A Clinical Analysis of Patients Who Visited Emergency Department due to Chemical Incident Occurred at Industrial Place: A Retrospective Study (산업장 화학 사고로 응급실에 내원한 환자의 임상적 고찰: 일개병원의 후향적 연구)

  • Purpose: The purpose of this study is to understand what kinds of chemical substances have been used annually and to investigate incidents that occurred due to chemical hazard release and to analyze statistically clinically chemical injury patients who visited one regional emergency medical center in Gumi city with documented references review. Methods: Annual chemical waste emission quantity (Kg/Year) (Cwep) was reproduced using national web site data governed by the Ministry of Environment and 5 years (from 1 .Jan. 2010 to 31. Dec. 2014) of medical records of chemical injury patients who visited our emergency department were reviewed retrospectively. By applying exclusion criteria, 446 patients of 460 patients were selected. Results: Dichloromethane, Toluene, Trichloroethylene, and Xylene were always included within Top 5 of Cweq. Six cases of chemical incidents were reported and in 3 of 6 cases involving Hydrogen fluoride were included during the study period. Male gender and twenties were the most prevalent group. Injury evoking chemicals were Hydrogen fluoride, unknown, complex chemicals (over 2 substances) in sequence. The most frequent site of wounds and injuries was the respiratory tract. Gas among status, intoxication among diagnosis, and discharge among disposition was most numerous in each group. Conclusion: There have been no uniform clinical protocols for chemical wounds and injuries due to various kinds of chemicophysical properties and ignorance of antidotes. Therefore conduct of a multicenter cohort study and experiments for ruling out chemicals according to chemicophysical priority as well as development of antidotes and clinical protocols for chemical injury patients is needed.

The level of urinary malondialdehyde concentrations in vinyl chloride monomer exposure workers (염화비닐 노출 근로자의 요중 Malondialdehyde 농도 수준)

  • The aim of this study was to investigate whether the exposure to vinyl chloride monomer(VCM) induces lipid peroxidation in workers by evaluating the concentration of malondialdehyde(MDA) in the urine in order to assess worker's oxidative stress due to exposure of vinyl chloride monomer. The subjects investigated in the study were divided into the experimental group; 18 workers exposed to VCM, and the control group; 19 workers unexposed to VCM. A gas chromatography/pulsed flame photometric detector(GC/PFPD) was utilized to analyze thiodiglycolic acid(TDGA), which was methylated with trimethylsilyldiazomethane (2.0M in diethyl ether) in urine and the urinary MDA, the product of lipid peroxidation, was determined by high-performance liquid chromatography/ultraviolet-visible detector after derivatized with 2,4-dinitrophenylhydrazine(DNPH). The concentrations of urinary TDGA in controls and VCM exposure workers were 0.13(2.01)mg/g Cr. GM(GSD) and 0.35(1.96)mg/gCr. GM(GSD), respectively. The concentrations of urinary MDA were $0.12(2.21){\mu}mol/gCr$. GM(GSD) in controls and $1.35(1.79){\mu}mol/gCr$. GM(GSD) in VCM exposure workers. As a result of simple regressions analysis between urinary concentration of TDGA and MDA in VCM exposure workers, it was found that the $R^2$ value was 0.261 (p=0.03) and the drinking and smoking did not affect their level. In conclusion, the workers exposed to VCM have a potentially to suffered by oxidative stress due to VCM exposure and the urinary MDA can be applicable to the marker of effect to assess the level of worker's VCM exposure.

  • Substantial alterations in cerebral blood flow(CBF) are known to occur during cardiopulmonary bypass(CPB). Many investigators have speculated that these changes may be responsible for both minor and major cerebral damages after CPB. More recently, these changes in CBF have been observed to be intimately related to the arterial carbon dioxide tension(Pa$CO_2$) maintained during CPB. The present study was prospectively designed to investigate the clinical effects of normocapnic and hypercapnic CPB on the cerebral oxygen metabolism in cardiac surgery Material and Method: Thirty-six adult patients scheduled for elective cardiac surgery were randomized to either normocapnic group (Pa$CO_2$35~40 mmHg, n=18) or hypercapnic group(Pa$CO_2$, 45~55 mmHg, n=18) with moderately hypothermic nonpulsatile CPB(nasopharyngeal temperature of 29~3$0^{\circ}C$). In each patient, middle cerebral artery blood flow velocity( $V_{MCA}$), cerebral arteriovenous oxygen content difference (C(a-v) $O_2$), cerebral oxygen extraction(COE), cerebral metabolic rate for oxygen(CMR $O_2$), cerebral oxygen transport( $T_{E}$ $O_2$), $T_{E}$ $O_2$/CMR $O_2$ ratio, cerebral desaturation(internal jugular bulb blood oxygen saturation $\leq$ 50%), and arterial and jugular bulb blood gas were evaluated throughout the operation. Postoperative neuropsychologic complications were assessed in all patients. All variables were compared between the two groups. Result: VMCA(169.13 $\pm$ 8.32 vs 153.11 $\pm$8.98%), TE $O_2$(1,911.17$\pm$250.14 vs 1,757.40$\pm$249.56), $T_{E}$ $O_2$,/CMR $O_2$ ratio(287.38$\pm$28.051 vs 246.77$\pm$25.84), $O_2$ tension in internal jugular bulb (41.66$\pm$9.19 vs 31.50$\pm$6.09 mmHg), and $O_2$saturation in internal jugular bulb(68.97$\pm$10.96 vs 58.12$\pm$12.11%) during CPB were significantly lower in normocapnic group(p=0.03), whereas hypercapnic group had lower C(a-v) $O_2$(3.9$\pm$0.3 vs 4.9$\pm$0.3 mL/dL), COE(0.3$\pm$0.03 vs 0.4$\pm$0.03), CMR $O_2$(5.8 $\pm$0.5 vs 6.8$\pm$0.6), and arterial blood pH(7.36$\pm$0.09 vs 7.46$\pm$0.07, p=0.04) during CPB. Hypercapnic group had lower incidence of cerebral desaturation than normocapnic group(3 vs 9 patients, p=0.03). Duration of the neuropsychologic complication(delirium) were shorter in hypercapnic group than in normocapnic group(36 vs 60 hrs, p=0.009). Conclusion: These findings suggest that hypercapnic CPB may have salutary effects on the cerebral oxygen metabolism and postoperative neurologic outcomes in cardiac surgery.surgery.

High Fat Diet or Exercise Training Alters Hepatic Total and Phospholipid fatty Acid Compositions in Rats (고지방식이 및 운동훈련이 흰쥐 간조직의 총지방산 및 인지질 지방산패턴에 미치는 영향)

  • Effects of high fat diet and/or endurance exercise training on hepatic total and phospholipid(PL) fatty acid compositions were evaluated in rats fed one of the following diets for 31 days. control diet(CD, 5 wt% corn oil) or high fat diet(HFD, 35 wt% corn oil). Half of the rats in each group were exercise-trained regularly on a treadmill for 90 minutes/day during the entire feeding period. Total and PL fatty acid compositions of hepatic lipid extracts were determined by a gas-liquid chromatograph),. Endurance exercise training did not change the daily food intake, but significantly reduced body weight gain and feed efficiency ratio of rats, which were most prominent in animals fed HFD. Exercise training did not significantly change the percentages of ∑saturated fatty acids (SFA) and ∑polyunsaturated fatty acids(PUEA), but decreased the percentage of ∑monounsaturated fatty acids(MUFA) in hepatic total fatty acids, which might be associated with the decrease in (equation omitted) 9-desaturation index of hepatic total fatty acid metabolism. Exercise training significantly lowered the percentages of 16 : 0 and 22 : 5$\omega$3, and increased the percentages of 20 : 1 and 20 : 3$\omega$3 in both total and PL fatty acid compositions in rat liver. Both total fatty acid and PL fatty acid compositions of rat liver responded more sensitively to changes in dietary fat content than to endurance exercise training in this study. Feeding HFD, whoch contains high level of linoleic acid(LA, 18 : 2$\omega$6), significantly decreased the percentages of ∑SFA and $\Sigma$MUFA, and increased the percentages of ∑PUFA and ∑$\omega$6 fatty acids of hepatic total fatty acids. Hepatic total fatty acid composition was affected by dietary fat content and dietary fatty acid composition more sensitively than those found in hepatic PL fatty acid composition. HFD significantly decreased most of desaturation indices, while exercise training significantly decreased elongation index(20 : 5$\omega$3⇒22 : 5$\omega$3) of hepatic total and PL fatty acid metabolism in rats. A Study on Pyrolysis of Cellulosic Organic Solid Wastes (셀룰로오스질 유기고형폐물의 열분해에 관한 연구)

  • Cellulosic organic solid wastes such as bark and sawdust, and filter papers as a pure cellulose were pyrolyzed at $300^{\circ}C$ under nitrogen current and mixed current of nitrogen and air. Amounts of condensates collected in air, water, and dry ice-acetone cooling traps, noncondensable gases, and carbonized residues were surveyed. The components of volatile liquids condensed in dry ice-acetone trap were separated by means of gas chromatograph and identified by retention times and syringe reactions. Pyrolysis under nitrogen current produced 13.4∼29.6${\%}$ of tar, 0.01∼0.12${\%}$ of aqueous liquids, 0.24∼1.43${\%}$ of volatile liquids, 9.84∼42.41${\%}$ of noncondensable gases, and 44.0∼65.81${\%}$ of carbonized residues. Pyrolysis under mixed current decreased tar and condensable liquids, but increased noncondensable gases.Volatile liquids collected under nitrogen current separated into the same 19 components by Porapak Q column regardless of the materials and only difference among materials was relative amounts of components. Volatile liquids collected under mixed current separated into six components and mainly lower molecular weight compounds such as methanol and formaldehyde were produced. According to the retention times and syringe reactions, methanol, formaldehyde, acetone, acetaldehyde, acetic acid, and three other compounds presumably containing hydroxyl group in the molecular structure were identified out of 19 compounds.

Geochemical Evolution and Deep Environment of the Geothermal Waters in the Bugok Area: Reconsideration on the Origin of Sulfate-type Geothermal Water (부곡 지열수의 심부환경과 지화학적 진화: 유황형 지열수의 생성과정 재해석)

  • The deep environment and geochemical evolution of the Bugok geothennal waters, located in the Kyeongnam Province, was re-interpreted based on the hydrochemical and isotopic data published by Yun et al. (1998). The geothermal waters of the Bugok area is geochemically divided into three groups; Geothennal water I, II and III groups. Groups I and II are geochemically similar; high temperature (55.2-77.2$^{\circ}$C) and chemically belonging to Na-S04 types. However, pH and Eh values are a little different each other and Group II water is highly enriched in S04 compared to Group I water. Group III water, occurring from peripheral sites of the central part of the geothennal waters, shows temperature range of 29.3 to 47.0$^{\circ}$C and belongs to $Na-HCO_3-S0_4$ types. The deep environment and geochemical evolution of the Bugok geothennal waters, showing the diversity of geochemistry, can be interpreted as follows; I) Descending to great depth of meteoric waters that originated at high elevation and reacting with sediments and/or granites in depth. The $S0_4$ concentration of the waters has been increased by the dissolution of sulfate minerals in sediments. 2) During the continuous descending, the waters has met with the reduction environment, producing the $H_2S$ gas due to sulfate reduction. The waters has been heated up to 130$^{\circ}$C and the extent of water-rock reaction was increased. At this point, pH of waters are increased, S04 concentration decreased and calcite precipitated, therefore, the waters show the $Na-S0_4$ type. 3) Ascending of the geothennal waters along the flow path of fluids and mixing with less-deeply circulated waters. The $S0_4$ concentration is re-increased due to the oxidation of $H_2S$ gas and/or sulfide minerals in sediments. During continuous ascending, these geothennal waters are mixed with shallow groundwater.

  • The chemical structure of glycolipid of Selenomonas ruminantium cell wall was to be elucidated. The bacterial cells were treated in hot TCA and the glycolipid fractions were extracted by the solvent $CHCl_3\;:\;CH_3OH$ (1 : 3). The extracted glycolipids fraction was further separated by acetone extraction. The acetone soluble fraction was named as the spot A-compound. The acetone insoluble but ether soluble fraction was named as the spot B-compound. These two compounds were examined for elucidation of their chemical structure. The results were as follows: 1. The IR spectral analysis showed that O-acyl and N-acyl fatty acids were linked to glucosamine moiety in the spot A-compound. However in the spot B-compound in addition to O and N-acyl acids phosphorus was shown to be attached to glucosamine. 2. It was recognized by gas liquid chromatography that spot A compound contained beta-OH $C_{13:0}$ fatty acid in predominance in addition to the fatty acid with beta-OH $C_{9:0}$, whereas the spot B compound was composed of the predominant fatty acid of beta-OH $C_{13:0}$ with small amount of beta-OH $C_{9:0}$. 3. According to the paper chromatographic analysis of hydrazinolysis products of the spot A compound, a compound of a similar Rf value as the chitobiose was recognized, which indicated a structure of two molecules glucosamine condensed. The low Rf value of the hydrazinolysis product of the spot B-compound confirmed the presence of phosphorus attached to glucosamine. 4. The appearance of arabinose resulting from. ninhydrin decomposition of the acid hydrolyzate of the spot A compound indicated that the amino group is attached to $C_2$ of glucosamine. 5. The amount of glucosamine in the N-acetylated spot A compound decreased in half of the original content by the treatment. with $NaBH_4$, indicating that there are two molecules of glucosamines in the spot A compound. The presence of 1, 6-linkage between two molecules of glucosamine was suggested by the Morgan-Elson reaction and confirmed by the periodate decomposition test. 6. By the action of ${\beta}-N-acetyl$ glucosaminidase the N-acetylated spot A compound was completely decomposed into N-acetyl glucosamine, whereas the spot B compound was not. This indicated the spot A compound has a beta-linkage. 7. When phosphodiesterase or phosphomonoesterase acted on $^{32}P-labeled$ spot B compound, $^{32}P$ was not released by phosphodiesterase, but completely released by phosphomonoesterase. This indicated that one phosphorus is linked to glucosamine moiety. 8. The spot A compound is assumed to have the following chemical structure: That is glucosaminyl, ${\beta}-1$, 6-glucosamine to which O-acyl and N-acyl fatty acids are linked, of which the predominant fatty acid is beta-OH $C_{13:0}$ fatty acid in addition to beta-OH $C_{9:0}$ fatty acid 9. The spot B compound is likely to have the linkage of $glucosaminyl-{\beta}-1$, 6-glucosamine to which phosphorus is linked in monoester linkage. Furthermore both O-acyl and N-acyl fatty acids contained beta-OH $C_{13:0}$ fatty acid predominantly in addition to beta-OH $C_{9:0}$ fatty acid.

Effect of Modified Atmosphere Packaging on Postharvest Quality of Kohlrabi (콜라비의 수확 후 MA 포장에 따른 품질 유지 효과)

  • The effect of modified atmosphere packaging (MAP) on kohlrabi (Brassica olerace L. gongulodes group)'s quality and antioxidant molecule during storage was examined to determine the optimal film package for maintaining freshness. To extend shelf life, MAP was tested using PE $50{\mu}m$ and oriented polypropylene (OPP) films with oxygen transmission rate (OTR) at 3,000, 10,000, $15,000mL/m^2/day/atm$. The OPP film packaging with modified oxygen transmission rate showed a delay in a weight loss and extended storage period. The package with OTR 3000 attained the desired gas composition of $O_2$ 3.2-6.7 kPa and $CO_2$ 13.1-19.8 kPa, in storage at room temperature. Kohlrabi stored in this package showed the lowest weight loss and the highest visual quality. Deterioration and off-odor were developed more rapidly in PE $50{\mu}m$ towards the end of the storage at room temperature. However, there are no differences among OTR films in visual quality and off-odor until 60 days at cold storage. Vitamin C content of kohlrabi was reduced rapidly in OTR 15000 with high transmission rate and showed less loss in PE $50{\mu}m$ and OTR 3000 in both room temperatures and cold storage. Results revealed that an OPP film with OTR 3000 extended the shelf life of kohlrabi in storage with maintained quality and vitamin C.

A Comparative Study of Two Different Heel Lancet Devices for Blood Collection in Preterm Infants (미숙아에서 채혈을 위한 발뒤꿈치 천자시 두 종류 천자 기구의 비교)

  • Purpose: To evaluate two different heel lancet device in terms of pain response and success of the procedure in the preterm infants undergoing heel puncture. Methods: 100 preterm infants undergoing capillary blood gas analysis or capillary bilirubin monitoring underwent heel puncture, were randomly allocated to blood sampling from the heel with either a conventional manual lancet or an automatic incision device. Primary outcome measures included the Premature Infants Pain Profile (PIPP) score, total duration of procedure, number of heel puncture and number of bruise. The pain response was evaluated using PIPP score and the effectiveness was evaluated using three criteria: total duration of blood sampling, number of puncture, bruising of the heel or ankle. Statistical analysis was performed using the SPSS ver. 13.0 program. Difference between the groups were analysed with t test (continuous variables) and the Chi square test or Fisher test (categorical variables). Results: The mean PIPP score was 4.91 for the automatic lancet group compared with 5.84 for the conventional manual lancet group (P=0.0255).The number of pain scores above 7 during blood collection did not differ between two groups (P=0.2167). The procedure took less time to perform in the automatic lancet group (mean, 30.69 seconds) than in the conventional lancet group (mean, 48.92 seconds) (P<0.0001). Conclusion: This study demonstrated that the automatic lancet device causes less pain and a shorter procedure time than the conventional manual lancet in preterm infants undergoing heel puncture. On the basis of these results the automatic lancet device is very useful method for blood collection in preterm infants by heel puncture.

The Prognostic role of Electrocardiographic Signs of Cor Pulmonale in Chronic Obstructive Pulmonary Disease (폐성심을 시사하는 심전도 소견 유.무에 따른 만성폐쇄성 폐질환 환자의 예후)

  • Background : In patients with chronic obstructive pulmonary disease(COPD), several factors have been associated with a poor prognosis. These include old age, low $FEV_1$ low diffusing capacity, high alveolar-arterial oxygen pressure difference, and finally cor pulmonale. This study was done to investigate if the ECG signs suggesting cor pulmonale were independent prognostic factors in patients with COPD. Method : We analyzed ECG, pulmonary function data and arterial blood gas values in 61 patients who were admitted through the emergency department with an acute exacerbation of COPD. The ECG signs reflecting cor pulmonale were right strial overloading(RAO), right bundle branch block, right ventricular hypertrophy and low-voltage QRS. The 61 patients were divided into 2 groups ; group I with no ECG signs(n=36) and group II with one or more ECG signs(n=25) suggesting cor, pulmonale. Results : Poor, prognostic factors by univariate analysis were low $FEV_1$, $FEV_1$ % pred., VC % pred., DLco, DLco % pred., $PaO_2$ and $SaO_2$ high $PaCO_2$ presence of ECG signs reflecting cor pulmonale, presence of mental status change, use of mechanical ventilator, and long term use of glucocorticoid. A multivariate analysis indicated that age(risk ratio=1.13, 95% confidence interval 1.05-1.23), DLco % pred. (risk ratio=0,97. 95% confidence interval 0.94-0.99), $PaO_2$ (risk ratio=0.95, 95% confidence interval 0.90-0.99) and RAO(risk ratio=5.27, 95% confidence interval 1.40-19.85) were independent prognostic factors of survival. There was a significant difference in survival between the patients with and without RAO(p=0.038). The survival rates at 1, 2, and 5 years were 94.5%. 81.4%, and 50.0% in patients without RAO and 82.4%, 70.6%, and 27.5% in patients with RAO, respectively. Conclusion : These results suggest that the presence of ECG signs reflecting cor pulmonale is a predictor of survival and that RAO of these ECG signs is a significant independent predictor of survival in patients with COPD.
