The Role of Neoadjuvant Chemotherapy for Advanced Stage Wilms Tumor (진행된 Wilms 종양에서 수술 전 항암화학요법의 의의)

  • Purpose : This study was designed to exclude radiation in advanced(stage 3, 4) Wilms tumor (WT) by increasing the chance of complete surgical removal with preceding neoadjuvant chemotherapy, thereby reducing the incidence of late effects. Methods : Between December 1998 and July 2002, we conducted neoadjuvant chemotherapy after needle aspiration biopsy on patients who had advanced WT. If needle biopsy was accessible, we conducted neoadjuvant chemotherapy(vincristine, adriamycin, dactinomycin) for 12 weeks and then performed surgical removal, excluded radiation therapy and conducted postoperative chemotherapy (vincristine, dactinomycin${\pm}$adriamycin). In other cases, we firstly conducted the operation and then performed radiation and postoperative chemotherapy. Results : Of the 17 patients diagnosed as WT, 12 patients had an advanced stage of disease. In two of the 12 patients, initial surgical removal was conducted. The median age of patients was 21 months(5-103 months). Of the 10 the patients who received neoadjuvant chemotherapy, eight patients were stage 1, one patient was stage 2, and the other was stage 3 at operation. In nine patients except one with stage 3 disease, we could perform complete surgical resection and therefore could omit radiation. In four cases we could also exclude adriamycin after operation. All but one patient was alive, disease-free, for a median follow-up of 21 months(9-43 months). Conclusion : After neoadjuvant chemotherapy, we could increase the chance of complete tumor resection, exclude radiation and decrease the intensity of postoperative chemotherapy in selected cases. Long term follow-up is needed to determine whether our method would significantly decrease late effects.

Clinical Characteristics in Patients with Kawasaki Disease Who Received Intravenous Gamma-globulin Retreatment (가와사끼병에서 감마글로불린 재투여가 필요했던 경우의 임상적 특징)

  • Purpose : Although the use of intravenous gamma-globulin(IVGG) in Kawasaki disease(KD) is effective in reducing clinical symptoms and coronary artery complications, 20-30% of patients have persistent or recrudescent fever and ongoing clinical symptoms. In these patients, the additional infusion of IVGG is considered. The authors studied the characteristics of patients who received IVGG retreatment, and compared them with the patients who did not need IVGG retreatment, for determination of IVGG retreatment. Methods : We reviewed the medical records of 117 KD patients who could be followed up at least six months. We studied the conventional laboratory findings, electrocardiogram(EKG), signal averaged ECG(SAECG) and echocardiogram. Results : Twenty three patients had early cardiac complications during the six months of follow-up. Four patients had late cardiac complications after six months. The early cardiac complication rate was higher in the IVGG retreatment group than the single infusion group(P<0.0001). The late complication rate was also higher in the retreatment group(P<0.0001). The patients who received methyl-prednisolone(m-PD) pulse therapy had much higher rates of early and late cardiac complications than those who received a single IVGG infusion. Among the clinical data and laboratory findings, only CRP increased significantly in patients who have had the cardiac complications. The IVGG retreatment group had increased CRP than the single infusion group. Conclusion : The patients with increased initial CRP may have an increased incidence of complications and an increased possibility of IVGG retreatment. We thought that retreated KD patients might have inflammations severe enough to need high dose IVGG as shown by high CRP levels, and IVGG retreatment could not prevent coronary artery lesions sufficiently.

Selection of Forage Corn Varieties Adapted to High Latitude (The South of Mt. Suyang) (고위도 기후대 재배 적합 국산 사료용 옥수수 품종 선발)

  • Since maize (Zea mays L.) originated in central and south America, it requires warm climate conditions throughout its growing season. Growth halts when night-time temperatures drop below 10℃, and the plant may die if temperature reach -1.7℃. Thus, temperature should be maintained between 10 and 30℃ from seeding to maturity. The germination temperature for maize should be at least 8-11℃, whit an optimal range 32-34℃. Since temperature significantly affects the germination rate and period, it plays a crucial role in maize growth. In this study, we evaluated the quantity and feed value of 11 major varieties to determine those best suited for maize cultivation as feed in higher latitude, specifically in Democratic People's of Republic of Korea, below 38 degrees north. A cultivation test was also conducted in Suwon in Republic of Korea, to assess adaptability in areas south of Mt. Suyang. Among the varieties tested, Shinhwangok2 reached silking the fastest, in 65 days, while Gwangpyeongok took the longest at 75 days. The stem length of all varieties exceeded 230 cm. Gwangpyeongok had the tallest stems, while Daanok and Shinhwangok2ho displayed the highest ear ratios. Dacheongok presented the highest values in both dry matter and TDN quantity, with 31,420 kg/ha and 21,66 kg/ha respectively. Pyeonggangok had the highest crude protein content at 8.0%. TDN (%) ranged from 57-68%, with Hwangdaok reaching up to 68%. Based on these findings, Dacheongok and Pyeonggangok appear to be the most suitable varieties for cultivation in terms of both quantity and feed value.

Factors Influencing Use of Mental Health Helping Systems among College Students in Korea (한국대학생의 정신건강 원조체계 활용에 영향을 미치는 요인)

  • Objective: This study examined the use of mental health helping systems among college students in Korea. A behavioral model of health service utilization was applied to examine factors influencing different types of helping systems. Methods: A total of 454 college students from four universities participated in the survey. A self-administered questionnaire measuring help-seeking behaviors including formal health and mental health services, informal helping system, peer group, and family support was used. Results: Respondents frequently turn to family members and peer group when in need, and age, sex, mental Health status, and attitude and knowledge on mental illness were significant factors affecting help-seeking behaviors of Korean students. While older students and those with more severe symptoms were more likely to seek help from formal resources, students with more severe symptoms were less likely to seek help from informal resources. Male students and those with negative attitude toward mental illness were less likely to ask peer groups for help. Conclusions: Study results indicate that informal resources and peer groups can be significant sources of social support for individuals in their late adolescence and young adulthood, however, their role as gateways to professional help is limited. Student counseling centers should take on a more active role in reaching out; implications for developing peer leaders as counselors and self-help groups are discussed.

Differential Excretion of Urinary Eosinophil Protein X after Methacholine Challenge Test in Children with Asthma (천식 환아에서 요중 Eosinophil Protein X의 메타콜린 기관지 유발 시험 전·후의 변화)

  • Purpose : Eosinophil is one of the important inflammatory cell involved in the airway inflammation in childhood asthma. It has been demonstrated that markers of eosinophil activation, including eosinophil cationic protein or eosinophil protein X(EPX), are increased in childhood asthma. Furthermore, they are related to disease activity and are assumed to be helpful in monitoring the treatment effect as urinary EPX(U-EPX) can be obtained easily and in a noninvasive way in children of all ages. Methods : Twenty-five children(22 male and three female) aged $11.87{\pm}3.82$ years with stable asthma were challenged with methacholine and urine was collected from each child during the following periods; before methacholine challenge test(MCT); 0-3 hr after the end of MCT; 4-7 hr after the end of MCT; and 8-24 hr after the end of MCT. Bronchial reactivity was determined by using Dosimeter( Jeager, Germany) with serially diluted methacholine from 0.05 to 25.0 mg. The $FEV_1$ less than 80% of baseline value were classified into positive MCT. U-EPX was measured with a sensitive and specific radioimmunoassay(Pharmacia & Upjohn AB, Uppsala, Sweden). Results were expressed as ${\mu}gEPX/mmol$ creatinine. Results : An early airway response after MCT was associated with an increase of U-EPX excretion for 0-3 hr after methacholine inhalation in comparison with beseline values. Most subjects showed a small increase in U-EPX excretion during late asthmatic response for 4-7 hr, which then decreased to normal level in 8-24 hr. Also, a tendency for a higher increase of U-EPX was associated with a lower threshold of methacholine challenge and a longer duration of asthma. Conclusion : Measurement of EPX in urine is a noninvasive and easy method to assess the severity of airway inflammation in asthmatic children. It may be a helpful index of the events underlying the airway inflammatory responses during nonspecific bronchial challenge, and in monitoring asthma management.

Developmental Difference in Metacognitive Accuracy between High School Students and College Students (메타인지 정확성의 발달 차이 연구: 고등학생과 대학생 데이터)

  • Metacognitive monitoring refers to high dimensional cognitive activities. Understanding one's own cognitive processes accurately can make effective controls for their performance. Brain area related with metacognition is PFC which is completed the order of late and it can be inferred that monitoring abilities is developing during late adolescent. In this study, we explored the developmental difference in monitoring accuracy between high school students and college students using by measuring JOL(Judgment of Learning). Participants was asked that they study Spanish-Korean word pairs and judge their future performance of memory. In the result, people in both groups thought that they could remember word pairs better than their actual performance. Absolute bias scores which mean the degree to predict their performance apart from true scores showed the interaction between subject groups and task difficulty. Specifically, people judged their learning state quite accurately in easy task condition. However, in difficult task condition, both groups showed inaccuracy for predicting their learning and the magnitude of the degree was bigger in the group of high school students.

Effects of Hyperosmolar Stimuli on Activation of Human Eosinophilic Leukaemia EoL-1 Cells (고삼투압 자극이 호산구의 활성화에 미치는 영향)

  • Purpose : Airway dehydration and subsequent hyperosmolarity of periciliary fluid are considered critical events in exercise-induced bronchoconstriction. The aim of this study was to establish if a hyperosmolar challenge could induce activation of eosinophils. Methods : Human eosinophilic leukaemic cell lines, EoL-1 cells were incubated with hyperosmolar solutions for 15 minutes. Activation of EoL-1 cells was monitored by degranulation and superoxide anion production. In addition, we examined surface expression of CD69 and ICAM-1. Results : Hyperosmolar stimuli didn't induce superoxide anion production and degranulation. In addition, EoL-1 cells cultured with hyperosmolar medium at 930 mOsm/kg $H_2O$ resulted in no significant increment in fluorescent intensity of CD69 and ICAM-1 expression compared with results for cells incubated with isomolar medium. Conclusion : We found that hyperosmolar stimuli don't cause activation of EoL-1 cells, but further studies are required to determine the role of eosinophil in the mechanism of exercise-induced asthma.

A Study on the Improvement of Counseling Education for Counseling Capabilities Rising of Military Officers (군 간부의 상담능력향상을 위한 상담교육 개선방안)

  • The purpose of this study is to find improvements in current issues faced in counseling training within the military and also show limits in counseling capabilities of military officers who are in charge of leading and counseling their troops. In reference to previous studies made on this topic, we looked at issues rising from military troops who are mainly in their post-adolescents having difficulties in adopting themselves to military life style. We also studied current status of counseling within the military and the counseling difficulties faced by military officers who are closely related to their troops.Survey and in-depth interviews were conducted to achieve the purpose of this study. 100 military officers were surveyed for the frequency of the counseling sessions, and to supplement the survey, in-depth interviews were conducted 3 military officers and analyzed qualitatively. As a result of the survey and the in-depth interviews 76.5% of the military officers never had counseling training prior to joining the military and only 32.9% received counseling training after joining the military. 61% of the survey participants expressed difficulties in counseling their troops. Most of them expressed difficulties in lack of training and issues on theory based training. For the improvements following have been identified; "understanding the troops", "Identifying the problems", "Execution of the training" and "Techniques for Military Training". This study has a meaning in terms of identifying improvement areas in counseling training in the military through researching into current limits in military counseling and military officers counseling capabilities when the social issue of new troops having difficulties in adopting to military life style are on the rise.

Clinical improvement in a case of atypical infantile onset Pompe disease with enzyme replacement therapy (효소 보충 치료로 호전을 보인 비전형적 영아형 Pompe 병 1례)

  • Pompe disease is a genetic disorder caused by a deficiency of acid ${\alpha}$-glucosidase (GAA). Infantile onset Pompe disease is uniformly lethal. Affected infants generally present in the first few months of life with hypotonia, generalized muscle weakness, and a hypertrophic cardiomyopathy, which is rapidly followed by death, usually by the age of one. The late-onset form is characterized less severe symptoms and prognosis. Therapy for Pompe disease is intended to directly address the underlying metabolic defect via intravenous infusions of recombinant human GAA to replace the missing enzyme. We report a case of atypical infantile-onset Pompe disease that presented symptoms in infancy but had less severe clinical manifestations and improved after GAA enzyme replacement ($Myozyme^{(R)}$, Genzyme Co., MA, USA) therapy. It is very important that pediatricians become aware of signs and symptoms of Pompe disease, such as a nasal voice or a waddling gait at an early stage so that these patients can benefit from appropriate GAA replacement therapy as soon as possible.

Content analysis of the way university students appreciate the meaning of life (대학생이 인식하는 삶의 의미에 대한 내용분석)

  • The purpose of this study is to understand the way university students appreciate the meaning of life. The subjects were 1, 2, 3, and 4 grade students from four universities located in Gyeonggido and Gangwondo Provinces; This study was a descriptive research project conducted from March to June 2015 for the purpose of understanding how university students appreciate the meaning of life. The data, gathered from interviews with a number of university students, who are at the last stage of adolescence, were classified into 102 statements and 30 categories through a series of content analyses of certain subjects, such as the definition and importance of the meaning of life, the endeavor to make life meaningful, the moments they feel the meaning of life, and the methods they adopt to feel the meaning of life. We analyzed the ways in which university students appreciate the meaning of life. We hope this study will provide a small guide for them so that the meaning of their lives can be enriched and their feeling of well-being enhanced. In addition, it is necessary to develop appropriate measures for college students to deal with crises in their life and, thus, the results of this study could be used as an important material for the systematic development of support programs for college students. Finally, this study will provide the basis for multifaceted interventions designed to improve the physical and mental health of college students and help them to adapt to college life.
