The Effects of Mechanical Stimulation using Graston on Changing Trigger Point Pressure Pain Threshold and Muscle Tone of the Same Spinal Segment in Neck Disk Patient (목 디스크 환자에게 그라스톤을 이용한 기계적 자극 시 동일 척수 분절의 통증 유발점 압통 역치 및 근 긴장도에 미치는 영향)

  • The purpose of this study was to identify whether cutaneous sensory (CS) changes induced by mechanical intervention(MI) increases the trigger point threshold and muscle tone of the same spinal segment to neck disc patients. Thirty persons with Neck disc patients were recruited in this experiment. The subjects consisted of 10 men and 20 women. The mechanical stimulus group induced CS changes for 5 minutes using the Graston instrument and the control group received no action. The CS changes were estimated by using the Von Frey Filament, PPT changes were measured by using the pressure threshold meter and msucle tone changes were measured by using Myotone pro. CS threshold increased significantly when MI was applied (p<0.05). On the same spinal segment, increases in the right infraspinatus PPT and muscle tone was observed (p<0.05) and decreases in the right trapezius PPT was observed(p<0.05). However, the PPT and muscle tone changes in other muscles were not significantly different. Furthermore, the control group CS, PPT and muscle tone were not significantly different. As a result, CS changes induced by MI make to change PPT and muscle tone on the same spinal segment. Therefore, application of MI to the same spinal segment may be of clinical significance as a new rehabilitation method for increasing pain threshold, muscle tone and pain control in neck disc patients.

  • The primary goal of this study was to provide a systematic methodology of utilizing 3D technology for tight-fit performance sportswear using information of skin deformation in various posture. Technical tools used in this study are Cyberware whole body scanner, RapidForm2004, 2C-AN 3D pattern development program, and YukaCAD. Analysis of the 3D skin deformation while knee joint was bent from $0^{\circ}$ to $60^{\circ}$ revealed that the length of dermatomes L4 was remained consistent during knee bending. Therefore, L4 was chosen as a major cutting line. To develop a highly ergonomic pattern, replicas of static and dynamic postures were developed and integrated using two methods, one is morphing method (Sqirlz Morph), and the other is AutoCAD. Experimental tight-fit garments called 'Derm-Mov Pattern' was designed using dematomes L4, L2, and inner line under knee and compared with four other patterns. As results, AutoCAD was appropriate as a integrating method of various postures. In wear test, 'Derm-Mov Pattern' was rated high (p < .001), in terms of pressure comfort especially around front crotch area. However, wear sensation was not signipicantly different in other area due to highly extensible property of materials. Pressure distribution was relatively even in these experimental garments.

  • Wolf's isotopic response is defined as the occurrence of a new skin disorder at the site of another unrelated skin disease that has already healed. In most cases of isotopic response, the initial dermatosis is herpes infection, and the most frequent second dermatoses are granulomatous reactions. Various interpretations of this phenomenon have been attempted. However, the exact mechanism has not been identified yet. Herein, we report a case in which the secondary disease was segmental vitiligo that appeared over the same dermatomes of herpes zoster. A 71-year-old woman presented with well-defined, depigmented patches on the left chest and back. She had been diagnosed with herpes zoster on the same dermatomes and treated with an antiviral agent 3 years ago. Histological examination showed decreased basal melanin pigments and melanocytes. Consequently, the patient was diagnosed with segmental vitiligo based on the clinical and histological findings.

  • A 45 year old man was admitted for further examination of an abnormal shadow of the right posterior mediastinum. The patient suffered from dysesthesia in the right thoracic wall of dermatome T7. CT scan and MRI revealed that two separate tumors had developed in the right paravertebral area linked to the vertebral canal via an intervertebral foramina. One-stage removal of the tumors were performed safely through the right posterolateral thoracotomy following the resection of the rib head and vertebral pedicle. The tumors were confirmed as histologically neurilemomas. The postoperative course was uneventful.

The Effects of Systemic Morphine to Analgesic Level in Spinal Anesthesia (Morphine 정주가 척추마취의 레벨에 미치는 영향)

  • 척추마취는 국소마취제를 지주막하강에 주입하여 척수신경 전근과 후근을 차단하는 방법으로 하복부나 하지 수술 뿐 아니라 만성 통증과 암성 통증의 치료에도 이용되고 있는데 마취시간이나 제통시간의 연장 및 적절한 피부분절의 마취나 진통의 달성은 척추마취에서 중요한 사항이다. 본 연구에서는 morphine정주가 척추마취에 어떤 영향을 주는지 알아보기 위해 척추마취하에서 하지 수술을 받은 40명의 환자를 대상으로 척추마취를 시행한 80분에 척추마취 레벨, 수축기 및 이완기 혈압, 맥박 그리고 호흡수를 조사한후 morphine 10 mg을 정맥내로 주사후 20분후에 척추마취 레벨과 혈압, 맥박, 호흡수를 조사하여 다음과 같은 결과를 얻었다. 1) 척추마취 레벨은 morphine 투여진 $T_{7.5{\pm}0.32}$에 비해 morphine 투여 20분후에 $T_{6.0{\pm}0.31}$로 의의있게 상승하였다 (p<0.005). 2) 수축기 및 이완기 혈압과 맥박수는 morphine투여전과 투여후에 의의있는 변화가 없었다. 3) 호흡수는 morphine 투여전에 비해 투여후 감소가 있었다(p<0.005). 이상의 결과로 척추마취하에서 수술을 시행할 때나 통증치료시 전신적으로 morphne을 투여하여 마취와 진통부위를 넓일 수 있을 것으로 사료된다.

A Case of Cockayne Syndrome with Focal Segmental Glomerulosclerosis (국소성 분절성 사구체 경화증(FSGS)을 동반한 Cockayne 증후군 1례)

  • Cockayne syndrome is a rare autosomal recessive disorder characterized by cachectic dwafism, mental retardation, loss of facial subcutaneous adipose tissue, microcephaly and photosensitive dermatitis. It is associated with renal abnormalities characterized by hyalinization of glomeruli, atrophy of tubules and interstitial fibrosis. To our knowledge, this is the first report of a case of Cockayne syndrome with FSGS in Korea. A 7-year old boy was admitted for evaluation of hypertension and proteinuria, which were detected 2 month ago. He was followed for short stature(<3 percentile), mental retardation(IQ 55), strabismus and dental caries since 3 years ago. He also showed microcephaly, a bird-like face and relatively large hands and feet. Laboratory findings showed decreased creatinine clearance($C_{Cr}$ 76.1 mL/min/$1.73m^2$) and proteinuria(1,548 mg/day). Renal biopsy demonstrated focal segmental glomerulosclerosis of the hilar type with large hyaline deposits, moderate tubular atrophy and interstitial fibrosis. His cardinal features, mental retardation, and renal biopsy findings were consistent with Cockayne syndrome. We report here a very rare case of Cockayne syndrome with FSGS presenting with proteinuria and hypertension.

Induction of Apoptosis by Ursolic Acid in F9 Teratocarcinoma Cells (F9 기형암종세포에서 Ursolic acid의 apoptosis 유도기작)

  • The apoptosis-inducing activity of ursolic acid (UA) was examined in mouse F9 teratocarcinoma cells on the bases of biochemical and morphological characteeristics. UA, pentacyclic trierpene acid, exhibits antitumor activities including inhibition of skin tumorigenesis, induction of tumor cell differentiation and antitumor promotion. Treatment with UA showed that the decrease of cell viability was dose-dependent. UA also induced genomic DNA fragmetation, a hallmark of apoptosis, indicating that the mechanism of UA-induced F9 cell death was through apoptosis. When the morphology of the F9 cells was examined by electron microscopy, the cells treated with UA showed the charcteristic morphological features of apoptosis such as chromatin condensation and nuclear fragmentation. DNA fragmentations by UA were inhibired by cycloheximide, which suggest that de novo protein synthesis was required for DNA fragmentation by UA. Inaddition, the expression of c-jun was increased, but those of c-myc and laminin B1 were decreased during apoptosis induced by UA in F9 cells. These results suggest that UA causes an apoptosis in F9 cells. Further, the increased expression of c-jun may be involved in the UA-induced apoptosis of f9 cells.

Myofascial restriction affect and change the physical posture by losing the muscle elasticity and flexibility and by losing muscle supporting ability from gravitation. 5. The partial myofascial restriction affect the muscle and the adjoining joint supporting gravitation and cause the unbalance of the entire body.

  • Ceotrichum candidum was identified from 2 dogs among 52 dogs which had various dermatological problems. One was 5-month-old male Labrador and the other was 7-month-old female German Shepherd. The diagnosis was confirmed by direct microscopy of the specimen as well as cultured isolation of the fungus from the cutaneous lesions. The. organism showed septate hyphae and rectangular to cylindrical arthrospores and no blastospores. Urea was not utilized and no assimilation of maltose was observed. Clinical improvement was occurred by application of 2% gentian violet and 2% miconazole solutions. It is suggested that this organism should be included in the differential diagnosis of canine dermatitis.

  • Epidural adhesiolysis is a convenient and safe method for the management of back pain. However, we experienced a case where a patient developed sensory change to S2 area after epidural adhesiolysis. Male patient, 43 years old, was admitted to our pain clinic for epidural adhesiolysis for back pain. Patient was experiencing pain radiating to left thigh, and sensory change and motor disturbance to the S1 area. Patient's symptoms and signs were much improved on the first day of epidural adhesiolysis. Patient, however, complained of numbness of perineal and S2 areas after the next day of injections. We postulated the cause of this complication was due to: compression of nerve root by the large volume of injectate and hematoma, and the side effect of local anesthetic, hypertonic saline and steroid.
