety should be strengthened. The purpose of this study was to determine how often young children have accidents and what factors affect the accident rate. The sample consisted of 771 children who were in the second, third and forth grades of two elementary schools located in Kyung-gi Province. One school had students from middle class families living in apartment complexes and the other, students from lower income families mainly living in single houses. The questionnaires included items on the occurance of accidents and the parents' attitudes regarding accidents during the academic year from March 1997 to February 1998. The Questionnaires were distributed to conventiently selected students to be compeleted by their parents and collected during the period of May 28, to June 6, 1998. The data were analyzed using SAS PC statistical package. The results of the study are as follows ; 1. Of 771 student subjects, 393 had 887 accidental injuries during the study period. 2. The month, the day and the time with the highest accident rate were May, Sunday, and between 1 and 4 p. m. each. 3. In the analysis of the location where the injury took place, the most frequent place was on around their homes followed by school and, then, inside the home 4. Most of the accidents were caused by carelessness on the part of the children and the most frequent type of injury was an abrasion. 5. Children most injured their legs 6. They were treated at home most often and usually emergency treatment was performed by family members with, disinfection being the main type of first aid. Cost of the treatment ranged from 8,000 to 20,000 won in most cases. 7. House type and parents' education level were statistically significant in chi-square analysis. 8. Parents educate their children about traffic safety most frequently followed by fire safety and, then, prevention of violence. 9. Parents think that prevention of violence should be the most important part of injury prevention education both at school and home. 10. To identify factors related to accident occurrence, multiple logistic regression was performed and the main factors were birth order and house type.

The Effects of Emotional Intelligence on the Career Decision-Making Self-Efficacy and Career Decision Levels of the Nursing Students (간호 대학생의 감성지능이 진로결정 자기효능감과 진로결정수준에 미치는 영향)

  • This research is a descriptive study intended to identify the factors involved in the influence of the emotional intelligence of nursing students on their career decision-making self-efficacy and career decision levels. The correlations among the major variables were analyzed by calculating the Pearson's correlation coefficients. To identify the effects of the subjects' general characteristics and emotional intelligence on their career decision-making self-efficacy and career decision levels, a multiple regression analysis was conducted. The analysis results showed that there were positive relationships among emotional intelligence, career decision making and career decision-making self-efficacy. The factors influencing career decision-making self-efficacy were gender (${\beta}=-.155$), motivation to find employment (${\beta}=-.145$), use of emotions (${\beta}=.236$), emotional control (${\beta}=.197$), and understanding of others' emotions (${\beta}=.157$). In addition, the factors affecting the subjects' career decision level were the year level (${\beta}=.273$), motivation to find employment (${\beta}=-.145$), motivation to obtain others' recommendation (${\beta}=-.138$), and emotional control (${\beta}=.223$). Hence, sufficient endorsement from school or home is required with regard to the roles and duties of a nurse, in order to motivate future students to select the nursing degree. Furthermore, it is suggested that emotional intelligence may be improved when the degree program provides a higher proportion of emotional education.
