Screening Strabismus of Elementary School (초등학생의 사시에 관한 연구)

  • Screening of strabismus was followed in 1306 healthy elementary school students from 8 to 13 years old. The cover-uncover test with M1N GLASSES was used to screen strabismus for all of students and then we examined abnormal students by the prism cover test. Finally 16 students were strabismus. So the cover-uncover test with MIN GLASSES was a good method to screen strabismus because of easy and simple testing. The number of strabismus was 16 patients who consisted in from 10 to 13 years old, and girls(2.4%) was 3 times more than boys(0.9%). The type of 16 strabismus was only heterophoria which consisted in 8 patients of exophoria, 7 patients of esophoria and 1 patient of hyperphoria. So the rate of exophoria and esophoria was almost same. 3 patients was ametropic amblyopia by testing visual acuity, and all of patients were normal color vision by color testing.

Clinical Findings of Mycoplasma pneumoniae pneumonia under 3 Year-Old Children (3세 이하 Mycoplasma pneumoniae 폐렴환자의 임상적 고찰)

  • Purpose : Mycoplasma pneumoniae pneumonia has been to be developed frequently in school age children and adolescence and hard to see under 3 year-old children. But it seems to be increased in number of patients with Mycoplasma pneumoniae pneumonia under 3-year old in clinical practice in these days. We have aimed to examine the characteristics of clinical findings of Mycoplasma pneumonia under 3 year-old children. Methods : We had performed retrospective review of medical records of 30 patients with Mycoplasmal pneumonia under 3-year old children who admitted to Department of Pediatrics, Kyunghee University Hospital from Jan. 1994 to Dec. 1997. The diagnostic criteriae was Cold agglutinin titer>1:64 or Mycoplasma antibody titer>1:80. Results : Mycoplasmal pneumonia was 30 out of 235 cases(12.7%) of total pneumonia under 3 year old children. Male female ratio was 1.3 : 1 and age distributions were 0~1y : 0, 1~2y : 8, 2~3y : 22 cases. Clinical symptoms and signs were cough(100.0%), sputum(83.3%), fever(80.0%) rhinorrhea(33.3%), vomiting(33.3%), moist rale(86.7%), decreased breathing sound(26.7%), wheezing(20.0%), and pharyngeal injection(30.0%). Thirteen out of 30 cases(43.3%) had unilateral infiltration, 10 cases(33.4%) had bilateral infiltration, 1 case(3.3%) had pleural effusion, and 6 cases(20.0%) had negative findings on chest radiography and there was no cases of atelectasis. On laboratory findings, 6 out of 30 cases(20.0%) had leukocytosis, 1 case(3.3%) had neutrophilia, 10 cases(30.0%) had eosinophilia, 17 cases(56.7%) had increased ESR, and 18 cases(60.6%) had positive CRP. Positive cold agglutinin titers(>1 : 64) were 19 cases(63.3%), and positive mycoplasma antibody(M-ab) titers(>1 : 80) were 27 cases(93.3%). Mycoplasma antibody test was more valuable than cold agglutinin test for the diagnosis of Mycoplasmal pneumonia and there was no correlation between cold agglutinin titer and mycoplasma antibody titer. Mycoplasma-polymerase chain reaction(M-PCR) was done with 13 cases, 12 out of 13 cases(92.3%) were positive. M-PCR test was valuable to the diagnosis of Mycoplasmal pneumonia but it will be needed to further study for their clinical application. Among 30 cases, 5 cases(16.7%) had complications, 3 cases(10.0%) had skin rash, 1 case(3.3%) had pleural effusion, 1 case(3.3%) had arthralgia, but all complications were mild and recovered without residual sequelae. Conclusion : The occurrence of Mycoplasmal pneumonia under 3 year-old children was not rare from this study. Clinical characteristics of Mycoplasmal pneumonia under 3-year old were normal radiologic findings in many cases, low complication rate, mild clinical course, and tend to rapid recovery compared with general manifestations of Mycoplasmal infectionsin children and adolescence. There were likely to be missed patients with Mycoplasmal pneumonia which did not diagnose by conventional serologic tests that had low sensitivity and specificity. We have to pay attention to the Mycoplasmal infection of the young children with pneumonia during epidemic periods of Mycoplasmal infection.

A test of Visual Acuity and Refractive error to Kindergarten's Children (유치원 어린이 나안시력과 굴절이상 측정)

  • Unaided visual acuity was tested by ACP-7 TOPCON chart projector on 376 kindergarteners and objective refraction error was measured by NIDEK ARK-700A auto-refractokeratometer on 554 eyes aged 3 to 5. The results were as follows ; The average unaided visual acuity of children aged 3 was 0.82, aged 4 was 0.90 and aged 5 was 0.92 respectively theerfore children s visual acuity has been gradually developed with their age. The kind of refractive error was 1% for hyperopia, 14% for hyperopic astigmatism, 3% for myopia, 50% for myopic astigmatism, 18% for mixed astigmatism and 14% for emmetropia respectively.

Analysis of Urinary Mass Screening for Elementary, Middle and High School Children Over a 3-year Period(1995-1997) in Seoul (서울지역내 초.중.고 학생들에 실시된 3년 동안의($1995{\sim}1997$) 집단뇨검사 결과 분석)

  • Purpose : The urinary mass screening program for the detection of proteinuria in school aged population has been performed in Seoul since 1981. Systematic evaluation in corporation with the Seoul School Health Center for students with proteinuria identified in the mass screening has been performed from 1987. The results of urinary mass screening up to 1994 was reported. I report here the results of urinary mass screening from 1995 to 1997 and compare them with previous results and attempt to reveal the significance of urinary mass screening. Objects and Methods : In the 3-year period between 1995 and 1997, annually about 460,000 students comprising 3 different age groups; 5th grade of elementary school, 2nd grade of middle school and 2nd grade of high school were chosen, corresponding to the approximate ages of 11, 14, and 17 years, respectively. These subjects accounted for 26% of total school aged children in Seoul. The screening program was carried out in 3 steps. The 1st test was performed with dipstick at school and the 2nd at the Seoul School Health Center. Those students who showed proteinuria in the 1st and 2nd tests were referred to the hospital. Laboratory examinations including renal biopsies were performed to those students with pathologic proteinuria to clarify the incipient renal diseases. Results : 1) The prevalence of asymptomatic proteinuria was 0.28% in the 1st test. It peaked at the group of 14 years old as 0.34%, compared with 0.26% at the group of 11 years old and 0.24% at the group of 17 years old. It reached to 0.26% in male and 0.30% in female. 2) 25 percent of those having proteinuria at the first test were positive at the second test. 3) The proportion of patients with proteinuria by 3rd test were as follows; 25% of transient proteinuria, 55% of orthostatic proteinuria, 6% of constant proteinuria, 12% of proteinuria with hematuria, and 2% of transient proteinuria with isolated hematuria. Pathologic proteinuria were totaled as 20%. The prevalence of renal diseases among the age group of 7-18 years old was estimated to be 1.4 per 10,000. 4) Renal biopsy performed on 38 children with proteinuria at the third test revealed IgA nephropathy in 17(44%), focal segmental glomerusclerosis in 5(13%), minimal change disease in 4(11%), membranoproliferative glomeronephritis in 3(8%), $Henoch-Sch\"{o}nlein$ purpura nephritis in 3(8%), and others in 6(16%). Therefore, the prevalence of IgA nephropathy among the age group of 7-18 years old was estimated to be 0.64 per 10,000. 5) The prevalence of chronic renal failure was estimated to be 5.7 per 1 million of 7 to 18 years age group. Conclusions : 1) The prevalence of proteinuria in the first screening test was 0.28% and finally only 5% of them showed the pathologic proteinuria at the third test. 2) The prevalence of IgA nephropathy and chronic renal failure were 0.63 per 10,000 and 5.7 per 1 million, respectively among school-aged children in Seoul.

The prevalence of temporomandibular disorders in $16\~18$ aged subjects at Yongin, Kyungkido, Korea (경기도 용인지역 $16\~18$세 연령자의 측두하악장애 유병율에 관한 연구)

  • An epidemiological investigation was carried out at Yongin, Kyungkido, South Korea to determine the prevalence of symptoms of temporomandibular disorders and parafunctional habits in adolescent aged 16, 17 and 18 years. 2,098 students(male 507, female 1,591) were randomly selected and investigated with the questionnaire. The results were obtained as follows, 1. The prevalence of symptoms of temporomandibular disorders was $80.9\%$ total, $77.3\%$ for male and $82.0\%$ for female with significance between male and female(P<0.05). 2. The prevalence of symptoms of temporomandibular disorders was gradually increased according to increasing age with significant difference among ages(P<0.001) 3. The most frequent symptom in the temporomandibular disorders is pain on chewing($65.9\%$), and then clicking($50.7\%$), Pain on mouth opening($41.8\%$), and pain on TMJ($36.8\%$) 4. There was little corelationship between clenching habits and symptoms of temporomandibular disorders(r=0.166), and also between bruxing habits and those(r=0.057).

PHYSIOLOGIC DRIFT OF THE MANDIBULAR DENTITION FOLLOWING THE EXTRACTION OF FOUR FIRST PREMOLARS (상, 하악 제1소구치 발치 후 하악치열의 생리적 치아이동에 관한 연구)

  • Retrostpective study of two groups of patients was conducted to evaluate the physiologic drift of the mandibular teeth following the extraction of four first premolars. The concept of physiologic drift, commonly referred to as 'driftodontics', following first premolar extractions has been gaining acceptance in the orthodontic community, the exact nature and amount of drift has not been adequately documented. There were also no guide lines as to when drift should be allowed to now. The purpose of this study was to quantify physiologic drift of the untreated mandibular dentition following extraction of the four first premolars during the early permanent and late permanent dentition stages. The early permanent dentition extraction sample(Group 1) included 26 Patients and the mean age at pretreatment was approximately 13.5 years. The observation period following extraction was approximately 6.96 months. The late permanent dentition extraction sample(Group 2) included 31 patients. The mean age at pretreatment was 21.3 years, followed by a observation period of 7.26 months. During the observation period, except for the extractions, no other mandibular therapy was rendered. Pre-and post-treatment lateral cephalograms and dental casts were analyzed. The obtained results were as follows 1. Group 2 showed marked changes in movements of the mandibular incisors and canines but minimal changes in molars. 2. The amount of changes in movements of the mandibular incisors and canines were significantly greater in Group 1 than in Group 2. The results showed no differences in rates of molar movements between groups. 3. Physiologic drift of the dentition produced desirable changes such as decreased Incisor Irregularity.

A Survey on the Hearing Disturbance of High School Students in Korea (한국고교생(韓國高校生)에 대(對)한 난청실태조사(難聽實態調査))

  • As a link of chain study program of school health, a survey was made up by the screening test with audiometry for hearing disturbance on 18,675 high school students who are mainly aged in 15-19 years from November 5.1969 to October 30. 1970. The results obtained were summerized as follows. According to our criteria as table 3, the rates of the profound, the severe and the moderate who required the appropriate hearing aids were 0.02%, 0.03% and 0.14% respectively:-the cumulative percentage was 0.197. When the marginal, 0.23% should be included the cumulative rate was 0.41%. But there was no-significance by sex and school classes. If we will make the special classes for them one class would be estimated out of 10,000 persons when a class is formed with about 15 persons. Otherwise when we examined that according to each ear of persons, the rates of the profound, the severe and the moderate were 0.17%, 0.22% and 0.33% respectively and their cumulative percentage wag 0.72. There was no significance also by sex and age. By the way, the rate of hearing disturbance in urban high school students tended to lower than rural. And the perceptive disturbance was higher than rural in rate. The conductive disturbance tended to oppose in comparison with the above.

Study about Vaccination of Patients Diagnosed by Antimeasles Antibody in Measles Out break between 2000 and 2001 (2000~2001년 홍역 유행시 홍역 항체 유무로 진단된 환아의 홍역백신 접종 여부에 관한 연구)

  • Purpose : Despite of the appropriate measles vaccination programs, epidemics occur every 2~3 years and especially occurred in large group in late of 2000 and early of 2001. To evaluate the effect of the vaccination, needs for revaccination and to determine the optimal age for revaccination, we examined measles specific IgG and IgM in mealses patients and investigated different antibody appearance according to vaccination history. Methods : Anti-measles antibodies were checked in sera of 201 patients(male : 117, female : 84) that are responsible for Criteria for Disease Control among 298 patients that are suspicious of measles including inpatients and outpatients in Wonju Christian Hospital from June in 2000 to June in 2001. They were checked by immunofluorescent assay. Then we classified them according to sex, month, distribution of age due to vaccination and appearance of measles antibody. Results : The ratio of male and female was 1.4 : 1. The maximum incidence was 38 cases(18.9%) in May in 2001. Incidence was increased from November in 2000 to January in 2001 and decreased in February and March in 2001. Thereafter it was increased from April in 2001 again and decreased from June. There were 93 cases(46.3%) in vaccinated group and 108 cases(53.7%) in unvaccinated group. In the distribution according to age in vaccinated group, there were 54 cases(58.1%) in more than 10 years old, 15 cases(16.0%) between 7 and 10 years old, 12 cases(12.9%) between 15 months and 3 years old, 6 cases (6.5%) between 4 and 6 years old and 6 cases(6.5%) between 6 months and 14 months old. In the distribution according to age in unvaccinated group, there were 88 cases(81.5%) between 6 months and 14 months old, 9 cases(8.3%) between 15 months and 3 years old, 7 cases(6.5%) less than 6 months old, 3 cases(2.8%) more than 10 years old and 1 case(0.9%) between 7 and 10 years old. In the distribution of measles specific IgG and IgM, 78 cas (87.6%) were IgG(+), IgM(+) and 11 cases(12.4%) are IgG(+), IgM(-) in vaccinated group. In unvaccinated group, there were 69 cases(63.9%) of IgG(+), IgM(+) and 39 cases (36.1%) of IgG(-), IgM(+). Con c lu s i on s : We thought that measles incidence was peaked between 6 months and 14 months old in unvaccinated group because of maximum decrement of transplacental matenal antibody and was peaked in more than 10 years old in vaccinated group because of maximum decrement of measles specific IgG. We think that measles revaccination as well as vaccination and especially optimal age for revaccination is very important to prevent measles successfully.

Effect of Bisphenol A, Nonylphenol, Pentachlorophenol on the Proliferation of MCF-1 and PC-3 Cells (Bisphenol A, Nonylphenol, Pentachlorophenol이 MCF-7 및 PC-3 세포 증식에 미치는 영향)

  • In the present study, we have analyzed effects of the endocrine distruptors, such as bisphenol A, nonylphenol and pentachlorophenol, on cell proliferation in the human breast cancer cell line, MCF-7, and the human prostate cancer cell line, PC-3, with MTT method. A dose dependent analysis of the cell proliferation of MCF-7 cells after administration of bisphenol A, nonylphenol and pentachlorophenol revealed a significant induction of cell proliferation. Maximum induction of cell proliferation was observed at concentrations between 10$\^$-7/ and 10$\^$-6/ M. Whereas, these chemicals had little effect on proliferation of PC-3 cells. These results demonstrated that bisphenol A, nonylphenol and pentachlorophenol do not induce proliferation of PC-3 cells but exhibit a significant induction of MCF-7 cell proliferation, suggesting all these chemicals are a estrogen mimic.

A Study on the Stages in the Development of Geographic Concept: The Conception of 'Place' (지리개념의 발달단계에 대한 연구: '장소' 개념을 중심으로)

  • This paper examines the cross-sectional development of children's conception of place. Previous research by Piaget and Weil, Jahoda, Daggs has questioned how young children develop the ablity to comprehend place. Oral interview and graphic test were made vy the children at the age of 5-14, in order to attain information on their knowledge, understanding and feeling about place. These data were used ti indentify developmental stages through cluster analysis. The results suggest that young children's conception of place develops with an identifiable 4 stages. There is no significant difference in development of conception of place between the 2nd year of kindergarten and the 1st year of elementary school, or the between the 5th, 6th year of elementary school and 1st, 2nd year of middle school. Rather, the 4th year of elementary school is an important and crucial truning point in the development of children's conception of place. It is concluded that children's conception of place develops in line with Piaget's general cognitive developmental theory in which the pre-conceptual stage ends at age 7 and the concrete-operation stage starts at age 11 to 12.
