  • Bell's palsy is an isolated facial paralysis of sudden onset caused by a neuritis of the seventh nerve within the facial canal. It occurs often in the adult man with a history of recent exposure to local cold, such as sleeping next to an open window, or in some cases it occurs after infections of the nasopharynx or masticator spaces. Especially, this neuropathy have linked with the major collagen disorders (diabetes mellitus). A segmental demyelination develops rapidly, with vascultitis in microinfarcts and ischemia to the nerve segment. The authors experienced about the bizarre neurological symptom of Bell's palsy after inferior alveolar nerve block anesthesia and TMJ dislocation in diabetic mellitus. The early and correct consultation with the multiple medical and dental departments was important to prevent the inadequate care & medicolegal problems.

  • Most of the dental treatments have been routinely performed under the local anesthesia and the effectiveness has also been proved safe. However, even not frequently dentists face some complications associated with the local anesthesia. In this report, the experience of the paresthesia after mental nerve block anesthesia for an implant placement of the mandibular premolar was presented to raise awareness of the complications related with local anesthetic procedure and to discuss about the causes, the proper treatments and the preventive approach of the paresthesia.

  • Modified surgical technique for transposition of the inferior alveolar nerve followed by immediate placement of endosseous implants in mandibles with moderate to severe atrophy are presented. Five transpositions of the inferior alveolar nerve together with the installation of 10 implants were performed in four patients. The mean postoperative follow-up time was 17 months, with a range of 8 to 20 months. All implants with functioning pontics remained stable, with no mobility or symptoms of pain and infection during the follow-up period. Neurosensory evaluation was performed using the two-point discrimination test. Two patients had objective neurosensory dysfunction at postoperative, but all the nerve function were reported as normal by the patients 4 months postoperatively.

  • Glial fibrillary acidic proteins (GFAP) are a group of intermediate filaments that are distributed in the cytoplasm of glial cells. GFAP immunoreactivity (GFAP-IR) increase after central and peripheral nerve injuries. The purpose of this study was to determine change of GFAP-IR in rat trigeminal ganglion satellite cells following the axotomy of inferior alveolar nerve(IAN). The immunohistochemistry was carried out using the avidin-biotin-peroxidase complex(ABC) method. 1. Control group : Astrocytes in central root of trigeminal ganglion had strong GFAP-IR, but satellite cells of trigeminal ganglion occasionally had GFAP-IR. The patterns of reactivity in satellite cells of trigeminal ganglion were not concenturated in any specific region of trigeminal ganglion. 2. Three day group after IAN axotomy : There were highly GFAP-IR in satellite cells of trigeminal ganglion in mandibular region. GFAP-IR in maxillary and ophthalmic regions were less intense compared to mandibular region. 3. Seven day group after IAN axotomy : GFAP-IR that were increased compared to control group were seen in the mandibular region. But GFAP-IR were less intense compared to three day group. These results suggest that GFAP-IR increase in specific region of trigeminal ganglion following peripheral axotomy. therefore we suppose that GFAP study offer research tool in trigeminal neuralgia.

가토 하치조신경 재건에 있어 정맥이식통로를 이용한 신경재생유도에 관한 실험적 연구 (VEIN GRAFT REPAIR COMPARED WITH NERVE GRAFTING FOR INFERIOR ALVEOLAR NERVE REGENERATION IN RABBITS)

  • This study was conducted to compare the vein graft with the nerve graft, and evaluated the availability of the vein graft on the reconstruction of the inferior alveolar nerve defect. The experimental animals were 12 rabbits weighing $1.5{\sim}2.0kg$, divided into 3 groups : sham operation group, vein conduit group and nerve graft group. All nerves were excised and histomorphometric analysis was performed at 2, 4, 6, 12, 16 weeks after operation. The obtained results were as follows. 1. Histologic examination revealed the regenerated nerve fibers within the lumen of the vein graft and nerve graft at 6 weeks after repair. 2. Axon diameter was significantly larger in nerve graft group(p<0.05) than in vein graft group at 6weeks, and larger in nerve graft group than in vein graft group at 16weeks. 3. Axon density was higher in the vein graft group at 16 weeks. 4. The myelin of the regenerated nerve fibers in distal segment of the vein graft group was thick, approaching the proximal segment at 16weeks. This means remyelination in distal segment in the vein graft group. These results suggested that autogenous vein graft may be used as an alternative to autogenous nerve graft.

하악 제3대구치 발치 후 발생한 하치조신경 및 설신경 손상에 관한 연구 (National survey of inferior alveolar nerve and lingual nerve damage after lower third molar extraction)

  • This retrospective study was to analyze the inferior alveolar nerve and lingual nerve damage after the removal of mandibular third molars. In this questionnaire study, the subjects chosen for this study were 2472 dentists who answered the questionnaire about numbness after the extraction of lower third molars. The data collected by E-mail and web site included the incidence of removal of the lower third molars, the incidence and the experience of numbness of the inferior alveolar nerve and lingual nerve, rate and duration of recovery, the influence in day life after the long-term sensory loss, the period and amount of the indemnity in the case of medical dispute. The results are summarized as follows. 1. The experience rate and the incidence rate of the inferior alveolar nerve numbness by oral surgeons in the past year were19.9% and 0.14%. Those of the lingual nerve by oral surgeon were 7.7% and 0.05%.2. The experience rate and the incidence rate of the inferior alveolar nerve numbness by the dentists except oral surgeons in the past year were 9.7% and 0.19%. Those of the lingual nerve by the dentists except oral surgeons were 5.5% and 0.11%.3. The recovery rate of the inferior alveolar nerve after 1 year and 2 years were 85.6% and 91.3%. The recovery rate of the lingual nerve after 1 year and 2 years were 84.8% and 89.3%.In conclusion, most of numbness may be recovered within 2 years. However the possibility of long term and persistent numbness should not be neglected. Therefore practitioner must inform the possibility of nerve injury and include this possibility in the consent forms.

하치조신경 손상시 턱끝신경 체성감각유발전위검사의 진단적 유용성에 관한 연구 (DIAGNOSTIC EFFICACY OF MENTAL NERVE SEP(SOMATOSENSORY EVOKED POTENTIALS) FOR THE INJURED INFERIOR ALVEOLAR NERVE)

  • Sensory dysfunction following the injury of the inferior alveolar nerve requires objective examination to get a reproducible data and to provide necessary treatment. This study was designed to evaluate if the SEP(somatosensory evoked potentials) of the mental nerve can be used as an objective method for the diagnosis of nerve injury and sensory disturbances. The subjects were nineteen patients ($37.4{\pm}11.3$ years old) who had been suffered from sensory disturbance of the unilateral lower lip and mental region for over 6 months after the inferior alveolar nerve injuries confirmed by the microsurgical explorations. The clinical neurosensory tests as SLTD(static light touch discrimination), MDD(moving direction discrimination), 2PD(two point discrimination), PPN(pin prick nociception) and accompanied pain were preceded to electro-physiologic examinations as SEP. The score of sensory dysfunction (sum score of all sensory tests) ranged from 0 to 8 were compared to the latency differences of the mental nerve SEPs. The correlation between clinical sensory scores and SEPs were tested by Spearman nonparametric rank correlation analysis, the differences in SEP latency by Kruskal-Wallis test and the latency differences according to PPN and accompanied pain by Mann-Whitney U test. This study resulted that the difference of the latencies between normal side and affected side was $2.22{\pm}2.46$ msec and correlated significantly with the neurosensory dysfunction scores (p=0.0001). Conclusively, the somatosensory evoked potentials of the mental nerve can be a useful diagnostic method to evaluate the inferior alveolar nerve injuries and the change of sensory dysfunction to be reproduced as an objective assessment.

  • 양측 하악지 시상 골절단술은 악안면 기형 및 부정교합을 치료하기 위해 흔히 사용되는 필수불가결한 술식이라고 할 수 있다. 하악지 시상 골절단술 후 발생할 수 있는 합병증으로는 하치조 신경 손상, 출혈, 측두하악장애, 부적절한 골면의 유합 및 골절, 재발 등이 있다. 악교정 수술 후 안면신경 마비의 발생 유병율은 최근 0.1 퍼센트로 보고되고 있다. 증상 발생의 원인으로는 안면신경의 압박, 신경의 불완전 또는 완전 손상, 신경의 견인, 마취제에 의한 신경의 허혈 등이 있다. 술 후 발생된 안면 신경의 마비는 환자의 삶의 질을 저해하고 사회활동을 기피하게 함으로써 가장 심각한 합병증 중 하나라고 볼 수 있다. 본 증례에서는 양측 하악지 시상 골전달술을 시행 후 발생한 안면신경 마비에 대하여 보고하고 있다.

치근단낭과 유사한 stafne 골낭종의 증례 보고 (Anterior stafne bone cyst mimicking periapical cyst: a case report)

  • Stafne 골낭종은 악하선의 발생 시 생긴 하악골의 결손부로 주로 하악골의 후방부나 하악 우각부에 발생한다. 대부분 하치조 신경관 하방에 발생하며 하악골 전방부에 발생하거나 하악 치아와 연관된 경우는 드물다. 또한 하악골의 천공을 일으키는 경우는 더욱 드물다. 본 증례는 하악골 전방부에 발생하여 하악골 천공을 일으킨 치근단낭과 유사한 병소가 stafne 골낭종으로 진단된 희귀한 증례로, 이와 유사하게 하악골에 발생 가능한 병소의 감별 진단과 함께 보고하고자 한다.

치과 수술 후 뉴로미터를 이용한 전류인지역치의 변화의 분석 (Analysis of Current Perception Threshold(CPT) Change after Dental Surgery using Neurometer)

  • 이 연구의 목적은 하악골 주위의 감각이상을 초래하는 치과 시술 후 하치조신경 및 그 분지부의 술후 통증, 또는 감각감퇴 나 감각이상을 호소하는 환자들의 전류인지역치를 측정하여 정상인과 비교할 때 어떠한 변화와 차이가 있는지 살펴보고 이를 임상에 활용하기 위함이다. 조선대학교 치과병원에서 하악 부위의 치과수술을 받은 후 지각이상을 주소로 구강내과로 전원된 환자(발치 14명, 임플란트 식립 7명, 악골 골절수술 3명, 안면부위 외상환자 3명, 낭종적출술 및 인공골 이식 환자 각 1명) 29명과 안면부의 통증이나 지각이상, 전신질환 등이 없는 건강한 정상인 25명을 대상으로 뉴로미터를 이용하여 2000 Hz, 250 Hz, and 5 Hz 주파수로 삼차신경 하악분지의 전류인지역치를 측정하였다. 전류인지역치는 남 녀 성별에 따른 비교에서 대조군의 $A{\beta}$ 신경섬유 (2000 Hz)에서 남자가 여자보다 유의하게 높게 나타난 것을 제외하고는 $A{\delta}$ 신경섬유(250 Hz), 신경섬유(5 Hz)의 전류인지 역치는 환자군과 대조군 모두 전류인지역치가 모든 주파수에서 유의한 차이가 관찰되지 않았고, 환자군과 대조군을 비교할때 모든 주파수 영역에서 환자군이 대조군보다 유의성있게 높게 나타났고, 환자군의 이환측의 전류인지역치도 비이환측 보다 역시 높게 나타났다. 이 결과로 볼 때 치과 수술 후 감각이상을 호소하는 환자의 수가 계속 증가하고 있는 추세여서 수술후 감각이상을 초래할 가능성이 있는 환자들에 대하여 수술 전 전류인지역치를 미리 측정함으로써 수술 전후 측정치와 비교하여 실제로 감각의 변화가 있는지 환자에게 설명할 수 있으며, 또한 의료분쟁을 사전에 방지하고 해결하는 차원에서도 전류인지역치검사는 매우 유용할 것으로 사료된다.
