Postural Aberrations in Female Patients With Chronic Low Back Pain (만성요통 여성 환자들에서 발생되는 자세의 변화)

  • 이 연구의 목적은 만성요통 여성 환자들에서 나타나는 자세 이상을 평가하고 방사선적 변수들과 임상적인 변수들과의 상관성을 알아보는 것이다. 이 연구의 대상자는 만성요통 여성 환자 38명과 대조군 32명이었다. 변수의 측정은 모든 대상자들이 이완된 자세로 기립한 상태에서 방사선 검사를 실시한 후 경추전만각, 흉추후만각, 요추전만각, 그리고 요추경사각을 측정하였다. 대상자들의 연령과 통증기간은 의무기록지를 참고하였다. 자료분석은 요통군과 대조군의 방사선적 변수들 간의 유의한 차이가 있는지를 알아보기 위해 독립된 t-검정을 시행하였고, 방사선적 변수들과 임상적 변수들과의 상관성을 알아보기 위해 피어슨 상관분석을 시행하였다. 연구결과 만성요통 환자군과 대조군사이의 경추전만각(p<.05), 요추전만각(p<.001), 그리고 천추경사각(p<.01)에서 통계학적으로 유의한 차이가 보였다. 통증기간은 요추전만각과 통계적학으로 유의한 상관관계가 있었으며(p<.05), 경추전만각은 흉추후만각과 통계학적으로 유의한 상관관계가 있었다(p<.05). 또한 흉추후만각, 요추전만각, 그리고 천추경사각은 각각 상호간에 상관관계가 있었다(p<.05). 만성요통은 요추만곡의 변화를 초래하여 흉추부 및 경추부의 자세이상을 유발할 수 있다. 그러므로 요통으로 인하여 올 수 있는 신체 전반적인 자세이상을 미리 예측하고 이를 교정하여야만 장기적으로 올 수 있는 합병증들을 예방할 수 있을 것이다.

Differences in Male and Female Spinopelvic Alignments in Middle School and High Schools Students of Gyeongnam Areas in Korea : a Three Dimensional Analysis Using Rasterstereography (경남 일부지역 중-고교생의 성별에 따른 시상면상 척추-골반 정렬의 특성 비교 연구 : Rasterstereography를 이용한 3차원 영상 분석)

  • This study investigated the patterns and correlations of spinopelvic alignments in in middle school and high schools students of Gyeongnam areas in Korea using rasterstereography. Sixty-one subjects were recruited from May to October 2013, the average age of subjects was $16.40{\pm}2.20$ years. In the present results, PSIS ratio was statistically different between the sexes(male $21.15{\pm}2.40$%, female $23.41{\pm}3.28$%)(p<.01) and lordotic angle was statistically different between the sexes(male $33.44{\pm}8.46^{\circ}$, female $38.96{\pm}8.11^{\circ}$)(p<.001), but other parameters were not statistically different between the sexes. However, we verified that lordotic angle was significant correlation separately with pelvic tilt(r=.348), kyphotic angle(r=.609). Surface rotation was significant correlation separately with kyphotic angle(r=-.278), lordotic angle(r=-.256), trunk inclination(r=.493). These finding could be used as basic data research to confirm normal pattern of spinopelvic alignment and balance in health adolescents, and might help understand adolescents with structure problem in spine and pelvis.

A Self-gravitational Acupressure Corrects the Standing Posture in the Sagittal Plane: A Retrospective Study (자가 중력 지압의 시상면 기립 자세 교정 효과: 후향적 관찰 연구)

  • Background: Manual acupressures in the correction and recovery of spinal deformities are known to be effective. However, they rely on therapists' skill and often require a long treatment time. The shortcomings of the manual therapy are anticipated to be effectively mitigated through self-gravitational acupressures (SGA) which stimulate multiple acupoints simultaneously with acupressure tools. Objective: The study aims to verify the effect of SGA for the correction of sagittal standing posture. Methods: A retrospective analysis was conducted on the postural improvements of the 93 subjects who underwent the SGA intervention, leveraging self-gravity for stimulating neck, back, pelvis, and calf areas both overall and sequentially using a set of acupressure tools equipped with multiple acupressing rods. Degree of posture abnormality was assessed before and after the SGA intervention using the photographic images of the subjects' sagittal standing postures, based on the angles of inclination of the upper body (from external auditory meatus to pelvis) and the lower body (from pelvis to malleolus) from the gravitational line passing through the hip joint. Results: After the SGA intervention, the upper body inclination angle was observed to decrease from an average of 3.2° to 1.6° (50.0% reduction, p<.001), and the lower body inclination angle decreased from an average of 3.5° to 3.2° (8.6% reduction, p<.01). Conclusion: The observed results underpin that SGA is effective for the correction of postural deformities. For the clinical application of SGA, prospective research is needed to optimize SGA protocol and acupressing tools and to validate long-term clinical efficacy.

Evaluation of the Effect of Location and Direction of the Scoliotic Curve on Postural Balance of Patients with Idiopathic Scoliosis (특발성 척추측만증 환자의 척추 만곡 위치와 방향이 자세 균형에 미치는 영향성 평가)

  • This study examined the effects of the location and direction of the scolioti curve on postural balance in patients with idiopathic scoliosis. Fifteen subjects were divided into three groups: right thoracic curve group, left lumbar curve group, and double curve group. The dynamic trunk motion (angle variation in the lumbar, thoracolumbar, lower thoracic and upper thoracic region) and plantar pressure distribution (maximum force and peak pressure) were assessed using an ultrasound-based motion analysis system and Emed-at platform system. From the results, it was confirmed that patients with idiopathic scoliosis showed postural imbalance with an increased angle and pressure asymmetry according to the location and direction of the scoliotic curve for dynamic trunk motion and plantar pressure distribution. In addition, there were differences in the postural balance pattern between the single curve and double curve groups. Further studies for developing a rehabilitation training device will be conducted to improve the postural control ability and trunk balance as well as treat scoliosis based on the results of this study.
