A Comparative Study on the Quality of Living for Therapeutic Cancer and Hospiece Patients (치료 암환자와 호스피스 환자에 대한 삶의 질 비교)

  • This study carried a comparative analysis of quality of living perceived by cancer and hospiece patients who received radiotheraphy, and influential factors in order to provide basic data for nursing goals and establishment of strategy. The subjects of the study were 50 cancer patients who were more than twenty years old and was receiving radiotheraphy in therapeutic radiology department of C university hospital, and fourteen hospiece patients who were in J hospital in Gwangju. They were conveniently sampled according to the selection standard, and researchers personally interviewed them using questionnaire and patient scripts to obtain necessary data. The results were presented as follows: 1. When cancer and hospiece patients were examined demographically, the number of 60 year-old patients were the most. The subjects whose marriage period was more than thirty-one years were the most. In medical expense, more than $70.0\%$ of the patients bore their expenses themselves. 2. When disease-related characteristics of the cancer and pospiece patients were exmained, more than $75\%$ of the patients had experience of being in hospital, and more than $60.0\%$ experienced operation. However, for prevalence period, $57.5\%$ of the cancer patients had less than six months, and $64.3\%$ of the hospiece patients had more than two years. 3. For physical symptoms of cancer patients, $77.5\%$ had fatigue, $60.0\%$ had loss of appetite, and $52.5\%$ had loss of weight while for the hospiece patients, $100\%$ had loss of weight, and $92.9\%$ had fatigue and loss of appetite. For the cancer patients, $0.0\%$ had swelling, and $7.5\%$ had bleeding, For the hospiece patients, $7.1\%$ had change in skin, and $14.3\%$ had diarrhea. 4. Mean score of the cancer subjects were as follows: family support, social support, emotional and spiritual support, physical symptoms, and periods were 3.87, 2.88, 3.10, 2.80, and 2.94 respectively. Those of the hospiece patients were 3.80, 1.96, 1.58, 2.64 and 3.24 respectively. 5. Mean score of family support of both patients were almost identical, but in character, a considerable difference was found: 3.10 and 1.58. In qualify of living, the mean score of hospiece patients was slightly lower.

Analysis of target volume motion followed by induced abdominal compression in tomotherapy for prostate cancer (전립선암 환자의 복부압박에 따른 표적 움직임 분석)

  • Purpose : To evaluate the changes of the motion of abdominal cavity between interfraction and intrafraction by using abdominal compression for reducing abdominal motion. Materials and Methods : 60 MVCT images were obtained before and after tomotherapy from 10 prostate cancer patients over the whole radiotherapy period. Shift values ( X -lateral Y -longitudinal Z -vertical and Roll ) were measured and from it, the correlation of between interfraction set up change and intrafraction target motion was analyzed when applying abdominal compression. Results : The motion changes of interfraction were X-average $0.65{\pm}2.32mm$, Y-average $1.41{\pm}4.83mm$, Z-average $0.73{\pm}0.52mm$ and Roll-average $0.96{\pm}0.21mm$. The motion changes of intrafraction were X-average $0.15{\pm}0.44mm$, Y-average $0.13{\pm}0.44mm$, Z-average $0.24{\pm}0.64mm$ and Roll-average $0.1{\pm}0.9mm$. The average PTV maximum dose difference was minimum for 10% phase and maximum for 70% phase. The average Spain cord maximum dose difference was minimum for 0% phase and maximum for 50% phase. The average difference of $V_{20}$, $V_{10}$, $V_5$ of Lung show bo certain trend. Conclusion : Abdominal compression can minimize the motion of internal organs and patients. So it is considered to be able to get more ideal dose volume without damage of normal structures from generating margin in small in producing PTV.

  • For the determination of principal taste components in L. chinensis M., A. acutiloba K., S. chinensis B. and A. sessiliflorum S., which were extracted with water and ethanol, contents and composition of free sugars, free amino acids, non-volatile organic acids and tannins were analyzed. The extraction yield of them was high when using the water as an extraction solvent, the optimal ethanol concentration with high yield was in the range of 50-75%. The contents of free amino acids in their extract $(25^{\circ}Bx)$ was high when using the ethanol as the extraction solvent. The amino acid content was in the range of 123-159 mg%. in samples. The free sugar of extracts contained 2-12% in samples which were consisted of glucose, fructose and sucrose. The pH and acidity range of extracts were 4.4-5.1 and 1.01-2.27% respectively, and especially extracts of S. chinensis B. showed to be strong acid as pH 2.9 and addity 11.93%. The composition of non-volatile organic acid in extracts of S. chinensis B. contained citric acid and malic acid 3.90% and 3.92% respectively as major components. The tannin content of extracts was considerably high when extracting with 50% ethanol, especially A. sessiliflorm S. contained $3.35%{\sim}3.85%$.

Preparation and Characteristics of Bread by Medicinal Herb Composites with Immunostimulating Activity (면역활성을 가진 생약복합물을 이용한 빵의 제조 및 특성)

  • In this study, the breads with medicinal herbs (MH) composites showing immunostimulating activity were prepared and their characteristics were examined. Fourteen kinds of medicinal herbs were extracted with hot water and divided into 3 groups (MH-1, MH-2, MH-3) based on their contents. All groups showed immunostimulating activity in terms of macrophage phagocytosis, nitrite production, cytostatic activity and cytokine production. In the preparation of breads containing MH extracts of various contents (0, 30, 50, 70, and 100%), there was no significant difference among dough pHs of all groups after first fermentation, but loaf volume was significantly (p<0.05) increased in 70% added group while decreased in 30%, 50%, and 100% added groups compared to the control. The "a" and "b" values of bread crumb increased with the content of MH extracts while "L" value decreased, but these values of bread crust were similar to the control group. Most improvements in hardness, adhesiveness, gumminess and chewiness of bread were noticed by the addition of 70% MH extracts, but those of springiness, cohesiveness and resilience were mostly by the 50% addition ones. Through the sensory evaluation, it was revealed that mouth feeling, taste and overall preference decreased at breads containing 70% and 100% extracts, although appearance and crumb texture were not significantly (p<0.05) different among all groups.

Correlation between Smartphone Addiction and Eating Behaviors of Elementary School Students in Ansan Area (안산지역 초등학생의 스마트폰 중독과 식행동과의 상관성)

  • This study investigated the correlation between smartphone addiction and eating behaviors of 288 elementary school students (fifth and sixth grades) in the Ansan area. Addiction and normal groups consisted of 29 (10.1%) and 259 students (89.9%). Average usage time of the smartphone addiction group was higher than that of the normal group (P<0.001). Percentages of using a smartphone when eating meals as well as skipping meals due to using a smartphone were also higher in the addiction group than the normal group (P<0.01). Degree of reduction of physical activity and sleep disorders due to using a smartphone were higher in the addiction group than the normal group (P<0.001). However, nutritional knowledge related to protein and dietary life scored lower in the addiction group than the normal group (P<0.05). Correlation between smartphone usage time and eating behaviors was as follows: smartphone usage time on weekdays increased with elevated usage time on weekends in the addiction group (+0.742) (P<0.001) and the normal group (+0.762) (P<0.001). Frequency of snack intake in the addiction group increased with elevated usage time on weekdays (+0.460) (P<0.05). Sleeping time of the normal group decreased with elevated usage time on weekdays (-0.150) (P<0.05), and good eating behaviors decreased with elevated usage time the on weekend (-0.143) (P<0.05). Consequently, smartphone addiction in elementary school students showed negative eating behaviors. Therefore, effective nutritional education programs and national support policy are required to solve problems of unhealthy eating behaviors from smartphone usage in elementary school students.

Content and Characteristics of Forest Cover Changes in North Korea (북한(北韓) 지역(地域) 산림면적(山林面積) 변화(變化)의 규모(規模)와 특성(特性))

  • It has been rare to obtain reliable information related to the size of forest land in North Korea. Several sources of forest statistics, ranging from the first map of forest distribution in Korean Peninsula produced in 1910 to official data reported by the North Korea Government in 1997, were gathered and analyzed to define the characteristics of forest cover changes over years. In addition, Landsat satellite data obtained from 1973 to 1993 were processed for the two study areas of the provinces of Pyungyang and Heasan, where the topography and land use pattern are significantly different each other. Using three sets of multitemporal Landsat imagery, land cover ma-ps were produced by computer classification. Although forest statistics reported before 1990 are somewhat inconsistent, they mere gradually decreasing over years. The estimates of 1991 satellite data and the recent statistics reported in 1998 shows very steep decline in forest lands as compared to the ones before 1990. The abrupt decrease of forest lands after 1990 was also found on the detailed analysis of Landsat data for the two study areas of Pyungyang and Heasan. The rapid decline of forest lands may have something to do with the poor economic situation of the country and the continuing natural disasters of severe flooding and drought. Unstocked forest, which was not classified into forest land, was a very distinct and pervasive land cover type that can be easily observed on satellite imagery. Since unstocked forest land in North Korea may be a critical factor for degrading environmental quality as well as for the continuing natural disasters, further analysis is necessary to define the exact extent and the physical characteristics of the cover type.

The Meta-Educational Implications of Toson-Shibigok ("도산십이곡"에 나타난 메타교육적 함의)

  • Toson-Shibigok(Twelve Songs of Toson) which T'oegye, Yi Hwang, composed has received a lot of attention in terms of its educational implications. These implications have become the essential part of Gojeon Sigas(classic songs and poems), and solid evidence has also been accumulated in the literature. Therefore, the purposes of this study are twofold, that is 1) to explore the impact of T'oegye's fundamental structure of the first education at that time and 2) describe how these educational attitudes will eventually yield meta- educational values. What T'oegye meant by fundamental structure of the first education was that Sijos originated from his own experience should be the vital part of teaching method. One can say that T'oegye's teaching method is more integrative than that of modern literature education in that it includes songs. Not only using his own teaching method, but T'oegye tried to demonstrate his own literary structure to his colleagues and the future generations. In the end, Meta-Education which comprises the heart of recent educational theories well reflects Toegye's theoretical propositions. Rather than giving practical knowledge, his method offers the paradigms of thought patterns and shows the learners' attitudes in approaching the truth. Moreover, T'oegye's teaching method has become a good example to the future generations as well as his contemporary followers. In sum, although a lot of years have passed since he died, T'oegye's Sijo works and life still have invaluable impact on the field of education.

The taxonomic consideration of leaf epidermal microstructure in Korean Thymelaeaceae Adans (한국산 팥꽃나무과 잎표피 미세구조의 분류학적 검토)

  • To examine the leaf epidermal microstructure, nine species in five genera (Daphne L. - 4 spp., Diarthron Turcz. - 1 sp., Edgewarthia Meisn. - 1 sp., Stellera L. - 1 sp., Wikstroemia Endl. - 2 spp.) of the Korean Thymelaeaceae were investigated by light microscopy (LM) and scanning electron microscopy (SEM). The stomata of stuo야ed taxa were 'hypostomatic type' and the size range of guard cell was $13.8-34.4{\times}8.7-22.9{\mu}m$: the smallest size of stomata was found in Diathran linifolium ($15.9{\pm}2.6{\times}10.0{\pm}1.3{\mu}m$), while the largest one was measured to Daphne adara ($32.8{\pm}1.6{\times}20.7{\pm}1.3{\mu}m$). The stomatal complex was anomocytic in the most studied taxa, except Daphne kiusiana by having combined with anisocytic together. The shapes of epidermal cells are undulate anticlinal wall. The size range of epidermal cell was $20.7-61.0{\mu}m$; the smallest size of epidermal cell was found in Stellera charnaejasme ($26.0{\pm}1.9{\mu}m$), on the other hand the largest one was found in Edgeworthia chrysantha ($53.6{\pm}3.1{\mu}m$). The well-developed flaky epicuticular waxes can be divided three kinds of pattern - (1) smooth in comparison, not entire platelets and scattered, (2) isolated flake-like platelets, mostly paralleled, sparsely, (3) flake-like platelets, flat, membraneous, protruding from the surfaces at varying angles and densely. Two types of trichome are recognized; (1) Type I: uniseriate trichome of striate surface (D. genkwa, Diarthron linifalium, E. chrysantha, W. ganpi and W. trichotama), (2) Type II: multicellular trichome of papillose surface, uncinated 3-4 nodes (Diathron linifolium). Finally, the systematics significance of the leaf micromorphological features in identification and elucidation of Korean Thymelaeaceae, especially between or within the genera including among the species is also briefly discussed.

Inhibitor activity of Ulmus davidiana Planch(UD) Herbal Acupuncture Solution on Cathepsin having bone resorption activity (유근피 약침액이 골재흡수 중 Cathepsin에 대한 억제 작용)

  • It was clarified that ethanol extract herb-acupuncture solution (EE-UD) and hydrotherapy herb-acupuncture solution (WE-UD) in Ulmus davidiana Planch (Ulmaceae), are the excellent inhibitors of cathepsin K and L. WE-UD inhibited cathepsin K when IC50 value was 5.32 ${\square}g$/ml, and suppressed cathepsin L when IC50 value was 6.34 ${\square}g$/ml. However, EE-UD indicated the activity of inhibiting cathepsin K and L in the level of 1.45 ${\square}g$/ml and 2.43 ${\square}g$/ml, thus it showed more significance than WE-UD. It could be observed that EE-VD is an excellent inhibitor to cathepsin K with Ki value of 0.8 ${\square}g$/ml. This activity is increased by 10-fold even in the analytical experiment when having operations like glutathione in pH 7.0. Also, this supports the mixture of GSH thiolate anion, thus it was thought that this increase in effectiveness is probably attributable to the enhanced chemical function in the combinations of herb-acupuncture solution towards a place of activity in enzyme. WE-UD showed the time-dependent inhibiting property, thus it allowed to know the disunion and the compounding speed in constant cathepsin K during the process of experiment. Finally, EE-UD was proved to suppress the absorbent bone ash in the experiment related to osteoclast in rats for test, and to the bone in rodent. It was proved that WE-UD has the effect of inhibiting the protease in cathepsin K and L, and in collagen of bone. These results strongly suggest that it is effective in preventing the progress of bone damage, which was induced due to cathepsin K. Also, it obtained the conclusion that it is effective to the reabsorption activity of bone in the bone marrow cells.

Nerve Growth Factor and Sensory Neuropeptide Levels in Plasma and Saliva of Various Orofacial Pain Patients (다양한 구강안면통증환자의 혈장 및 타액에서의 신경성장인자와 감각성 신경펩티드 농도에 관한 연구)

  • Nerve growth factor (NGF) and sensory neuropeptides are involved in the process of nociception at peripheral nerve fibers and wide spread in central nervous system. The aims of this study were to investigate NGF and sensory neuropeptides (substance P [SP] and calcitonin gene-related peptide [CGRP]) levels in human plasma and saliva, and the associations between these sensory neuropeptides levels and chronic orofacial pain symptoms. NGF, SP, and CGRP levels in plasma and resting whole saliva samples collected from 67 orofacial pain patients (joint pain, dental or periodontal pain, mucosal pain) and 36 pain free control subjects were measured by enzyme immunoassay. The characteristic pain intensity of each subject was measured using the Graded Chronic Pain Scale and the flow rate of resting whole saliva was measured. Joint pain patients group showed significantly higher plasma NGF level compared to each of dental pain patients (p<0.01), mucosal pain patients (p<0.01), and control group (p<0.01). Plasma NGF level of dental pain patients group was significantly higher than that of control group (p<0.01). Saliva SP level of dental pain patients group (p<0.05) and saliva CGRP level of mucosal pain group (p<0.05) were significantly higher than that of control group. Plasma and saliva SP levels of joint pain patients was significantly associated with pain intensity (plasma: standardized coefficient=0.599, p<0.01, saliva: standardized coefficient=0.504, p=0.05). In dental pain patients group, plasma SP (standardized coefficient=0.559, p<0.01), saliva SP (standardized coefficient=0.520, p<0.01) and saliva CGRP (standardized coefficient=0.599, p<0.01) levels were significantly associated with age. In mucosal pain patients group, plasma SP (standardized coefficient=0.495, p<0.05), saliva SP (standardized coefficient=0.500, p<0.05), and saliva CGRP (standardized coefficient=0.717, p<0.01) levels were significantly associated with age. NGF and neuropeptides may play a role in the maintenance of various orofacial pain symptoms. The examination of those levels in plasma and saliva helps understanding the mechanism of orofacial pain, and furthermore, can be applied to the diagnosis and therapy of orofacial pain.
