Objective: To evaluate the effects of facemask therapy, which was anchored from the zygomatic buttresses of the maxilla by using two miniplates, in skeletal Class III patients with maxillary deficiency. Methods: Eighteen skeletal Class III patients (10 girls and 8 boys; mean age, $11.4{\pm}1.28$ years) with maxillary deficiency were treated using miniplate-anchored facemasks, and their outcomes were compared with those of a Class III control group (9 girls and 9 boys; mean age, $10.6{\pm}1.12$ years). Two I-shaped miniplates were placed on the right and left zygomatic buttresses of the maxilla, and a facemask was applied with a 400 g force per side. Intragroup comparisons were made using the Wilcoxon test, and intergroup comparisons were made using the Mann-Whitney U-test (p < 0.05). Results: In the treatment group, the maxilla moved 3.3 mm forward, the mandible showed posterior rotation by $1.5^{\circ}$, and the lower incisors were retroclined after treatment. These results were significantly different from those in the control group (p < 0.05). No significant anterior rotation of the palatal plane was observed after treatment. Moreover, changes in the sagittal positions of the maxillary incisors and molars were similar between the treatment and control groups. Conclusions: Skeletally anchored facemask therapy is an effective method for correcting Class III malocclusions, which also minimizes the undesired dental side effects of conventional methods in the maxilla.

To investigate the relationship between the calcification stages of mandibular canines and the skeletal maturity stage of the hand-wrist in subjects with normal occlusion and Class III malocclusion, hand-wrist radiographs and panoramic radiographs were taken from subjects of normal occlusions(94 males, 88 females) and Class III malocclusions(75 males, 76 females) who had no systemic diseases and no history of orthodontic or prosthodontic treatment. Fishman's method for the skeletal maturity stages of the hand-wrist and Demirijian's method for the calcification stages of mandibular canines were used and analyzed. The results were as follows : 1. In subjects with normal occlusion and Class III malocclusion, skeletal maturity of the hand-wrist and calcification of mandibular canines at various ages occured earlier in females than in males(p<0.05). 2. Comparing the skeletal maturity stages of the hand-wrist and the calcification stages of mandibular canines between subjects with normal occlusion and Class III malocclusion, there were no significant differences between the groups. 3. The correlation coefficient between the calcification stages of mandibular canines and the skeletal maturity stages of the hand-wrist. in subjects with normal occlusion and Class III malocclusion showed a high association(p<0.01). 4. In stage 4 of the skeletal maturity of the hand-wrist, the frequency distribution of calcification G stage among the various calcification stages was highest both in normal occlusion and in subjects with Class III malocclusion. However, there was no significant difference in the frequency distribution of calcification stages between the groups.

Maxillary protraction is the conventional treatment for growing Class III patients with maxillary deficiency, but it has undesirable dental effects. The purpose of this report is to introduce an alternative modality of maxillary protraction in patients with dentoskeletal Class III malocclusion using a modified C-palatal plate connected with elastics to a face mask. This method improved skeletal measurements, corrected overjet, and slightly improved the profile. The patients may require definitive treatment in adolescence or adulthood. The modified C-palatal plate enables nonsurgical maxillary advancement with maximal skeletal effects and minimal dental side effects.
Class III malocclusion patients can be approached with many different types of treatment methods, and thus, each patient's problems must be accurately evaluated to allow selection of the best possible treatment method. Cephalometric analysis is an essential part of diagnosis and treatment planning of orthodontic patients, and it would certainly be helpful if reliable cephalometric guidelines could be set. The author divided 482 Class III malocclusion patients(253 males and 229 females) into fourgroups according to different types of treatment methods they have received to correct imbalance between upper and lower jaws: 1) orthopedic appliance (face mask & RPE), 2) camouflage treatment with fixed appliance, 3) surgical-orthodontic treatment, 4) cross-bite correction with removable plates/ functional appliance. Cephalometric values at the time of first clinical examination were compare among the four groups. Cephalometric analysis indicates the following results: 1)the amounts of antero-posterior and vertical skeletal discrepancies and dental compensation were greatest in surgery group 2) SNB, Wits, distance from Nasion Perpendicular Plane to point a facial angle, facial convexity, and APDI were greater in orthopedic appliance group than fixed appliance(camouflage) group, but there was no statistical difference 3) removable plates/ functional appliance group showed least amounts of skeletal discrepancies and dental compensation with statistical significance.

Childrens between the ages of 13 and 15 years, living in Seoul, were examined in order to determine the prevalence and severity of malocclusion in the permanent dentition. This survey encompassed 981 children and an individual chart was prepared for each subject recording an original HMAR score and classification of occlusion according to Angle. Also, subjective evaluation of "treatment needs" was carried out in 581 children. The results were as follows: 1. Of the 981 children in this survey, 12,1 percent showed excellent occlusion. (0 point) 2. The 14 percent of the children who had a score of 24 and above all appeared to belong to the "treatment highly desirable" or "treatment mandatory" category. 3. The incorporation of SAR (Supplementary Assessment Record) into the HMAR can provide more sensitive method for evaluating severity of malocclusion. 4. According to Angle's classification 77.4 percent of all malocclusion belonged to Class I, 1.3 percent to Class II, Division 1, 0.9 percent to Class II, Division 2, and 11.3 percent to Class III. 5. The mean HMAR score for Class I was significantly lower than for either Class II, Division 1 or Class III. (P < 0.05) 6. A close relationship was found between the degree of "treatment needs" and the value obtained by the HMAR scoring. (chi-square test, p < 0.05) The differences between the mean HMAR scores of the various subjectively defined categories were statistically significant. (P < 0.001)
This study was undertaken to investigate soft tissue profile changes by orthodontic treatment in female patients. Traditional cephalometric appraisal yields data of dubious scientific value, the soft tissue profile forms were evaluated by finite element method. The subject was divided into three groups according to Angle's classification and each group was composed of 25 female patients averaged aged 12-14 years at the start of treatment. The changes in soft tissue form were evaluated by computing the degree of distortion in each triangle after treatment compared with the triangle before treatment. The conclusions were as follows; 1. The soft tissue profile forms were evaluated by finite element method and independent evaluation of each element by local changes was possible. 2. Maximum and minimum principal strains showed marked variability depending on the particular finite element and each group and Class II, III sample was greater than Class I sample. 3. Soft tissue size changes as a result of orthodontic treatment was not related to those of shape. 4. Soft tissue changes by orthodontic treatment were variable in individual patient, and were not related to Angle's classification.
From October 1993 to April 1997, 37 patients(30 male and 7 female patients), ranging in age from 23 to 73 years, were operated for annuloaortic ectasia with or without aortic dissection. Four patients were in New York Heart Association Class I, 10 in Class II, 20 in Class III, and 3 in Class IV. In cases of annuloaortic ectasia with aortic dissection, the surgical treatment in all cases consisted of total replacement of the ascending aorta with composite graft and reimplantation of the coronary arteries to the tube graft. The postoperative complication, as postoperative bleeding or LV dysfunction, was 18%(7 cases) and late mortality for entire group was 2.7%(one death). The period of follow-up ranged from 1 month to 36 months(average 9.6 months). In conclusion, Bentall operation for annuloaortic ectasia with or without aortic dissection is reliable method with low mortality and excellent short-term results.
In accordance with the Watershed Sewer System Maintenance Plan enforced on February 2, 2013, the different compliance concentration of effluent limit be applied to effluent discharged from public sewage treatment works(PSTWs) in each watershed on the basis of water quality thereof. With the introduction of watershed sewer system, it is necessary to set the compliance concentration of effluent limit for PSTWs situated in the watershed, by region and PSTW size, to achieve water quality criteria for regional watersheds or target water quality under TMDL program. Watershed Environmental Agencies establish the Watershed Sewer System Maintenance Plan and set the compliance concentrations of effluent limit for PSTWs under the plan. The agencies plan to apply tougher effluent BOD concentration limits in Class I to IV areas. Effluent BOD concentration limits will be toughened from 5~10 mg/L to 3 mg/L in class II~III areas, from 10mg/L to 5mg/L in class IV areas. Uniform application of effluent BOD concentration limits to PSTWs in the watershed sewer system need to be complemented considering type of sewage treatment technology employed and watershed characteristics. Therefore, this study presents method to determine the compliance concentration of effluent limit from PSTWs in the watershed.
From April, 1981, to April, 1990, 20 male and 7 female patients ranging in age from 17 to 63, were operated on for aortic insufficiency with an aneurysm of the ascending aorta. Ten patients were in New York Heart Association functional class II, 7 in class III, and ten in class IV. The surgical treatment in all cases consisted of total replacement of the ascending aorta with composite graft containing a prosthetic aortic valve and reimplantation of the coronary arteries by an intermediate tube graft. In 15 patients an uncomplicated annulo-aortic ectasia existed, and in 12 an aortic dissection; three of the latter group were operated during the acute phase. 17 patients showed typical Marfan syndrome, and 3 patients showed severe ascending aortic aneurysm secondary to the aortic valve disease. The overall operative mortality was 7%[2 deaths]. Those 2 deaths occurred following emergency operation due to associated aortic dissection, but no death during elective operation. All survivors have been followed-up during a period ranging 1 to 108 month[average 34 months]. There was no late mortality. Among the survivors, clinical improvement is readily apparent[2,3 in class I, 2 in class II ]. In conclusion, the treatment of aortic insufficiency associated with an aneurysm of the ascending aorta by insertion of a composite graft and reimplantation of the coronary arteries through an intermediate Dacron tube is a reliable method with low mortality and excellent results.
Since malocclusion affects a large segment of the population, it is by definition a public health problem. The etiology ana treatment of malocclusions have been studied by clinicians; however epidemioloic aspect of tile problem have been neglected. This study was undertaken using Angle's classification to obtain and to evaluate epidemiologic data on the prevalence of malocclusion in a group of 2,378 Yonsei University students, 17 to 23 years of age. All freshmen were selected, except for those students receiving orthodontic treatment and those few with too many missing teeth which prohibits classification by Angle's method. The following results were obtained: 1) Almost $91\%$ of students had malocclusion of the teeth severe enough to require correction. 2) There was a statistically significant difference in malocclusion between males and females($93.66\%$ malocclusion in males, $79.13\%$ malocclusioa in females). 3) Crowding was most pravalent in class I malocclusion. 4) There appeared to be a specific association between the number of lost first molars and Angle's classification. 5) In this study, more class II, Div.2 malocclusion appeared than in Massier's and Frankel's study of Caucasians, which used similar criteria. Class III malocclusion was more prevalent than normal occlusion in the Korean students studied, but in Caucasians' normal occlusion was more prevalent.
