Purpose: The novelty of intensive mobility training (IMT) is its intensive nature. The purpose of this study was to examine the effect of IMT in patients with Parkinson's disease. Methods: Subjects participated in 3 hours/day for ten days (30 hours). Gait parameters of interest were the timed up-and-go test, 10-m walk test, and step length and width. Measures were made at baseline before commencement of training (pre-training) and at the end of the two-week training period (post-training). Results: Seven patients with Parkinson's disease enrolled in the study. On average, participants are able to tolerate 141 minutes of activity during a 180-minute session. Results showed that, after 10 consecutive days training, subjects significantly improved for all parameters; the timed up-and-go test, 10-m walk test, and stride length and step width. Conclusion: This study's findings show that gait properties in patients with Parkinson's disease can be improved with IMT.
Body weight support treadmill training is a new and promising therapy in gait rehabilitation of patients with hemiplegia. The purpose of this study was to identify the effects of body weight support treadmill training on gait and standing balance in patients with hemiplegia. Eighteen patients with hemiplegia participated in the study. A 10 m-timed walk test, measurements of step length and standing balance score were administered. Intervention consisted of body weight support treadmill training five times a week for 2 weeks. The data were analyzed by paired t-test. Body weight support treadmill training scoring of standing balance, step length and 10 m-timed walk test showed a definite improvement. Body weight support treadmill training offers the advantages of task-oriented training with numerous repetitions of a supervised gait pattern. The outcomes suggest that patients with hemiplegia can improve their gait ability and standing balance through body weight support treadmill training.
Purpose : This study aimed to conduct a comprehensive meta-analysis to evaluate the effect size for pulmonary function and gait capacity of treadmill exercise in stroke patients. In addition, we aimed to examine the current status of treadmill interventions and compare the effect sizes of treadmill training to provide evidence-based practice for future development and application. Methods : The meta-analysis study was conducted using research articles that applied treadmill exercise to stroke patients and were published between January 2005 and February 2020. For a systematic meta-analysis, we conducted a search using the PICOS framework and selected 25 domestic stroke- and treadmill-related studies. The Comprehensive Meta-Analysis program was used to calculate the effect sizes for lung function and gait capacity (6-minute walk test and 10 m walk test). As Cohen's d has a tendency to overestimate the effect size, we used Hedge's g to increase the accuracy in computing the effect size. Results : Based on the results of the meta-analysis, the total effect size of treadmill exercise was 0.535, which was statistically significant, with a medium effect size (p<0.05). The effects of treadmill exercise in stroke patients were divided into dependent variables, namely pulmonary function (0.372) and gait capacity (0.584). In addition, for gait capacity, the effect sizes were evaluated for both the 6-minute walk test (0.756) and the 10 m walk test (0.514). Conclusion : This study provides objective evidence of the effectiveness of treadmill-based gait training programs. We hope that the results of this study will help support the development and implementation of treadmill-based gait training in stroke patients. Treadmill training is expected to improve not only pulmonary function, but also the gait capacity of stroke patients. Long-term investigations on the effects of treadmill training in stroke patients are necessary.
Background: There are many situations where walking in an actual community needs to change direction along with walking on a straight path, and this situation needs to be reflected in assessing walking ability of the community. Therefore, in this study, we tried to determine whether the assessments can distinguish the level of walking in the community. Design: Retrospective cohort study. Methods: Fifty-two survivors with chronic stroke have participated in the study. According to the evaluation result of 10mWT, the subjects of 0.8m/s and above were classified as the group who could walk in the community (n=22), and the subjects of 0.4m/s~0.8m/s were classified into the group who could not walk in the community (n=30). Modified Rivermead Mobility Index, Postural Assessment Scale for Stroke, Fugl-Meyer Assessment, Berg Balance Scale, 10-meter Walk Test (10mWT) were used to evaluate the motor skills. Furthermore, Activities-specific Balance Confidence Scale was used to evaluate psychological factors, and Timed Up & Go Test (TUG), Figure-of-Eight Walk Test (F8WT), Four Square Step Test (FSST), Step Test (ST) were applied to evaluate dynamic balance and mobility. Results: As a result for distinguishing walking levels in the community, TUG was 14.25 seconds, F8WT was 13.34 seconds, FST was 19.43 seconds, and ST of affected side and non-affected side were 6.5 points and 7.5 points, respectively. TUG (AUC=0.923), F8WT (AUC=0.905), and FST (AUC=0.941) were highly accurate, but the ST of affected side and non-affected side (AUC=0.806, 0.705) showed the accuracy of the median degree, respectively. Conclusion: To distinguish walking levels in the community of survivors with chronic stroke, TUG and FSST have been found to be the best assessment tool, and in particular, FSST could be very valuable in clinical use as the most important assessment tool to distinguish walking levels in the community.
본 연구에서는 산사태 발생시 붕괴토사량을 산정하기 위하여 산사태 발생면적과 붕괴토사량의 상관관계를 활용하고, 붕괴토사량의 총 이동거리를 예측하기 위하여 붕괴토사량과 붕괴사면의 높이 및 붕괴토사의 도달거리를 활용하였다. 그리고 토석류의 이동경로를 예측하기 위하여 붕괴토사량의 유동 및 퇴적특성을 경사도의 인자로 단순화시킨 랜텀워크 모델을 적용하였다. 산사태 발생지점에서 이동경로 및 피해범위를 산정하기 위하여 토석류 이동 확률계산을 몬테카를로 시뮬레이션을 통하여 10,000회 반복적으로 수행하였다. 이때 계산된 다양한 랜텀워크의 궤적을 피해영역으로 제시하였다. 제안된 랜텀워크 모델을 이용한 산사태 피해범위 산정기법의 정확도를 확인하기 위하여 지리산 천왕봉 일대에서 발생된 산사태 발생이력을 적용하였다. 제안된 모델의 적용성을 검토한 결과 비교적 정확하게 피해범위를 산정하는 것으로 나타났으며, 10 m × 10 m 크기의 셀을 활용하는 것이 실제 피해범위에 대한 정확한 재현이 가능한 것으로 확인되었다.

Objective: Pilates is a low/mid-intensity exercise that can be easily performed by elderly individuals as it is an individual body-oriented exercise. It is also a cardio workout that can be performed anywhere to develop strength and flexibility. Therefore, we investigated the effects of 8 week Pilates program on the balancing ability of elderly individuals. Method: The research participants were selected from elderly residents in B city. Ten individuals voluntarily signed an agreement to undergo free measurements as well as to participate in the workout program. (Height: $157.1{\pm}11.9cm$, Weight: $61.7{\pm}8.0kg$). The Pilates exercise was performed 60 minutes a day, three times a week for a total of eight weeks. The measurement variables used to test balance were the vestibular test, 5 m habitual and maximum walk test and 3 m tandem walk test. A series of paired t-test were conducted using IBM SPSS Statistics 23.0 to analyze all the research data collected in order to determine the balance ability of the participants before and after the Pilates program. Additionally, the statistically significant level for all analysis was set to ${\alpha}=.05$. Results: In the vestibular test, some meaningful changes were observed in the length envelope area (ENV) while standing on one foot, but there were no significant differences in the ENV, rectangle(REC), root mean square, and total length. Results also revealed that statistically significant differences existed in the 5 m habitual and maximum walk test, as well as the 3 m tandem walk test. Conclusion: To summarize the findings, the 8 week Pilates program employed in this study significantly improved the dynamic balance of the elderly participants. Thus, elderly individuals that frequent perform Pilates are expected to enjoy positive benefits such as increased balance and fewer falling accidents.
Purpose: The aim of our study was to evaluate the therapeutic effects of walking and turning plus treadmill training on the functional balance and walking ability of individuals with Parkinson's disease (PD). Methods: Twenty-four participants with Stage 1 to 3 ($2.13{\pm}0.64$) PD based on the Hoehn and Yahr scale were randomly allocated to the experimental group (EG) and control group (CG), with 12 participants in each group. The measured outcomes included the motor subscale of the unified Parkinson's disease rating scale (UPDRS-M), the Berg balance scale (BBS), the Timed Up and Go (TUG) test, the 10-meter walk test (10MWT), and the 6-minute walk test (6MWT). Results: Pre-to-post intervention improvements were noted for all the outcome measures for both groups (p < 0.05). Post intervention, there was a significant improvement in the EG compared to the CG for the following measured outcomes (p < 0.05): UPDRS-M (p = 0.021; 95% CI, 0.081-6.519), BBS (p = 0.042; 95% CI, 1.375-4.541), TUG (p = 0.034; 95% CI, -3.315--0.143), 10MWT (p = 0.011; 95% CI, -2.032--0.289), and 6MWT (p = 0.002; 95% CI, 24.39-91.273). Conclusion: Our study suggests that walking and turning plus treadmill training improves balance and walking compared to treadmill training only in patients with PD.
Objective: Based on the results of previous studies, it is necessary to analyze gait and discuss and present the effects of aquatic exercise for chronic stroke. The purpose of this study was to present objective data on the effect of aquatic exercise on the gait of persons with stroke by performing a meta-analysis. Design: A systematic review and meta-analysis. Methods: We performed a meta-analysis of 23 studies that investigated the effects of aquatic exercise performed between 2006 and 2017. The studies were searched on the basis of the participants, intervention, comparison, outcomes standard. The quality of the research method was assessed using a tool that can assess the risks posed by each study design. A meta-analysis software program was used to calculate the mean effect size, effect size by intervention, and effect size by outcome. We also performed a meta-regression analysis and an analysis of publication bias. Results: The mean effect size of the patients' gait was 0.65 (p<0.05). The largest effect size by outcome was observed at the 6-m walk test, followed by the 6-minutes walk test, 10-m walk test, and the walking equipment test (p<0.05). The meta-regression analysis showed that the effect size increased with increased duration, number, and length of sessions. Conclusions: Aquatic exercise appears to show a moderate effect on the gait of chronic stroke survivors. Meta-analyses on the effects of aquatic exercise in other patient populations are needed. This study suggests standard criteria establishments for the effect of aquatic exercise on the walking ability of persons with chronic stroke.
PURPOSE: This study aimed to analyze the factors influencing the improvement of global synkinesis (GS) and gait in stroke patients by the weight shift exercise. METHODS: Twenty stroke patients were randomly assigned to experimental group I (n = 10) and experimental group II (n = 10). In experimental group I, weight shift with upper extremity exercises in a standing position were mediated and in experimental group II, upper extremity exercises in a standing position were mediated. These interventions were conducted in both groups for three sessions per week, 30 minutes per session over four weeks. Before the interventions, leg GS was measured using surface electromyography and the 10m walk test (10MWT). The same parameters were measured four weeks later. RESULTS: The GS and 10MWT of the experimental group I showed that there was a statistically significant difference between dorsiflexion (p < .05) and plantarflexion (p < .01) and 10MWT (p < .01). In experimental group II, there was a statistically significant difference only in 10MWT (p < .01). The comparison between the changes in both groups showed that in the case of GS, there was a statistically significant difference in the dorsiflexion (p < .01) and plantarflexion (p < .05), and in the 10MWT too (p < .05). CONCLUSION: GS of the lower extremities was decreased by improving posture control and enlarging soft tissues due to the symmetrical weight shift in experimental group I, which turned out to be effective in improving the gait speed.
Background: Hemiplegic patients have the problems of the balance and weight shifting to the affected leg in walking. The aim of this study was to investigate the effect of unilateral stepping exercise combined with auditory feedback on balance and walking ability of the hemiplegic patients. Methods: Thirty hemiplegic patients were allocated in study group (n=15) or control group (n=15). General exercise and weight supporting exercise were conducted for the control group, and general exercises and unilateral stepping exercise combined with auditory feedback were conducted for the study group. Exercise were conducted three times a week for six weeks. Balance ability was measured by Berg Balance Scale (BBS), postural assessment scale for stroke (PASS), and performance-oriented mobility assessment (POMA). Walking ability was measured by time up and go test (TUG), 10m walk test (10mWT), and six minutes walk test (6MWT). Results: Balance and walking ability were significant increased in both group (p<.05). Balance and walking ability of the study group were more increased than that of the control group (p<.05). Conclusions: Unilateral stepping exercise combined with auditory feedback is more effective than weight supporting exercise to increase on balance and walking ability for the hemiplegic patients.
