In order to quantify the contribution of cosmic-ray ionizing component to the dose given by natural background radiation, a series of measurement has been carried out using LiF TLDs for about one and a half years on quarterly basis. Three different types of LiF TLDs namely, chips and PTFE based disks of $^{7}LiF$, and the same disks of $^{6}LiF$ for identifying possible contribution of neutron component were used. Measurements were made by placing badge-incased TLDs in a lead castle of 10 to 15cm thick installed in a room on the third floor of a four-story building in CNU Daedeok campus for 5 cycles of 90 days. For comparison a series of spectrometric study was also performed for the energy region over 3MeV using a 3'${\phi}\;{\times}\;3$'NaI(Tl) scintillation detector in association with an MCA of 1024 channels, and it was found that the data obtained by the TLDs placed in the lead castle indicate 75% of the dose given by outdoor cosmic-ray component. The results obtained by the TLDs through correction for shielding loss show that the outdoor dose contribution of ionizing component of cosmic rays at this campus is $34.3{\pm}1.1nGy/h$ which satisfactorily agrees with that expected for our particular location of measurement.
Shielding for reducing exposure dose can make the diagnosis limited. The purpose of this study is to increase the efficiency of radiation protection and minimize the loss of image information by producing the shielding made of the water and the contrast medium which has different proportion and finding out the ideal proportion of them. Each shielding materials were made of water and water-soluble iodine contrast medium with the different proportion. The attenuation rate of absorbed dose was evaluated by the shielding materials in the plastic contents for measuring the efficiency of the radiation protection. As a result, the higher ratio of the contrast medium, the more efficient it is for radiation attenuation. The anatomical structure was observed most properly in case of the solution with 20 ml of the contrast medium and most difficultly in case of more than 60 ml of the contrast medium. In case of the signal intensity between skeleton and gas, the difference of average value had a significant as p < 0.001. Shielding with contrast medium attenuates less than the conventional shielding but in the examination for the sensitive part to radiation, it can be used to minimize the loss of the image information and reduce the exposure dose.
Fundamental data on the safety status is necessary to develop and operate the additional policies related to the radiation safety, however, few studies have been conducted to investigate the status quantitatively. In this study, out of the areas related to radiation safety, the status of the radiation worker, ambient radiation and the environmental radiation were examined in the quantitative aspect using diverse statistical methods. In the results of that, following information were obtained. First, out of the radiation workers engaged in 9 businesses as of end of 2014, the business having highest exposure dose is NDT showing significantly high than other businesses. Second, the average difference of the exposure doses in radiation workers in 9 businesses in 2014 compared to that in 2010 was not statistically significant at significance level of 5% (p=0.221). Third, the average of the environmental radiation in Gyeonggi-do was high than the Jeollabuk-do. The results of this study are anticipated to assist to utilize the basic data for the workers on radiational works, daily living radiation, and the safety control related to the environmental radiation, and to establish the policy of radiation protection in the future.
When a radiation generating device is installed in an export container due to COVID-19, the purpose of this study was to measure the space dose in the radiation room and to study the effectiveness of the shielding wall in the laboratory. Air dose measurement method was set behind the X-ray tube, 50 cm, 100 cm, 200 cm, and measured 12 locations. The dose values before and after the use of the movable radiation shielding wall were compared by measuring 3 locations behind the X-ray tube using the movable radiation shielding wall. The measured values were 50 cm on the left behind the X-ray tube: 1.446 μSv, behind the X-ray tube: 0.545 μSv, and 50 cm on the right behind the X-ray tube: 1.466 μSv. Measurements behind the radiation barrier were 0.190 μSv, 0.204 μSv, and 0.191 μSv. As a result of performing the corresponding sample t test of the average value according to the use of movable barrier walls, p <0.001 was found. As a result of the actual measurement, the medical exposure of the examiner due to the shielding wall in the laboratory decreased to 82.3%. In order to reduce occupational exposure in screening radiological laboratories, it is recommended that sufficient separation from radiation sources and the use of shielding walls are recommended.
In this study, we analyzed the incidence of side effects of photoneutron dose according to the number of beams during intensity-modulated radiotherapy of prostate cancer using 15 MV. The radiation treatment plan design for intensity-modulated radiation therapy for prostate cancer was established with a prescription dose of 220 cGy per dose and a total of 7260 cGy for 33 treatments. The linear accelerator used in the experiment is Varian's True Beam STx (Varian, USA). Photoneutron dose was generated by using 15 MV energy in the planning target volume (PTV). The treatment plan was designed with IMRT 5, 7, and 9 portals using the Eclipse System (Varian Ver 10.0, USA). An optically stimulated luminescence albedo neutron dosimeter (Landauer Inc., USA) was used to measure photoneutron dose. IMRT 5 portals, 1.7 per 1,000, 7 portals, 1.8 per 1,000, 9 portals, 2.0 per 1,000 were calculated as the probability of experiencing side effects on the thyroid gland due to photoneutron dose. This study studies the risk of secondary radiation exposure dose that can occur during intensity-modulated radiation therapy, and it is considered that it will be used as useful data in relation to stochastic effects in the future.
Our purpose is to specify behavior and environmental factors aimed at reducing the exposed dosage caused by PET-CT and to develop radiation safety management guidelines adequate for domestic circumstances. We have used a multistep-multimethod as the methodological approach to design and to carry out the research both in quality and quantity, including an analysis on previous studies, professional consultations and a survey. The survey includes responses from 139 practitioners in charged of 109 PET-CTs installed throughout Korea(reported by the Korean Society of Nuclear Medicine, 2010). The research use 156 questions using Cronbach's ${\alpha}$ (alpha) coefficients which were: 0.818 for "the necessity of setting and installing the radiation protective environment"; 0.916 for "the necessity of radiation protection", "setting and installing the radiation protective environment"; and 0.885 for "radiation protection". The check list, derived from the radiation safety management guidelines focused on behavior and environment, was composed of 20 items for the radiation protective environment: including 5 items for the patient; 4 items for the guardian; 3 items for the radiologist; and 8 items applied to everyone involved; for a total of 26 items for the radiation protective behavior including: 12 items for the patient; 1 item for the guardian, 7 items for the radiologist; and 6 items applied to everyone involved. The specific check list is shown in(Table 5-6). Since our country has no safety management guidelines of its own to reduce the exposed dosage caused by PET-CTs, we believe the guidelines developed through this study means great deal to the field as it is not only appropriate for domestic circumstances, but also contains specific check lists for each target who may be exposed to radiation in regards to behavior and environment.
The purpose of this study was to investigate changes in surface dose due to increased scattering of gamma rays from patients injected with 99mTc and 18F, which are radioactive isotopes, in close contact with materials with high atomic number such as the walls of the stable room. Prepare 99mTc and 18F by injecting 20 and 10 mCi respectively into the NEMA phantom, and then measuring the surface dose for 60 minutes by positioning the phantom at a height of 1 m above the surface, at a distance of 0, 5 and 10 cm from the wall, and at the same location as the phantom facing the wall. Each experiment was repeated five times for reproducibility of the experiment and one way analysis of variability (ANOVA) was performed for significance testing and Tukey was used as a post-test. The study found that surface doses of 220.268, 287.121, 243.957, and 226.272 mGy were measured at 99mTc, respectively, in the case of empty space and in the case of 0, 5 and 10 cm, while those of 18F were measured at 637.111, 724.469, 657.107, and 640.365 mGy, respectively. In order to reduce changes in surface dose depending on the patient's location while waiting, it is necessary to keep the distance from the ground or the wall where the patient is closely adhered to, or install an air mattress, etc., to prevent the scattered lines as much as possible, considering the scattered lines due to the wall etc. in future setup of the patient waiting room and safety room, and in addition to the examination, the external skin width may be reduced.
High-dose $^{131}I$ therapy has been generally carried out to remove remaining thyroid tissue or to cure metastasize lesion of patients who received full thyroidectomy due to differentiated thyroid cancers. In case high-dose $^{131}I$ therapy is carried out for a patient, the patient should be hospitalized being isolated for a certain period in order to restrict the amount of exposure to radiation of people at large from the patient within the limit of a level of radiation. Effective half-life is an important value to calculate how family members are exposed to radiation from a patient or to decide the period of isolation of the patient from the family members. Therefore, in this study we calculated the effective decay constant, effective half-life and period of isolation of high-dose $^{131}I$ therapy patient using NM670 SPECT/CT. As a result of carrying out this study, the effective half-life of high-dose $^{131}I$ therapy patients was derived and the time to reach the discharge level of 1.2 GBq was confirmed. When they were compared with each other in each of curative doses, the effective half-life did not have significant difference, but the time when the level of radiation remaining in the interior of the body to reach the criteria of isolation and discharge showed significant difference and it could be confirmed that the higher the curative dose the longer the period of isolation becomes. When the effective half-lives in each type of preparation were compared with each other, they did not show significant difference. However, When the times to reach the level of radiation that is the criteria of isolation and discharge in each type of preparations, they showed significant difference. The cause of the shortening of the isolation period for rhTSH patients group is decided to be low curative dose. Accordingly, if the current national health insurance (the insurance is applied to using of rhTSH in 3.7 GBq or lower) is maintained, while discerning them in each of types of preparation, we would be able to discharge patients at the time earlier than the current period of isolation (2 nights and 3 days).
Purpose : To improve the local control of patients with nasopharyngeal cancer, we have implemented 3-D conformal radiotherapy and forward intensity modulated radiation therapy (IMRT) to used of compensating filters. Three dimension conformal radiotherapy with intensity modulation is a new modality for cancer treatments. We designed 3-D treatment planning with 3-D RTP (radiation treatment planning system) and evaluation dose distribution with tumor control probability (TCP) and normal tissue complication probability (NTCP). Material and Methods : We have developed a treatment plan consisting four intensity modulated photon fields that are delivered through the compensating tilters and block transmission for critical organs. We get a full size CT imaging including head and neck as 3 mm slices, and delineating PTV (planning target volume) and surrounding critical organs, and reconstructed 3D imaging on the computer windows. In the planning stage, the planner specifies the number of beams and their directions including non-coplanar, and the prescribed doses for the target volume and the permissible dose of normal organs and the overlap regions. We designed compensating filter according to tissue deficit and PTV volume shape also dose weighting for each field to obtain adequate dose distribution, and shielding blocks weighting for transmission. Therapeutic gains were evaluated by numerical equation of tumor control probability and normal tissue complication probability. The TCP and NTCP by DVH (dose volume histogram) were compared with the 3-D conformal radiotherapy and forward intensity modulated conformal radiotherapy by compensator and blocks weighting. Optimization for the weight distribution was peformed iteration with initial guess weight or the even weight distribution. The TCP and NTCP by DVH were compared with the 3-D conformal radiotherapy and intensitiy modulated conformal radiotherapy by compensator and blocks weighting. Results : Using a four field IMRT plan, we have customized dose distribution to conform and deliver sufficient dose to the PTV. In addition, in the overlap regions between the PTV and the normal organs (spinal cord, salivary grand, pituitary, optic nerves), the dose is kept within the tolerance of the respective organs. We evaluated to obtain sufficient TCP value and acceptable NTCP using compensating filters. Quality assurance checks show acceptable agreement between the planned and the implemented MLC(multi-leaf collimator). Conclusion : IMRT provides a powerful and efficient solution for complex planning problems where the surrounding normal tissues place severe constraints on the prescription dose. The intensity modulated fields can be efficaciously and accurately delivered using compensating filters.
Since 1977, KAERI has conducted the fundamental R&D on the permanent disposal of potential HLW repository in Korea. The first ten year project is divided into three short-term phase studies. The first phase study which shall be finished in March of 2000, has the prime target to develop the disposal concept of HLW. Throughout this study the preliminary and generic disposal repository system has been introduced. The potential repository is proposed to be emplaced into crystalline rocks which is the most common rock types in Korea. The proposed depth of the repository is between 300 to 700 meter. The numerical code, MASCOT-K was developed to asserts the long term safety of the proposed repository concept. Based on this conceptual design preliminary safely assessment was performed. Results show that for the given disposal system the potential radioactive release it well below the regulatory limit.
