Reversed Adipofascial Flap for Hindfoot Soft Tissue Defect combined with Open Calcaneal Fracture: A Case Report (개방성 종골 골절과 동반된 후족부 연부조직 결손에서의 역행성 지방근막 피판술: 1예 보고)

  • Open calcaneal fractures are potentially devastating hindfoot injuries, in which the status of soft tissue envelope is very important. The reversed adipofascial flap has a merit of simplicity and minimal complication compared to free tissue transfer. We report of a case of open calcaneal fracture with soft tissue defect of hindfoot, which was successfully treated with reversed adipofascial flap.

  • The A detailed 3D finite-element analysis model of a human foot has been developed by converting CT scan images to 3D CAD models in order to analyze the distribution of plantar pressure. The 3D foot model includes all muscles, bones, and skin. On the basis of this model and the pressure distribution results, shoes for diabetes patients, which can make the plantar pressure distribution uniform, may be designed through finite-element contact analysis.

  • Microfracture procedure has been widely recognized as the primary surgical treatment for an osteochondral lesion of the talus. However, if symptoms persist after initial surgery, selection of a secondary procedure can be difficult. The author desciribes the advantage and value of autologous osteochondral transplantation as a secondary procedure after failed microfracture for osteochondral lesion with a review of sevral previous published articles.

  • Acupuncture has been widely used to treat a variety of disease and symptoms. But various complications have been reported. Among them, peripheral nerve injuries have been reported less frequently than other complications. The purpose of this report is to describe what we believe to be the first case of delayed superficial and deep peroneal nerve compressive neuropathy caused by fibrotic mass formed by neglected broken acupuncture needle.

  • 당뇨병성 족부병변은 당뇨병성 신경증, 허혈증, 그리고 골관절병 등에 의해서 감염, 궤양 등 하지의 조직이 파괴된 상태로 당뇨병환자의 하지절단, 사망률 등을 높이는 중요한 원인이다. 전세계적으로 하지절단의 가장 많은 원인이 당뇨병성 족부병변이며, 당뇨병환자는 정상인에 비해 하지절단의 위험이 약 15배 가량 높다. 또한 5$\sim$15% 정도의 당뇨병환자가 일생 동안 하지절단에 직면하게 된다고 하고, 한쪽 다리를 절단하면 다른 한쪽도 절단할 확률이 1$\sim$3년 이내에 40%, 3$\sim$5년 사이에 58% 정도이며, 일단 하지절단 후에는 3년 생존률이 50%에 이른다는 보고도 있다.

  • Heterotopic ossification of Achilles tendon is known to be related with history of prior Achilles tendon surgery, trauma, Achilles tendon rupture. We report a case of heterotopic ossification of partially ruptured Achilles tendon and treated by surgical removal of ossification and V-Y advancement with tendon repair.

  • 족부의 교정을 위해서는 비정상적인 상태 및 원인 그 치료법에 대해서 충분히 이해해야 할 것으로 생각되며, 비 척행족에 대해서는 먼저 정확한 원인 인자의 파악과 수술전 평가, 수술의 선택, 수술의 범위등을 정해야 한다. 이렇게 함으로써 변형에 대한 충분한 교정이 가능하며, 수술후 예후가 좋은 척행족으로 될 것이다. 그리고 위에서 밝힌 치료의 원칙은 변하지 않겠지만, 관절 고정술을 대체하여 다른 남은 관절에 영향을 주지 않으면서 더 많은 유연성을 가질 수 있는 기능있는 족부를 만들 수 있는 새로운 수술방법이(관절 치환술등) 더 개발되어 져야할 것으로 생각된다.

  • The surgical repair of an Achilles tendon acute rupture is a proven, traditional treatment for optimal functional recovery. However, concerns regarding complications such as re-rupture, wound problems and infections are driving new techniques, including minimally invasive approaches and nonoperative treatments. If we understand the characteristics and contemplate treatment strategies for possible complications, the surgical repair of the Achilles tendon is an attractive option and can be expected to yield satisfactory functional recovery.
