  • 컴퓨터지원진단(Computer Aided Diagnosis; CAD) 시스템은 방사선 의사들이 흉부 X-ray 영상에서 결절을 탐지하는데 있어 실제적으로 발생할 수 있는 오진율을 줄이고, 폐 결절이 존재하는 폐야에서 결절의 존재 유무를 판단하여 검출을 표시함으로써 진단율을 개선시킬 수 있도록 하였다. 본 논문은 흉부 X-ray 영상에서의 폐 결절을 추출하는데 유전자 알고리즘(Genetic Algorithm)을 이용한 템플릿 매칭(Template Matching) 방법을 제안한다. 제안한 방법은 흉부 X-ray 영상에 존재하는 결절과 레퍼런스 이미지를 매칭시켜 적합도를 계산한 후, 그 값을 통하여 수치가 낮은 개체를 선택하여 높은 개체와 교차시킨다. 그리고 레퍼런스 이미지는 결절이 존재하는 환자 X-ray 영상에서 샘플 노듈을 추출한 후 가우시안 분포를 갖는 512개의 레퍼런스 이미지를 생성하였다. 본 논문에서 사용된 영상은 결절 50개, 비결절 30개와 흉부 X-ray 영상에서 육안으로 판별이 가능한 결절 영상을 20개를 포함하여 총 100개 영상을 사용하였다. 실험 결과 83%의 결절을 자동 추출 하였으며, 가장 적절한 레퍼런스 이미지를 발견하고 이를 흉부영상에 매칭시켜 정확한 결절의 위치를 확인하였다.

Analysis of the Influence of Examination Gowns on the Image and the Suitable Fabrics for Chest AP Examinations on DR X-ray Systems (디지털 X-선 시스템에서 흉부 전·후 방향 검사 시 검사복이 영상에 미치는 영향과 적정 검사복 원단의 분석)

  • The purpose of this study was to analyze fabrics suitable for use as examination gowns to determine whether examination gowns affect imaging during anterior to posterior chest examinations(Chest AP) on a digital X-ray system. Examination gowns in use at five medical centers in Seoul were collected and included modal, tencel, cotton, and rayon fabrics. The selection of fabrics was based on studies that reported fabrics with good tactile, absorbent, stretchable, and wrinkle resistance. Phantoms of five hospital gowns and four fabrics, arranged in overlapping layers from one to eight, were created and examined on a digital X-ray system in both Chest AP examination. The images examined were subjected to a first-step profile analysis, a second-step signal intensity averaging analysis, and a third-step microscopic analysis. The results showed that all nine materials had an increasing impact on the image as the number of layers of fabric increased, with the modal fabric having the least impact on the image in the first, second, and third analyses. In conclusion, as the resolution of digital x-ray systems increases, the impact of examination clothing on the image will increase, and research to find suitable materials for examination clothing will continue to be necessary.

Dose Assessment during Pregnancy in Chest PA Examination (흉부 후전방향 검사 시 임산부의 선량 평가)

  • One of the causes of death for pregnant female is embolism, when a chest PA examination is performed. In addition, due to small doses, the examinations are performed for the purpose of preparing for pre-delivery emergency surgery or basic examination for pregnant female. Evaluating fetal doses through actual measurements is subject to ethical problems, Monte Carlo simulations assesses the organ and fetal doses of pregnant females according to week of pregnancy. The results of the simulations showed that the fetal dose decreased according to weeks of pregnancy and it showed a dose of about 0.1 mGy. The higher the density and thickness of the shielding material, the better the shielding effect. In addition, the dose reduction effect for each shielding material is between 40 and 98%. Afterwards, it is deemed necessary to study the reduction of fetal doses through various shielding characteristics and methods.

CT Densitometry of Normal Tissue and Mass of Lung according to Reconstruction Algorithm (재구성 연산 방식에 따른 흉부의 정상 조직과 종괴의 CT 밀도 측정)

  • Fifty patients with lung mass were studied to evaluate the effect of reconstruction algorithm on the CT number of lung mass and normal thoracic tissues. In each examination, the CT image of the lung mass was reconstructed using soft, standard, detail and bone algorithm. The results were shown as follows 1. the average maximum difference of lung mass density on the ROIs using 4 different algorithms was less than 1HU. 2. The maximum difference in the degree of lung mass enhancement was respectively $0.1{\sim}3.2HU$ (ROI $0.5\;cm^2$), $0.1{\sim}2.8HU$(ROI $3\;cm^2$) and $0.0{\sim}2.1$(ROI $6\;cm^2$). 3. The mean density of the normal thoracic tissues was highest in the bone algorithm, though there was no significant between 4 different reconstruction algorithms(p = 1.00).

Calculation of Effective Dose on Domestic Chest PA X-ray Examinations (국내 흉부 X-선 검사에 따른 유효선량 계산)

  • Research on effective dose analysis of actual conditions of use based on large data is scarce. In this study, the exposure conditions of Chest X-Ray examinations used by 324 medical institutions in Korea were calculated and evaluated using computer simulations. As a result of the experiment, the effective dose in the low energy parameter bands was 0.024 mSv, followed by spleen, adrenal glands, and lung. The effective dose in the high-energy exposure parameter band was 0.123 mSv, followed by height, spleen and adrenal glands. The effective dose was 0.017 mSv when the optimal conditions considered the quality and exposure proposed in Park's study were used. The results of the study will be a reference for chest X-rays and will help reduce patient exposure.

Analysis of Image Quality and Scan Dose when Applying Reconstruction Algorithm Changes to Chest CT Scans (흉부 CT 스캔에서 재구성 알고리즘 변화적용 시 화질과 스캔 선량 분석)

  • In this study, among chest CT examination conditions, the tube voltage was changed to 100 and 80 kVp and the reconstruction algorithm was changed to FBP, ASIR-V, and DLIR to compare and analyze changes in examination dose and image quality. As a result, when applying ASIR-V and DLIR at a tube voltage of 100 kVp, which is lower than the existing tube voltage, the dose is lowered while achieving image quality most similar to that when applying 120 kVp and FBP. especially, DLIR reconstructed images had excellent SNR and CNR at all tube voltages. In addition, the SSIM index was analyzed to be closest to 1, showing the highest similarity to the original image. Therefore, when performing repeated chest CT examinations, the application of DLIR can reduce the examination dose by about 29.7%, which is expected to help solve some of the biggest problems with CT examinations, namely radiation exposure due to the examination.
