The influence of various core designs on stress distribution in the veneered zirconia crown: a finite element analysis study

  • PURPOSE. The purpose of this study was to evaluate various core designs on stress distribution within zirconia crowns. MATERIALS AND METHODS. Three-dimensional finite element models, representing mandibular molars, comprising a prepared tooth, cement layer, zirconia core, and veneer porcelain were designed by computer software. The shoulder (1 mm in width) variations in core were incremental increases of 1 mm, 2 mm and 3 mm in proximal and lingual height, and buccal height respectively. To simulate masticatory force, loads of 280 N were applied from three directions (vertical, at a $45^{\circ}$ angle, and horizontal). To simulate maximum bite force, a load of 700 N was applied vertically to the crowns. Maximum principal stress (MPS) was determined for each model, loading condition, and position. RESULTS. In the maximum bite force simulation test, the MPSs on all crowns observed around the shoulder region and loading points. The compressive stresses were located in the shoulder region of the veneer-zirconia interface and at the occlusal region. In the test simulating masticatory force, the MPS was concentrated around the loading points, and the compressive stresses were located at the 3 mm height lingual shoulder region, when the load was applied horizontally. MPS increased in the shoulder region as the shoulder height increased. CONCLUSION. This study suggested that reinforced shoulder play an essential role in the success of the zirconia restoration, and veneer fracture due to occlusal loading can be prevented by proper core design, such as shoulder.

From diagnosis to treatment of mucopolysaccharidosis type VI: A case report with a novel variant, c.1157C>T (p.Ser386Phe), in ARSB gene

  • Mucopolysaccharidosis type VI (MPS VI) is an autosomal recessive lysosomal disorder caused by the deficiency of arylsulfatase B due to mutations in the ARSB gene. Here, we report the case of a Korean female with a novel variant of MPS VI. A Korean female aged 5 years and 8 months, who is the only child of a healthy non-consanguineous Korean couple, presented at our hospital for severe short stature. She had a medical history of umbilical hernia and recurrent otitis media. Her symptoms included snoring and mouth breathing. Subtle dysmorphic features, including mild coarse face, joint contracture, hepatomegaly, and limited range of joint motion, were identified. Radiography revealed deformities, suggesting skeletal dysplasia. Growth hormone (GH) provocation tests revealed complete GH deficiency. Targeted exome sequencing revealed compound heterozygous mutations in the ARSB genes c.512G>A (p.Gly171Asp; a pathogenic variant inherited from her father) and c.1157C>T (p.Ser386Phe; a novel variant inherited from her mother in familial genetic testing). Quantitative tests revealed increased urine glycosaminoglycan (GAG) levels and decreased enzyme activity of arylsulfatase B. While on enzyme replacement therapy and GH therapy, her height increased drastically; her coarse face, joint contracture, snoring, and obstructive sleep apnea improved; urine GAG decreased; and left ventricular mass index was remarkably decreased. We report a novel variant-c.1157C>T (p.Ser386Phe)-of the ARSB gene in a patient with MPS VI; these findings will expand our knowledge of its clinical spectrum and molecular mechanisms.

구리이온을 함유한 PCB 폐에칭액의 Cross-flow 나노여과 (Cross-flow Nanofiltration of PCB Etching Waste Solution Containing Copper Ion)

  • 본 연구는 인쇄회로기판(PCB) 제조 시 에칭공정에서 발생되는 구리이온($Cu^{+2}$)을 고농도로 함유한 황산 폐에칭액을 NF 막분리법을 사용하여 에칭액 회수와 구리이온 처리를 효율적으로 수행하기 위한 NF 막여과 공정의 운전조건을 설정하기 위한 기본 자료를 확보하는데 있다. 이를 위해 미국 Koch사의 SelRO MPS-34 4040 NF 막을 대상으로 구리이온을 고농도(5~30 g/L)로 함유한 모의 황산 폐에칭액의 cross-flow 나노여과 실험을 수행하여 투과 플럭스와 구리이온의 총괄 배제도를 측정하였다. 이 결과 투과 플럭스는 황산 폐에칭액 내 구리이온의 농도가 증가할수록, 황산 폐에칭액의 pH가 낮을수록 작아졌으며, 그 값은 최소 $4.5L/m^2{\cdot}h$에서 최대 $23L/m^2{\cdot}h$이었다. 황산 폐에칭액 내 구리이온의 총 배제도는 구리이온의 농도가 클수록, 용액의 pH가 낮을수록 그리고 폐에칭액의 순환유량이 작을수록 낮아졌으며, 황산 폐에칭액의 pH가 1 이상인 상태에서 70% 이상의 구리이온 배제가 가능하였다. NF 막을 12개월 동안 황산용액 내에 보관하여도 투과 플러스 와 구리이온 배제도의 유의한 변화가 없어 SelRO MPS-34 막모듈을 강산 조건에서 1년이상 막모듈의 교체 없이 산성 폐에칭액 처리에의 사용이 가능하였다.

국내 6형 뮤코다당증의 임상 양상, 분자유전학적 특징 및 효소치료의 효과에 대한 고찰 (Clinical Features, Molecular Analysis, and Outcome of ERT in Korean Patients with Mucopolysaccharidosis Type VI)

  • 뮤코다당증 6형(Mucopolysaccharidosis type VI)은 ARSB 유전자의 변이로 인하여 dermatan sulfate 대사에 관여하는 arylsulfatase B 효소의 결핍으로 유발되는 상염색체 열성 리소좀 축적 질환이다. 본 연구는 국내에서 뮤코다당증 6형으로 진단된 3명의 환자를 대상으로 하였다 3명의 환자는 최초 진단 시 뮤코다당증 6형의 특징적인 증상인 저신장, 다발성 골형성 부전, 심장 판막 이상 및 각막 혼탁을 보였으며 이후 손목 터널 증후군 및 제한성 폐질환의 양상을 보였다. 모든 환자에서 소변의 GAG (glycosaminoglycan)는 증가되어 있었으며 피부 섬유아세포 또는 백혈구에서 측정한 arylsulfatase B는 모두 감소되어 있었고 중합효소 연쇄반응-염기서열분석법에 의한 ARSB 유전자 검사에서는 c.200T>G (p.Ile67Ser), c.982G>A (p.Gly328Arg), c.571C>T (p.Arg191*), c.1054G>A (p.Asp352Asn)의 이형접합체 돌연변이 및 c.512G>A (p.Gly171Asp) 동형접합체 돌연변이를 발견할 수 있었다. recombinant human Arylsulfatase B (rhASB)를 통한 효소 대체 치료는 2008년부터 국내에서도 허가되었으며 효소대체치료 시행 후 소변 GAG (glyco-saminoglycan)는 감소하였으며 관절 운동 및 지구력은 향상되었다. 그러나 관절 침범 및 각막 혼탁, 심장 및 폐 침범 소견은 호전되지 않았다. 본 연구는 뮤코다당증 6형의 특징적인 증상을 보였으며 생화학적 검사 및 분자유전학적 검사를 통해 뮤코다당증 6형으로 확진된 3명의 환자를 대상으로 시행한 효소대체치료의 효과를 보고하는 바이다.

Surface Deformation Measurement of the 2020 Mw 6.4 Petrinja, Croatia Earthquake Using Sentinel-1 SAR Data

  • By the end of December 2020, an earthquake with Mw about 6.4 hit Sisak-Moslavina County, Croatia. The town of Petrinja was the most affected region with major power outage and many buildings collapsed. The damage also affected neighbor countries such as Bosnia and Herzegovina and Slovenia. As a light of this devastating event, a deformation map due to this earthquake could be generated by using remote sensing imagery from Sentinel-1 SAR data. InSAR could be used as deformation map but still affected with noise factor that could problematize the exact deformation value for further research. Thus in this study, 17 SAR data from Sentinel-1 satellite is used in order to generate the multi-temporal interferometry utilize Stanford Method for Persistent Scatterers (StaMPS). Mean deformation map that has been compensated from error factors such as atmospheric, topographic, temporal, and baseline errors are generated. Okada model then applied to the mean deformation result to generate the modeled earthquake, resulting the deformation is mostly dominated by strike-slip with 3 meter deformation as right lateral strike-slip. The Okada sources are having 11.63 km in length, 2.45 km in width, and 5.46 km in depth with the dip angle are about 84.47° and strike angle are about 142.88° from the north direction. The results from this modeling can be used as learning material to understand the seismic activity in the latest 2020 Petrinja, Croatia Earthquake.

다상유동형 입자법을 이용한 Rayleigh-Taylor 불안정성의 수치해석 (Numerical Study on Rayleigh-Taylor Instability Using a Multiphase Moving Particle Simulation Method)

  • 하나의 시스템 내에 2개 이상의 상이 다른 유체가 존재할 시에는 다상유동에 의한 복장성이 존재하며, 이는 해석의 어려움이 따른다. 두 개 이상의 상이 다른 다상유동은 유동 및 경계면에 영향을 끼치지 때문에, 불안정성과 같은 비선형 유동이 나타나게 된다. 여러 종류의 불안정성 중 레일리히-테일러 불안정성은 대표적인 예로 알려져 있다. 본 연구에서는 밀도차가 레일리히-테일러 불안정성에 미치는 영향을 조사하기 위해 다양한 Atwood 수를 선정하였으며, 초기 경계면 형상 역시 다양한 형태를 설정하고 시뮬레이션 하였다. 본 연구에서 사용된 입자법인 MPS(Moving particle simulation)은 이러한 다상유동에서 널리 쓰이지는 않았으나, 다상유동을 위한 입자간 상호 연성 모델인 자가-부력 항, 표면 장력 항과 경계면 경계 조건 항을 추가로 사용하여 수치해석이 가능하게 하였다. 본 연구에서 새로이 개발된 다상유동형 입자법을 이용하여 고려된 경우들에 대해 수치해석을 수행하였으며, 각각의 결과들을 비교 분석하였다. 또한 레일리히-테일러 불안정성에 기인한 유동의 속도를 측정하여 포텐셜 기반의 이론값과의 비교를 통해 경향성이 일치함을 알 수 있었다. 이론값과의 크기의 차는 포텐셜 기반의 이론값에서는 고려가 힘든 비선형성에 기인한다고 사료된다.

Influence of Metal Oxide Particles on Soil Enzyme Activity and Bioaccumulation of Two Plants

  • Particle size and metal species are important to both soil microbial toxicity and phytotoxicity in the soil ecosystem. The effects of CuO and ZnO nanoparticles (NPs) and microparticles (MPs) on soil microbial toxicity, phytotoxicity, and bioaccumulation in two crops (Cucumis sativus and Zea mays) were estimated in a soil microcosm. In the microcosm system, soil was artificially contaminated with 1,000 mg/kg CuO and ZnO NPs and MPs. After 15 days, we compared the microbial toxicity and phytotoxicity by particle size. In addition, C. sativus and Z. mays were cultivated in soils treated with CuO NPs and ZnO NPs, after which the treatment effects on bioaccumulation were evaluated. NPs were more toxic than MPs to microbes and plants in the soil ecosystem. We found that the soil enzyme activity and plant biomass were inhibited to the greatest extent by CuO NPs. However, in a Biolog test, substrate utilization patterns were more dependent upon metal type than particle size. Another finding indicated that the metal NP uptake amounts of plants depend on the plant species. In the comparison between C. sativus and Z. mays, the accumulation of Cu and Zn by C. sativus was noticeably higher. These findings show that metal oxide NPs may negatively impact soil bacteria and plants. In addition, the accumulation patterns of NPs depend on the plant species.

Superconducting Magnet Power Supply System for the KSTAR 2nd Plasma Experiment and Operation

  • The Korea Superconducting Tokamak Advanced Research (KSTAR) device is an advanced superconducting tokamak to establish scientific and technological bases for attractive fusion reactor. This device requires 3.5 Tesla of toroidal field (TF) for plasma confinement, and requires a strong poloidal flux swing to generate an inductive voltage to produce and sustain the tokamak plasma. KSTAR was originally designed to have 16 serially connected TF magnets for which the nominal current rating is 35.2 kA. KSTAR also has 7 pairs of poloidal field (PF) coils that are driven to 1 MA/sec for generation of the tokamak plasma according to the operation scenarios. The KSTAR Magnet Power Supply (MPS) was dedicated to the superconducting (SC) coil commissioning and $2^{nd}$ plasma experiment as a part of the system commissioning. This paper will describe key features of KSTAR MPS for the $2^{nd}$ plasma experiment, and will also report the engineering and commissioning results of the magnet power supplies.

만성 난치성 상부 등 통증에서 초음파 유도 하 흉추 주위 공간 차단술 (Ultrasound Guided Thoracic Paravertebral Space Block for Chronic Intractable Upper Back Pain)

  • There are some cases of myofascial pain syndrome (MPS) with chronic upper back pain that does not respond to dry needling or trigger point injection, well-known treatments for MPS. A 67-year-old female developed a stabbing upper back pain with trigger point at left T7~8 levels 10 years ago. She complained of the pain with Numeral Rating Scale (NRS) 8 points. Myofascial release technique and trigger point injection had no effect. Under ultrasound guidance 20 ml of 1% lidocaine was injected into thoracic paravertebral space. Immediately, the pain was reduced to NRS 4 points. One week later, the second block was performed in the same way as the first, and the pain was reduced to NRS 2 points. The stabbing pain disappeared, and oral opioids were discontinued. Ultrasound guided thoracic paravertebral space block is an effective and safe treatment for refractory MPS with chronic upper back pain.
