A Case Report of Soeumin Atopic Dermatitis Patient Improved by Gwakhyangjeonggi-san (곽향정기산(藿香正氣散)으로 호전된 소음인(少陰人) 아토피 피부염 환자 치험 1례)

  • 1. Objectives This study is about a Soeumin Atopic Dermatitis Patient. In this study, we report the effects of Sasang constitutional treatment to this patient. 2. Methods This patient was treated by Soeumin's constitutional medications and acupunture according to the result of Sasang constitutional diagnosis. 3. Results and Conclusions This patient's chief complaints were edema, palule, erythema, squama and pruritus. By using Gwakhyangjeonggi-san, she showed positive response about her symptom. This study shows that Gwakhyangjeonggi-san has effect to Atopic Dermatitis in Soeumin.

  • We experienced a case of 63 years old male patient who had synchronous rectus abdominis intramuscular schwannoma and chest wall lipoma. Schwannoma is rare benign tumor which derived from nerve sheath and mainly peripheral nerve of flexor part. The authors report rare synchronous schwannoma and lipoma development.

A Case Report of Castleman's Disease of the Neck (경부에 발생한 Castleman씨 질환 2례)

  • Castleman's disease was originally described as a localized mediastinal lymph node enlargement characterized by angiofollicular hyperplasia and intrafollicular capillary proliferation, with surgical removal of mass the only treatment required. It has been divided into two distict histologic types. The hyaline-vascular type is more common and characterized by small hyaline-vascular follicles and interfollicular proliferation. The plasma-cell type is occurred less frequent and more likely to present with constituitional symptoms. It commonly involves the mediastinal and pulmonary lymph nodes, with neck involvement in only 15% to 20% of cases. We report two cases of hyaline-vascular type of Castleman's disease located in the neck area with references to recent literature.

Case Report : Brain Fog Treated by Bekhogainsam-tang (백호가인삼탕(白虎加人蔘湯)으로 호전된 브레인 포그(Brain fog) 1례 임상보고)

  • Objective: This study aimed to report the effect of Baekhogainsam-tang on brain fog. Methods: A 59-year-old female patient complained of brain fog and waking up frequently during the night. Based on the Shanghanlun disease pattern identification diagnostic system, the patient was treated with Baekhogainsam-tang. The result was evaluated using the score. Results: After administration of Baekhogainsam-tang for 45 days, the MFI score decreased from 89 to 40. The average number of instances of waking up during the night decreased from 3 to 1.5. Conclusions: Some cases of brain fog can be treated by Baekhogainsam-tang.

A Case Report on the Shelter for Dementia Programs based on the Primary Health Care Post (보건진료소 중심 치매 쉼터 프로그램 사례)

  • Purpose: This study developed a shelter for dementia programs based on primary health care posts; and examined its effectiveness in rural older adults. Methods: This study used a case report provided by a primary healthcare post in Jeonnam, Korea, in 2023. A single group pre-test post-test design was used; for three months, and 10 older adults participated in the shelter for dementia programs based on the primary health care post. The effectiveness of the intervention was measured immediately after entering the shelter for dementia programs. Results: There were significant differences between the pre-test and post-test on the cognitive impairment screening test (p=.005). Conclusion: The shelter for dementia programs based on primary health care posts was effective in improving cognitive impairments of rural older adults.

Supratentorial Gangliocytoma Mimicking Extra-axial Tumor: A Report of Two Cases

  • We report two cases of supratentorial gangliocytomas mimicking an extra-axial tumor. MR imaging indicated that the tumors were extra-axial, and meningiomas were thus initially diagnosed. Relative to gray matter, the tumors were hypointense on T1-weighted images and hyperintense on T2-weighted images. On contrast-enhanced T1-weighted images, homogeneous enhancement was observed, while CT scanning revealed calcification in one of the two cases.

  • Deep vein thrombosis and pulmonary embolism are serious fatal complication after orthopedic surgery. Most of them occur after large operation such as total knee/hip arthroplasty. Some reports about deep vein thrombosis after simple arthroscopic procedure were on the Western literatures, but there was no report on the Korean literature. So we report two cases of pulmonary embolism after knee arthroscopic procedures.

Diphyllobothrium nihonkaiense Infections in a Family

  • Diphyllobothrium latum and Diphyllobothrium nihonkaiense are morphologically similar to each other, and only genetic method can differentiate clearly between the 2 species. A strobila of diphyllobothriid tapeworm discharged from a 7-year-old boy was analyzed to identify the species by mitochondrial cytochrome c oxidase subunit 1 (cox1) gene sequencing. He and his family (total 4 persons) ate slices of 3 kinds of raw fish 16 days before visiting our outpatient clinic. All family members complained of abdominal pain and watery diarrhea. They all expelled tapeworm strobilae in their stools. They were treated with a single oral dose of praziquantel and then complained of no more symptoms. The cox1 gene sequencing of the strobila from the boy revealed 99.9% (687/688 bp) similarity with D. nihonkaiense and only 93.2% (641/688 bp) similarity with D. latum. Thus, we assigned this tapeworm as D. nihonkaiense. This is the first report of D. nihonkaiense infection in a family in Korea, and this report includes the 8th pediatric case in Korea. The current report is meaningful because D. nihonkaiense infection within a family is rare.

Kretzschmaria quercicola sp. nov., an Undescribed Fungus from Living Oak in Mt. Daeryong, Korea

  • We encountered an unfamiliar ascomycete fruiting body, fitting characteristics of the genus Kretzschmaria, which features in a stipitate ascigerous stroma with carbonaceous interior and disintegrating perithecia. In this study, we report and characterize a new species of the decaying fungus. Compared to other species, one of the notable features of this specimen (TPML150908-046) is its stromatal size (up to 15 cm). Although TPML150908-046 is morphologically similar to K. milleri and K. sandvicensis, it differs sharply from both species in apical ring size (TPML150908-046, $6.5{\sim}10.5{\mu}m$; K. milleri, $11{\sim}16{\mu}m$) and ascospore width (TPML150908-046, $10.5{\sim}17{\mu}m$; K. sandvicensis, $8.5~11.5{\mu}m$). Phylogenetic trees based on ${\beta}$-tubulin, ITS, and RPB2 sequences showed that our collection clustered with K. sandvicensis, with the respective similarities for these sequences being 95.6%, 91.3%, and 97.7%, signifying it as another species. With these results, we report it as a new species, which we call Kretzschmaria quercicola sp. nov.

Slowly Recovering Isolated Bilateral Abducens Nerve Palsy after Embolization of Ruptured Anterior Communicating Artery Aneurysm

  • Bilateral abducens nerve palsy related to ruptured aneurysm of the anterior communicating artery (ACoA) has only been reported in four patients. Three cases were treated by surgical clipping. No report has described the clinical course of the isolated bilateral abducens nerve palsy following ruptured ACoA aneurysm obliterated with coil. A 32-year-old man was transferred to our institution after three days of diplopia, dizziness and headache after the onset of a 5-minute generalized tonic-clonic seizure. Computed tomographic angiography revealed an aneurysm of the ACoA. Magnetic resonance imaging showed focal intraventricular hemorrhage without brain stem abnormalities including infarction or space-occupying lesion. Endovascular coil embolization was conducted to obliterate an aneurysmal sac followed by lumbar cerebrospinal fluid (CSF) drainage. Bilateral paresis of abducens nerve completely recovered 9 weeks after ictus. In conclusion, isolated bilateral abducens nerve palsy associated with ruptured ACoA aneurysm may be resolved successfully by coil embolization and lumbar CSF drainage without directly relieving cerebrospinal fluid pressure by opening Lillequist's membrane and prepontine cistern.
