CLEANLINESS AND WALL MOPHOLOGY OF ULTRASONIC ROOT-END RETROGRADE CAVITY (초음파로 형성된 치근단 역충전와동의 정화도 및 와벽형태)

  • The purposes of this study were to evaluate the degree of cavity cleanliness and to observe cavity wall morphology when root-end retrograde cavity preparation was done with ultrasonics. Root resections were done on 20 extracted human maxillary central incisors after canal filling with gutta-percha, and retrocavities were prepared using a slow-speed round bur as a control, and stainless steel ultrasonic tips of power settings of 2 and 6 ($Miniendo^{TM}$, EIE, SA, USA) as experimentals. The degree of the remaining cavity debris and smear layer, and wall morphology were evaluated under the scanning electron microscope. The results were as follows : Cavity prepared with ultrasonics of either power setting showed significantly less smear layer than did slow-speed preparations (p<0.01). However, there was no significant difference in canal debris (p<0.05). Cavity prepared with ultrasonics showed hatcheted appearance of wall, while slow-speed preparation showed relatively plain one.

  • Molar-incisor malformation (MIM) is characterized by malformation in the root with a normal crown. While MIM mostly occurs in the permanent first molar, it has also been reported in the maxillary central incisor and the primary second molar (PSM), but anatomical analysis of the primary teeth with MIM has not been studied to date. In this case report, a patient with MIM was reported, and an extracted PSM with MIM was analyzed with micro computed tomography (CT). A cervical constriction morphology of the cementoenamel junction (CEJ) can be observed in extracted PSM. In micro CT analysis, characteristics such a mineralized plate (cervical mineralized diaphragm) in the CEJ area, complex root canal morphologies, a calcified mass inside the pulp chamber, and constricted pulp chamber of crown portion were observed.

EFFECT OF SMEAR LAYER ON THE APICAL LEAKAGE OF WARM VERTICAL CANAL FILLING (도말층이 가온 수직가업 근관 충전후 치근단 누출에 미치는 영향)

  • The purposes of this study were to observe the difference in the root canal wall after hand instrumentation or engine-driven Ni-Ti instrumentation under the scanning electron microscope, and to evaluate the apical leakage provided by continuous wave of canal filling technique with or without root canal sealer and smear layer. Twenty recently extracted human maxillary anterior teeth were instrumented with K-type files or engine-driven Ni-Ti files, Quantec series 2000$^{TM}$ and irrigated with 5.25% NaOCl alone or 15% EDT A and final flush of 5.25% NaOCl. After the instrumentation and flushing, teeth were split in half with a knife and a mallet. They were then examined with a scanning electron microscope Forty-four recently extracted human maxillary anterior teeth were divided into four groups with and without smear layer and then warm vertical canal filling using System-B with or without sealer. The extent of leakage was scored after immersion in India ink for 1 week. The results were as follows: 1. No significant difference of smear layer was observed between K-type file-instrumented group and engine-driven Ni-Ti file-instrumented group. 2. A group without smear layer showed significantly less apical leakage than a group with smear layer when sealer was used for the canal filling (p<0.01). 3. There was no significant difference between a group without sealer and smear layer and a group without sealer and with smear layer (p<0.01). 4. In groups without smear layer, a group with sealer showed significantly less apical leakage than a group without sealer (p<0.01). 5. When root canals were irrigated with NaOCl alone, a group with sealer showed significantly less apical leakage than a group without sealer (p<0.01).

THE EFFECT OF SMEAR LAYER TREATMENT ON THE MICROLEAKAGE (Smear layer 처리에 따른 미세누출에 대한 연구)

  • The purpose of this study was to compare the sealing ability of root canal obturation with or without the treatment of smear layer. Eighty extracted human teeth with one canal were selected Instrumentation was performed with crown-down technique. After instrumentation, root canals of the NaOCl group and NaOCl-6 group were irrigated with 3% NaOCl. EDTA group and EDTA-6 group were irrigated with 17% EDTA. Then all teeth were obturated using continuous wane obturation technique NaOCl group and EDTA group were immersed in methylene blue solution for 84hours. NaOCl-6 group and EDTA-6 group were immersed in methylene blue solution for 6months. The teeth were sectioned at 1.5 mn (Level 1), 3.0 mm (Level 2) and 4.5 mm (Level 3) from the root apex. The length of dye-penetrated inter-face and the circumferential length of canal at each level were measured using Sigma-Scan Pro 5.0. 1. The mean leakage ratio was decreased cervically. 2. NaOCl group showed higher mean leakage ratio than EDTA group at each level. But there was significant difference at level 1 only (p < 0.05). 3. NaOCl-6 group showed higher mean leakage ratio than EDTA-6 group at each level. But there was significant difference at level 1 only (p < 0.05). 4. NaOCl-6 group showed higher mean leakage ratio than NaOCl group at each level. But there was significant difference at level 1 only (p < 0.05). 5. EDTA-6 group showed higher mean leakage ratio than EDTA group at each level. But there was no significant difference. 6. In NaOCl group and NaOCl-6 group, scanning electron micrographs of tooth sections generally covered with smear layer. In EDTA group and EDTA-6 group, tooth sections showing the penetration of sealers to opened dentinal tubules. The results suggest that removal of smear layer was effective to reduce the apical microleakage of the root canal.

  • 저자는 임상에서 널리 사용되는 근관충전재료인 산화아연유지놀, N2, MN2 및 Triozinc Paste의 살균효과를 비교하기 위하여 한천배양기 및 혈액가한천배양기상에서 백색포도상구균 및 α-용혈성연질상구균의 두균주를 사용, 실험하여 다음과 같은 결과를 얻었다. 1) 백색포도상구균을 심은 한천배양기상에서 비건조도판을 사용한 실험군에서의 균성장억제대는 MN2, 6. 83; Triozinc paste, 5. 77; N 2, 5. 61; 산화아연유지놀, 3.49mm였다. 2) 백색포도상구균을 심은 한천배양기상에서 건조도판을 사용한 실험군에서의 균성장억제대는 MN2, 4.40; Triozinc paste, 1.46; 산화아연유지놀, 0.99; ㅜ2, 0.84mm였다. 3) 백색포도상구균을 심은 혈액가한천배양기상에서 건조도판을 사용한 실험군에서의 균성장억제대는 MN 2, 1.15; Triozinc paste, 0.55; 산화아연유지놀, 0.54; N2, 0.43mm였다. 4) α-용혈성연질상구균을 심은 용혈가한천배양기상에서 건조도판을 사용한 실험군에서의 균성장억제대는 MN 2, 2.14; Triozinc paste, 1.00; 산화아연유지놀, 0.75; N 2, 0.43mm였다.

THE EFFECT OF THE CANAL IRRIGANTS ON THE ELECTRONIC WORKING LENGTH DEVICE (전자 근관장측정기 사용에 있어서 근관세척제의 사용이 측정에 미치는 영향에 관한 연구)

  • Electronic working length determination is based on the electrical impedence between the root canal and the periodontal ligament Different canal irrigants with different electric conductivity may effect on the accuracy of the electronic mechanism. 0.9% Saline, 2.5% NaOCl and 3% $H_2O_2$ were evaluated either in wet or in dry condition. The results were as follows. 1. The electrical resistance mearured by Impedence Meter (Model #253, portland, USA) were 1 $K{\Omega}$ in 0.9% Saline, 0.1$K{\Omega}$ in 2.5% NaOCl and 48 $K{\Omega}$ in 3% $H_2O_2$. 2. Saline measured -0.57mm from the actual canal length and had significant difference. When dried with paper points, however, it measured -0.25mm demonstrating no statistical difference. 3. 2.5% NaOCl measured -4.07mm in wet and -2.26mm in dry condition and both showed significant difference from the actual canal length. 4. 3% $H_2O_2$ measured +0.02mm in wet and -0.09mm in dry canals and both appeared to be within acceptable range for the electronic working length mechanism.

DENS INVAGINATUS IN MAXILLARY LATERAL INCISORS: REPORT OF 2 CASES (상악 측절치의 치내치에 대한 증례보고)

  • Dens invaginatus is a malformation of tooth resulting from an infolding of the enamel epithelium during tooth development. This malformation shows a broad spectrum of morphologic variations. This invagination frequently allows the entry of irritants and microorganism, which usually lead to necrosis of the adjacent pulp tissue and then to periapical or periodontal abscess. Root canal treatment of such tooth is often difficult because of the un usual form and complicated pulpal space. This article reports 2 cases of dens invaginatus in maxillary lateral incisors. The first case was successfully treated with $Ca(OH)_2$. In the second case, involved tooth was extracted and this extracted tooth was observed using the micro-computed tomography.
