A Comparative Study on Isoflavone Intakes and Blood Lipids between Hypertensive and Normotensive (고혈압 환자와 정상인의 이소플라본 섭취와 혈중 지질상태 비교연구)

  • It has been suggested that isoflavones protect the cardiovascular system, in part by attenuating blood pressure. The purpose of the present research was to compare the isoflavone intake and blood lipids between hypertensive and normotensive and to examine dietary management for prevention of hypertension. Anthropometrical measurements, blood pressures, nutrients and isoflavone intakes using the 24-hour recall method, and serum lipids of 81 hypertensives and 77 normotensives were estimated. The average age, height, weight, and BMI were 58.2 years, l58.5 cm, 64.3 kg, and $25.6kg/m^2$ for the hypertensive and 58.0 years, 159.4 cm, 63.0 kg, and $24.8kg/m^2$ for the normotensive, respectively. Systolic and diastolic blood pressures of the hypertensive were significantly higher than those of the normotensive (p<0.001, p<0.001). The daily food intake of the hypertensive was significantly lower than that of the normotensive (p<0.01). The daily energy intakes of hypertensive and normotesive were 1479.8 kcal and 1590.9 kcal and there was no significant difference between the two groups. Carbohydrate intake of the hypertensive was significantly higher than that of the normotensive (p<0.05). However, daily intakes of plant protein, fiber, ash, calcium, sodium, potassium, and vitamin $B_1$ of the hypertensive were significantly lower than those of the normotensive (p<0.05, p<0.01, p<0.01, p<0.01, p<0.05, p<0.05, p<0.05). The daily intakes of daidzein, genistein, isoflavone of the hypertensive were significantly lower than those of the normotensive (p<0.05, p<0.05, p<0.05). However, isoflavone intakes per 1000 kcal showed no significant difference between the two groups. Serum lipids of the hypertensive and normotensive were l89.6 mg/dL and l87.2 mg/dL for total cholesterol, 157.3 mg/dL and 161.9 mg/dL for triglyceride, 42.9 mg/dL and 43.5 mg/dL for HDL-cholesterol, 115.2 mg/dL and 111.4 mg/dL for LDL-cholesterol, and 3.5 and 3.4 for atherogenic index. In the total subjects, there was a significantly negative correlation between systolic blood pressure and genistein intake (p<0.05). Based on these results, we concluded that the daily intakes of food, energy, and isoflavones of hypertensives were lower than normotensives. Therefore, it should be emphasized that proper dietary management considering these dietary factors for prevention of hypertension.

The Effects of Milk Group Intake to Dietary Diversity Score and Nutrient Adequacy Ratio among Toddler (유아의 우유군의 섭취가 식이의 다양성과 영양소 섭취의 적정도에 미치는 영향)

  • This research was conduced to 1) examine the influence of milk group intake on the dietary, considering dietary diversity and nutrient adequacy, 2) identify which foods truly contribute to the intake of specific nutrients 3) point out the dietary problems of this age group. The subjects were 664(male 357, female 307) healthy toddlers of the ages of 12-36 months, who had neither been taking any medicine-type supplements nor had any diseases that affect the dietary intakes, chosen among the outpatients of university and private hospitals in Seoul and Kyonggi province during Mach-August, 1999. The 24 hour recall method and CAN Pro(food analysis program) were used to analyze the amount of nutrient intake. The grouping of the subjects for the analysis by monthly age was based on the 98 Korean Child growth standard. The results that the milk group intake showed significant negative correlations with the grain group(r=-0.194, p<0.01), the vegetable group(r=-0.115, p<0.01) and the fruit group(r=-0.125, p<0.01). It also showed that the more the milk group intake, the lower the Dietary Diversity Score(r=-0.131, p<0.01), and the more the intake of raw milk, the more dificient the intake of iron(r=-0.211, p<0.01), vitamin A(r=-0.169,p<0.01), vitamin B$_1$(r=-0.078, p<0.05) and vitamin C(r=-0.187, p<0.01). Foods contributory to nutrient intake for of 12-14 months of were mostly infant formula, and the contribution ratio was high. In the age of 15-17 months, the subjects obtained most of the nutrients from raw milk., liquid-type yoghurt, soybean milk, cooked rice, etc. As for iron intake, the age of 21-23 month obtained the most from ionic drinks, and 24-29 months from raw milk, chocolate cakes, ionic drinks, etc. Though the NAR(Nutrient Adequacy Ratio) and MAR(Mean Adequacy Ratio) generally tended to increase accordingly with the DDs, as for nutrients Ca, vitamin A, vitamin B$_1$, and vitamin C the NAR marked the highest when the DDS marked the lowest(due to infant formula). And for iron, even when the DDS was 5 the NAR value was low. Im conclusion, the more the milk group intake, the lower was the dietary diversity. High dependency on infant formula and others from milk group over the age of 12 months proved to result in a decrease of nutritious solid food intake, iron deficiency and a low dietary diversity score, which can lead to an unbalanced diet. Therefore, nutrition education towards mothers/caretakers of toddler is of urgent need, in order to reduce the fluid milk group intake of low nutritional density and to provide various solid foods the children. Comparative Analysis and Evaluation of Dietary Intakes of Koreans by Age Groups: (5) Meal Patterns (서울 및 근교에 거주하는 한국인의 연령별 식생활 비교 및 평가: (5) 끼니별 섭취)

  • This study was performed to investigate the characteristics of meal patterns among Koreans of different age groups. The study subjects consisted of preschool children (PC), elementary school children (EC), high school students (HS), college students (CS), and adults over 30 years old living in Seoul and the surrounding areas. A dietary survey was conducted using 24-hour recall method from 1996-1998 and data were collected from 2392 subjects. The subjects over 30 years old were classified as 30∼49 years, 50∼69 years, and over 70 years. EC, HS, and CS groups consumed less than 20% of their total energy intake from breakfast. PC and EC consumed over 30% of their energy from snacks. In all age groups, the proportion of subjects who skipped a meal was highest for breakfast and lowest for dinner. Especially, 31.4% of the CS group skipped breakfast. Breakfast and snacks provided higher quality meals than the others when each meal was assessed by the index of nutritional quality (INQ). Especially, lunch of the HS group, each meal of the EC group and subjects over 70 years of age had a lower diet quality than the others did. The major snack items for PC, EC, HS, and CS groups were milk, ice cream, and snack cakes, but those for subjects over 30 years of age were fruits. The PC group obtained 35.5% of their energy from snacks, but the proportion of snack energy decreased with increasing age. PC and EC groups obtained a great proportion of calcium, vitamin B2, fat, and cholesterol intake from snacks. Snack foods provided 53.2%, 42.4%, 45.4%, and 24.6% of those nutrients, respectively, for the PC group and 54.2%, 43.5%, 37.4%, and 26.8%, respectively, for the EC group. These results suggested that each age group had specific meal patterns. Especially, breakfast and snack intake seemed to complement overall diet quality. However, more intervention is needed to improve overall dietary nutrition, e.g., efforts to decrease fat and cholesterol proportion and increase breakfast consumption. These dietary characteristics among different age groups present foundation data for the development of methods to resolve the nutritional problems of each age group.

Comparative Analysis of Obesity by $\beta$-II, III, Adrenergic Receptor Gene Polymorphism in Korean Young Females ($\beta$-II, III Adrenergic Receptor 유전자 다형성에 따른 20대 한국여성의 식이 섭취량, 비만도 및 체성분의 비교연구)

  • The purpose of this study was to investigate the obesity and state of dietary intake of 216 young Korean females, and the influence of $\beta$-II, III Adrenergic receptor (AR) gene polymorphism upon obesity and dietary intake. The average weight, height and BMI of the subjects were 160 cm, 54 kg, and 20.9 kg/$m^2$, respectively. The average triceps skinfold thickness, waist circumference, hip circumference and WHR were 21.7mm, 73.1cm, 93.3cm and 0.78, respectively. The results of body composition measurement using bioimpedance method, average body fluid, body protein, mineral mass and body fat were 29.271, 7.22 kg, 6.79 kg and 19.16 kg, respectively. A dietary survey was conducted using 24-hour recall method. Average calorie intake was 1621 ㎉, which is 81% of Korean RDA. We detected 182 (84.3%) Gln27 (QQ) homozygotes and 34 (15.7%) Gln27Glu (QE) heterozygotes for $\beta$-II AR polymorphism. For $\beta$-III AR polymorphism, we detected 163 (75.5%) Trp64 (WW) and 53 (24.5%) Trp 64Arg (WR). The results of comparing of obesity by $\beta$-II AR gene polymorphism, obesity index and BMI of QE type were slightly higher than those of the QQ type. For $\beta$-III AR gene polymorphism, the mean BMI, obesity index, fat mass and percent body fat (%) of the WR type were significantly higher than those of the WW type (p < 0.05). These findings suggest that genetic variability in the human $\beta$-III AR is associated with obesity among young Korean females. We also evaluated the effect of the simultaneous presence of the $\beta$-II AR and $\beta$-III AR polymorphism on obesity. We found that the BMI and obesity index of the mutant type in both $\beta$-II AR and $\beta$-III AR were significantly higher than those of the type that has only one gene mutation or has no mutation (p < 0.05), indicating a synergistic effect of $\beta$-II AR and $\beta$-III AR polymorphism on obesity. No association was found between $\beta$-II Ad or $\beta$-III AR polymorphism and dietary intake.

Assessment of Nutrients Intake and Evaluation of Nutritional Adequacy of Adults Living in Kyungpook Area (경북지역 일부 성인남녀의 영양소 섭취량과 영양섭취의 균형도 평가)

  • To provide the baseline information for establishing community based nutritional service system in the context of health promotion, we conducted nutrition survey for 196 adults (male ; 99, female ; 97) in Kyungpook area. Individual food intake was measured by 24-hour recall method, and then diet quality was evaluated by Index of Nutritional Quality (INQ) and Mean Adequacy Ratio (MAR). Average dietary intake of Ca and Vit A of total subjects were lower than Korean Recommended Dietary Allowances (Ca; 83% of RDA, Vit A; 77% of RDA). On the whole, mean nutrients intake of male subjects were higher than that of female subjects. It also appeared that overall nutrients intake of younger women (20~29 yr) were lower than that of other age group. When we evaluated the nutrients intake of the subjects by INQ, Vit C (=2.39) showed the highest score. Ca (=0.89) and Vit A (=0.84) were revealed to be concerned nutrients in both quantity and quality. There was almost no difference in MAR between men and women (Men ; 0.88, Women ; 0.86). MAR showed significantrly positive correlation with age (p<0.05), meal frequency (p<0.001) and negative correlation with education level (p<0.01) in women, whereas no statistically significant relationship among these variables was observed in men. Age and meal frequency showed significantly positive correlation with most nutrients intakes among women. In conclusion, nutrition education strategies for sound food choice and regular meals need to be developed to improve the nutritional adequacy of female adults in the 20's.

A Study on Relation among Habitual Isoflavone Intake, Blood Pressure, and Serum Lipid Parameters in Korean Men and Women over 20 Years Old (20세 이상 남녀의 일상 식이중 이소플라본 섭취와 혈압, 혈중 지질과의 관련성)

  • There is some evidence that soy isoflavone has beneficial effects on the concentration of blood lipids. In this study, we investigated habitual isoflavone intake levels and the relation among dietary isoflavone intake, blood pressure, and blood lipids of adult men (n = 149) and women (n = 205). Anthropometric measurements including blood pressure, dietary in-take assessment using 24-hour recall method, and biochemical assessment using blood were conducted. The average age, height, weight, and BMI were 54.7 years, 168.5 cm, 67.3 kg, and 24.5 $kg/m^2$ for men and 53.9 years, 153.8 cm, 59.2 kg, and 25.0 $kg/m^2$ for women, respectively. The mean daily intakes of total food and energy were 1219.1 g and 1740.9 kcal for men and 1071.3 g and 1432.6 kcal for women, respectively. The mean daily isoflavones (daidzein + genistein) intake of men and women were 20.0 mg and 14.2 mg, respectively. Blood pressure of the subjects was 128.3/75.5 mmHg for men and 124.1/73.7 mmHg for women. Serum lipids of men and women were 180.2 and 184.9 mg/dL for total cholesterol, 160.8 and 137.6 mg/dL for triglyceride, 41.5 and 44.7 mg/dL for HDL-cholesterol, 106.6 and 112.7 mg/dL for LDL-cholesterol, and 3.5 and 3.3 for atherogenic index, respectively. Serum triglyceride and atherogenic index of men were significantly higher than those of women. In men, isoflavone intake and the level of total serum cholesterol were negatively (p < 0.05) correlated, after adjusted age. In women, there were significant correlations between isoflavone intake and blood pressure (systolic and diastolic), after adjusted age (p < 0.05). Based on these results, we concluded that higher isoflavone intake seemed to be related to a better lipid profile in men and lower blood pressure in women. But more epidemiological studies and controlled clinical trials would help to confirm the optimal amount required for the prevention and treatment of cardiovascular disease.

The Nutrient Intakes and their Relationships with the Use of Diuretics, Symptom Severity and Physical Functioning in Heart Failure Patients (심부전 환자의 이뇨제 사용, 증상의 중증도 및 신체활동능력과 영양소 섭취 상태와의 관련성)

  • Objectives: The purpose of this study was to identify the differences in nutrient intake according to using diuretics, symptom severity and degree of physical functioning in heart failure patients. Methods: A secondary data analysis was conducted by using baseline data of an intervention study for heart failure patients. In this study, 131 heart failure patients were included. Data were collected using medical records, NYHA (New York Heart Association functional classification) class, and 6-minute-walking test and 24-hour diet recall. Data were analyzed using descriptive statistics and Chi-square test by SPSS 21.0. Nutrient intake was assessed using CAN-pro 2.0. Results: Majority of the participants consumed total calorie less than Estimated Energy Requirement (EER) and consumed carbohydrates more than 65% of their total calorie intakes. 24.4% of the participants consumed fat more than 30% of their total calorie intakes and 23.7% consumed saturated fat more than 7% of their total calorie intakes. 100.0% of the participants consumed protein less than 7% of their total calorie intakes and 73.3% of the participants consumed more than recommended intakes of sodium. More than 90.0% of the participants consumed less than adequate intakes of potassium (90.1%) and Vitamin D (91.6%), respectively. 100% and 62.6% of the participants consumed less than Estimated Average Requirement (EAR) of magnesium and Vitamin $B_1$, respectively. Nutrient intakes in heart failure patients were different for potassium intake according to the usage of diuretics. The participants with symptom severity tended to intake protein less properly and the participants walking more than 300.0 m tended to intake sodium improperly high. Conclusions: The findings of this study indicated the need for screening nutrient intakes of heart failure patients. It is necessary to increase the intake of total calories and most nutrients and to restrict sodium intakes among heart failure patients.

Iron Nutritional Status of Infants and Young Children in the Seoul Area

  • The purpose of this study is to evaluate the iron nutritional status by investigating dietary intake and analyzing the hematological iron status indices including serum transferrin receptor (sTfR) in 8 to 28 month old infants md young children taking supplementary foods. The nutrient intake of 60 healthy infants and young children from 8 to 24 months of age was investigated by means of a 24-hour recall method, and the subjects were divided into 2 groups (8- 12 months and 13-28 months) according to age. Venous blood samples from these groups were collected and measured for the following : hemoglobin(Hb), hematocrit(Hct) , mean corpuscular volume (MCV), mean corpuscular hemoglobin, mean corpuscular hemoglobin concentration (MCHC), serum ferritin, serum iron, total iron binding capacity (TIBC), and sTfR. Anemia is defined as hemoglobin < 11g /dl , serum ferritin level < 10ng1m1 for iron deficiency , serum transferring receptor(sTfR) > 4.5mg / 1 for iron deficient erythropoiesis. Total daily calorie intake was 934.6 ${\pm}$ 284.5kcal (98.32% of RDA) on average. Average daily iron intake in infants aged 8 to 12 months was 8.92 ${\pm}$ 3.32mg. The mean daily iron intake in infants aged 13 to 28 months was 7.15 ${\pm}$ 3.35mg (90% of Recommended Dietary Allowance, RDA). Mean values for Hb, Hct sew ferritin and sTfR were 12.10 ${\pm}$ 0.77g141,36.02 ${\pm}$ 2.31%,20.91 ${\pm}$ 11.58ng/m1 and 3.78 ${\pm}$ 1.47mg /1, respectively. In the young children from 13 to 28 months of age, the prevalence of anemia was 5.6%. The prevalence of iron deficiency was 9.5% in those from 8 to 12 months of age, and 27.8% in those from 13 to 28 months of age. The prevalence of iron deficient erythropoiesis was 16.7% in infants aged 8 to 12 months and 44.4% in those aged 13 to 28 months. The prevalence of both serum ferritin level < 10ng/m1 sTfR > 4.5mg/1 was 22% in the young children aged 13 to 28 months. The measureand ment of sTfR may be a promising new tool in diagnosis of iron deficiency in early childhood when the iron deficiency is prevalent. It seems appropriate to emphasize nutritional education and evaluation to promote the iron nutritional status of infants and young children.

A Study on Eating Behavior, Recognition of Body Image, Weight Control Method, Nutritional Status, and Blood Parameters of Obese Children in Iksan City (익산지역 비만 아동의 식습관, 체형 인식, 체중 조절 행동, 영양 섭취 상태와 혈액성상에 관한 연구)

  • The factors for the childhood onset obesity were assessed to provide informations for the nutrition education program targeting the primary school children. The subjects of this study were 529 primary school children in Iksan City, Jeon-buk Province. Children were classified into obese(236) and control(293) groups by the obesity index. Anthropometric measurement dietary intake through 24-hour recall method, blood analysis and questionnaire response including nutritional knowledge and attitude, body image recognition and eating habits were conducted The mean age of subject was 12.0 years. The average body weight(p<0.001), BMI(p<0.001), and obesity index(p<0.001) in obese group were significantly different from those in control group; their values in the former were 26.5 kg/m2 and $37.7\%$ and 43.0 kg, 19.8 kg/m2 and $3.2\%$ in the latter, respectively. The average scores of self-satisfaction were significantly different in two groups (p<0.001), 31.2, and 34.1, respectively. Among the obese subjects, $60.6\%$ were unsatisfied or very unsatisfied with their body image, and $76.5\%$ had experiences of weight control and $67.5\%$ took diet control as a weight control method Among mothers of obese subjects, $89.1\%$ were unsatisfied with their children's body image. $64.4\%$ of obese subjects had obese parents. Average intake of energy were $76.7\%$ of RDA in obese group and $74.9\%$ of RDA in control group. Average intake of cereals(p<0.01), potatoes & starch(p<0.01) and eggs(p<0.01) between obese and control subjects were significantly different; 295.4g, 76.2g, 55.6g in the former and 277.9g, 38.1g, 45.6g in the latter, respectively. The mean serum triglyceride(p<0.05), GPT(p<0.05), hemoglobin(p<0.001) concentration of the obses and control subjects were significantly different; in the former, 103.6mg/dL, 24.6U/L, 13.4g/dL, and in the latter, 93.7mg/dL, 19.9U/L, 13.1g/dL, respectively, The results suggest that practical nutrition education for the prevention of childhood onset obese should be provided to primary school children.

Correlations among Blood Pressures, Dietary Intakes and Other Environmental Factors in Korean Adult Males (성인 남성의 혈압과 식이섭취 실태 및 기타 환경 요인과의 관계에 대한 사례연구)

  • The present survey was designed to investigate correlations among blood pressure, nutritional intakes, and various environmental factors in Korean adult males, and to observe the difference in dietary intakes between hypertensive patients and normotensive subjects. Two hundred forty-three adult men from Seoul and rural areas were interviewed about their food intakes, dietary patterns, food habits, and general environmental factors from December 20, 1982 to February 15, 1983. Their dietary intakes were surveyed by means of 24-hour recall diet method. Dietary score card was used in order to evaluate the quality of routine diet of the subject. Variables of environmental factors and factors related to hypertension were expressed as percentage, and the differences in nutrient intake and food score of each sub-group were examined by means of t-test. Correlation of all the variables were analyzed by Pearsons Correlation Coefficient. The results obtained are summarized as follows: 1. 38% of the subjects were between the ages of 40~49, and about one-half of the subjects were middle or high school graduates. 28% of the subjects earned 210, 000~350, 000 Won per month, and income distribution was almost identical to that of the urban population of Korea. 2. There was more overweight or obese found in the hypertensive patients than in the normotensive subjects. About 40% of the hypertensive patients reported a family history of hypertension, and their knowledge about hypertension was rather poor, but slightly better than the normotensive subjects. The patient group had less adherence to smoking and alcohol compared with the normotensive subjects, but no difference was observed in coffee drinking. The mean value of the blood pressure for the normotensive subjects was 127/86mmHg and 175/109mmHg for the hypertensive group. 3. Nutrient intake for the hypertensive group was lower for most of nutrients than in the healthy group. Hypertensive patients on diet therapy consumed less calories and carbohydrates compared with those who do not follow diet therapy. 4. There was no difference intake of these food groups between patients and the healthy group. Food score was strongly related to consumption of animal protein, saturated fats, Calcium, Iron, Vitamin A, Riboflavin, and Vitamin C, which are prone to be deficient in the typical Korean diet. 5. Blood pressure had a strong positive correlation with age, body weight, and socioeconomic level. In addition, monthly income was positively related with age and education level. 6. Three was a positive relationship between blood pressure and consumption of vegetable protein, and a negative relationship with consumption of Calories, Carbohydrates, Calcium, Iron, Vitamin A, Vitamin B1, Niacin, and Vitamin C. Animal protein intake increased as income and education levels increased, but this tendency was opposite for carbohydrates. In summary, nutritional intakes are strongly correlated with various environmental factors and the level of blood pressure. There was no difference between normotensive and hypertensive subjects with respect to the quality of food consumed except restricting energy and carbohydrate in the patient group.
