The Influence of Vocal Cords and Intensity by Hemodialysis in End Stage Renal Disease (말기 신부전 환자에서 혈액투석 치료가 성대 및 강도의 변화에 미치는 영향)

  • Kidney diseases rates in modern Korean society, which is entering an aging society, are growing. These diseases diagnosed at early stage can be cured with the drug or dietary treatment. If, however, at worsening stage, kidney dialysis or transplant is needed. In this context, we analyzed the relationship between kidney and voice by measuring and analyzing the changing rate of the vocal chord and intensity according to hemodialysis. For this, we collected voice samples before and after hemodialysis, and compared and analyzed them through application of vocal analytical element. Namely, based on oriental medical diagnosis theory that voice is quieted down when kidney is abnormal, we collected voice samples before and after hemodialysis, and applied trembling and intensity analysis of vocal chord.

  • 뇌졸중은 뇌에 혈액을 공급하고 있는 혈관이 막히거나 터져서 혈액 공급이 되지 않는 부분의 뇌가 손상되어 신체장애가 오는 질환이다. 이런 뇌졸중은 인구 고령화 및 만성질환의 증가로 인해 매년 환자 수가 증가하고 있고, 우리나라에서 사망원인 중 두 번째로 많은 원인을 차지하고 있다. 요즘처럼 날씨가 쌀쌀해지면서 기온이 낮아지면 혈관이 수축하고 혈류의 혈압이 증가하는 경우가 있어 뇌졸중 예방에 각별히 주의해야 한다.

Stomach Cancer Secondary to Hematologic Diseases (혈액질환에 속발하는 이차성 위암)

  • Purpose: Patients with hematologic diseases such as chronic myeloid leukemia (CML) or chronic lymphoid leukemia (CLL) are known to have an increased chance of acquiring a secondary neoplasm. Stomach cancer is one of the most common malignant diseases in Korea, and we investigated whether the incidence of secondary stomach cancer in patients with a hematologic disease increases, in order to determine if a more intensive screening program for detecting secondary gastric cancer was required. We also investigated the safety of performing a gastrectomy in hematologic disease patients. Materials and Methods: From 1992 to 2006, the medical records of 8376 patients diagnosed with one of the six common hematologic diseases were reviewed. Results: Nine secondary stomach cancers were found among the 8376 patients during the 15-year observation period. No surgical-related complications occurred, and there was no recurrence of stomach cancer if detected early. Conclusion: It seems that a more intensive screening program for detecting secondary gastric cancer in hematologic disease patients is not required, and surgery is not risky in these patients.

The Study on the Elderly Homodialysis Patients' Transition of Health Condition and Health-related Quality of Life (혈액투석노인의 건강상태 변화인식과 건강관련 삶의 질에 관한 연구)

  • The purpose of this study examines the elderly hemodialysis patients' Health-related quality in life(HRQL) and investigates the relationship between HRQL and transition of health condition. The subject were 443 elderly hemodialysis patients(over 65 years old) in Seoul and Gyeonggi province. The Health-related quality of life scale was used in this research, in which were analysed a wide range of statistical descriptive analysis, Pearson's correlation, reliability analysis, multiple regression and the Scheffe test. The results were as follows. The HRQL were statiscally significant difference by Gender, Educational level, Live with spouse, Best supporter, and Number of Disease. Also the factor related to transition of health condition was Number of Disease. The transition of health condition accounted for 13.9% of variance in HRQL and Number of Disease accounted for an additional 7.4% of HRQL. The factors, which influenced finally on the HRQL in the elderly hemodialysis patients, were transition of health condition and number of disease. In conclusion adequate hemodialysis treatment and social services should be provided in accordance with the factors affecting HRQL in order to improve the HRQL for elderly hemodialysis patients.

  • 혈관은 혈액을 온몸으로 순환시키는 관으로 인체의 모든 장기는 혈관을 통해 영양을 공급받는다. 그러므로 혈관 내에 찌꺼기가 쌓이고 두꺼워지면 심근경색, 뇌졸중, 혈류장애 등과 같은 혈관질환 발병률을 높이고 최악에는 사망으로 이어진다. 통계청의 자료에 의하면 작년 한 해 뇌혈관질환이 사망률 2위, 심장질환이 3위를 차지하면서 한국인의 사망원인 대다수가 혈관질환과 관련 있는 것으로 나타났다.

Pulmonary Resection for Invasive Pulmonary Aspergillosis in Hematological Malignancy Patients (혈액암 환자에서 합병된 침습성 폐아스페르길루스증의 수술적 치료)

  • Background: Invasive pulmonary aspergillosis, a frequent fungal infection in immunocompromised patients, is known to have a poor prognosis despite the use of antifungal therapy in leukemic patients. We studied the outcome of surgical resection of invasive pulmonary aspergillosis where bleeding tendency, localized recurrence of infection, and incidence could be reduced. Material and Method: We retrospectively reviewed 14 patients with a hematological malignancy where invasive pulmonary aspergillosis was diagnosed during the 10 years between 1998 and 2007. From the medical records, we reviewed the type and treatment of the hematological malignancy, including the diagnostic methods of invasive pulmonary aspergillosis, the preoperative hematological conditions and their management, and the surgical methods and records. We also analyzed the development of postoperative complications and patient mortality, the recurrence of postoperative invasive pulmonary aspergillosis, and if the patients had a bone marrow transplant. Result: Fourteen patients with invasive pulmonary aspergillosis and a hematological malignancy underwent a pulmonary lobectomy. One patient had a complication of bronchopleural fistula, but there were no other serious complications such as bleeding or wound infection, and none of the patients died postoperatively. Conclusion: We have shown that pulmonary lobectomy is a safe and effective therapy for invasive pulmonary aspergillosis in patients with hematological malignancies that allow further treatment of the hematological malignancy.
