근시(近視)의 병인병기(病因病機)에 관(關)한 문헌적(文獻的) 고찰(考察) (A Study on the Literatures of Oriental Medicine Which Deals with the Treatment Principle on Myopia)

  • Objectives : Myopia is a disease which can be easily found in the child and it considers as a prevalent disease. This study has a reference of the literatures of oriental medicine on the myopia to investigate treatment principles of the myopia. Methods : This study is based on the literatures of oriental medicine. Results : According to the literatures of oriental medicine, there were two opposite opinions those could cause myopia. One was 'lack of Yinqi(陰氣)', the other was 'lack of Yangqi(陽氣)'. Originally many scholars thought that myopia was caused by laking of Yinqi(陰氣). The representative scholar was LiGao, and his work is Dongwonsunsangsihyobang(東垣先生試效方). But, some scholars insisted the opposite opinions, and the next generations accepted this opinion. The representative scholar was WangHaoGu(王好古), his work was Chasanangi(此事難知). Euihakgangmok(醫學綱目) published conversely the opinion of LiGao. Therefore, after ages appreciated that LiGao and WangHaoGu(王好古) insisted the same opinions-the cause of the myopia was lack of Yangqi(陽氣). So the most latest generation scholars knew that the cause of myopia was lack of Yangqi(陽氣). Conclusions : Historically many scholars insisted that myopia was caused by lack of 'Yangqi(陽氣)'. But that is the result of false transmission through the generation.

오수관 설계유량 산정법이 설계유속에 미치는 영향 (Design Flow Velocity Changes According to the Design Flow Determination Methods in the Sanitary Sewer)

  • The present study analyzed actual cases of designed flow estimation method and designed flow rate of sewage pipe lines. In order to examine the effects of peak-hour demand factor estimation with given daily highest peak loading, we analyzed its effects on designed flow rate with changing the peak-hour demand factor from 2.0 to 10.0. The results of this study are as follows. When reviewing the recent designs, we found that 59.4% of pipe line with 250mm and 300mm diameter, which fall under minimum allowable pipeline did not meet the minimum velocity which is specified as 0.6m/sec in design standards. The pipe line that have minimal access population or have very low slope did not satisfy the minimum velocity. In estimating the designed sewage flow, the applied daily highest peak loading and hourly highest peaking loading were the load factor for the entire population of the planned area, and for the peak loading of the initial pipes connected to a very small population, we applied the same factor as that applied to the entire area and, as a result, the hourly highest flow was underestimated. Because, in case of the initial pipes, the method of applying the same peak loading to all subject areas is highly possible to produce underestimated design flow, when estimating the designed flow of the initial pipes connected to a small population need to adopt a rational flow factor according to the size of population. For this, it is considered to investigate and analyze raw data on daily and hourly variation of sewage flow.

도심지 노면하부 지반함몰 및 공동탐사 사례 연구 (Study on Subsurface Collapse of Road Surface and Cavity Search in Urban Area)

  • 최근 도심지 도로 하부 지반함몰 사고는 상수관로, 하수관거, 통신관로, 가스관로, 배전지중관로, 지하철 등의 다양한 공익시설 및 지하구조물이 시공되고 있으며 이러한 시설물의 부적절한 시공, 노후로 인한 시설물 손상, 유지관리 소홀이 그 원인으로 추정되고 있다. 도로함몰은 도로하부에서 매설관 파손 등의 원인으로 공동이 발생한 후 공동이 성장 확대되어 포장면 하부까지 도달하고 도로를 통행하는 차량의 하중을 도로포장 부분이 지탱할 수 없을 정도의 소성변형이 발생하여 갑자기 함몰이 발생할 경우 인명피해 및 교통장애를 초래하는 등 사회전반적인 안전과 경제 발전에 위협이 될 수 있다. 서울시에서는 도로 하부의 공동이 함몰되기 이전 단계에서 3D GPR 탐사를 통해 발견하여 복구함으로써 지반함몰에 따른 피해를 사전에 차단하는 노력이 우선적으로 진행되고 있으며 그 사례를 소개하였다.

도시 하수 폐기물의 고주파 건조 (Drying by Microwave Irradiation of Sewage Waste)

  • 고주파를 이용한 오니의 건조특성에 대하여 살펴보고 재래식 건조특성과 비교 분석하였다. 고주파 건조는 빠른 속도와 높은 효율을 보였다. 재래식 건조에서는 항율건조기간이 나타나지 않았고 수분은 모세관현상에 의하여 이동하였다. 오니의 수분 중 자유수와 결합수는 존재하지 않았다. 고주파 건조에서 감율건조기간이 두 부분으로 나타났으며, 수분의 이동은 확산에 의하여 일어났다. 증발 표면적이 커질수록 수분의 제거가 촉진되었고, 오니의 지름이 커질수록 넓은 범위의 항율건조 기간이 나타났으며, 모양에 따라서도 건조특성이 다르게 나타났다. 고주파 건조에서는 오니의 중심부에서 가장 먼저 온도가 상승하였으며, 항율건조기간에서 $80{\sim}100^{\circ}C$로 일정하게 유지되면서 가장 높았다.

합류식 하수관거 월류수 및 우수관거 유출수의 수리동력학적 오염부하저감장치의 분석 (Analysis of Hydrodynamic Separators for Combined Sewer Overflows and Stromwater Runoff Control)

  • Appropriate removal of pollutants from combined sewer overflows(CSOs) and stormwater runoff is of primary concern to watershed managers trying to meet water quality standards even under a wet weather condition. Harmful substances associated with particles besides TSS and BOD are subjected to removal prior to discharge into the natural waters. Effectiveness of five major hydrodynamic separation technologies, Vortechs, Downstream Defender including Storm King for CSOs control, CDS, Stormceptor, and IHS, were evaluated in this study. There is not sufficient information for accurate evaluation of the removal efficiency for the pollutants from the stormwater runoff and CSOs. Based upon limited engineering data, however, all technologies were found to be effective in separation of heavy particles and floating solids. Technologies utilizing screens seem to have advantage in the treatment capacity than the other technologies relied fully on hydrodynamic behavior. The IHS system seems to have a strong potential in application for control of CSOs because of unique hydrodynamic behavior as well as a flexibility in opening size of the screens. Size of the particulate matter in the CSOs and stormwater runoff is found to be the most important parameter in selection of the type of the hydrodynamic separators. There exists an upper limit in the solids removal efficiency of a hydrodynamic separator, which is strongly dependent upon the particle size distribution of the CSOs and stormwater runoff.

정중곁 중뇌경색으로 인한 편측 안검하수에 대한 치험 1례 (A Case Report of Unilateral Ptosis with Paramedian Midbrain Infarction)

  • Objective: The purpose of this case report was to evaluate the effectiveness of Korean medicine for treating unilateral ptosis with paramedian midbrain infarction. Methods: One patient with unilateral ptosis with paramedian midbrain infarction was treated with acupuncture, electroacupuncture and herbal medication for 32 days. We evaluated changes in ptosis by measuring the vertical distance for the frontal gaze of the right eye. Result: After 32 days of Korean medicine treatment the vertical distance for the frontal gaze of the right eye increased from 0 to 9 cm. Conclusion: The study findings suggest that Korean medicine might be effective in treating ptosis due to a paramedian midbrain infarction.

전이성 유방암 환자의 통합 암 치료에 대한 증례보고 (A Case Report of Metastatic Breast Cancer Patient Treated with Integrative Cancer Treatment)

  • Objective : The purpose of this study is to report and observe effects of Integrative Cancer Treatment (ICT) on metastatic breast cancer patient. Method : A left breast cancer patient diagnosed with recurrence on liver and bones April 2018. The patient had received paclitaxel chemotherapy for two months and discontinued it because of tumor progression and side effects. The patient has been treated with ICT since March 2018 and has been taking capecitabine since October 2018. The clinical outcomes were measured by computed tomography, laboratory findings including tumor markers (CEA, CA15-3). The clinical outcomes were measured by computed tomography, laboratory findings including tumor markers (CEA, CA15-3), liver function test (AST, ALT), Eastern Cooperative Oncology Group performance status (ECOG PS), and numeric rating scales (NRS). Results : After the ICT, tumor size was partially decreased accompanying by reducing the levels of tumor markers. Major clinical symptoms induced by paclitaxel chemotherapy were improved. There were no severe adverse events induced by ICT based on National Cancer Institute Common Terminology Criteria for Adverse Events (NCI CTCAE) version 5.0. Conclusion : This case suggests that ICT may have synergistic effect for the treatment of metastatic breast cancer patient in conjunction with capecitabine.

  • Objective: The purpose of this study is to report the therapeutic possibility of a primary peritoneal carcinoma (PPC) patient with Traditional Korean Medicine (TKM). Methods: A 37 years old female patient diagnosed with PPC suffered from operation site pain, neck pain, nausea, headache and fatigue after undergoing operation and chemotherapy (Paclitaxel/Carboplain). The patient was treated with acupuncture, pharmacoacupuncture, moxibution and herbal medicine. Laboratory study and National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE), version 5.0 were used to evaluate the safety and adverse events of treatments. The performance status was measured by Eastern Cooperative Oncology Group (ECOG) scale. Results: After treatments, cancer did not show any progression. Related symptoms such as postoperative pain, nausea, fatigue and quality of life have improved. Conclusion: This case study implies that TKM may have therapeutic possibility in treating PPC patient.

수레바퀴 암치료법을 시행한 전이성 부신피질암 환자의 증상 개선 및 삶의 질 향상에 대한 증례보고 (A case study on a metastatic adrenocortical carcinoma patient treated by Wheel Balanced Cancer Therapy with improvement of quality of life and related symptoms)

  • Objective: The purpose of this study is to report a metastatic adrenocortical carcinoma patient treated with wheel balanced cancer therapy (WBCT) regimen. Methods: A 52-year-old female patient diagnosed and incised with metastatic adrenocortical carcinoma visited the East West Cancer Center (EWCC) on Sep. 1st, 2018 for WBCT. The patient was treated with WBCT for an approximately 2 months, from Sep. 1st to Nov. 9th. Computed tomography (CT) was used to follow-up the tumor site. Laboratory analysis and National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE), version 5.0 were used to evaluate the safety of WBCT. Results: The surgical beds after surgery (Lt. Laparoscopic adrenalectomy) were maintained without recurrence at follow up chest CT, and related symptoms and quality of life (QOL) were improved during the WBCT. Conclusion: This case study suggests that WBCT may help to improve QOL of adrenocortical carcinoma patient.

40mm 저속유탄(K200) 신관 품질개선을 통한 불발율 감소에 관한 연구 (Research on the Decrease of Dud Ammunition Rate of 40mm Grenade(K200) Fuze through Quality Improvement)

  • 최근 군에서 사격 훈련 중 1990년에 제조된 40mm 저속유탄(K200)은 탄착불발에 의한 탄약 악작용이 발생하였다. 악작용 내용은 총 72발의 사격 중 11발의 불발탄이 발생되어 불발율은 15% 정도였다. 탄약은 장기 저장 후 사용하는 품질특성을 가지고 있으므로 장기 저장시에도 성능 및 안전성 등의 확보가 필수적이다. 따라서 본 연구에서는 40mm 저속유탄용 신관의 품질개선을 통해 불발율을 최소화하고자 하였다. 첫 번째로 기폭관 성능시험, 발사시험 등을 수행한 결과 불발의 원인을 신관 화공품인 기폭관의 경시변화에 의한 성능저하로 판단하였다. 두 번째로 불발율을 최소화하기 위해 기폭관의 화약량을 증가시키고 화약량 증가에 따른 부품의 치수를 변경하는 등 40mm 저속유탄 신관의 품질개선을 추진하였다. 마지막으로 품질 개선 전 후 기존품과 개선품의 가속노화시험을 통해 저장수명을 예측해 본 결과 기존품은 6.5년, 개선품은 45.5년으로 개선되어 신관의 수명이 약 7배 향상된 결과를 도출하였다. 이에 품질 개선된 40mm 저속유탄 신관은 장기 저장시 불발율이 감소되어 40mm 저속유탄의 안전성, 신뢰성, 성능 향상에 기여할 것으로 판단된다.
