  • The use of the finite element method for biomechanical analysis is increasing rapidly in recent years. Since biomechanical models are usually in very complex shapes, it takes a lot of time and efforts to build reasonable finite element models. In this paper, a new tetrahedral meshing algorithm from the series of 2-D computed tomography(CT) images has been proposed. In this scheme, the planar sections of three-dimensional objects and the side surfaces between two planar sections are triangulated first, and then an advancing front algorithm is employed to construct tetrahedral elements by using basic operators. A sample finite element model for thoracic vertebra is presented.

  • 목뼈는 척주 중에서 두개골과 등뼈(흉추) 사이의 부분으로 목 부분을 형성하는 뼈 구조물이다. 위치는 머리뼈(두개골) 밑에서부터 갈비뼈가 붙는 등뼈(흉추)의 윗부분까지를 이른다. 본 논문에서는 목을 구부려 턱이 가슴에 닿도록 하는 굽힘과, 목을 들어 천장을 보는 것처럼 자세를 취하는 폄에 대하여 각을 측정한다. 굽힘의 각도는 $45{\sim}50^{\circ}$가 기본이며 최대의 가동 범위는 $80{\sim}90^{\circ}$이다. 폄의 각도 또한 $40^{\circ}{\sim}50^{\circ}$가 기본이며 정상은 $70^{\circ}$로 제한된다. 굽힘과 폄의 각도가 기본에 못 미치거나 통증을 유발할 때에는 능동과 수동 돌림의 치료법으로 치료하도록 한다.

  • Tuberculous spondylitis is the most common manifestation of musculoskeletal tuberculosis (TB). The progression of the disease is usually slow and insidious. The main symptom, back pain, is not specific, which frequently results in a delayed diagnosis resulting in neurologic deficits and more advanced vertebral destruction. It is more difficult to diagnose the disease if the involved area is an uncommon sites, such as the upper thoracic, cervical or sacral region. It is important to make an early diagnosis and treatment to achieve a better treatment outcome. We reported a 29 year old female with upper thoracic TB spondylitis(T2-8) and pulmonary TB complaining of back pain that persisted for 5 months and fever. TB spondylitis was not suspected to be due to upper thoracic involvement despite her pulmonary tuberculosis. Chest CT for the evaluation of pulmonary TB found T5 destruction and the paravertebral abscess that was consistent with TB spondylitis. Her spine was examined by MRI, which made an early diagnosis before the neurologic deficit had developed. She was treated with surgical intervention due to the spinal instability and anti-TB medication for 1 year with excellent results.

Immediate Effects of the Downhill Treadmill Walking Exercise on Thoracic Angle and Thoracic Extensor Muscle Activity in Subjects With Thoracic Kyphosis (내리막 경사로 트레드밀 걷기 훈련이 흉추 뒤굽음증의 흉추각도와 흉추기립근 활성도에 미치는 영향)

  • Background: In previous studies, changes in postural alignment were found when the slope was changed during walking. Downhill walking straightens the trunk by shifting the line of gravity backward. Objects: This study investigated the effect of the downhill treadmill walking exercise (DTWE) on thoracic angle and thoracic erector spinae (TES) activation in subjects with thoracic kyphosis. Methods: A total of 20 subjects with thoracic kyphosis were recruited for this study. All the subjects performed the DTWE for 30 minutes. A surface EMG and 3D motion capture system were used to measure TES activation and thoracic angle before and after the DTWE. Paired t-tests were used to confirm the effect of the DTWE (p<.05). Results: Both the thoracic angle and TES activation had significantly increased after the DTWE compared to the baseline (p<.05). An increase in the thoracic angle indicates a decrease in kyphosis. Conclusion: The DTWE is effective for thoracic kyphosis patients as it decreases their kyphotic posture and increases the TES activation. Future longitudinal studies are required to investigate the long-term effects of the DTWE.

Evaluation of quantitative on T-spine exhalation technique and T-spine breathing technique of natural breathing (T-spine exhalation technique과 자연스런 호흡 상태에서 촬영하는 T-spine breathing technique의 정량적 평가)

  • Measurements of CNR(Contrast to Noise Ratio) and SNR(Signal to Noise Ratio) of T-spine breathing technique (TBT) using spontaneous breathing and T-spine exhalation technique (TET) with full exhalation were carried out, and with which the more appropriate method was suggested. Both TBT and TET were examined in a sample of fifty-three patients who visit to our hospital for spinal disease from June 2012 to November 2012. All images were evaluated with CNR measured from the differences between the mean pixels and contrast density as setting ROI of spinous process, pedicle, vertebral body, intervertebral foramen, and intervertebral disk using Image J. SNR was measured with the mean pixels and the standard deviation as setting ROI of vertebral body using Image J. In CNR comparison and SNR comparison of TET and TBT, TBT was indicated as excellent in ROI of pedicle, vertebral body, intervertebral foramen and intervertebral disk, and statistical analysis were significant(p<.01). As TBT indicated excellent images compared to the existing T-spine lateral radiography, T-spine lateral radiography would be reestablished and significant as applying to various medical institutions.

A Thoracic Spine Segmentation Technique for Automatic Extraction of VHS and Cobb Angle from X-ray Images (X-ray 영상에서 VHS와 콥 각도 자동 추출을 위한 흉추 분할 기법)

  • In this paper, we propose an organ segmentation technique for the automatic extraction of medical diagnostic indicators from X-ray images. In order to calculate diagnostic indicators of heart disease and spinal disease such as VHS(vertebral heart scale) and Cobb angle, it is necessary to accurately segment the thoracic spine, carina, and heart in a chest X-ray image. A deep neural network model in which the high-resolution representation of the image for each layer and the structure converted into a low-resolution feature map are connected in parallel was adopted. This structure enables the relative position information in the image to be effectively reflected in the segmentation process. It is shown that learning performance can be improved by combining the OCR module, in which pixel information and object information are mutually interacted in a multi-step process, and the channel attention module, which allows each channel of the network to be reflected as different weight values. In addition, a method of augmenting learning data is presented in order to provide robust performance against changes in the position, shape, and size of the subject in the X-ray image. The effectiveness of the proposed theory was evaluated through an experiment using 145 human chest X-ray images and 118 animal X-ray images.

종료종모돈이 3원 교잡종의 육질에 미치는 영향

  • 3원교잡종의 지육지방축척과 육질에 대한 종료종모돈(두록종:D, 버크셔종:B, 매산돈:M)의 영향을 보기 위해, 랜드레이스종과 대요크셔종의 F1(LW) 종빈돈에 D, B 또는 M종의 3품종의 종모돈을 교배시켜 얻은 3원교 잡종(LWD, LWB, LWM)에 대해서 지육의 지방 축적량, 로스육의 물리적 특성과 화학적 조성 또는 근육섬유의 특성, 근육과 피하지방의 지방산 조성을 비교 검토했다. 시험돈은 3원교잡종 각각에 대해서 거세돈 수컷과 암컷을 각각 6두씩, 합계 36두를 공시했다. 단사(單飼), 무제한 급여 하에서 체중이 30kg에서 105kg가 될 때까지 사육했다. 도살한 돼지의 지육우반환(枝肉右半丸)의 제5-6, 8-9, 11-12, 13-14 흉추(胸椎) 간, 제16흉추-제1요추(腰推) 또는 최후요추-제1선추(仙推)간의 횡단면지방의 면적비율은 어떠한 부분에도 LMW이 LWD, LWB보다 통계적으로 유의가 많았다. 로스육의 물리적 특성치인 Tenderness는 $LWM961.3Kgw/cm^2)$ of $LWD(84.13KgW/cm^2)$$LWD(87.22KgW/cm^2)$보다 유의적으로 부드럽고, Pliability는 역으로 LWM이 LWD, LWB보다 유의 있게 낮았다. 또 로스육의 근육내 지방함량은 $LWM(3.23\%)$$LWD(2.32\%)$$LWB(2.35\%)$보다 유의성 있게 많았고, 가열 손실율(加熱損失率)은 LWM이 LWD, LWB보다 유의성 있게 높았다. 근섬유경(筋纖維經)은 LWD가 LWB, LWM보다 큰 경향으로 인정되어 지고, 근속(筋束)의 횡단면적은 LWD가 LWB 또는 LWM보다 유의 있게 조밀했다. LWM은 근육 내 지방산인 오레인산이 LWD, LWB보다 훨씬 높았고, 불포화 지방산의 비율이 높았다. 이상의 결과 종료종모돈이 3원교잡종의 육질에 영향을 미친다는 것이 명확하게 되었다.

  • Schwannoma is a rare intramedullary tumor. There has been no reported case about interval aggravation on follow-up MR imaging. We report initial and follow-up MR findings of a thoracic intramedullary schwannoma with syringomyelia and edema in a patient without neurofibromatosis. Intramedullary schwannoma should be included for differential diagnosis of intramedullary tumor even though followup MR imaging shows interval increase in size of the tumor and extent of associated edema to mimic malignancy.

A Bile Duct Cancer Patient Whose Stent Shifted Significantly Over the Course of External Beam Radiotherapy (외부조사 방사선치료 기간 중 총담도 스텐트의 위치 변화가 컸던 사례)

  • The author reports a bile duct cancer patient whose stent shifted significantly from right to left over the course of radiotherapy. The 80-year-old female patient had a short stature with thoracic kyphosis and mutiple spinal compression fractures. She was also emaciated and very lean. By comparing the weekly scanned computed tomography images, the author found her stent to have shifted by more than 4 cm from right to left over the course of external beam radiotherapy. The results of this case study suggest that for a very lean and emaciated kyphotic bile duct cancer patient, the possibility of large interfractional movement of the bile duct or stent during radiotherapy should be considered.
