  • 음성의 생성기전은 폐의 호기에 의하여 성대가 진동하여 원음이 발생하고, 원음이 하인두, 구인두, 비인두, 구강 등의 성도를 통과하면서 공명이 일어나며, 혀와 입술의 운동에 의하여 조음이 일어나서 결국은 음성이 생성되게 된다. 저자는 음성을 생성하는 후두 및 성도의 음성외과학적 구조 및 생리에 대하여 주로 언급하고, 성도에 발생하는 다양한 질환중 음성에 영향을 주는 질환만을 선택하여 간략히 설명하고자 한다. (중략)

Hypopharyngeal Foreign Body Migration Induced Retropharyngeal Abscess (후인두공간으로 이동하여 농양을 유발한 하인두 이물 1예)

  • Foreign bodies in upper aerodigestive tract migrating into retropharyngeal space are rarely encountered emergent cases in otolaryngologic fields. A 60-years-old female presented throat pain and lump sense after a meal. Computer tomography showed metallic foreign body impacted in the retropharyngeal space. A hypopharyngeal perforation was suspected by through flexible laryngoscopy and gastrointestinal endoscopy. It was successfully removed by external cervical approach, and we report this case with a review of the related literatures.

  • Synovial sarcoma is commonly found in the extremities of mesenchymal origin, but rare in the head and neck area. Histopathology is diagnostic and it shows a biphasic pattern with two neoplastic elements; spindle cell, sarcoma like stroma and gland like clefts lined by epitheloid cells. Synovial sarcoma is high-grade neoplasm that expresses epithelial as well as supporting features. This paper presents a case of synovial sarcoma of hypopharynx on 25 years old male. He was treated by surgical excision and postoperative chemoradiotherapy. There were no evidences of local recurrence & distant metastasis for 19 months postoperatively.

  • Neuroendocrine carcinomas are rare neoplasms of the hypopharynx. Neuroendocrine neoplasms are divided into four main types : carcinoid, atypical carcinoid, small cell carcinoma and paraganglioma. The diagnosis is primarily based on light microscopy and should be confirmed by immunohistochemical investigation. Small cell neuroendocrine carcinoma of the hypopharynx is extremely uncommon tumor and surgical results for this tumor have been disappointing. Chemotherapy and radiotherapy currently appear the most effective forms of therapy. We report our case of small cell neuroendocrine carcinoma of the hypopharynx with the brief review of literatures.

  • Fibromatosis is a broad group of benign fibroblastic proliferation that shows locally aggressive growth but never metastasize. Common anatomic sites include abdominal wall, extremity, and mesentery. Little is reported about clinical features and outcome of fibromatosis of the head and neck. The treatment of choice is wide excision, which is often difficult. Postoperative recurrence rates are high. We recently confirmed a unique case of fibromatosis occurred on the hypopharynx of 44-year-old male patient and report this interesting case with review of literature.

A Case of Hypopharyngeal Liposarcoma (하인두에 발생한 지방육종 1예)

  • Liposarcoma is one of the most common malignancies originating from human soft tissue. Because most of liposarcoma occur in the lower extremities or retroperitoneum, there are few reports about liposarcoma of head and neck region, including larynx and pharynx. Histologically, there are four types of liposarcoma, and prognosis and recurrence rate are different according to histologic subtype. Wide excision is the treatment of choice and liposarcoma hardly respond to primary radiotherapy. A 56-year-old man presented with voice change and foreign body sensation in laryngopharynx. Hypopharyngeal liposarcoma was diagnosed by surgical biopsy and performed totallaryngopharyngoesophagectomy and gastric pull-up. Here we report our experience on this case with review of literature.
