Objectives: This survey research was conducted to evaluate real-time non-face-to-face education in clinical Korean medicine conducted during the COVID-19 outbreak. Methods: This study was conducted with all 3rd and 4th grade students of the Department of Korean Medicine, Gachon University. The Korean Internal Medicine (Pulmonary) lecture course was held from April 1st to July 1st, 2020 for one semester. The lectures were conducted through real-time non-face-to-face education using the Cisco Webex program, and the lectures were mainly conducted by sharing a PowerPoint summarizing textbook. The questionnaire consisted of 3 parts and a total of 18 questions. The survey was conducted anonymously to ensure the confidentiality of all responses. Results: The overall score for real-time non-face-to-face education was 74.69±18.15 points. The efficiency of real-time non-face-to-face education was 2.12±0.78 points, and opinions on the expansion of future use were 2.08±0.91 points and were generally positive. The ratio of non-face-to-face education and face-to-face education in future clinical Korean medicine theory lectures was about 63:27, with a higher proportion of non-face-to-face education. Conclusions: Promoting clinical Korean medicine as a non-face-to-face education has generally been positively evaluated in terms of efficiency and overall, and its use should be expanded in the future. In particular, it will be necessary to use non-face-to-face education in the theory classes of clinical Korean medicine. However, technical problems involving internet, video, equipment, and communication tools will need to be improved in the future.
The aim of this study is to find out benefits of symptom questionnaire for oriental medical treatments. We analyzed symptom questionnaire of Korean association of abdomen diagnosis and treatment. Conclusions are as below. 1. Appetite, taste and digestion have a deep connection with accumulation and muscular contracture, with which the method of reinforcement and reduction is decided. 2. Feces, urine and, sweat have a deep connection with accumulation and water, with which the method of diaphoresis, emesis and purgation is decided. 3. Thirst and chillness and fever have a deep connection with water poisons and feverish feeling, based on which the cold and heat is classified. 4. we can find out the lesions with departmentalized physical sections of the head and the face, the chest, the abdomen, the articulations, the limbs and the skin. 5. It is possible to select appropriate prescription and assess before & after treatment through the questionnaire.
1. 도입 및 배경 Lou Gehrig 병이라고 알려져 있는 근위축성 측삭경화증은 중추신경계 중 운동신경의 퇴행성 변화에 의한 진행성 질환이다. 이 병은 근육의 마비와 약화, 강직의 진행과 더불어 언어장애, 연화장애, 호흡장애가 동반된다. ALS는 환자나 가족들에게 엄청난 고통을 주는 질환으로 이병은 발병 후 3-5년 내에 약 $75\%$가 사망한다. 그리고 연하장애가 주된 증상이거나 고령에서 발병하였을 때에는 생존기간이 더욱 단축되는 경향을 나타낸다. 그리고 치명적인 호흡장애로 인해 대부분 사망하게 된다. 현재 의학계에서는 이 병의 원인도 알고 있지 못하고, 증상에 대한 아무런 치료방법도 제시하지 못하고 있는 실정이다. 2. 역학 원발성 운동신경원성 질환의 발생률은 100,000당 약 7명이고, ALS는 이 중 약 $65-85\%$를 차지한다. 전 세계적으로 1년에 약 2-4명/100,000이 발생하고 있으며 시간이 경과할수록 점차 증가하고 있는 추세이다. 3. 연구방법 EMG나 Biopsy 등을 통해 신경과 전문의가 ALS로 진단한 환자 중 상지대학교 부속한방병원에서 3개월 이상 입원치료를 받은 환자 18명을 대상으로 임상연구를 진행하였고, 설문조사는 외래에 내원한 환자 중 설문에 응답한 환자 28명을 포함한 총 46명의 환자들을 대상으로 시행하였다. 치료방법은 침, 약침, 봉약침, 한약 등을 환자의 체질을 고려하여 변증에 따라 적용하였고, 환자의 상태 변화는 ALSFRS(ALS Functional Rate Scale)를 사용하여 관찰하였다. 4. 결과 본 연구결과 ALS의 치료에 한의학적 치료방법은 유효한 것으로 나타났다. 특히 본 질환이 시간이 경과하면서 $100\%$ 악화되는 질환임을 감안한다면 한의학적 치료를 통해 약 $40\%$의 환자에서 몸의 상태나 언어장애, 만성적 피로 등이 호전되었고, 근력도 증가되는 결과를 나타내었다.
Objectives: Attention-Deficit/Hyperactivity Disorder (ADHD) is characterized by a persistent pattern of inattention and/or hyperactivity impulsivity that interferes with function or development in children. In traditional Korean medicine (TKM) and traditional Chinese medicine (TCM), ADHD is classified by several patterns based on symptoms and signs. However, currently, there is no objective diagnostic tool for ADHD in traditional medicine. The objective of this study was to develop the Pattern Identification Questionnaire for ADHD (parents-survey style) to be used in Korean medicine, through a literature review and consultation with groups of experts. Methods: The types of pattern identifications of ADHD mentioned in 13 pieces of Korean and Chinese literatures and their symptoms and signs were analyzed. The advisory committee (15 Neuropsychiatrist and 11 Pediatrist in Korean Medicine) assessed the appropriateness of the literature selection and the types of pattern identification selection and their symptoms and signs, and weighed the significance of the symptoms and signs. The Pattern Identification Questionnaire for ADHD was developed using the calculated weights by evaluated significance. The translation of symptoms and signs to the Korean language was achieved through consultation with expert translators. Results: 1. Four pattern identification types and their symptoms and signs were selected according to frequency of appearance in the Korean and Chinese literatures, and were reviewed by the advisory committee: Kidney yin deficiency and liver yang ascendant hyperactivity (腎虛肝亢), Dual deficiencies in the heart and spleen (心脾兩虛), Phlegm-fire harassing the heart (痰火擾心), and Spleen weakness and liver energy preponderance (脾虛肝旺). 2. The weights of all the symptoms and signs in the four patterns were calculated using the means and standard deviations of the symptoms and signs' importance that were obtained from specialists' significance weighting. 3. The Pattern Identification Questionnaire for ADHD (parents-survey style) in Korean medicine composed of 38 questions was suggested. Conclusions: Using a review of the literature and expert advice, Pattern Identification Questionnaire for ADHD (parents-survey style) in Korean medicine was developed. Further clinical study is required to develop a final version of the questionnaire through the evaluation of reliability and validity.
Objectives : This study was performed to develop Hwabyung clinical guideline. Acupuncture, moxibustion, negative therapy and psychotherapy were selected, and herbal therapy was excluded in this study. Methods : 27 questions were composed to investigate nonherbal therapy of Hwabyung. They were answered by the experts. They are the professors of oriental neuropsychiatry in Korea. Results and Conclusions : Most experts use acupuncture mainly in treating Hwabyung. They think that acupuncture therapy is needed by all means. Moxibustion, negative therapy and psychotherapy are also utilized mainly or partially to treat Hwabyung, Moxibustion and negative therapy are usually done for somatic symptoms, and psychotherapy is for mental symptoms.
본 논문에서는 사상체질분류검사 설문지를 이용하여 사상체질을 진단할 때, 진단의 정확도를 향상시키기 위한 사상체질 분류 함수를 개발하기 위하여 퍼지 분류기를 이용한다. 본 연구에서 사용하는 데이터는 9개 한의과대학의 10개 부속한방병원에서 치료를 받은 환자들 중 각 병원의 사상체질전문의로부터 체질진단을 받고 최소한 4주 이상 사상체질 처방을 사용한 후 주 증상이 전반적으로 호전되어 체질이 확인된 환자 1,914명을 대상으로 하고 있다. 본 연구는 사상체질의학의 광제설을 토대로 환자의 성별을 분리 하였을 뿐만 아니라, 비만도를 추가적으로 분류하였으며, 체형기상, 용모사기, 성질재간, 병증약리 중 체형기상을 토대로 분류하였으며, 사상체질을 판별할 수 있도록 설계되고 구현되었다.
Objectives : Consumption is a chronic wasting disease and major portion of Oriental Medicine's therapy. However, there is no standard diagnostic method for consumption that is $q{\grave{i}}-x{\bar{u}}$, $xu{\grave{e}}-x{\bar{u}}$, $yang-x{\bar{u}}$, $y{\bar{i}}n-x{\bar{u}}$. Methods : A questionnaire which includes symptoms and signs for diagnosis of $q{\grave{i}}-x{\bar{u}}$, $xu{\grave{e}}-x{\bar{u}}$, $yang-x{\bar{u}}$, $y{\bar{i}}n-x{\bar{u}}$ was evaluated by Delphi technique. Each question was valuated by interviewing 27 oriental medicine doctors. Then. we choose questions given over 5 points and reorganized some items according to the recommendations by interviewed-doctors. We then accessed the value of re-organized questions composing of the questionnaires. Conclusion : We finally chose each 9 items of $q{\grave{i}}-x{\bar{u}}$, $xu{\grave{e}}-x{\bar{u}}$, $yang-x{\bar{u}}$, $y{\bar{i}}n-x{\bar{u}}$'s questionnaire. Further study is necessary for modification of questionnaire by statistics and certification by clinical trial.
Objectives : The purpose of this survey is to examine Korean medical doctor's prescription status of Jaungo. Methods : The questionnaire was distributed via e-mail to Korean medical doctors who were registered in the association of Korean medicine from December 26th 2017 to February 5th 2018. We screened out respondents who didn't meet screening criteria. Results : Total 325 respondents completed answers, and the computerized data were analyzed. 74.7% of respondents were provided Jaungo from outside (extramural herbal dispensaries, pharmaceutical company). Almost all of the respondents (98.2%) were prescribing Jaungo in the form of ointment, and the cream container was the most commonly used container. In chief Indications of Jaungo were skin dryness (83.1%), pruritus (67.7%), burns (64.0%), and atopic dermatitis (60.9%). More than 60% of the respondents were prescribing Jaungo without additional herbs or additives. Most of patients were educated to apply Jaungo thinly to the affected area once or twice a day or frequently. Reason for dissatisfaction about Jaungo of patient were due to staining clothes (59.1%), offensive smell (44.0%), and poor texture (26.5%). Conclusions : Jaungo is a topical herbal agent frequently used in the Korean medicine. This is the first survey figure out current status of topical herbal agent in Korean medical doctors. The result of this study would contribute to progressing treatment method using topical herbal agent.
Objectives: The aims of study were developing cut-off value of Yin-deficiency questionnaire (YDQ) for diagnosis of Yin-deficiency (YD) and compare diagnostic ability between YDQ and Yin-deficiency scale score (YDS) in xerostomia patients. Methods: We recruited 58 xerostomia patients. They were diagnosed YD or non-YD by 3 Korean medicine doctors (KMD). We assessed YD using YDQ and YDS. We evaluated xerostomia using VAS, Dry Mouth Symptom Questionnaire (DMSQ), Salivary Flow Rate (SFR), oral moisture on buccal mucosa and tongue surface (OMB and OMT). We surveyed tongue coatings using Winkel Tongue Coating Index (WTCI). Results: We diagnosed 23 patients YD and 35 patients non-YD. There were no significant differences of age, sex and body mass index between the YD and non-YD groups. Using receiver operating characteristic curve analysis, the optimal cut-off value of YDQ was defined as 304. Sensitivity, specificity and Youden index of YDQ were 86.96%, 71.43% and 1.5839 respectively. Using Cohen's coefficient of agreement, we found that degree of agreement between KMD and YDQ diagnosis was moderate (${\kappa}$=0.524, p<0.001). Using Pearson's correlation analysis, we found concurrent validity of YDQ and YDS were significant correlated. Using area under curve value, we found diagnostic ability between YDQ and YDS were not significantly different (p=0.505), but there were more strong correlations between DMSQ-symptoms and YDQ (r=0.731, p<0.001) than correlations between DMSQ-symptoms and YDS (r=0.418, p<0.01). Conclusions: The cut-off value of YDQ can diagnose YD in xerostomia and diagnostic ability of YDQ in xerostomia is better than YDS.
Objectives The purpose of this study is to evaluate the safety of Non-toxic bee venom (BV) and observe VAS change before and after Non-toxic BV treatment in pain patients. Methods We surveyed the clinical practitioners who treated with Non-toxic BV in pain patients who visited the Korean medical clinic. The questionnaire survey was conducted for clinical practitioners who agreed to participate after hearing the explanation for the purpose and characteristics of the questionnaire. Patients in the questionnaires were reviewed based on their medical records from July 1, 2016 to October 28, 2016. Results We received 445 cases and selected 403 cases finally. 2 cases, however, were not able to continue treatment for 3 weeks and were eliminated. Depending on when the pain occurred, we divided the 401 cases into three groups (Acute, Subacute, Chronic group). In all groups, VAS scores were significantly decreased after treatment. Adverse reactions following Non-toxic BV treatment had occurred was 16 cases (3.60%). Except for 3 cases with hives, most of adverse reactions were mild or moderate and were not in need of extra treatment. The total safety of treatment for 3 weeks was mostly safe. The number of cases discontinued treatment was 42 cases (9.44%). Most of these cases, treatment was stopped for personal reason unrelated to the Non-toxic BV treatment. Conclusions These results suggest that the Non-toxic BV treatment has no serious adverse reactions and is a relatively safe treatment. Further studies are needed to prove the efficacy and clinical safety of Non-toxic BV treatment.
