Analysis of the Treatment and Adverse Events of Whiplash Associated Disorder Patients During Pregnancy in a Korean Medicine Hospital (임신 중 교통사고로 일개한방병원에 내원한 환자의 치료 분석 및 이상반응 보고)

  • Objectives: The purpose of this study is to analyze both treatment and adverse events of Korean medicine treatment for whiplash associated disorder patients during pregnancy. Methods: We investigated the medical records of 44 pregnant patients who had got into car accidents from April 1, 2018 to May 31, 2019. Results: 100 percent of the inpatients and 88 precent of the outpatients were treated by acupuncture. 79 percent of the inpatients and 72 precent of the outpatients were treated by chuna, and 86 percent of the inpatients and 64 precent of the outpatients were treated by pharmacopuncture. Out of 32 patients with low back pain, 94 percent were treated by acupuncture, 47 percent by phramacopuncture and 38 precent by chuna. 90 percent of 31 patients with cervical pain were treated by acupuncture, 71 percent were treated by pharmacopuncture and 77 percent were treated by chuna. There were adverse events like cough, rhinorrhea, dilatation of cervix and worsen low back pain during or after the treatment. Conclusions: Korean medicine treatment including chuna and pharmacopuncture can be applied to pregnant women injured by traffic accident.

A Case Report of Improvement in Abdominal Distension in a Patient with Functional Dyspepsia Using Hyeolgusamhwa-tang (혈구삼화탕 투약 후 복부팽만이 호전된 기능성 소화불량 환자 증례보고)

  • Objective: The aim of this case report was to describe the effect of Hyeolgusamhwa-tang (絜矩三和湯 ) on abdominal distension caused by Qi (氣脹) in a patient with functional dyspepsia. Methods: A female Korean patient was treated with Hyeolgusamhwa-tang and acupuncture for 14 days. We observed objective symptoms, such as bloating, nausea, belching, loss of appetite, and heartburn, and we also measured her abdominal circumference. Results: After treatment, her abdominal circumference was decreased from 87 cm to 83.5 cm, and her food intake was improved from the baseline. The other objective symptoms, like general weakness, loss of appetite, abdominal discomfort, and heartburn, were improved or disappeared without any adverse events reported. However, we could not find any significant differences for some symptoms, such as nausea. Conclusions: This case suggests that Hyeolgusamhwa-tang and acupuncture therapy could be effective in reducing functional distension in patients diagnosed with distension caused by Qi (氣脹).

  • A patients with quadriplegia and a history of pancreatectomy was administered Socheongryong-tang to reduce sputum. The patient's sputum decreased, but a sudden increase in blood sugar was observed. Ephedra is known to stimulate the sympathetic nerves to increase the blood pressure and blood sugar, but in healthy people, changes in blood sugar are not significant due to corrections by hormone regulation. However, this case suggests that ephedra can cause significant blood sugar increases in patients who undergo pancreatectomy or who have diabetes mellitus. Even though ephedra is highlighted for its usefulness in sputum reduction and in weight control, it should be carefully prescribed to patients with diabetes.

A Case Report on Korean Medical Treatment for a Patient with Chronic Nausea and Vomiting Syndrome (만성 오심 구토 증후군 환자의 한의치료 1례)

  • This study investigated the case of a nineteen-year-old female patient with chronic nausea, vomiting, and abdominal distension, who was diagnosed with CNVS and Spleen-Qi deficiency. Subjective symptoms were recorded with the Numerical Rating Scale (NRS) every morning, and the Gastrointestinal Symptom Rating Scale (GSRS) was used on the days of admission and discharge. For eleven days following admission, she took Bojungikki-tang-gagam and received acupuncture and moxibustion therapy. After three days of treatment, her nausea and vomiting ceased, and abdominal distension improved from NRS 3 to 0 after seven days of treatment. The GSRS score for the specific symptoms improved from 3 to 2; however, the total score remained largely unchanged (from 20 to 21). This case suggests that Korean medical treatment may improve CNVS.

Korean Medical Treatment of Sequela Following Chemotherapy for Recurrent Ovarian Cancer after Hysterectomy: a Case Report (재발성 난소암 환자의 화학적 항암요법 후유증 치험 1례)

  • Objectives: Most of the time after chemotherapy for ovarian cancer, the immune system is getting even worse with reduction of whole blood cell. The purpose of this study is to report the effect of korean medical treatment on Sequela including pancytopenia after chemotherapy for recurrent ovarian cancer after hysterectomy. Methods: The patient having treated with hospital management from June 8th in 2017 until July 5th in 2017, participated. Her chief complaints were fatigue, tinnitus, dizziness, decreased appetite due to glossodynia and constipation. We diagnosed her as pancytopenia based on her whole blood test result. We treated the patient with acupuncture, herbal medicine, cupping and moxibustion. The efficacy of treatment was evaluated with Numerical Rating Scale (NRS) and blood test. Results: During hospitalization period, the symptoms including fatigue, tinnitus, dizziness, decreased appetite, constipation and pancytopenia were reduced. After discharging, we identified that her blood levels are maintained and ovarian cancer no longer recurred. Conclusions: To recover from Sequela including pancytopenia following chemotherapy for recurrent ovarian cancer after hysterectomy, Korean medical treatment is considered to be effective.

  • Objectives: The purpose of this study was to report a case of a pregnant woman diagnosed with lumbar disc herniation that improved in lumbar pain and radiating leg pain through combined korean medical treatment including pharmacopuncture at lumbar facet joints in situations surgical and pharmacological treatment is limited due to pregnancy. Methods: A combination of treatment including pharmacopuncture at lumbar facet joints, acupuncture, cupping, and herbal medicine was provided for 1 month to a woman at 21 weeks pregnant who was diagnosed with lumbar disc herniation. Her progress was assessed with Numeric Rating Scale (NRS), Oswestry Disability Index (ODI), EuroQol-5Dimension (EQ-5D) before and after treatment. Results: After the treatment, the pain decreased and the quality of life was enhanced. No adverse events were observed. Conclusions: The findings indicate that combined korean medical treatment including pharmacopuncture at lumbar facet joints can be safe and effective alternative to treat lumbar disc herniation during pregnancy.

  • Objective: The purpose of this study was to report the effect of acupuncture on the splenius muscle and the effect of combined Korean medicine treatment on acute dizziness in patients due to whiplash injury. Methods: Three patients with acute dizziness were treated with acupuncture of the splenius muscle and combined Korean medicine. A numeric rating scale (NRS) and the Korean dizziness handicap inventory (K-DHI) were checked every day from admission to discharge to confirm symptom improvements. Results: In the acute stage of dizziness caused by whiplash injury, acupuncture of the splenius muscle and combined Korean medicine improved the NRS and K-DHI scores. Conclusions: For patients with acute dizziness caused by whiplash injury, a combination of acupuncture on the splenius muscle and Korean medicine treatment may be a solution.

  • Objective: This study reports on the management of a patient with myasthenia gravis (MG) following myasthenic crisis (MC) using Korean medicine. Methods: A 77-year-old Class V MG patient was treated with acupuncture, moxibustion, and Korean herbal medicine. A manual muscle test was used to derive a Medical Research Council (MRC) score and the patient's subjective view of his dyspnea was measured to assess MG symptoms. Results: The MRC grade indicated significant deterioration after 65 days of treatment, although there was no change in the patient's subjective dyspnea assessment. But no other MG or MC complications occurred. Conclusion: This study suggests that Korean medicine could be used for the management of Class V MG patients.

Combined Korean Medicine Treatment of Lumbosacral Transitional Vertebra with Localized Bone Marrow Edema: A Case Report (이행성 척추에 동반된 골수부종 환자의 복합한방치료: 증례보고)

  • This study was to investigate the effectiveness of combined Korean medicine treatment on a patient who was diagnosed on lumbosacral transitional vertebra (LSTV) with localized bone marrow edema. The patient was treated with acupuncture, pharmacopucture, cupping and herbal medicine. We evaluated patient's progress using range of motion (ROM), EuroQol-5 dimensions (EQ-5D), Oswestry disability index (ODI), numerical rating scale (NRS), patient global impression of change, self-walkable distance. NRS of back pain decreased from 7 to 5, at the same time NRS of lower limb decreased from 4 to 3. EQ-5D score increased from 0.24 to 0.646. ODI score improved 80 to 62. Self-walkable distance and lumbar spine ROM also increased. As a result, clinical improvements were found in a patient. In conclusion, this study shows that Korean medicine treatment can be considered as effective conservative care for LSTV with localized bone marrow edema.

A Review of Clinical Studies for Chinese Medicine Treatment of Idiopathic Edema Using the CNKI Database (특발성 부종의 중의치료에 대한 임상연구 동향 - CNKI검색을 중심으로)

  • Objectives: This study investigated the latest clinical studies on Korean medicine treatment of idiopathic edema in the Chinese National Knowledge Infrastructure Database (CNKI). Methods: We searched the last 9 years of clinical studies discussing Oriental medicine-based treatments for idiopathic edema in the CNKI database. The search focused on the authors, publication year, type of study, purposes of study, method and duration of treatment, evaluation criteria, and results of the selected articles. Results: The search identified 7 randomized controlled trials (RCTs) and 9 others. All studies used oral herbal medicine for treatment. The most commonly used herbs were Poria Sclerotium (茯苓), Atractylodis Rhizoma Alba (白朮), and Alismatis Rhizoma (澤瀉), which are the constituent herbs of Oryeong-san (五苓散). All 16 studies confirmed the efficacy of Oriental medicine treatments. Conclusion: More varied and scientifically designed clinical studies are required to develop treatments for idiopathic edema. The results of this study could be used as basic data for further studies on idiopathic edema.
