Genetics of Hereditary Peripheral Neuropathies (유전성 말초신경병의 유전학)

  • Hereditary peripheral neuropathies can be categorized as hereditary motor and sensory neuropathies (HMSN), hereditary motor neuropathies (HMN), and hereditary sensory neuropathies (HSN). HMSN, HMN, and HSN are further subdivided into several subtypes. Here, we review the most recent findings in the molecular diagnosis and therapeutic strategy for hereditary peripheral neuropathies. The products of genes associated with hereditary peripheral neuropathy phenotypes are important for neuronal structure maintenance, axonal transport, nerve signal transduction, and functions related to the cellular integrity. Identifying the molecular basis of hereditary peripheral neuropathy and studying the relevant genes and their functions is important to understand the pathophysiological mechanisms of these neurodegenerative disorders, as well as the processes involved in the normal development and function of the peripheral nervous system. These advances and the better understanding of the pathogenesis of peripheral neuropathies represent a challenge for the diagnoses and managements of hereditary peripheral neuropathy patients in developing future supportive and curative therapies.

  • TMost of the medical images that are being used today are to confirm the existence of lesions. These kind of medical images can not only be used to confirm lesions but if we could use it for academic studies or for analysis and reference materials for surgical operations, it could offer great help in medical studies. Furthermore, we could reduce the number of errors in treatment through simulations in the surgical operations and treatments. In this paper, the necessary parts regarding the medical image visuals were extracted through the point cloud and by using the information obtained through it, it manufactures the necessary information for academic studies, diagnoses or operations. By designing and realizing the system which makes simulation possible with the manufactured information, the paper attempts to provide a more effective way to approach academic studies or giving appropriate diagnoses in the treatment of patients.

  • Successful treatment of any pathological condition of the shoulder requires an accurate diagnosis. Physical examination represents an integral part of the assessment of shoulder disorders. Pain, loss of strength, decreased range of motion, and instability are the most common symptoms caused by a great variety of different shoulder pathologies. Therefore, a sophisticated clinical approach to the patient with shoulder pathology is essential. A standardized evaluation including a variety of diagnostic tests and clinical assessments improves the diagnostic accuracy by specifically examining one component of the shoulder complex. In most cases a careful physical examination will establish or suggest a diagnosis and special investigative techniques such as imaging procedures can be applied more selectively for confirmation or further evaluation of the pathological entity.

Pericardiectomy by a Left Limited Anterolateral Thoracotomy for Constrictive Pericarditis after Cardiac Surgery -2 case reports- (개심술 후 발생한 압축성 심막염에 대한 좌전측방 소개흉술에 의한 심막절제술 - 2예 치험 -)

  • Although it is a rare complication of cardiac surgery, constrictive pericarditis still remains a difficult problem that needs an appropriate treatment after cardiac surgery. We had two patients with constrictive pericarditis presenting with unexplained right heart failure early after cardiac surgery, and the diagnosis of constrictive pericarditis was made by a specific finding of septal bounce shown in echocardiographic study. On the postoperative 40th day and 31st day, they underwent pericardiectomy by a left limited anterolateral thoracotomy. For one to two weeks since pericardiectomy, the cardiac failure symptoms were gradually relieved. For patients without improvement of the constrictive symptom and sign even with conservative medical therapy for constrictive pericarditis developed early after cardiac surgery, pericardiectomy by a left limited anterolateral thoracotomy is considered as a useful therapeutic mode.

  • Erdheim Chester disease (ECD) is very rare non-Langerhans cell histiocytosis (LCH) which occurs in the skeletal system and multiple organs. As it is progressive, sometimes it causes fatal results. However, it is often misdiagnosed as LCH or multiple bone metastasis and, thus, is very difficult to diagnose. In Korea, only 10 cases were first reported in 1999. In particular, there have been a few orthopedic approaches or reports in English-speaking literatures, and no report has been issued in Korea. The authors performed bone biopsy in patients with knee and lower extremity pain who were referred for the integrated treatment. We attempts to report this diagnosis experience with literature review.

  • Objective : The main purpose of this research is to make a survey of the effective way of the Korean traditional medical care about Industrial accidental patients. Methods : In following research, 40 cases of industrial accidental patients who hospitalized in the Dong-Shin Korean Traditional Medicine Hospital from January 1, 1996 to October 21, 2002 were surveyed. Results : 1. The highest incidence was shown in the age of twenties to fifties as much as 92.5% and the ratio of malr to female was 35:5. 2. The highest type in the industrial is professional negligences. 3. The number of patients via other hospital(82.5%) was much than that of the first visitor to our hospital(17.5%) 4. In admission motives, the patients who had been treated by western medical treatment expressed the dissatisfaction of that treatment, so hoped to be treated by oriental medical styles. 5. CVA was 47.5%, disease of sinews and bones system was 52.5%. 6. Hemiplegia(47.5%) was the most frequent out of all CVA patients symptoms, back pain(32.5%) was the commonest pain region of all the condition of sinews and bones system. 7. At first stage of admission period, Acupunture and Herb-medication was frequently prescribed for hwalhyultonglakgige, but as going to end stage was frequently used for bogi, bohyul, gudam and ansingige. 8. The treatment methods which was used for treating industrial accident was acupunture, cupping therapy, physical therapy.

A Qualitative Study on the Treatment Process Experiences of Patients with Whiplash Associated Disorder Treated with Traditional Korean Medicine - Based on the Grounded Theory Approach - (교통사고 상해 증후군 환자들에 대한 한의치료 경험의 질적 연구 -근거이론 접근방법으로 -)

  • Objectives : The purpose of this study is to understand the treatment process experiences of patients with whiplash associated disorder treated with Traditional Korean Medicine. Methods : This study was based on grounded theory. We recruited a total of 10 participants between the ages of 19 and 65, who were hospitalized at Kyung Hee University Korean Medicine Hospital and received treatment after traffic accidents from January to October 2014. Data was collected from in-depth interviews and notes, and was analyzed using open coding, axial coding and selective coding. Results : The core category of the phenomenon: 'The experiences of the treatment process for patients with whiplash associated disorder treated with Traditional Korean Medicine', was elicited as 'The selection of Traditional Korean Medicine treatment over other treatments for holistic healing that supplement for the deficiencies of Western medical treatment'. Conclusion :' The experiences of treatment process for patients with whiplash associated disorder treated with Traditional Korean Medicine', was elicited as 'The selection of the Traditional Korean Medicine treatment over other treatments for holistic healing that complements deficiency of the Western medical treatment'.

Development of Design Techniques of Plastic Ankle Foot Orthosis for the Hemiplegics(I) (편마비 환자용 플라스틱 단하지보조기의 설계기술개발 (I) - 응력 해석을 통한 접근 -)

  • In this study, a stress analysis is made fur an ankle foot orthosis (AFO) in the view point of structural stability. The investigated AFO is a solid and standard type. To analyze the stress distribution on the neck of AFO, the maximum AFO angular translation data is obtained by gait analysis. The material test of polypropylene is made to obtain the mechanical properties of AFO. The maximum dorsiflexion appears at the midstance in the gait analysis. The experimental angular translation at the top of AFO is about 10.3$^{\circ}$ at mid stance. Three models of AFO with different width of neck are made and analyzed with ABAQUS 6.1. The stress levels and distributions of 3 different width(W$_1$, W$_2$=0.85W$_1$, W$_3$=0.60W$_1$) AFO are investigated. As a result, the standard type(W$_1$) appears to the maximum stresses at the medial edge of cutout area surrounding ankle joint. The maximum stresses of the narrower type(W$_2$) are occured on medial edge and center of ankle. The narrowest type(W$_3$) appears to the maximum stresses at center of ankle. The maximum stresses become smaller as ankle width of AFO is narrower.

Three Cases of Fever Unknown Origin with Lymphoproliferative Features and a Unique Pattern of 18-FDG Uptake on the Fusion PET/CT

  • Evaluation of a fever of unknown origin (FUO) is complex. Recently, PET scanning has been approved for screening in FUO evaluation. We treated three cases of FUO associated with increased FDG uptake in the bone marrow of the femur and tibia on the fusion PET/CT; all three had the same pattern of uptake. Bone marrow biopsies revealed mature lymphocyte and histiocyte infiltration and myxoid changes in one case, and cortical bone involvement in another case. The cases were all young females who had fever with neutropenia and relative lymphocytosis that lasted for several weeks and then remitted spontaneously. Even though the results of the studies were not diagnostic, the unique uptake pattern on PET/CT and the histology might be related to the cause of the illness and should be studied further to assess the association with classic FUO.

Clinical Analysis of Traumatic Diaphragmatic injuries Report of 28 cases (외상성 횡경막 손상 28례 분석보고)

  • The records of 28 patients with traumatic diaphragmatic injuries seen at Masan Samsung Hospital from march 1986 o March 1995 were reviewed. We treated 21 male and 7 female patients ranging in ages from 5 to 68 years. Thc diaphragimatic injuries were due to blunt trauma in'20 cases(Trawc accident 18, compression injury 1, Human trauma 1) and penetrating injuries 8 cases (all stab wound). Most common symptoms were dyspnea 27 (96%), chest pain 26 (93%), abdomianl pain 8 (29%), comatose mentality 36 (11 %). Chest X-ray were elevated diaphragm in 20 cases(71%) find hemothorax in 18 cases (64%) and 25 cascs (89%) were diagnosed or suspected as diaphragmatic inju,rims prcopcrativcly. The repair of 28 cases were performed with thoracic approach in 20 cases, abdominal approach 7, thoracoabdominal approach in 1 cases. There were 5 death (18%) and all death related to the severity of associated injury. (hypovolcmic shock 3, combined head injury 1, renal failure 1).
