THE STUDY ON THE PREDICTION OF THE MESIODISTAL DIAMETERS OF UNERUPTED CANINES AND PREMOLARS IN KOREAN MALE AND FEMALE (미맹출 영구 견치 및 소구치 근원심 폭경 추정에 관한 연구)

  • The purpose of this study was to establish regression equations and probability charts for predicting the sum of mesiodistal crown diameters of unerupted unilateral canine and premolars from the sum of mesiodistal crown diameters of four mandibular incisors in Korean male and female. The plaster casts of 162 children(75 boys and 87 girls) among the contestees in 1994-2001 Healthy Dentition Contest in Seoul were measured. Sex differences are compared and the following results were obtained: 1. Bilateral comparison of sum of widths of permanent canine and premolars showed no significant differences for either sex(p>0.05). Sum of widths of permanent canine and premolars of male were significantly larger than that of female(p<0.01). 2.   • Supernumerary teeth are defined as an excess in the number of teeth when compared to the normal dental formula. They are more prevalent in the permanent dentition than the primary dentition. Supernumerary teeth can occur in the maxilla, mandible, or both. But the majority are found in the maxilla and most of it is found in the premaxilla region The present cases documents about the uncommon cases of supernumarary teeth on maxillarty premolar area and mandibular incisal area.

  • When a delayed eruption of tooth is diagnosed, the causative factor should be detected before an establishment of treatment plan, if possible. Although a panoramic radiograph is enough to evaluate the position of tooth and the stage of tooth development, a 3-D Dental CT would be a powerful tool to reveal a spatial relationships between objects. The reported case showed a delayed eruption of lower left permanent canine and a mini-plate with screws adjacent to the impacted canine. Although the screws adjacent to the root of impacted tooth showed a close proximity, it was not presumed that these screws would interfere the eruption of the tooth. The impacted canine did not show any spontaneous eruption during observation. After the mini-plate and screws were removed, an orthodontic traction using elastic power chain was performed. The position of mini-plates and screws should be carefully designed to avoid damaging the tooth follicles or tooth roots in the jaw. Also the screws should be removed before an orthodontic traction to prevent damaging the root surface of impacted tooth.

  • The primary objective of this study was to make the prediction percentile tabulation of the sum of mesiodistal widths of the unerupted permanent canines and premolars derived from the sum of mesiodistal widths of the erupted permanent incisors in normal and crowding groups. Crowding group consisted of 85 pairs of dental casts (42 males and 43 females) with more than 4mm of crowding in anteriors. Normal group consisted of 109 pairs of dental casts (65 males and 44 females) with less than 1mm of spacing or crowding in anteriors. The mesiodistal crown diameters of permanent teeth were measured from the dental casts using the sliding calipers (Mitutoyo Co., Japan). From the study, the obtained results were as follows: There were not significant differences between male and female in both groups on the basis of the ratios between the sum of mesiodistal widths of incisors and the sum of mesiodistal widths of canines and premolars. There was not significant difference between two groups on the basis of the ratio between the sum of mesiodistal widths of mandibular incisors and the sum of mesiodistal widths of mandibular canines and premolars and was significant differences between two groups on the basis of the ratio between the sum of mesiodistal widths of mandibular incisors and the sum of mesiodistal widths of maxillary canines and premolars.   • Transposition has been described as an interchange of position of permanent tooth and is a relatively rare dental anomaly. Transposition of teeth may occur both in the maxillary and mandibular arches. but it appears more often in the maxilla of individual teeth, the maxillary canine is the most often involved. A canine transposes most often with a first premolar and less frequently transposes with a lateral incisor. Incomplete transposition is a condition describing an interchange in the position of the crowns of two permanent teeth, while the root apices remain in their relative position. Complete transposition is a situation in which both the crowns and entire root structure are transposed. The etiologic factors of transposition are tooth buds interchange, retained deciduous canines, migration of the erupting canine, trauma to deciduous teeth etc. This report describes a case of a transposition between a maxillary left canine and a lateral incisor and impaction of a maxillary left central incisor due to trauma to deciduous dentition.

THE BEST TEETH COMBINATION TO PREDICT MESIODISTAL DIAMETERS OF THE UNERUPTED CANINE AND PREMOLARS OF KOREANS (한국인에서 미맹출 견치와 소구치의 근원심 폭경 예측을 위한 최적의 치아조합)

  • The probability table of Moyers and prediction equation of Tanaka and Johnston that have been the most frequently used, cannot produce accurate prediction when used in Korean because they are based on the Caucasian popularity of the Northern European race. The method of Moyers or Tanaka and Johnston predicts sizes of the unerupted canine and premolars on the basis of the sizes of mandibular incisors. However, some of the recent papers raise a question as to whether the mandibular incisors are the best combination to predict the sizes of the unerupted canine and premolars. The purpose of this study is to determine which sum or combination of sums of permanent tooth widths present the best prediction for the unerupted canine and premolars in a Korean sample, to calculate a specific linear regression equation for this population, and to evaluate the clinical significance. A new linear regression equation was calculated based on the data of 178 Korean young adults(70 women, 108 men, mean age 21.63 years) with complete permanent dentitions. Fifty three more children(28 girls, 25 boys, mean age 14.22 years) were used as a validation sample for the application of the multiple linear regression equation. The conclusions were as follows: 1. The combination of the sums of permanent upper central incisors, lower lateral incisors and upper first molars was the best predictor for the unerupted canine and premolars in this sample($r=0.65{\sim}0.80$). 2. The multiple linear regression equation was calculated including sex and arch as additional predictor variables. male, upper: $Y\;=\;0.332{\times}X_0\;+\;6.195$ male, lower: $Y\;=\;0.332{\times}X_0\;+\;5.269$ female, upper: $Y\;=\;0.332{\times}X_0\;+\;5.929$ female, lower: $Y\;=\;0.332{\times}X_0\;+\;5.003$. The determination coefficient of the equation was 64% and a standard error of the estimate was 0.71mm. 3. In about 97% of the validation sample, the estimation of the tooth width sums of unerupted canine and premolars using the new multiple linear regression equation was smaller than 1mm compaired with the actual values.

  • Ectopic eruption should be understood as a change in the course of the normal eruption path of a dental bud at any moment its origin. An example of this alteration is the dental transposition, a rare and more specific dental anomaly that may be defined as a change of position between two teeth. This case shows ectopic eruption of transposed mandibular lateral incisor beneath primary first molar at the first transitional period of the mixed dentition The crown of the lateral incisor has tipped distally, compelling root resorption and exfoliation of the adjacent primary cuspid and primary first molar. The reason for such eruption is not clearly understood, but it may involve; (1)trauma history, (2)prolonged retention of the deciduous teeth, (3)premature exfoliation of the deciduous teeth, and (4)genetic factor. Treatment is divided into interceptive and definitive treatment. Ectopically erupting mandibular incisor tends to become transposed with the adjacent cuspid and thus seems to warrant early orthodontic intervention. Early treatment may obviate later extraction or transposition of the incisor and canine in the permanent dentition. Timing is an important factor to be considered regarding in the correction of the lateral incisor transposition. This case advocates treatment with an active orthodontic therapy at the early stage of the mixed dentiton, before the eruption of the permanent cuspid.

A STUDY ON THE WIDTH OF ATTACHED GINGIVA IN CHILDREN (아동의 부착치은 폭경에 대한 연구)

  • The aim of this study is (1) to establish the baseline information concerning the width of keratinized gingiva, depth of gingival sulcus and width of attached gingiva on the buccal surface of the teeth: and (2) to determine the relationship between the above values and tooth eruption: and (3) to estimate the frequency of mucogingival problems. The results were as follows; 1. The mean width of attached gingiva of the children aged $6\sim12$ proved to be wider in the maxilla than in the mandible. Of the primary teeth, the widest width was found in the areas of maxillary primary lateral incisors and maxillary primary canines(3.50mm and 3.55mm). The narrowest was noted in the area of mandibular first primary molars(1.34mm) In the permanent dentition, the greatest width was found in the areas of maxillary permanent lateral incisors (3.00mm). The narrowest was noted in the area of mandibular first premolars(0.55mm). 2. In the primary dentition, the width of attached gingiva of primary canines and first and second primary molars became wider from the age of six as the age increased. In the permanent dentition of the boys, only mandibular central incisors and maxillary first molars showed the tendency towards increase in the width of attached gingiva with increasing age. In the permanent dentition of girls, central and lateral incisors of both jaws and maxillary first molars showed statistically significant increase in the width of attached gingiva with increasing age(p<0.05). 3. At the age of tooth change, the attached gingiva of primary teeth were almost wider than those of successive permanent teeth (p<0.05). 4. During the period of 6 to 12 years of age, the width of keratinized gingiva and the depth of gingival sulcus of permanent tooth at the age of twelve were larger than those of primary tooth at the age of six (p<0.05). 5. The maximum in the frequency of mucogingival problems was found in the areas of upper and lower first primary molars of primary dentition, and in the upper and lower first premolars of permanent dentition regardless of sex. The frequency was higher in primary teeth than in the corresponding successive permanent teeth These teeth showed tendency towards increase in mucogingival problems with age.

  • Estimating the size of unerupted teeth is an essential aspect of orthodontic diagnosis and treatment planning in the mixed dentition. Several methods were introduced and used for the prediction. The most common methods among these would be Moyers probability chart and Tanaka and Johnston equations. These are currently used widely, but they were developed for Caucasians. Because there are clear racial differences in teeth size, the objectives of this study were to produce correlation coefficients between the combined mesiodistal widths of the permanent mandibular incisors and those of the canines and premolars for each quadrant, and prediction tables with regression equations, specifically for Korean. 178 young adults (70 women, 108 men, mean age 21.63 years) were selected from the College of Dentistry, Yonsei University, Seoul, Korea. The mesiodistal crown diameters of the permanent teeth were measured with calipers. Significant sexual dimorphism was found in tooth sizes. The correlation coefficients between the total mesiodistal width of the mandibular permanent incisors and those of the maxillary and mandibular canines and premolars were found to be between 0.52 and 0.64. The standard error of the estimatation was better (0.60) for women and the ${\gamma}^2$ values ranged from 0.27 to 0.41 for both sexes Prediction tables were prepared for Korean. This study showed larger canine and premolar diameters than Tanaka and Johnston's and Moyers' studies which might be due to the racial differences. Further investigations with a larger sample size will be needed for more representative data on the Korean population.

Evaluation of Characteristics of the Dental Maturity in Girls with Precocious Puberty (성조숙증으로 진단된 여자 어린이의 치아 성숙도 특성 평가)

  • Precocious puberty (PP) is defined as the early development of secondary sexual characteristics (before the ages of 8 years in girls and 9 years in boys). The aim of this study is to identify characteristics of the dental maturity in girls with PP that discriminate them from normal healthy girls. This study included 99 girls aged 6 - 8 years with PP and 99 girls without past medical history of same chronological age (control group). The study was performed on 198 panoramic radiographs (99 PP girls, 99 control group girls). Demirjian method was used to evaluate the panoramic radiographs and determine dental maturity. Difference in dental maturity score and tooth formation stages between the two groups were analyzed. The PP group showed significant higher maturity score than control group. Among mandibular teeth, mandibular 2nd premolar and 2nd molar were significantly more mature in the PP group than control group. Logistic regression analysis showed that mandibular 2nd molar was only significant predictor for PP girls.
