  • Long-term exposure of cigarette smoke or air pollutants to human can induce damages in the airway mucociliary function and can be closely related to the irritation and sputum formation in the respiratory system. This study was undertaken to investigate whether rat trachea can be used as a tool for evaluating the cigarette smoke quality. It was identified that, through the examination with inverted microscope, ciliary beating in 1 mm long cut of nat trachea ring was continued for at least 48 hours in saline solution at $25^{\circ}C$. The ciliostasis time in a KCN solution as a positive control was decreased with increasing the concentration of KCN. There were no significant differences in the ciliostasis time by body weight and individual variation of rats. Ciliotoxicity of whole smoke trapped in saline was not significantly changed by aging for more than 6 hrs. The ciliostasis time was in inverse proportion to the number of sample cigarettes applied. As moisture in the cigarette was increased, ciliostasis time was linearly increased. Therefore, these data indicate that the ciliotoxicity test using rat trachea in vitro can be applied to evaluate the cigarette smoke quality and to search factors for the irritation and sputum formation by cigarette smoke as well as air pollutants.

  • Papillary thyroid carcinoma is known as its relatively high cure rate after surgical treatment. But invasion of the trachea by thyroid carcinoma is poor prognostic factor and the best management is en bloc surgical resection of the tumor invading the trachea. A 55-year-old man was diagnosed as papillary thyroid cancer with tracheal invasion. We treated the patient by total thyroidectomy with window resection of invading trachea followed by immediate reconstruction with sternohyoid muscle flap and tracheostomy. At 48 days after surgery, tracheostoma was closed and the patient had no functional complication by the surgical process. Until 10 months after surgery, there was no sign of recurrence and the patient led social life without any discomfort. We present this case with a review of the related literatures.

  • Primary neoplasms of the trachea are rare, but are a very serious critical life-threatening disease. Nearly all the lesions of the trachea are presented as obstructive lesions. Bronchoscopic examination including chest C-T, tomogram and air tracheogram are essential for the further definition of these lesions. The need for removal of tracheal tumors whether complete or incomplete, is clear enough regardless of the histology of the tumor. We have experienced 9 cases from Jan. 1965 to June, 1985. One patient with tracheal hamartoma was cured with complete resection through rigid bronchoscopy and another patient with fibrous histiocytoma was treated with re-excision and laser evaporation through superior mediastinotomy due to recurrence, 1 year later. The remaining patients were treated with mass excision or segmental resection and end-to-end anastomosis through collar incision and superior mediastinal sternotomy. The remaining two patients were operated with and segmental resection and end-to-end anastomosis of trachea using partial cardiopulmonary bypass. The histologic diagnosis were adenoid cystic Ca[5], fibrous histiocytoma[1], mucoepidermoid Ca[1]. hamartoma[1], anaplastic Ca.[1]. Three patients were treated post-operatively with radiation; with adenoid cystic Ca.[2] and anaplastic Ca.[1]. Their post-operative courses were uneventful during the follow-up from 2 months to 7 years.

  • Tracheal rupture by a blunt trauma is an uncommon injury, and its clinical presentations are variable. It is a kind of the modern hazard. Herewith, we report a successful management of the tracheal rupture. A 22 year-old female was transferred from other hospital 4 hours after a car crash. Physical examination, simple chest X-ray, Chest CT and fiberoptic bronchoscopy revealed rupture of the membranous portion of the trachea about 5cm in length extending to the right main bronchus. Ruptured membraous portion of the trachea was sutured directly with absorbable suture. Her postoperative course was uneventful, and follow-up fiberoptic bronchoscopy revealed intact membranous portion of the trachea.

  • Primary benign tracheal tumors are exceedingly rare and leiomyoma has the least frequency among them. It is important to recognize early without misdiagnosis such as asthma because it is curable. A 41 year-old female was admitted for dyspnea and choking since November 1991. Under the impression of asthma, she received medication. Symptom was not improved and thus chest CT scan was performed. There was endotracheal tumor mass which was located just above the carina and arose from the right lateral tracheal wall with broad base. We successfully resected the tumor mass including trachea and the defected area of trachea was reconstructed with autologous graft using pericrdium & rib cartilage. During the follow up period, no complication was developed.

Bronchoscopic examination including chest CT, tomogram and air tracheogram are essential for the further definition of these lesions. This is a case report of adenoid cystic carcinoma in a 47 years old male patient. The tumor was located in cervical trachea with wide base and obstructing the lumen almost completely. The patient`s symptom was productive cough and hoarseness for 4 months. The tumor was resected and End-to-End anastomosed. The tumor was confirmed to be adenoid cystic carcinoma histopathologically. The need for removal of tracheal tumor whether complete or incomplete, is clear enough regardless of the histology of the tumor. This patient was treated post-operatively with radiation.

  • Background and Objectives : The aim of the this study is to determine the efficacy of an external prosthesis made of ringed Polytetrafluoroethylene to prevent Postanastomotic stenosis after surgical correction of extensive tracheal defects in rabbits. Materials and Methods : Thirty rabbits were used, divided into two groups of 15 animals each. Group A rabbits underwent resection of six-ring segments of the cervical trachea and tracheal end-to-end anastomosis. The Procedure used in group B was similar to that used in group A. but the tracheal anastomosis was supported by an external ringed polytetrafluoroethylene prosthesis. After six months, rabbits were killed and tracheas were resected and then compared the postanastomotic tracheal stenosis using morphometry. Results : Anteroposterior diameter, transverse diameter, cross sectional area and intra luminal perimeter of trachea was greater in group B than group A. Also inflammatory changes of mucosa and submucosa were greater in group A than group B. Conclusion : A ringed PTFE as a external stent was effective to prevent tracheal stenosis resulting from the extensive tracheal resection and tracheal reconstruction in rabbits.

  • This experiment was performed in order to study the effects of haepyoyijintang on the injured tracheal tissue induced $SO_{2}$ in rats. Healthy adult male rats weighting about 250g were divided into 4 groups-the Normal group, the Control group, the group of Haepyoyijintang administration for 5days after $SO_{2}$gas exposure (Sample I), and the group of Haepyoyijintang adiministration for 10 days before and for 5days after $SO_{2}$ gas exposure (Sample II). The results were obtained as follows; 1. In the trachea Control group, the lesion of the ciliated epithelium was and the mucus secretion of the respiratory tract was increased iginificantly. 2. In the trachea of SampleIgroup, the lesion of the ciliated epithelium and the mucus secretion of the respiratory tract were decreased compared with Control group. 3. In the trachea of SampleIIgroup, the lesion of the ciliated epithelium and the mucus secretion of the respiratory tract were decreased compared with Control and Sample Igroup. According to the above results, Haepyoyijintang has significant effects on the injuried tracheal tissu caused by $SO_{2}$ in rats.

  • A glomus tumor is composed of modified smooth muscle cells that are similar to those of the glomus body. Its occurrence in the trachea is quite rare. To our knowledge, only 20 cases of glomus tumor of the trachea have been reported worldwide and there has been only 1 case in Korea. We report one case of a 58-year-old man with dyspnea who had a glomus tumor in the upper trachea with a review of literatures. The chest CT scan and bronchoscopy demonstrated a 2.5 cm sized lobulated tumor at the posterior wall of the upper trachea. It had an elongated shape with a broad base and with a highly vascularity. A simple resection of 3 levels of the trachearing was with a tumor and end-to-end anastomosis performed. Microscopic and immunohistochemical staining of the tumor revealed the characteristics of a glomus tumor.
