  • Clinical results with the xenograft cardiac valves were reviewed for 212 patients who underwent heart valve replacement from January 1981 to December 1987. One hundred and twenty-four Carpentier-Edwards k 88 Ionescu Shiley valves were used. Overall operative mortality was 11 out of 212[5.1%]: 5 out of 153[3.39o] for mitral valve replacement [MVR], 2 out of 34[5.9%] for aortic valve replacement [AVR], 0 out of 4[0%] for Tricuspid valve replacement [TVR], and 4 out of 21[19.1%] for double valve replacement [DVR;MVR+ AVR]. Two hundred and one operative survivors were followed up for a total of 824.3 patient-years [a mean 3.9*1.8 yrs], and the follow up was 78.1%. The linealized complication rates were 0.1% emboli / patient-year, 1.0% endocarditis/ patient-year and 2.2% overall valve failure / patient-year. A linealized rate of primary tissue failure was 0.7*/o/ patient-year. The actuarial survival rates including the operative mortality were 92*2.8% at 4 years and 85*4.3% at 7 years after surgery using the Xenograft cardiac valves. Probabilities of freedom from thromboembolism and overall valve failure were 73*11.0% and 69*2.4% at 7 years after surgery using the Xenograft cardiac valves respectively. The intrinsic durability of the Xenograft cardiac valves appears to be relatively well satisfactory over the long term [4 to 7 years] and the risk of failure appears well balanced by the advantages of a low incidence of thromboembolism and no mandatory anticoagulant therapy.

We classified 377(male 146, female 231)eyes showing good naked vision at least 1.0, with a spherical equivalent refraction of under ${\pm}0.50D$ and cylindercal refraction of under -0.75D, from 10 to 59(mean $25.8{\pm}12.4$) years into 5 groups with 10 year interval and analyzed change of the near astigmatism and accommodative response amount to distinction of age. Age comparison, total mean 1.47D, 10 years age group 1.63D, 20 years age group 1.57D, 30 years age group 1.71D, 40 years age group 0.97D, and 50 years age group 0.05D. It was no difference between 10 to 30 years age group, But it showed the decre8.se which is remarkable from above 40 years age group and it does not almost response from above 50 years age group. Sexual difference, the man appeared some highly in male(mean $26.4{\pm}12.3$ years) 1.56D m female(mean $26.4{\pm}12.3$) 1.43D. Near-astigmatism power change, mean +0.12D, 10 years age group +0.06D, 20 years age group +0.16D, 30 years age group +0.09D, 40 years age group +0.05D, and 50 years age group +0,06D. No significant difference of astigmatism powers among the an groups, but from all years age group the astigmatism power appeared more highly far-astigmatism than near-astigmatism. Near-astigmatism axis change in the far-astigmatism changes at under ${\pm}10^{\circ}$ 103 eyes, above ${\pm}10^{\circ}$ rotation clockwise direction 108 eyes, above ${\pm}10^{\circ}$ rotation counter-clockwise direction 101 eyes. It was analyzed near-astigmatism axis change.

  • Purpose: Gastric cancer often occurs in the elderly but is uncommon in young individuals. Whether young patients have different clinical behaviors and outcomes from those of older patients remain unclear. Materials and Methods: We identified 1,366 cases of newly diagnosed noncardia gastric adenocarcinoma from the Kaiser Permanente Northern California Cancer Registry between 2000 and 2010. We then compared the clinicopathological features and survival among the different age groups. Results: The male : female ratio differed significantly between the younger and older patient groups (0.84 in age <50 years vs. 1.52>60 years, P<0.01). More younger patients were Hispanic (54% patients <40 years vs. 19% patients ${\geq}70$ years, P<0.0001), while more older patients were Caucasian (49% patients ${\geq}70$ years vs. 15% patients <40 years; P<0.0001). The diffuse/mixed histological type was more prevalent in younger patients (70% patients <40 years vs. 27% patients ${\geq}70$ years; P<0.0001), whereas the intestinal type was more frequent in older patients (71% in patients ${\geq}70$ years vs. 30% in patients <40 years; P<0.0001). Poorly differentiated adenocarcinoma was more common in the younger patients (80% in patients <40 years vs. 60% in patients ${\geq}70$ years; P=0.016). Survival rates at 1, 2, and 5 years gradually declined with increasing age (overall P=0.0002). Conclusions: Young patients with gastric cancer had more aggressive disease but higher overall survival rates than older patients. Younger Hispanic patients and older Caucasian patients were more likely to be diagnosed with gastric cancer. These differences may be due to biological predisposition and/or environmental exposure.
