Extraction and analysis of rotator cuff tear area Using Clustering Based Quantization (클러스터링 기법 기반 양자화를 이용한 회전근개 건 파열 영역 추출 및 분석)

  • 본 논문에서는 기존의 회전근개 건 파열 추출 방법을 개선하기 위하여 초음파 영상에서 환자 정보를 제거하여 ROI 영역을 추출한다. 추출된 ROI 영역에서 명암 대비를 강조하기 위해 기존의 사다리꼴 형태의 퍼지 스트레칭 기법에서 소속 함수를 개선한 퍼지 스트레칭 기법을 적용하여 힘줄과 연골 영역을 효과적으로 강조한다. 강조된 ROI 영역에서 Max-Min 이진화와 8방향 윤곽선 추적 기법 및 Monoton Cubic Spline 기법을 적용한 후에 라벨링 기법을 적용하여 힘줄 및 연골 영역을 추출한다. 추출된 힘줄과 연골 영역을 이용하여 회전근개 영역을 추출한다. 추출한 회전근개 영역에 SOM 기반 양자화 기법을 적용하여 회전근개 건 파열 영역을 추출한다. 제안된 회전근개 건 파열 영역 추출 방법을 다양한 초음파 회전근개 건 파열 영상을 대상으로 실험한 결과, 제안된 회전근개 건 파열 영역이 기존의 추출 방법보다 TPR 값이 증가되어 회전근개 건 파열 분석에 효과적인 것을 확인할 수 있었다.

  • With increased development of patch graft, the number of repair using patch graft in massive rotator cuff tear has increased. Understanding characteristics of various patch graft might be helpful for selection of type of patch, and to improve the outcomes for the treatment of massive rotator cuff tears using patch grafts. Therefore, this paper reviews various studies dealing with clinical outcomes of rotator cuff repair using diverse patch grafts in massive rotator cuff tears.

Correlation of Pain for Rotator Cuff Disease Using Ultrasonography and Stress (견관절 초음파검사를 이용한 회전근개 질환의 통증과 스트레스의 상관성)

  • We verified reason of pain by rotator cuff disease using shoulder sonography and found a correlation between shoulder pain and stress. To find out the accuracy of sonographic prediction of rotator cuff disease among the patients with shoulder pain we surveyed 184 patients in S hospital in Seoul Korea between January to October 2012. These patients were previously diagnosed with the torn rotator cuff, adhesive capsulitis and impingement syndrom with shoulder pain. In most times, the rotator cuff disease was diagnosed among the physical workers who use shoulder excessively and also in the women in their 50~60 years of age(144 patients, 78.3%). There were significant correlation between rotator cuff disease and the stress of pain, between sonographic prediction and pain(p<.05). There were significance between shoulder pain and stress in daily life according to result for survey of BEPSI-K(p<.05).

A Study on the Diagnostic Usefulness of Ultrasound and Magnetic Resonance Imaging for the Diagnosis of Shoulder Rotator Cuff Tear (어깨 회전근개 파열 진단을 위한 초음파 검사와 자기공명영상 검사의 진단적 유용성 연구)

  • Rotator cuff tears are a leading cause of shoulder pain in adults. Due to the increase in social activities, the number of patients complaining of shoulder pain is increasing, and interest in shoulder diseases is also increasing. With the development of ultrasound equipment, the sensitivity and specificity of diagnosis are high, and it is used to diagnose rotator cuff tears in musculoskeletal disease. Ultrasound is recognized as a complementary method to MRI examination in rotator cuff tears. Therefore, this study aimed to find out the diagnostic usefulness of ultrasound and MRI examinations in the diagnosis of shoulder rotator cuff tears.A retrospective analysis was performed on 262 patients who were diagnosed with final rotator cuff damage by arthroscopy after completing ultrasound and MRI examinations. Sensitivity, feature, positive predictive value, image predictive value, and touch were disassembled for the test results. In addition, the degree of clavicular tear was scored and recorded in 5 stages. Ultrasound examination was similar to MRI examination results for both full-thickness and partial tears, and there was no statistically significant difference. Partial tear test results showed higher positive predictive value and accuracy than MRI test. In conclusion, ultrasound can be fully utilized as a screening test for rotator cuff disease, and it is thought that it will be selected and used clinically according to the patient's constitution and situation.
