  • 본 연구에서는 근적외선대역의 광원을 이용하여 생체측정 시 에러로 작용하는 피하지방층의 두께를 추정하는 방법의 가능성에 대해 검토한다. 현재까지 피하지방층의 두께측정을 위해서는 초음파장비나 CT, Dexa 장비 등 고가의 장비로 측정을 하는방법 이외는 특별한 수단이 없는 것이 현실이다. 정확한 측정을 위해 피하지방층의 두께를 제거하는 것이 일반적인 연구의 목적이나 본 연구에서는 이들 피하지방 두께 측정을 최근 주목받고 있는 건강에 적용하고자 한다. 본 연구의 피하지방층의 두께추정방법의 가능성이 보여진다면 체지방측정에 있어 몸 전체의 체지방을 추측하는 방식인 BIA방식의 체지방측정방법과 비교하여 특정부위에 대한 피하지방층 측정도 가능할 것이라는 기대도 할 수 있다.

The Analysis of Relationship between Abdominal Adipose Tissue Distribution and Health Risk Factors in Adolescent Obese Man (청소년기 비만 남성의 복부지방조직 분포와 건강관련 위험지표의 관련성 분석)

  • In the current study, the relationships between abdominal adipose tissue depots and skinfold thickness, body circumference and composition, and blood lipids and pressure were investigated in adolescent obese man. Total abdominal adipose tissue (TAT) and subcutaneous adipose tissue (SAT) were correlated with skinfold thickness. Especially subscapular skinfold was highly correlated with TAT and SAT, and it was found that chest skinfold was the only predictor for estimating visceral adipose tissue (VAT). Body circumference and body composition were associated with TAT and SAT, not VAT, and correlation coefficient of waist circumference was highest. On the other hand, body fat mass was highly correlated with TAT and SAT than %body fat and fat free mass. It was also shown that blood triglyceride concentration and systolic blood pressure were the only predictor for estimating VAT and SAT respectively.

  • This study attempted to measure subcutaneous fat thickness by the use of ultrasonic wave in age and sex matched Korean subjects, and to observe correlation between fat thick-ness and physical indices. In male, fat thickness of suprailiac area showed the highest value of $9.40{\sim}9.51mm$ and then subscapular was $6.60{\sim}6.84mm$, femoral was $6.48{\sim}7.04mm$ and triceps regions was $3.48{\sim}3.69mm$. In female, femoral subcutaneous fat thickness was the higher. $11.85{\sim}12.15mm$and then suprailiac was $8.79{\sim}9.87mm$ subscapular was $6.20{\sim}6.91mm$ and triceps fat thickness was $4.80{\sim}4.93mm$. In male, fat thickness of triceps and relative body weight(RBW). body mass index(BMI), triceps or $R\ddot{o}hrer$ index were positively correlated. Correlations between suprailiac and weight, relative body weight(RBW). body mass index(BMT), $R\ddot{o}hrer$ index or subscapular were positively significant. In female. there were positive correlations between fat thickness of femoral and RBW, BMI, or $R\ddot{o}hrer$ index. And there were no positive correlations in other parts of the body.

20대 남성의 피하지방 분포경향에 관한 연구

  • 급속히 변화하는 현대사회에서 생활수준의 향상과 식생활의 서구화 및 신체의 활동량 감소, 과잉 영양공급으로 체중이 증가하여 체형의 변화가 일어난다. 이에 본 연구에서는 체형을 변화시키는 여러 요인 중에서 피하지방으로 인한 변화를 알아보기 위해 초음파 피하지망 계측기를 사용하여 20대 남성을 대상으로 19 .approx. 24세를 집단1, 25 .approx. 28세를 집단2로하여 각 집단간의 피하지방 분포경향과 각 집단간의 피하지방 계측피를 요인분석하여 남성복 의복제작 및 현대남성의 올바른 체형해석을 위한 기초자료 제공이 본 연구의 목적이다. 피하지방이 침착하기 쉬운 부위는 허리앞1/2과 배앞 1/2로 나타 났고, 피하지방이 침착하기 어려운 부위는 하퇴전면으로 나타났다. 피하지방두께 계측에서 집단간 유의 차 검증결과 20대 후반으로 연령이 증가함에 따라 작은 폭으로 지방침착이 일어났다. 집단1의 요인분석 결과 요인1은 허리부위, 요인2는 체간부1/2과 체간부 측면부위로 나타났다. 집단2의 요인분석결과 요인 1은 윗가슴부위와 허리부위, 요인2는 전체적부위로 나타났다. 전체집단에서는 요인분석결과 요인1은 상 지, 하지부위, 요인2는 허리부위로 나타났다. 연령의 증가와 함께 피하지방 침착이 체간부 하부인 허리 와 배를 중심으로 이루어지는 것으로 나타났다.

  • 본 논문에서는 요부 영상에서 근육을 추출하는 방법을 제안한다. 제안된 방법은 초음파 영상에서 왜곡이 존재하지 않는 영역을 측정 할 근육 영역을 설정한 후, 초기 초음파 영상에서 불필요한 잡음을 제거하고 Ends-in Search Stretching 기법을 적용하여 근육 영역의 명암 대비를 강조한다. 그리고 형태학적 특징을 이용하여 등뼈 영역과 피하지방을 분리한 후, 4 방향 윤곽선 추적 알고리즘을 적용하여 피하지방의 하단 부분을 추출한다. 또한 최대 및 최소 명암도를 조정하여 얻어진 등뼈의 후보 영역에서 형태학적 특징을 이용하여 잡음을 제거하고 최종적으로 등뼈 영역을 추출한다. 추출된 등뼈 영역에 대해 피하지방층과 등뼈 사이를 근육의 두께로 측정한다. 본 연구에서 제안된 방법을 요부의 초음파 영상에 적용하여 근육 영역을 추출한 결과, 제안된 방법이 초음파 영상에서 근육 영역들의 두께를 측정하는데 기존의 근육 측정 방법보다 효과적인 것을 확인 할 수 있었다.

Effect on the Subcutaneous Fat Thickness of the clothing Training in the Cold Condition (향한기의 표의훈련이 피하지방두께에 미치는 영향)

  • This study was intended to investigate the effect on the human body such as subcutaneous fat thickness the circumference of extremities etc. of the clothing training of putting on thin clothes periodically from the cold period. The subjects were divided into the clothing training group and the non-training group, The training group was asked to wear cool clothes in daily life and to wear the training clothes of T-shirts with half-length sleeves and pants and perform the clothing training for two hours daily three times a week in a cold environment over the period from November to February. The non-training group was asked to lead a life wearing comfortable clothes. Then a comparative experiment was conducted at 15$\pm$1$^{\circ}C$, 50$\pm$5% R, H and 0.25m/sec before and after the clothing training. After the clothing training regardless of gender subcutaneous fat thickness was more increased and total clothing weight per the surface area of the body was decreased in the training group than the non-training group. The training group showed lower skin temperature in the limbs and lower average skin temperature than the non-training group irrespective of gender which proved the effect of the clothing training. The training group was shown to have attendancy toward a greater sense of warmth and a less sense of discomfort which proved the effect of the clothing training.

Extraction and Measurements of Abdominal Muscles in Ultrasound Images Using Morphological Information of Abdominal Fasia (복부 근막의 형태학적 정보를 이용한 초음파 영상에서의 복부 근육 추출 및 측정)

  • 본 논문에서는 복부 초음파 영상에서 근육 영역의 명암 대비를 강조하기 위해서 Multiple 연산을 적용한 후, 명암 대비가 강조된 영상에서 수직 방향의 명암도가 200 이상인 픽셀들에 대해 퍼지 기법에 적용하여 이진화한다. 이진화된 영상에서 피부층과 외복사근 사이에 존재하는 피하지방을 추출하기 위해 Thick-Search 방법을 적용하여 피부층과 외복사근을 추출한 후, 피부층과 외복사근의 사이에 Up-Down Search 방법을 적용하여 피하지방층을 추출한다. 피하지방층이 추출된 영상에서 근막의 형태학적 정보를 이용하여 근막을 추출한다. 추출된 근막 사이에 대해 Up-Down Search 방법을 적용하여 근육의 후보 영역을 추출한 후, 근육의 형태학적 정보를 이용하여 최종적인 근육 층을 추출한다. 추출된 근육의 경계선을 Monotone Cubic Hermite 보간법을 이용하여 근육의 경계선을 보정한 후, 최소자승법을 이용하여 근육의 두께를 측정한다. 제안된 방법을 복부 초음파 영상에 적용하여 근막 및 근육 영역을 추출한 결과, 기존의 근육 추출 방법보다 정확하게 추출되었고, 근육의 두께 측정 결과도 전문의가 육안으로 측정한 결과와 근사한 것을 확인할 수 있었다.

Gluteal Subcutaneous Fat Thickness Measured by Computed Tomography as an Estimate of Proper Gluteal Intramuscular Injections in Korean Adults (전산화 단층촬영을 이용한 한국 성인의 둔부 피하지방두께 측정 -근육주사 바늘길이 산정을 위한 기초조사-)

  • Purpose: To study the thickness of gluteal subcutaneous fat (SCF) and propose an adequate length for needle for gluteal intramuscular injections based on computed tomography (CT) measurements. Methods: The thickness of gluteal SCF were measured and studied for 568 patients who visited a tertiary hospital in Seoul, Korea between January 2007 and February 2009 for routine health screening and who had abdominopelvic CT. Results: The average thickness of gluteal SCF was $15.92{\pm}4.08mm$ in males and $24.90{\pm}5.47mm$ in females. The thickness of gluteal SCF differed significantly according to gender. The gluteal SCF thickness was greater than 20.4 mm for 54 (12.3%) of the 440 male patients and 99 (77.3%) of the 128 female patients. Conclusion: The most common syringe needle used for gluteal intramuscular injections in Korea is a 23 G, 25.4 mm-needle. The SCF thickness must be less than 20.4 mm in order to reach the dorsogluteal muscles to a depth of at least 5 mm if this 25.4 mm needle is used. In many patients, especially in female patients, the 25.4 mm needles will result in improper intramuscular injections with the injection being into the SCF. Therefore an appropriate needle should be selected by considering the gender and SCF thickness of patients receiving gluteal intramuscular injections.

Extraction of Lumbar Multifidus Muscle using Ultrasound Imaging (초음파 영상에서 다열근 추출)

  • In this paper, we propose a new method for extracting muscles from lumbar images. The proposed method sets areas without distortions with field expert's assistance as areas of measuring interest and removing noises from initial ultrasonic videos. Then, the method emphasizes the brightness contrast with Ends-in search stretching algorithm and separate thoracic vertebra from subcutaneous fat area using morphological characteristics. 4-directions contour tracing algorithm is applied to extract the bottom of subcutaneous fat area. Extracting thoracic vertebra area requires noise removal and morphological characteristics as well among candidate areas obtained by controlling min-max brightness. The thickness of muscles is then defined as the length between subcutaneous fat area and extracted thoracic vertebra. The experiment which consists of 368 image analysis verifies that the proposed method is more effective in measuring the thickness of muscles than before.

Evaluation of Gestational Diabetes Mellitus Risk Factors Using Abdominal Subcutaneous Fat Thickness for Early Pregnancy in the US Imaging (초음파영상에서의 임신초기 복부피하지방두께를 이용한 임신성당뇨 위험인자 평가)

  • The purpose of this study was to investigate the relationship between abdominal subcutaneous fat thickness(ASFT) and maternal gestational diabetes mellitus(GDM) measured by ultrasound at period of pregnancy. We compared maternal age, pre-pregnancy body mass index, and weight gain during pregnancy in 286 pregnant women who were diagnosed with early pregnancy ASFT and high GDM screening test(50 g OGTT) of more than 140 mg/dL. ROC curve analysis was used to determine the cut-off value of ASFT for GDM prediction. Maternal age and weight gain during pregnancy were not related to GDM in the mid-trimester and pre-pregnancy body mass index and earely pregnancy ASFT were significantly different between normal and GDM high risk groups. The cut-off value of ASFT for GDM prediction was 2.23 cm(AUC 0.913. Sensitivity 76.19%, Specificity 93.72%). ASFT measured by ultrasound in early pregnancy was useful as an important index for predicting mid-trimester GDM prediction. Therefore, ASFT can be used as an auxiliary diagnostic index for early recognition of GDM.
