Clinical Application of Functional MRI : Motor Cortex Activities by Acupuncture

  • We report a preliminary fMRI evidence of modulation of somatomotor areas by acupuncture in GB34 acupoint. GB34, located in the back of the knee, is known to be effective in recovering motor function after stroke. Using 3T MR scanner, functional MR imaging was performed in five normal volunteers in two stimulation paradigms; acupuncture manipulation on GB34 and sham points. Group analysis form five individuals showed that bilateral sensorimotor areas (BA 3,4,6 and 7) showed stimulation related BOLD signal contrast of approximately 6% whereas very few areas were activated when sham stimulation is given. Our results suggest that acupuncture fMRI study can be safely conducted in 3T environment and stimulation in GB34 modulate the cortical activities of the somatomotor area in human.

MR머리 영상의 뇌 경계선 추출 및 디렉트 볼륨 렌더링 (Extraction of Brain Boundary and Direct Volume Rendering of MRI Human Head Data)

  • 본 논문은 MR 머리 영상 데이타를 디렉트 볼륨 렌더링하는 방법을 제안한다. MR 영상을 가시화하기 위해서는 서피스 렌더링을 많이 사용하나 이 방법은 면을 추출하는 과정에서 면 내부의 정보를 잃어버린다. 디렉트 볼륨 렌더링은 면 내부의 정보를 추출 할 수 있으나 데이타의 특성상 MR 머리 영상 데이타에 이 방법을 적용하기가 쉽지 않다. 이 논문에서는 MR 머리 영상 데이타를 뇌와 뇌 이외의 구성 요소로 분할한 다음에 뇌 복셀값을 증가시키고 원래의 영상과 다시 결합시켜 디렉트 볼륨 렌더링을 시도하였다. 뇌 경계선은 히스토그램 경계값, 모포로지 연산, 스네이크 알고리즘(snakes algorithm)을 이용하여 추출하였다. 추출된 뇌 경계선는 육안으로 추출한 것의 91~95%의 유사도를 보인다. 제안된 디렉트 볼륨 렌더링은 뇌와 뇌 이외의 구성 요소를 동시에 3차원 가시화하였다.

Brain MRI 검사 시 치아 임플란트 시술유무와 RF Pulse 세기에 따른 인체 영향에 관한 연구: XFDTD 프로그램을 이용 (Study on the Human Influence according to RF Pulse Intensity by use Dental Implant on BRAIN MRI: Using the XFDTD Program)

  • Brain MRI 검사에서는 영상을 얻기 위해 RF Pulse를 인체에 조사하게 되는데 이때 조사된 RF Pulse 에너지의 상당 부분은 우리 몸에 그대로 흡수되게 되고 이로 인해 인체 온도가 상승하게 되는데 노출 정도에 따라 인체에 영향을 주게 된다. 이러한 RF Pulse 에너지는 MRI 장비의 발전으로 인해 자기장의 세기가 1.5 Tesla에서 3 Tesla, 3 Tesla에서 7 Tesla로 높아지는 시점에서 자기장의 세기 변화에 따른 두부 전체의 SAR와 온도의 변화를 알아보고 위의 결과가 치아 임플란트 사용 여부에 따른 결과의 차이를 알아보기 위해 연구를 시작하게 되었다. 실험은 인체 두부 모델에 1.5 Tesla MRI 장비에서 발생되는 64 MHz RF Pulse 주파수, 3.0 Tesla MRI 장비에서 발생되는 128 MHz RF Pulse 주파수, 7 Tesla MRI 장비에서 발생되는 298 MHz RF Pulse 주파수를 조사하고 위의 실험을 치아 임플란트를 사용하였을 때와 사용하지 않았을 때로 나누어 XFDTD 프로그램을 사용하여 머리 주변의 SAR와 체온의 변화를 각각 실험하였다. 치아 임플란트를 하지 않았을 때보다 치아 임플란트를 하였을 때 SAR값은 7T의 RF Pulse 주파수 256MHz에서 최대 약 5800배의 차이를 나타냈으며 주파수가 증가할수록 치아 임플란트 사용으로 인한 SAR값의 차이는 크게 나타났다. 치아 임플란트를 하지 않았을 때보다 치아 임플란트를 하였을 때 두부전체의 온도변화는 2배에서 최대 4배 가까운 온도 상승을 나타내었다. 또한 RF Pulse 주파수가 증가할수록 SAR값은 증가하지만, 두부 전체의 온도는 감소하였는데 이는 주파수가 증가할수록 파장이 작을수록 인체표면에 흡수되는 양이 많아지기 때문이다. 향후 치아 임플란트 유무와 RF Pulse 주파수의 세기 변화로 인한 이상 반응 요인에 대한 연구를 통해 인체의 생리적 연구와 생화학적 연구 등 인체에 대한 영향과 관련하여 연구가 필요하다고 하겠다.

자기공명영상시스템에서의 의료용 리드선의 전자기적 호환 연구 (Electromagnetic Compatibility Study of a Medical Lead for MRI Systems)

  • In the presence of an electrically conducting medical lead, radio frequency (RF) coils in magnetic resonance imaging (MRI) systems may concentrate the RF energy and cause tissue heating near the lead. A novel design for a medical lead to reduce this heating by introducing pins in the lead is presented. Peak 10 g specific absorption rate (SAR) in heart tissue, an indicator of heating, was calculated and compared for both conventional (Medtronic) lead design and our proposed design. Remcom XFdtd software was used to calculate the peak SAR distribution in a realistic model of the human body. The model contained a medical lead that was exposed to RF magnetic fields at 64 MHz (1.5 T), 128 MHz (3 T) and 300 MHz (7 T) using a model of an MR birdcage body coil. The proposed design of adding pins to the medical lead can significantly reduce the heating from different MRI systems.

자연과 도시환경의 인체친화성에 대한 신경해부학적 평가: 기능적 자기공명영상법 (Neuro-Anatomical Evaluation of Human Suitability for Rural and Urban Environment by Using fMRI)

  • 본 연구에서는 3.0 Tesla 기능적 자기공명영상법(function magnetic resonance imaging, fMRI)을 이용하여 자연과 도시풍경의 시각 자극에 따른 대뇌중추의 활성화 영역의 차이점을 규명하고, 아울러 대뇌 부위별 활성화 양상의 차이점을 정량적으로 분석하여 자연과 도시의 생활환경을 인체친화성과 관련하여 신경해부학적 측면에서 연구하고자 하였다. 오른손잡이 정상인 27명(평균 $27.3{\pm}3.7$세)을 대상으로 3.0T MRI를 사용하여 기능적 자기공명영상을 얻었다. 대뇌의 활성화를 유도하기 위한 시각 자극 패러다임은 3번의 휴지기와 2번의 활성기로 구성되어 있고, 각각의 휴지기와 활성기 지속시간은 30초와 120초로 하였다. 전체 피험자 27예의 설문결과를 바탕으로 자연풍경을 선호한 26예와 도시풍경을 비선호한 14예를 두 그룹으로 나눈 후 활성화 양상을 비교하였다. 자연과 도시풍경 간의 대뇌 활성화 양상을 비교한 결과, 도시풍경에 비하여 자연풍경을 보았을 때 유의한 수준(p<0.05)에서 활성화를 보인 영역은 뇌섬엽, 중전두회, 설전부, 미상핵, 상두정회, 상후두회, 방추상회, 창백핵이었으며, 도시풍경에서는 하전두회, 해마방회, 중심후회, 상측두회, 편도체, 후대상회 영역에서 우세한 활성화가 관찰되었다. 자연풍경에서는 도시풍경에 비해 주로 긍정적 정서와 관련된 영역에서 활성화가 나타난 반면, 도시풍경에서는 두려움, 공포, 불쾌감을 느낄 때 반응하는 편도체와 같은 부정적 정서와 관련된 영역에서 활성화가 나타났다. 본 연구에서는 3.0T 기능적 자기공명영상법 이용하여 자연과 도시풍경의 시각 자극에 따른 대뇌중추의 활성화 양상의 차이점을 신경해부학적인 측면에서 정성 정량적으로 평가할 수 있었고, 이러한 결과는 인간이 본능적으로 느낄 수 있는 희로애락의 감정에 따른 대뇌 활성화와 관련하여 생태환경의 인체 친화성과 쾌적성에 대한 객관적인 평가 지표로서 응용될 수 있을 것으로 생각된다.

  • Temporomandibular joint disorders (TMD) often show muscular disorder, internal derangement and degenerative change of articular surface of the condyle. It is important to choose adequate diagnostic method for obtaining data for the diagnosis and treatment. Magnetic Resonance Imaging (MRI) has some advantages, because it shows good resolution image for the soft tissue and it is not invasive to the human body. We have performed serial research about the retrodiscal tissue of the TMJ according to the progress of the disease, using T2 weighted image, since 2002. In this study, we applicated resized observation area to compare signal intensity between the retrodiscal tissue of the TMJ and cerebral gray matter, using T2 weighted image.

Physical Modeling of Chemical Exchange Saturation Transfer Imaging

  • Chemical Exchange Saturation Transfer (CEST) imaging is a method to detect solutes based on the chemical exchange of mobile protons with water. The solute protons exchange with three different patterns, which are fast, slow, and intermediate rates. The CEST contrast can be obtained from the exchangeable protons, which are hydroxyl protons, amine protons, and amide protons. The CEST MR imaging is useful to evaluate tumors, strokes, and other diseases. The purpose of this study is to review the mathematical model for CEST imaging and for measurement of the chemical exchange rate, and to measure the chemical exchange rate using a 3T MRI system on several amino acids. We reviewed the mathematical models for the proton exchange. Several physical models are proposed to demonstrate a two-pool, three-pool, and four-pool models. The CEST signals are also evaluated by taking account of the exchange rate, pH and the saturation efficiency. Although researchers have used most commonly in the calculation of CEST asymmetry, a quantitative analysis is also developed by using Lorentzian fitting. The chemical exchange rate was measured in the phantoms made of asparagine (Asn), glutamate (Glu), ${\gamma}-aminobutyric$ acid (GABA), glycine (Gly), and myoinositol (MI). The experiment was performed at a 3T human MRI system with three different acidity conditions (pH 5.6, 6.2, and 7.4) at a concentration of 50 mM. To identify the chemical exchange rate, the "lsqcurvefit" built-in function in MATLAB was used to fit the pseudo-first exchange rate model. The pseudo-first exchange rate of Asn and Gly was increased with decreasing acidity. In the case of GABA, the largest result was observed at pH 6.2. For Glu, the results at pH 5.6 and 6.2 did not show a significant difference, and the results at pH 7.4 were almost zero. For MI, there was no significant difference at pH 5.6 or 7.4, however, the results at pH 6.2 were smaller than at the other pH values. For the experiment at 3T, we were only able to apply 1 s as the maximum saturation duration due to the limitations of the MRI system. The measurement of the chemical exchange rate was limited in a clinical 3T MRI system because of a hardware limitation.

Acupuncture stimulation for motor cortex activities: Evidence from 3T functional MRI study

  • Purpose: To investigate whether or not acupuncture of GB34 produces a significant response of the modulation of somatomotor areas by functional magnetic resonance imaging (fMRI) study. Methods: The acupoint, GB34, located in the back of the knee, is known to be effective in recovering motor function after stroke. Using 3T MRI scanner, functional MR imaging of the whole brain was performed in 12 normal healthy subjects during two stimulation paradigms; acupuncture manipulation on GB 34 and sham points. This study investigates the activation of the motor cortex elicited by a soft and an intensified stimulation of GB 34. Three different paradigms were carried out to detect any possible modulation of the Blood Oxygenation Level Dependent (BOLD) response in the somatomortor area to motor stimulation through acupuncture. Results: Group analysis from seven individuals showed that bilateral sensorimotor areas (BA 3,4,6 and 7) showed stimulation related BOLD signal contrast of approximately 6% whereas very few areas were activated when sham stimulation is given. Conclusions: The present study shows that acupuncture fMRI study can be safely conducted in 3T MRI environment, and acupuncture stimulation in GB34 modulates the cortical activities of the soma- to motor area in human. The present findings may shed light on the CNS mechanism of motor function by acupuncture and form a basis for future investigations of motor modulation circuits in the stroke patients.
