Effect of a Breastfeeding Management Program on Breastfeeding Duration (모유수유관리 프로그램이 모유수유지속에 미치는 효과)

  • Purpose: The purpose of this study was to measure the effects of a breastfeeding management program for promotion of breastfeeding. Method: The study design was a non-equivalent control group pre-post test. The study subjects were pregnant women receiving a breastfeeding education class in one hospital in Ilsan. The total number of study subjects was 153 pregnant women, namely 75 mothers in the experimental group and 78 in the control group. Data was collected between June and November, 2006. A breastfeeding management program was provided to the experimental group while admitted for delivery, and in weeks 2, 4, 8, 12, 16, 20, and 24 of the postpartum period. The rates of breastfeeding were measured in weeks 2, 4, 8, 12, 16, 20, and 24 of the postpartum period. Result: There were significant differences in the scores of rate of breastfeeding between the two groups(all cases), which were 93.1 in the experimental group and 85.8 in the control group at week 4; 88.1 vs 71.8 at week 8; 83.4 vs 56.7 at week 12; 71.4 vs 53.9 at week 16; and 65.7 vs 48.4 at week 20 respectively. However, there were not significant differences in the scores of rate of breast-feeding between the two groups, which were 97.7 in the experimental group and 96.0 in the control group in the early postpartum period at week 2, and 59.6 vs 45.3 at week 24. Add to this, the study analyzed only people who had jobs. As a result, the scores of rate of breastfeeding were 94.8 in the experimental group which were significantly higher than 88.5 in the control group at week 4; 93.2 vs 73.3 at week 8; 89.6 vs 57.2 at week 12; and 75.0 vs 46.0 at week 20 respectively. However, there were not significant differences at week 2, week 16 and week 24. Conclusion: The results of this study suggest that a breastfeeding management program during the last trimester of the perinatal period and postpartum period may increase the duration of breastfeeding in postpartum clients who intend to breastfeed. This study supports the literature which found that prenatal education and postpartum support are important to the outcome of breastfeeding.

Effects of a Chelate (DTPA) on Cucumber Growth and Soil Chemical Properties in Nutrient-accumulated Soil of Polytunnel Greenhouse

  • This study was conducted to evaluate the effects of a chelating agent on cucumber growth and changes in soil nutrients availability in polytunnel greenhouse fields. Diethylene triamine penta acetic acid (DTPA) was selected as a chelating agent. Two experiments were carried out as follows: i) For field experiment in the autumn season of 2010, each plot was treated by varying the concentration and the number of times being applied with DTPA; [DTPA (0.5 mM, 1 time/3 months), DTPA (0.06 mM, 1 time/1 week), DTPA (0.13 mM, 1 time/2 weeks), DTPA (0.06 mM, 1 time/1 week)+N]. Conventional practice was also investigated. ii) In the spring and summer seasons of 2011, each plot was treated by varying the concentration (0, 0.06, 0.13, 0.19 mM) of DTPA, chemical fertilizers (NPK), and combination of chemical fertilizers and DTPA 0.06 mM. The fruit yields of cucumber and soil chemical properties had no significant differences between treatments. However, in the spring season of 2011, DTPA 0.06 mM plot added 1 time per 2 weeks increased the yield of cucumber, but caused the reduction of yield in next cultivation season. This result showed that excess use of DTPA can cause the damage of crop growth. The inorgainc contents such as Ca and Mg absorbed by cucumber plant had significant differences between DTPA 0.19 mM (2 times/1 week) and fertilizers plus DTPA treatments [DTPA 0.06 mM (2 times/1 week) + 1/2 NPK, DTPA 0.06 mM (2 times/1 week) + NPK]. The input cost of fertilizers was saved when the concentration and the number of times added with DTPA was 0.06 mM and 1 time a week, respectively. This treatment used 67% less of applied fertilizers cost than the plot of conventional practice did. Thus, this research suggested that the application of DTPA 0.06 mM by 1 time a week can be effective for sustainability of crop production and reduction of fertilizers usage in polytunnel greenhouse.

A 4-week Repeated dose Oral Toxicity Study of Mecasin in Sprague-Dawley Rats to Determine the Appropriate Doses for a 13-week, Repeated Toxicity Test

  • Objectives: In this study, we investigated the 4-week repeated-dose oral toxicity of gami-jakyak gamcho buja decoction (Mecasin) to develop safe treatments. Methods: In order to investigate the 4-week oral toxicity of Mecasin, we administered Mecasin orally to rats. Sprague-Dawley (SD) rats were divided into four groups of five male and five female animals per group: group 1 being the control group and groups 2, 3, and 4 being the experimental groups. Doses of Mecasin of 500, 1,000, and 2,000 mg/kg of body weight were administered to the experimental groups, and a dose of normal saline solution of 10 mL/kg was administered to the control group. We examined the survival rate, weight, clinical signs, and gross findings for four weeks. This study was conducted under the approval of the Institutional Animal Ethics Committee. Results: No deaths occurred in any of the four groups. No significant changes in weights or food consumption between the control group and the experimental groups were observed. Serum biochemistry revealed that some groups showed significant decrease in inorganic phosphorus (IP) (P < 0.05). During necropsy on the rats, one abnormal macroscopic feature, a slight loss of fur, was observed in the mid dosage (1,000 mg/kg) male group. No abnormalities were observed in any other rats. In histopathological findings, the tubular basophilia and cast of the kidney and extramedullary hematopoiesis of the spleen were found. However, those changes were minimal and had occurred naturally or sporadically. No other organ abnormalities were observed. Conclusion: During this 4-week, repeated, oral toxicity test of Mecasin in SD rats, no toxicity changes due to Mecasin were observed in any of the male or the female rats in the high dosage group. Thus, we suggest that the doses in a 13-week, repeated test should be 0, 500, 1,000, and 2,000 mg/kg respectively.

  • Intestinal mucosal mast cell (MMC) responses were studied in rats experimentally infected with Metngonimur yokogawai (Dlgenea: Heterophyidael. Twenty Sprague-Dawley rats were fed each 2,500 metacercariae isolated from the sweetish and sacrificed on the week 1, 2, 3 and 4 post-Infection (PI). Recovery of worms was performed from the small intestine of each rat. To visualize the MMCs, duodenal and jejunal (upper, middle and lowers) tissue sections were made and stained with alcian blue/safranine-0. The average worm recovery rates were 16.2% and 13.8% on the week 1 and week 2, respectively, but they decreased rapidly to 4.1% and 4.2% on the week 3 and week 4 PI, respectively, which indicate spontaneous worm expulsion after the week 2. The MMC number In the Infected rats was, compared with uninfected controls, significantly Increased In the whole small intestine, through the whole period of observation. The peak level of mastocytosis was observed on the week 3 PI. It is strongly suggested that MMCs might be involved In the expulsion process of flukes from the rat intestine.

Leisure Time Physical Activity in Saudi Arabia: Prevalence, Pattern and Determining Factors

  • Background: Identification of reliable predictors of leisure time physical activity (LTPA) will enable healthcare providers to intervene and change the patterns of LTPA in the population to improve community health. Objectives: The objectives of this study were to determine prevalence and pattern of LTPA among adult Saudis aged 18-65 years, and to define the socio-demographic determinants that correlate with LTPA in Al-Hassa, Saudi Arabia. Subjects and Methods: A cross-sectional study of 2176 adult Saudis attending urban and rural primary health care centers were selected using a multistage proportionate sampling method. Participants were personally interviewed to gather information regarding socio-demographics, physical activity pattern using the Global Physical Activity Questionnaire (GPAQ). Physical activity (PA) in each domain was expressed in metabolic equivalents (METs). Results: The median total METs minutes/week for LTPA for both genders was 256, higher for men (636 METs minutes/week) compared to women (249METs minutes/week). Overall, only 19.8% of the total PA was derived from LTPA. Of the sampled population 50.0% reported doing no leisure activity. Using the cut off of 600 METs-minutes/day or 150 minutes of moderate intensity over 5 or more days/week, only 21.0% of the included sample were considered as being sufficiently active and 10.4% were in the high active category with beneficial health effects. Multivariate regression analysis showed that male, younger age (<35 years), absence of chronic disease conditions and moderate level of total PA were significant predictors for being active in the LTPA domain. Conclusion: The prevalence and intensity of LTPA among the included sample demonstrated low levels. Nearly 80% of the included sample population did not achieve the recommended LTPA level with beneficial health effects. Female gender, urban residence and associated chronic diseases correlated with a low LTPA.

Postmortem changes in physiochemical and sensory properties of red snow crab (Chionoecetes japonicus) leg muscle during freeze storage

  • In order to evaluate the maximal storable period of the raw crab for a non-thermal muscle separation, the quality changes of the leg meat of red snow crab (Chionoecetes japonicus) during freeze storage were investigated. Fresh red snow crabs were stored at $-20^{\circ}C$ for 7 weeks, and the leg muscle was separated by a no heating separation (NHS) method every week. During the storage, considerable loss of the leg muscle did not occur and microbiological risk was very low. In contrast, discoloration appeared at 2-week storage on around carapace and the leg muscle turned yellow at storage 3-week. In physiochemical parameters, protein and free amino acids gradually decreased with storage time, expected that proteolytic enzymes still activated at $-20^{\circ}C$. At 4-week storage, the sensory acceptance dropped down below point 4 as low as inedible and notable inflection points in pH and acidity were observed. The volatile base nitrogen was low, though a little increase was recorded. These results suggested that the maximal storable period at $-20^{\circ}C$ of the raw material was within 2 weeks and it was depended on external factor such as the discoloration. The present study might be referred as basic data for approaches to solve quality loss occurred in non-thermal muscle separation.

The Effect of Inspiratory Muscle Resistance Exercise with Aerobic Exercise on the Breathing Functions of Adults in their 20s Depending on Smoking or No smoking (유산소운동을 동반한 들숨근 저항운동이 20대 성인의 흡연 여부에 따라 호흡 기능에 미치는 영향)

  • Purpose : This study applies inspiratory muscle resistance exercise with aerobic exercise to smokers and nonsmokers and then determines whether subjects' breathing functions (FVC, FEV1) are increased and how much effect smoking has on the difference in the increase of breathing functions between the two groups. Methods : For this experiment, 26 male adults were selected and randomly allocated to the smoker group (n=13) and nonsmoker group (n=13). The smokers and nonsmokers performed the inspiratory muscle resistance exercise with aerobic exercise three times a week for four weeks. Regarding the breathing functions, the forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) were measured three times: week 0, week 2, and week 4. The aerobic exercise was performed using a stationary bicycle with 8 difficulty levels. The inspiratory muscle resistance exercise was performed using Power Breathe with 10 resistance levels. Results : The study found that the FVC and FEV1 values of the smoker group decreased slightly after four weeks of inspiratory muscle resistance exercise with aerobic exercise. In other words, the difference was not statistically significant. In contrast, the FVC and FEV1 values of the nonsmoker group increased by a statistically significant amount. In addition, the intergroup comparison of the average increases in FVC and FEV1 values showed statistically significant differences. Conclusion : The results of this study show that when inspiratory muscle resistance exercise with aerobic exercise was performed, the increase in the breathing functions of nonsmokers was higher than that of smokers. This confirms that, within the parameters of the study, smoking had a negative effect on the increase of breathing functions. This suggests that quitting smoking must be considered as an essential factor when applying a breathing physiotherapy or a breathing function improvement program in clinical settings

Thirteen-week repeated-dose oral toxicity study of KOB03, a polyherbal medicine for allergic rhinitis, in rats (알러지성 비염 한약제제 KOB의 랫드에서의 13주 반복 경구투여에 의한 독성 연구)

  • Objectives : To evaluate the safety of KOB, a polyherbal medicine for allergic rhinitis, we conducted a subchronic toxicology study. Methods : Dried extract of KOB(Lot. No. 11003, yield : 41.1%) was prepared from GLP company (Hanpoong Pharm & Food Co., Ltd). KOB was repeatedly administrated orally of male SD rats at daily dose levels of 500 (G2), 1250 (G3) and 5000 (G4) mg/kg/day for 13 weeks. We recorded the clinical signs of toxicity, body weight, food intake/consumption, optometry, urine analysis, organ weights, hematology, and conducted serum biochemical analysis, necropsy, gross and histological changes in target organs of Sprague-Dawley rats, and clinical chemistry analysis. Results : Neither death nor any toxicological signs were obserbed in KOB at all doses of 500, 1250 and 5000 mg/kg/day during the administration period for thirteen-week. Furthermore, there was no difference in body weight and food-take consumption, optometry, necropsy, organ weight, gross pathological findings, and urine analysis among the groups of rats treated with different doses of KOB, during at the observation period for thirteen-week. The hematological analysis and clinical blood chemistry data were revealed no toxic effects from repeated-dose administration of KOB in rats during the observation period. Conclusions : Based on these results, the no observable adverse effect level (NOAEL) of KOB was considered to be 5000 mg/kg/day for male rats under these study conditions.

Hip Function after Surgically Treated Isolated Traumatic Acetabular Fracture: A Prospective Series of Consecutive Cases

  • Purpose: Isolated acetabular fractures can occur as a result of a high energy impact on the hip joint. Surgery is required for most patients with an isolated acetabular fracture in order to alleviate pain, restore joint stability, and regain hip function. This study was conducted in order to examine the course of hip function in patients after surgical treatment of an isolated traumatic acetabular fracture. Materials and Methods: This prospective series of consecutive cases included patients who underwent surgery for treatment of an isolated acetabular fracture in a European level one trauma center between 2016 and 2020. Patients with relevant concomitant injuries were excluded. Scoring of hip function was performed by a trauma surgeon using the Modified Merle d'Aubigné and Postel score at six-week, 12-week, six-month, and one-year follow-up. Scores between 3-11 indicate poor, 12-14 fair, 15-17 good, and 18 excellent hip function. Results: Data on 46 patients were included. The mean score for hip function was 10 (95% confidence interval [CI] 7.09-12.91) at six-week follow-up (23 patients), 13.75 (95% CI 10.74-16.76) at 12-week follow-up (28 patients), 16 (95% CI 13.40-18.60) at six-month follow-up (25 patients), and 15.50 (95% CI 10.55-20.45) at one-year follow-up (17 patients). After one-year follow-up, the scores reflected an excellent outcome in 11 patients, good in five patients, and poor in one patient. Conclusion: This study reports on the course of hip function in patients who have undergone surgical treatment for isolated acetabular fractures. Restoration of excellent hip function takes six months.
