한국 성인 남성의 식사패턴과 주관적 스트레스와의 관련성: 2014~2016 국민건강영양조사 (A Study on the Relationship between Dietary Patterns and the Subjective Stress Perception of Korean Adult Men: Based on the Korea National Health and Nutrition Examination Survey, 2014~2016)

  • This study was conducted to identify the major dietary patterns of Korean adult men and to examine the relationship between subjective stress perception and these dietary patterns using data from the 2014~2016 Korean National Health and Nutrition Examination Survey (KNHANES). Of the 23,080 total subjects, adult men between the ages of 19 and 64 who did not take the questionnaire or answer the questions relating to depression, and cases where the daily energy intake was less than 500 kcal or more than 5,000 kcal in the Food Frequency Questionnaire were excluded. This left a total of 3,464 subjects who were included in the final analysis. We performed a factor analysis based on the yearly mean intake frequency of 41 food groups to identify the major dietary patterns. Three major dietary patterns were identified (factor loading >0.3), including the 'Healthy pattern', 'Processed meat pattern', and the 'Alcohol pattern'. The 'Healthy pattern' was characterized by higher intake of beans, tofu, vegetable, fish, and fruits. The 'Processed meat pattern' was characterized by high consumption of processed meats and instant foods. The 'Alcohol pattern' was characterized by a higher intake of alcohol. As a response to the subject's subjective perception of stress, the most answered 'a little' according to age, marital status, occupation, income, residential area, smoking status, drinking status, and exercise status. People whose diets followed the 'Processed meat' and 'Alcohol' patterns had significantly higher scores on subjective stress perception compared to people following the 'Healthy pattern'. To manage subjective stress, it is more effective to maintain health by relieving stress through a healthy method that combines healthy eating and exercise rather than following an unhealthy diet as characterized by the 'processed meat' and 'alcohol' dietary patterns.

농촌 노인의 마을 밥상 개선 프로그램 개발을 위한 도시와 농촌 노인의 식생활 행태 및 영양소 섭취 상태 비교분석 : 2014년 국민건강영양조사 자료를 이용하여 (Comparative analysis of dietary behavior and nutrient intake of elderly in urban and rural areas for development of "Village Lunch Table" program: Based on 2014 Korea National Health and Nutrition Examination Survey data)

  • 본 연구는 제 6기 2차년도 국민건강영양조사 (2014년) 자료를 이용하여 거주 지역에 따른 노인의 식생활 행태와 식품 및 영양소 섭취의 차이를 분석하였다. 대상자는 65세 이상 노인으로 건강 설문, 검진조사, 식품섭취조사에 모두 응답한 1,239명 (남 543명, 여 696명)을 선정하였으며, 거주지역에 따라 대상자를 농촌 노인 (867명)과 도시 노인 (372명)으로 분류하였다. 대상자의 끼니결식 여부, 주당 외식횟수, 영양교육 여부, 식품군별 섭취량, 영양소 섭취량을 분석하였으며, 권장섭취량 또는 충분섭취량 대비 영양소 섭취수준은 2015년 한국인 영양소 섭취기준 (KDRIs)과 비교하여분석하였다. 주요연구결과를요약하면다음과같다. 거주 지역에 따른 대상자의 조사 전날 아침, 점심식사의 결식 여부는 유의적인 차이가 나타나지 않았으나, 저녁식사의 경우 도시 노인의 결식률이 농촌 노인에 비해 유의적으로 높았으며 (p = 0.030), 주당 외식횟수도 도시 노인이 농촌 노인에 비해 유의적으로 높았다 (p < 0.001). 영양교육의 여부는 유의적인 차이가 나타나지 않았으나, 농촌, 도시 노인 모두 5% 미만이었다. 거주 지역에 따른 대상자의 식품군별 섭취량을 연령, 성별, 에너지 섭취량을 보정하여 분석한 결과, 농촌 노인의 경우, 물 (p = 0.028), 과실류 (p = 0.006), 해조류 (p = 0.040), 우유및유제품류 (p = 0.045) 섭취가 도시 노인에 비해 유의적으로 낮았다. 반면에 곡류 섭취는 도시 노인에 비해 유의적으로 높았다 (p = 0.027). 연령, 성별, 에너지 섭취량, 교육 및 소득수준을 보정하여 분석한 결과를 보아도 농촌노인의 물 (p = 0.039)과 과실류 (p= 0.006)의 섭취가 도시노인에 비해 유의적으로 낮았다. 두 지역 노인들의 영양소 섭취량을 비교해 보면, 연령, 성별, 에너지 섭취량을 보정하여 분석한 결과, 농촌 노인의 경우, 다가불포화지방산 (p = 0.025), n-6계 지방산 (p = 0.023), 인 (p = 0.039), 철 (p = 0.035), 비타민A (p = 0.009), 카로틴 (p = 0.013), 니아신(p < 0.001), 비타민 C (p = 0.004) 섭취량이 도시 노인에 비해 유의적으로 낮았다. 교육 및 소득수준까지 모두 보정한 경우에도 농촌 노인의 철 (p = 0.042), 비타민 A (p = 0.023), 카로틴 (p = 0.025), 니아신(p < 0.003), 비타민 C (p = 0.009) 섭취량이 도시 노인보다 유의적으로 낮았다. 이러한 결과는 농촌 노인들이 특히 비타민 C, 비타민 A, 카로틴, 니아신의 섭취량과 관련이 있는 과실류와 우유 및 유제품류의 섭취가 유의적으로 낮은 것과 관련이 있어 보인다. 한편 영양소섭취기준 대비 영양소 섭취수준은 농촌 노인의 경우, 비타민 A (p = 0.016), 비타민 C (p = 0.003) 섭취수준이 도시 노인에 비해 유의적으로 낮았으며, 리보플라빈, 칼륨의 섭취수준은 80% 에 미치지 못하였다. 특히 칼슘 섭취수준은 농촌, 도시 노인 모두 60% 이하였다. 이러한 영양섭취 문제는 특히 농촌 노인의 건강에 부정적인 영향을 미칠 수 있으며, 지역별 영양불균형을 초래할 수 있다. 도시, 농촌 지역의 특성에 따라 대상자들이 식품을 구입하고 이용하는 식생활행태가 다를 것이며, 이러한 식환경 특성은 대상자의 영양소 섭취에 영향을 미친다. 본 연구에서는 이러한 식생활 환경 전부를 반영하지 못한 아쉬운 점이 있으나, 두 지역에서 유의한 차이를 보였던 사회경제지표인 소득 및 교육 수준, 연령, 성별, 에너지 섭취량을 모두 반영해서 살펴보았고 이러한 점을 고려해 볼 때 여전히 농촌 노인의 식생활행태와 영양소 섭취는 도시 노인에 비해 문제가 심각함을 확인할 수 있었다. 따라서 농촌 노인의 식생활을 개선하기 위해서는 우선적으로 대상과 환경특성을 반영한 맞춤형 교육 프로그램의 개발이 필요하며, 식품이나 음식을 직접 제공하는 식생활 개선 프로그램이 마련되고활성화되어야할것이다.

한국 성인의 카페인 섭취 수준이 대사증후군 및 관련 질환과의 관련성 연구 : 2013~2016 국민건강영양조사 자료 활용 (Study on relationship between caffeine intake level and metabolic syndrome and related diseases in Korean adults: 2013 ~ 2016 Korea National Health and Nutrition Examination Survey)

  • 본 연구는 2013 ~ 2016년 국민건강영양조사 자료를 활용하여 19세 이상 성인의 카페인 섭취 수준이 대사증후군 발생에 미치는 영향을 규명하기 위해 실시하였다. 카페인 섭취 수준을 평가하기 위해 2014년 식품의약품안전평가원에서 수행한 주요급원식품의 카페인 함량 분석자료를 활용하여 1인당 카페인 섭취량과 주요급원식품을 통한 카페인 섭취량을 산출한 후 대사증후군 발생의 위험에 미치는 영향을 분석한 결과는 다음과 같다. 만 19세 이상 성인의 하루 평균 카페인 섭취량은 41.97 mg 이었고, 각 군별 평균 카페인 섭취량은 Q1 0 mg, Q2 1.42 mg (0 ~ 3.66 mg), Q3 6.94 mg (3.99 ~ 12.31 mg), Q4 21.65 mg (12.31 ~ 45.81 mg), Q5군 154.41 mg (${\geq}45.81mg$)으로 나타나 상위 20%에 해당하는 대상자들만이 하루 45.81 mg 이상의 카페인을 섭취하였다. 카페인 섭취의 주요 급원식품은 커피, 탄산음료 및 기타음료, 차류의 순이었으며, 이들 식품을 통한 카페인 섭취량은 97% 정도 이었다. 카페인 섭취 수준에 따라 대사적 지표 및 대사증후군에 미치는 영향을 분석한 결과 성, 연령 이외 이들 대사적 지표 발생에 영향을 미칠 수 있는 교육수준, 음주, 흡연, 신체활동, 에너지 및 당 섭취량에 보정 후 카페인 섭취량은 대사증후군, 고혈압, 저 HDL-콜레스테롤혈증 및 복부비만에 영향을 미치지 않았으나, 당뇨병과 고중성지방혈증의 경우 Q1군에 비해 Q3군에서 각각 0.76 (95% CI: 0.63 ~ 0.93), 0.87 (95% CI: 0.77 ~ 0.98)를 보였고, Q4군에서는 각각 0.66 (95% CI: 0.53 ~ 0.82), 0.83 (95% CI: 0.73 ~ 0.94)로 나타나 하루 3.66 ~ 45.81 mg 정도의 카페인 섭취는 당뇨병 및 고중성지방혈증의 낮은 유병률과 관련이 있었다. 본 연구결과 적정수준의 카페인 섭취는 당뇨병과 고중성지방혈증의 낮은 유병률과 관련이 있으므로 건강 유지에 도움이 되는 적정수준의 카페인 섭취량 선정 시 가이드라인으로 활용 될 수 있을 것으로 사료된다.

만성폐쇄성폐질환 유소견 성인의 폐쇄성 기류제한 상태 비인지 영향요인 (Risk Factors for Unawareness of Obstructive Airflow Limitation among Adults with Chronic Obstructive Pulmonary Disease)

  • Purpose: The purpose of the present study is to examine risk factors for unawareness of obstructive airflow limitation among adults with chronic obstructive pulmonary disease. Methods: Secondary data analysis was performed with the data from the 6th Korea National Health and Nutrition Examination Survey (KNHANES; 2013-2014). The data were analyzed with the IBM SPSS 22.0 version using frequency, percentage, odds ratio, and logistic regression. Results: Ninety-eight percent of subject with a pulmonary function test score of $FEV_1/FVC$<0.7 (N=833) did not recognize that their lung function was impaired. The heavy drink, absence of tuberculosis or asthma diagnosis, and no symptom of expelling phlegm were identified as major risk factors for unawareness of airflow limitation. Conclusion: In order to increase awareness of airflow limitation and to prevent the worsening of the condition, the pulmonary function screening test should be provided to community residents including those who do not show symptoms of respiratory illness.

한국 30세 이상 성인의 심혈관계 질환의 위험 요인 (Risk Factors for Cardiovascular Disease in Adults Aged 30 Years and Older)

  • Purpose : Cardiovascular disease is major factor of mortality in worldwide. Previous studies shown that the socioeconomic factors, nutrition factors, health behavior factors, biological factors and co-morbidity are increasing a prevalence of cardiovascular disease. Method : This study examined the risk factors for cardiovascular disease among adults aged 30 years and older using the data from the 2012 to 2014 Korean National Health and Nutrition Examination Survey (KNHANES). The study participants were 7,555 Cardiovascular disease includes hypertension, stroke, angina pactoris, and myocardial infarction. Descriptive statistic and multivariates logistic regression were calculated. Result : The overall prevalence of cardiovascular disease was 31.16% in the participants. Cardiovascular disease was significantly associated with gender, age, income, education, marital status as socioeconomic factors in unadjusted model. After adjusting socioeconomic status variables, past smoker (OR 1.27, 95% CI 1.05-1.55), obesity (OR 7.14, 95% CI 4.21-12.11), skipping a meal (OR 2.79, 95% CI 2.46-3.16), HDL-C (OR 0.99, 95% CI 0.98-0.99) and WC (OR 1.06, 95% CI 1.05-1.07) were associated with cardiovascular disease. Conclusion : The results marked the importance of finding high risk groups and an early management of cardiovascular disease.

Trends in nutrient intakes and consumption while eating-out among Korean adults based on Korea National Health and Nutrition Examination Survey (1998-2012) data

  • BACKGROUND/OBJECTIVES: Eating-out among Korean people has become an important part of modern lifestyle due to tremendous growth of the food service industry and various social and economic changes. This study examined trends in meal patterns and meal sources while eating-out among Korean adults aged 19 years and older. SUBJECTS/METHODS: Data were from the 1998-2012 KNHNES (Korea National Health and Nutrition Examination Survey) by the 24-hour dietary recall method. This study included 55,718 adults aged 19 years and older. For analysis of eating-out frequency, data were categorized by source of meals and serving place. RESULTS: Average frequency of meals consumed away from home increased from 1998 to 2012, although it remained lower than that of meals at home. In addition, male, unmarried, employed, higher educated, and high income individuals more frequently consumed meals away from home. Moreover, sodium intake while eating-out significantly increased from 2,370 mg in 1998 to 2,935 mg in 2012. Lastly, percentage contributions of daily total protein intake, fat intake, and sodium intake from eating-out increased to more than half (53-55%) in 2012 compared with 47-48% in 1998. CONCLUSIONS: As eating-out has grown in popularity, greater recognition of public health and nutritional education aimed at promoting healthy food choices is needed. In addition to developing consumer education for overall healthier eating patterns, individuals who are younger, unmarried, higher educated, and males are especially at risk and require attention.

Plain water intake of Korean adults according to life style, anthropometric and dietary characteristic: the Korea National Health and Nutrition Examination Surveys 2008-2010

  • BACKGROUND/OBJECTIVES: The objective of the study was to provide useful insights into plain water intake of Korean adults according to life style, anthropometric, and dietary characteristics. SUBJECTS/METHODS: The data from the 2008-2010 Korea National Health and Nutrition Examination Survey were used. The subjects were 14,428 aged 20-64 years. Water intake was estimated by asking the question "How much water do you usually consume per day?". Dietary intake was estimated by 24-hour dietary recall. A qualitative food frequency questionnaire including 63 food items was also administered. RESULTS: The mean plain water intake for men and women were 6.3 cup/day and 4.6 cup/day, respectively. Plain water intake increased as lean body mass, waist circumference, and body mass index levels increased, except for percentage of body fat. As energy and alcohol intakes increased, plain water intake increased. As total weight of food intake and total volume of food intake increased, plain water intake increased. Plain water intake increased as consumption of vegetables increased. Plain water intake increased as frequencies of green tea, alcoholic drink, and all beverages were increased in men. Plain water intake increased with increased frequencies of green tea, milk, soy milk, and alcoholic drink and decreased frequencies of coffee and soda in women. CONCLUSIONS: Our results suggest that persons who had a higher waist circumference or lean body mass and women with higher BMI consumed more plain water. The persons eating high quality diet, or the persons who had more vegetables, green tea, milk, soy milk, or alcoholic drink consumed more plain water.

Estimation of curcumin intake in Korea based on the Korea National Health and Nutrition Examination Survey (2008-2012)

  • BACKGROUND/OBJECTIVES: Turmeric and its active component curcumin have received considerable attention due to their many recognized biological activities. Turmeric has been commonly used in food preparation and herbal remedies in South Asia, leading to a high consumption rate of curcumin in this region. However, the amount of curcumin in the Korean diet has not yet been estimated, where turmeric is not a common ingredient. SUBJECTS/METHODS: This study utilized the combined data sets obtained from the Korea National Health and Nutrition Examination Survey conducted from 2008 to 2012 in order to estimate the curcumin intake in the Korean diet. The mean intake of curcumin was estimated from the amount of curcumin-containing foods (curry powder and ready-made curry) consumed using reported curcumin content in commercial turmeric and curry powders. RESULTS: Only 0.06% of Koreans responded that they consumed foods containing curcumin in a given day, and 40% of them were younger than 20 years of age. Curcumin-containing foods were largely prepared at home (72.9%) and a significant proportion (20.4%, nearly twice that of all other foods) was consumed as school and workplace meals. The estimated mean turmeric intake was about 0.47 g/day corresponding to 2.7-14.8 mg curcumin, while the average curry powder consumption was about 16.4 g, which gave rise to curcumin intake in the range of 8.2-95.0 mg among individuals who consumed curcumin. The difference in estimated curcumin intake by using the curcumin content in curry powder and turmeric may reflect that curry powder manufactured in Korea might contain higher amounts of other ingredients such as flour, and an estimation based on the curcumin content in the turmeric might be more acceptable. CONCLUSIONS: Thus, the amount of curcumin that can be obtained from the Korean diet in a day is 2.7-14.8 mg, corresponding to nearly one fourth of the daily curcumin intake in South Asia, although curcumin is rarely consumed in Korea.

흡연 여부에 따른 한국 성인의 식품 및 영양소 섭취와 치주염과의 상관성 (Association of Food and Nutrient Intakes with Periodontitis by Smoking Status among Korean Adults)

  • Periodontal disease is one of the most common chronic inflammatory diseases in the oral cavity, and this is the leading cause of loss of teeth. Studies on the association between diet and periodontal diseases are very limited. The purpose of this study was to investigate the association between food and nutrient intakes and the prevalence of periodontitis. Subjects were 13,391 adults participating in the 2008-2010 Korea Health and Nutrition Examination Survey. Periodontitis was diagnosed by dentists using the Community Periodontal Index. Nutrient intakes were estimated by the 24-hour dietary recall. Consumption frequencies of foods were from the food frequency questionnaire. Subjects were categorized into 'smoking' or 'non-smoking' groups. Multiple logistic regression analysis was applied to determine the association between diet and periodontitis. The proportions of subjects having periodontitis were 26% in the non-smoking group and 37.5% in the smoking group. In the nonsmoking group, intakes of fruits, dairy products, green tea, energy and vitamin C were inversely associated with the prevalence of periodontitis, but fish and coffee intakes were positively associated with the prevalence of periodontitis after adjusting for covariates. In the smoking group, protein and retinol intakes were inversely associated with the prevalence of periodontitis after adjusting for covariates. These results suggest that certain food and nutrient intakes such as fruits, dairy products, green tea, vitamin C, protein, or retinol intakes may affect the prevalence of periodontitis among Korean adults. Further studies are required to confirm these findings in other research settings.

Dietary pattern classifications with nutrient intake and body composition changes in Korean elderly

  • BACKGROUND/OBJECTIVES: The body composition changes in aging increased the risk of metabolic disorder. Recent dietary studies have increasingly focused on the correlations between dietary patterns and chronic diseases to overcome the limitations of traditional single-nutrient studies because nutrients in food have complex relations that interact. SUBJECTS/METHODS: This study was conducted to classify a dietary pattern among Korean elderly using cluster analysis and to explore the relationships between dietary patterns and body composition changes in Korean elderly aged 65 years or older. The study subjects (n=1,435) were individuals who participated in the Korean National Health Examination and Nutrition Survey (KNHANES) in 2011. RESULTS: There were three dietary patterns derived by cluster analysis in this study: 'Traditional Korean' (37.49% of total population), 'Meat and Alcohol' (19.65%) and 'Westernized Korean' (42.86%). The (1) 'Traditional Korean' pattern was characterized by high consumptions of white rice and low protein, low fat, and low milk products, while (2) 'Westernized Korean' pattern ate a Korean-style diet base with various foods such as noodles, bread, eggs and milk, (3) 'Meat and Alcohol' pattern had high consumptions of meat and alcohol. In body composition changes, compared with the 'Traditional Korean' pattern, the 'Meat & alcohol' pattern was associated with a 50% increased risk of having elevated BMI ($kg/m^2$), 'Westernized Korean' pattern was associated with a 74% increased abnormality of ASM/Wt (kg) by logistics analysis. Most of the Korean adult population continues to follow ether a traditional Korean having beneficial effects for successful aging. However, the 'Traditional Korean' pattern showed low protein intake (0.7 g/kg), calcium intake, and vitamin D intake as well as low of appendicular skeletal muscle mass (ASM (kg)) among 3 groups. CONCLUSIONS: Considering the low ASM, consumption of protein, calcium and vitamin D should be increased for Korean elderly health body composition.
