Comparison of Gas Exchange Parameters between Same Volume of $N_2-O_2$ and Heliox Inhalation (동일한 상시 호흡량의 $N_2-O_2$ 및 Heliox 투여 시 가스교환지표의 비교)

  • Background: Heliox is known to decrease $PaCO_2$ in patients with increased airway resistance by increasing minute ventilation and reducing work of breathing(WOB). Besides these effect, heliox is expected to decrease functional anatomic dead space owing to improvement of peak expiratory flow rate(PEFR) and enhancement of gas distribution. We investigated whether heliox can decrease $PaCO_2$ even at the same minute ventilation (VE) and WOB with $N_2-O_2$ to speculate the effect of the heliox on the anatomic dead space. Material and Method: The subjects were 8 mechanically ventilated patients with asthma or upper airway obstruction(M : F=5 : 3, $68{\pm}10$years) who were under neuromuscular paralysis. The study was consisted of three 15-minutes phases: basal $N_2-O_2$ heliox and washout Heliox was administered via the low pressure inlet of servo 900C, and respiratory parameters were measured by pulmonary monitor(CP-100 pulmonary monitor, Bicore, Irvine, CA, USA). To obtain the same tidal volume(Vt) in heliox phase, the Vt on monitor was adjusted by the factor of relative flow rate of heliox to $N_2-O_2$. Dead space was calculated by Bohr equation. Results: 1) Vt, VE, peak inspiratory pressure(PIP) and peak inspiratory flow rate(PIFR) were not different between $N_2-O_2$ and heliox. 2) PEFR was higher on heliox($0.52{\pm}0.19$L/sec) than $N_2-O_2$($0.44{\pm}0.13$L/sec)(p=0.024). 3) $PaCO_2$(mmHg) were decreased with heliox($56.1{\pm}14.1$) compared to $N_2-O_2$($60.5{\pm}15.9$)(p=0.027). 4) Dead space ventilation(%) were decreased with heliox($73{\pm}9$ with $N_2-O_2$ and $71{\pm}10$ with heliox)(p=0.026). Conclusion: Heliox decreased $PaCO_2$ even at the same VE and WOB with $N_2-O_2$, and the effect was considered to be related with the reduction of anatomic dead space.

시판 신선편이 채소류의 유통 중 품질변화 조사

  • 신선편이 채소류의 유통 중 품질변화를 조사하고자 대형 할인 매장에서 시판중인 8종의 포장된 제품을 구입하여 4$^{\circ}C$에서 냉장저장하면서 변화를 조사하였다. 선정 품목은 양상추, 양배추, 치커리, 버섯(양송이), 연근, 우엉, 도라지, 마늘로서 총 8가지 채소류를 대상으로 하였으며, 각 시료에 따라 엽채류는 2일, 버섯, 우엉, 도라지는 3일, 마늘은 4일, 연근은 5일 간격으로 다음의 항목을 조사하였다. 중량감소율, 경도, 가용성 고형분, pH를 측정하였으며, 색의 변화는 Hunter ‘a’, ‘b’ 및 ‘L’값으로 나타내었으며 표면의 갈변도를 기계적으로 측정하기 어려운 경우 마쇄 후 여액의 흡광도(420nm)로 측정하여 나타내었다. 이와 함께 포장지 내부의 기체조성 변화 측정을 통해 호흡특성을 함께 조사하였다. 중량감소율의 경우 모든 시료가 현저히 증가하였으나 진공포장된 연근, 우엉의 경우 중량감소율이 낮았으며, 특히 우엉은 호흡률이 높아 저장 6일째에 진공이 해제되었다. 마늘과 우엉의 pH는 감소하였고, 나머지는 전체적으로 증가하는 경향을 나타내었다. 특히 엽채류와 도라지의 O.D value의 증가와 버섯의 L value 감소가 현저하여 이 품목에서 갈변이 빠르게 진행됨을 알 수 있었다.

Effects of Nasal Continuous Positive Airway Pressure Application on Sympathetic Activation : Power Spectrum Analysis of Electrocardiogram in Obstructive Sleep Apnea Syndrome (폐쇄성 수면무호흡증에서 지속적 상기도 양압술 시행이 교감신경계 활성도에 끼치는 영향 : 심전도 스펙트럼 분석)

  • Objectives: Obstructive sleep apnea syndrome(OSAS) is known to be associated with the changes of autonomic nervous system (ANS). Nasal continuous positive airway pressure(nCPAP) treatment was found to correct abnormal ANS changes in OSAS but it remains to be further clarified. We aimed to assess the effects of nCPAP on ANS manifested on electrocardiogram, using spectrum analysis in the subjects with OSAS. Methods: Digital polysomnography was performed in 18 patients with OSAS(mean age $43.7{\pm}16.6$ years ; 17 males, 1 female ; mean respiratory disturbance index (RDI) $48.6{\pm}20.9$) for one baseline and another CPAP nights. From each night, 300 continuous beats of ECGs without artifact were chosen from both stage 2 sleep and REM sleep and they were used for power spectrum analysis. We compared between baseline and CPAP nights the heart rate variability including VLF(very low frequency power), LF (low frequency power), HF(high frequency power), R-R means, R-R variance, and LF/HF ratio, using Wilcoxon signed ranks test. Results: In all patients, nCPAP proved to be effective in relieving apneas and snoring. During nCPAP night compared with baseline night, decreases in VLF(p<0.05), LF(p<0.01), and R-R variance(p<0.05) were found in stage 2 sleep, and decreased LF(p<0.05) was found in REM sleep. No significant differences in each sleep stage were found in other variables between the two nights. Conclusion: Our findings suggest that OSAS increases the activity of sympathetic nervous system and nCPAP application effectively decreases the activity. And nCPAP does not appear to influence the parasympathetic nervous activity in OSAS.

중량별 신체부하가 호흡순환기능에 미치는 영향

  • 본 연구의 목적은 트레드밀 운동시 신체에 가해지는 중량별 부하가 심박수, 환기량, 호흡수, 이산화탄 소배출량, 산소섭취량, 체중당산소섭취량, 호흡교환률 등에 어떠한 영향을 미치는 지를 측정하여 훈련이 나 운동처방에 도움이 될 수 있는 자료를 얻고자 하였다. 피검자들은 성인 남녀 각 5명씩을 대상으로 하였으며, 신체 중량별 부하는 남자에게 안정시, 무부하, 5kg부하, 10kg부하, 15kg무하로 하였으며, 여자 에게는 안정시, 무부하, 5kg부하, 10kg부하로 구분하여 측정하였다. 운동부하는 트레드밀(TC-1200, Technogym, Italy)을 이용하여 중량별로 4km/hr로 3분간 실시하였다. 단계별 운동부하에 의한 인체에 미치는 호흡순환계의 변화를 조사하기 위해 호흡순환계의 변인을 측정 분석한 결과, 다음과 같은 결론 을 얻었다. 남자그룹의 경우는 심박수, 환기량, 이산화탄소배출량, 산소섭취량, 체중당산소섭취량, 호흡 교환률에 대한 중량별 평균차의 검증에서 안정시와 무부하, 5kg부하와 10kg부하, 10kg부하와 15kg부하 사이 에는 유의한 차이가 나타났고, 무부하와 5kg부하 사이에서는 유의한 차이가 없었다. 호흡수에있어서는 10kg 부하와 15kg부하 사이에서만 유의한 변화가 나타났다. 여자그룹의 경우는 심박수, 환기량, 이산화탄소배출 량에 대한 중량별 평균차의 검증에서는 안정시와 무부하, 무부하와 5kg부하, 5kg부하와 10kg부하 사이에서 유의한 변화가 나타났으며, 산소섭취량과 제중당산소섭취량에 있어서는 안정시와 무부하, 무부하와 5kg부하 사이네서만 유의한 변화가 나타났다. 또한 호흡교환률과 호흡수에 있어서는 안정시와 무부하 사이에서만 유의한 변화가 나타났으며, 무부하와 5kg부하, 5kg부하와 10kg부하 사이에는 유의한 차이가 없었다. 이와 같은 결과를 종합해 보면 남자에게는 걷기시에 단계별 무게부하가 호흡순환계에 영향을 미친다고 볼 수 있 으나, 5kg까지의 부하는 호흡순환계에 영향을 미치지 않고 일을 수행할 수 있음을 알 수 있다. 그리고 여자에게는 걷기시에 단계별 무게부하가 운동시 심박수와 환기량, 이산화탄소 배출량에는 영향을 미치고 호흡 교환률과 호흡수에는 영향을 미치지 않음으로보아, 여자에게는 걷기시에 단계별 무게 부하가 호흡순환계에 영향을 받고있음을 알 수 있다.

Effects on the Respiratory Function, Lower Extremity Muscle Activity and Balance for the Wellness of Stroke Patients - Focused on Whole Body Vibration Exercise Combined with Breathing Exercise - (뇌졸중 환자의 웰니스를 위한 호흡기능, 하지근활성도 및 균형에 미치는 효과 - 호흡운동을 결합한 전신진동운동을 중심으로 -)

  • The purpose of study was to compare respiratory function and quadriceps muscle activity in stroke patients by applying inspiratory muscle training combined with whole body vibration. In addition, the purpose of study is to present an exercise method for improving the respiratory function of stroke patients and the function of the lower limb muscles of stroke patients. Totally, 21 patients with Stroke patients were randomly assigned to two groups through clinical sampling. 11 patients who applied whole body vibration combined with respiratory exercise were randomly assigned to Experiment Group I, and 10 patients who applied placebo exercise combined with breathing exercise were randomly assigned to Experiment Group II. And for 5 weeks, 4 days/week, 1 time/day, 4 sets/1 time intervention program was implemented. Before intervention, the respiratory function was measured with a maximum inspiratory pressure meter, the lower extremity muscle activity was measured using the surface EMG, and the balance ability was measured using a bug balance test. And after 5 weeks, the post-test was re-measured and analyzed in the same way as the pre-test. In the comparison of changes within the group of experimental group I, there were significant differences in the activity and balance of the respiratory muscle strength, the biceps femoris, and the anterior tibialis muscle (p<.05). In the comparison of the changes in the experimental group I, there was a significant difference in respiratory strength and balance (p<.05). In the comparison of changes between groups, there was a significant difference in the activity of the biceps femoris and anterior tibialis (p<.01). In the future, research on protocols for respiratory exercise and whole body vibration to improve neuromuscular function is considered to be necessary.

Experimental Estimation of the Effect of Rainfall Interception on Soil Respiration in a Broad-leaved Deciduous Forest in Western Japan (일본 서부 낙엽활엽수림의 차단 강수가 토양호흡에 미치는 영향 평가)

  • To estimate the effect of rainfall on soil respiration, soil $CO_2$ efflux was measured with a manipulation of rainfall interception at a broad-leaved deciduous forest in western Japan from 20 February to 19 November 2001. The diurnal patterns of soil respiration observed at the intercepted subplot ($F_c$) were quite similar to those of soil temperature at 0.2 m depth with a maximum around midnight and a minimum from noon to early afternoon. Such diurnal patterns have not been observed in the previous studies at the same study site under natural conditions (which manifested no clear diurnal variations). Furthermore, the magnitudes of $F_c$ showed substantial differences (e.g., ~50% reduction under water-limited conditions) compared to those of the potential soil respiration under natural conditions ($F_{cal}$). These findings demonstrate that rainfall events not only affected the magnitude of soil $CO_2$ efflux but also modified the vertical structure of soil temperature, thereby altering diurnal patterns of soil respiration.

Respiration Rate in Each Organ of Ginseng Plant (인삼 식물체의 부위별 호흡량)

  • This study was conducted to know the respiration rate in different organ of 4 years old ginseng plant on June 14 and in different root conditions of 6 years old on October 1. Respiration rate of each organ was the increasing order of young berry, leaf, peduncle, stem and root at all temperature(15$^{\circ}C$ to 30$^{\circ}C$). Temperature coefficients of respiration rate from 15$^{\circ}C$ to 30$^{\circ}C$ of each organ were 2.39 in young berry, 1.75 in leaf, 1.57 in root, 1.17 in stem and 1.16 in peduncle. There was no difference between respiration rate of large size root (117${\pm}$8.8g) and that of small size (54${\pm}$4.0g) in 6 years old ginseng. Respiration rate was decreased with the decrease in the water content in root, especially in small size root. And respiration rate of red skin root was higher than that of healthy root.

  • The longer pollen stage grew to flowering stage, the higher anther respiratory rate in vivo became. and it was rapidly increased just before flowering. The anther respiratory rate in vitro showed the first and second peak points after 3-7 days and 9-1l days in culture, respectively, and fastest and highest in Daecheongbyeo with high sporophytic potentiality. It was lower in cold-pretreatment than non-treatment at the early days, but higher from 15 days after culture. The frequency of browning anthers was promoted by cold-pretreatment. The respiratory rate was not different between uncolored and browned anthers at 12 days, but it was higher in browned anthers after 24 days in culture.

The effect of postural abdominal breathing exercise on posture muscle activity and respiratory function in adults in their 20s (자세에 따른 복식호흡 운동이 20대 성인의 자세근육 활성도 및 호흡 기능에 미치는 영향)

  • This research is to find out which position is more effective on exercise. This study compared paricipants in their twenties on sitting position & standing position doing abdominal breathing of their respiration & abdominal muscle activation for 4 weeks. In sitting position, FEV1 was the biggest after breathing exercise. In standing position, the gap was biggest between the FEV1 & forced expiratory flow before and after the exercise. In standing and supine position, Lt. rectus abdominis before exercise and Rt. rectus abdominis after exercise muscle activities were the biggiest. Both posture exercise of repeated measure of analysis result, before and after the exercise of the rectus abdomonis muscle and external oblique muscle were not difference in both muscle activity(p>.05). Left Musculus obliquus externus abdominis before and after physical exercise, it's showed a meaningful difference(p<0.05). Before and after exercise, saw a meaningful difference the efforts of FEV1/FVC(p<0.05). The left external oblique abdominis muscle showed the most muscle activity in the supine position before/after exercising, the efforts expiratory showed the lowest muscle activity.The results from the repeated measures ANOVA regarding to before/after the volume and muscle activity, it showed significant difference between at the right rectus abdominis muscle(p<.05). This study finally gathered data about the muscle activity according to the position. This could be used to find in what position the abdominal breath exercise is the post effective & a new treatment for the patients who has worsened respiration function.

Extracorporeal Life Support with a Twin-pulse Life Support (T-PLS) System (이중 박동성 인공심폐기(Twin-Pulse Life Support, T-PLS)를 이용한 심폐순환보조)

  • A mechanical circulatory support system is a life-saving option for treating acute severe respiratory failure or cardiac failure. There are currently a few types of assist devices and the Twin-Pulse Life Support (T-PLS) system is a kind of pulsatile pump. We report here on three patients with severe life threatening cardiopulmonary dysfunction who had the T-PLS system used as an assist device. The indications for applying the T-PLS system were continuing respiratory or cardiac failure in spite of maximal ventilatory and inotropic support. There were two patients with acute respiratory failure due to infection and one patient with cardiac failure due to acute myocarditis. One respiratory failure patient and one cardiac failure patient survived after applying the T-PLS system for 3 days and 5 days, respectively. The T-PLS system is useful as an assist device and it should be considered before multi-organ failure occurs.
