주의력결핍 과잉행동장애는 미국에서 약 200만 명의 어린이에게 나타나는 심각한 만성 신경행동학적 장애이다. 주의력결핍 과잉행동장애 치료의 중심은 약물요법이다. 그러나 현재 환자의 약 30%는 주의력결핍 과잉행동장애를 적응증으로 갖도록 허가된 의약품들을 사용해도 적절한 치료효과를 얻지 못하고 있는 상황이다. Modafinil은 methylphenidate나 amphetamines와 같은 각성제와는 다른 약리학적 기전으로 중추신경계를 항진시킨다. 본 연구는 modafinil의 주의력결핍 과잉행동장애 치료효과에 대한 최신 지견을 얻고자, 1990년부터 2010년 3월까지 MEDLINE에 등재된 논문을 ADHD와 Modafinil이라는 MeSH terms로 검색하여 추출한 자료 중에서 대조군이 사용된 무작위 배정 및 이중맹검 임상연구 사례만을 선별하여 임상적 유용성을 평가하였다. 현재 modafinil은 주의력결핍 과잉행동 장애 치료제로 허가된 의약품은 아니지만 최근 여러 국가에서 시행된 연구들은 modafinil이 안전하고 효과적인 주의력결핍 과잉행동장애 치료제일 수 있다는 결과를 보여주고 있다.

The present study was designed to evaluate the effects of staff training in PBS on the behaviors of staff and children with developmental disabilities in an inpatient unit. The perceived significance of children's problem behavior and staff's caring stress significantly decreased with the intervention. A satisfaction survey reported staff's high satisfaction with the process and results of staff training. This study suggests that PBS is a socially valid and positive intervention with problem behavior of children with developmental disabilities. The implications and limitations of this study, with directions for future research, are also discussed.

Objectives:This study investigated the relationship among child's behavior problems, maternal depression, and parenting stress in children with psychiatric diagnoses and their mothers, and the effect of these variables to the mothers' parenting stress. Methods:Seventy-three children(31 externalizing, 24 internalizing, 18 mixed) and their mothers were involved in this study. The mothers of three groups completed MMPI, KPI-C(Korean Personality Inventory for Children), and PSI(Parenting Stress Index). To investigate the relationships among each variables, ANOVA, Pearson correlation, stepwise regression analysis were performed. Results:There was no significant difference in maternal depression among three groups of children. In 2-Way ANOVA, main effect of maternal depression was statistically significant on depression, parent health, and relationship with spouse subfactor in parent domain, parent domain total, and overall parenting stress. But the main effect of child group was statistically significant on distractability/hyperactivity subfactor in child domain only. In regression analysis, maternal depression explained the parent domain of parenting stress most effectively, and child's hyperactivity and anxiety explained the child domain of parenting stress significantly. Conclusion:These findings suggest that it is important to intervene maternal depression to reduce the parenting stress, along with the treatment of the child's behavior problems.
Objectives: REM sleep behavior disorder (RBD) has received little attention in epidemiologic studies. This study aimed to determine the prevalence of probable REM sleep behavior disorder (pRBD) in the elderly population and its clinical features. Methods: A random sample of 1,588 was selected from a roster of 14,050 elderly population living in Osan city. The subjects were asked to fill out the REM sleep behavior disorder screening questionnaire (RBDSQ). Subjects whose score were 5 or higher on RBDSQ underwent a diagnostic phase of person-to-person assessment by experts in RBD. Results: Among 1,588 subjects, 886 elderly subjects participated in the screening phase and 123 subjects were assessed in the diagnostic phase. Eleven subjects were diagnosed as having pRBD, so prevalence was 1.5% (95% CI=0.70-2.30%). The frequency of depression and cognitive decline was significantly increased in patients with pRBD compared to subjects without pRBD, and there was no difference in sleep disturbances between two groups. Conclusions: Probable REM sleep behavior disorder is not rare in the elderly but frequently under-recognized. More attention should be paid to evaluation and treatment of RBD.
The purpose of this study was to identify the current status of the challenging behaviors of the developmentally disabled living in residential facilities for the disabled in Gyeonggi-do and whether social workers in residential facilities recognize the challenging behaviors, and then to draw implications for establishing an intervention for the challenging behaviors. As a research method, a survey was conducted targeting 145 social workers in residential facilities for the disabled in Gyeonggi-do. The collected data were calculated as frequency, percentage, mean, and standard deviation using SPSS 25.0. As a result of the study, first, it was analyzed that the status of injuries related to challenging behaviors of persons with developmental disabilities was high. Second, it was analyzed that the social worker's perception of the challenging behavior of the developmentally disabled is that they are considering job change because of psychological trauma. Objectives: REM sleep behavior disorder (RBD), characterized by excessive motor activity during REM sleep, is associated with loss of muscle atonia. In recent years, it has been reported that RBD has high co-morbidity with CNS disorders (especially, Parkinson's disease, dementia, multiple system atrophy, etc.). We aimed to assess differences in clinical and polysomnographic findings among RBD patients, depending on the presence or absence of central nervous system (CNS) disorders. Methods: The medical records and polysomnographic data of 81 patients who had been diagnosed as having RBD were reviewed. The patients were classified into two groups: associated RBD (aRBD, i.e., with a clinical history and/or brain MRI evidence of CNS disorder) and idiopathic RBD (iRBD, i.e., without a clinical history and/or brain MRI evidence of CNS disorder) groups. Twenty-one patients (25.9%) belonged to the aRBD group and 60 patients (74.1%) belonged to the iRBD group. The clinical characteristics and polysomnographic findings of the two groups were compared. Results: Periodic limb movement disorder (PLMD), i.e., PLMI (periodic limb movement index)>5, was observed more frequently in the aRBD group than in the iRBD group (p<0.001, Fisher's exact test). Also, obstructive sleep apnea syndrome (OSAS), i.e., RDI (respiratory disturbance index)>5, was found more frequently in the aRBD group (p=0.0042, Fisher's exact test). The percentages for slow wave sleep and sleep efficiency were significantly lower in the aRBD group than in the iRBD group. Conclusion: We found that 1 out of 4 RBD patients had associated CNS disorders, warranting more careful neurological evaluation and follow-up in this category of RBD. In this category of RBD patients, we also found more frequent PLMD and OSAS. These patients were also found to have lower slow wave sleep and sleep efficiency. In summary, RBD patients with associated CNS disorders suffer from more disturbed sleep than those without them.
The purpose of this study was to investigate whether group play therapy is effective in reducing problem behaviors of children with developmental disabilities. The subjects of study were in special schools and 5 children with disabilities using short-term care centers for the disabled, The research period was 12 sessions of 40 minutes twice a week using winter vacation from January 2, 2018 to January 28, The measuring tool, K-CBCL, was used to observe behavioral changes qualitatively. The summary of this study is as follows. First, children with disabilities significantly reduced their aggressiveness, atrophy, depression, and anxiety, and increased their activity during each session. Second, problem behaviors decreased after the intervention of group play therapy, and there were more decreases in internalization problems than externalization problems. The results of this study suggest that group play therapy is meaningful in reducing problem behavior according to the individual characteristics of children with disabilities.
On a dairy farm, a Holstein cow had consistently shown an intersucking behavior within the milking herd. It has been considered as one of several behavioral disorders primarily coming from feeding management failure during weaning period. Due to their behavior, injured teats of other milking cows could decrease milk production. Through tongue surgery, by producing a convex shape of the dorsum of the tongue after suturing the excised ventral region, the intersucking behavior was forcefully corrected and disappeared thereafter within the dairy herd without any complications.
The authors intended to investigate personality characteristics and those influence on the outcome of cognitive behavioral therapy in patients with panic disorder. 167 patients who met DSM-IV criteria for panic disorder were assessed by the PDQ-R(Personality Disorder Questionnaire-Revision) and various self-report tools for assessing symptoms of panic disorder. The effect of therapy was measured by the changes of scores and the end state functioning before and after 12-sessions of CBT. The patients with panic disorder were more likely showed obsessive-compulsive, avoidant and paranoid personality disorder and also Cluster C. If is needed when patients were divided into two groups according to total scores of PDQ-R(high or low personality disorder groups), high personality disorder group showed many evidences for increased psychopathology at the start of treatments, this suggested the close linkage between panic disorder and personality disorder. Interestingly, there were no significant differences between both groups in scores of clinical variables and the end state functioning. In conclusion, although patients with high tendency of personality disorder had more generalized problems at the beginning of treatments, they could improve as much as the patients with low tendency of personality disorder. They can be helped by cognitive behavioral therapy for panic disorder and seem to profit as much as patients with low tendency of personality disorder. If is needed to seek other factors in poor responders for cognitive behavioral therapy.
