Amisulpride-Induced Hyperprolactinemia: Preliminary Study (아미설프라이드로 유발된 고프로락틴혈증: 예비 연구)

  • Objective: Hyperprolactinemia is common side effect associated with antipsychotics use. Nevertheless, hyperprolactinemia is relatively neglected by clinician. Especially, there is no study related to amisulprideinduced hyperprolactinemia in korea. This study aimed to determine whether amisulpride can be induced hyperprolactinemia in Korean psychiatric patients. Methods: This study methodology consisted of a retrospective review of medical charts and prolactin levels. Serum prolactin levels were measured in 24 Korean patients(12 males and 12 females) with psychosis who were treated over 400mg of amisulpride per day. Results: All patients had hyperprolactinemia. Prolactin levels significantly increased after receiving amisulpride(z=-3.702, p=0.000). The prolactin level was significantly higher in females($156.29{\pm}63.75$ng/mL) than in males($69.04{\pm}39.91$ng/mL) after administering amisulpride(p=0.000). There was a correlation between dosage and prolactin levels(r=0.61, p=0.002). However, there was no correlation between duration of treatment and prolactin levels. Conclusions: Antipsychotics, especially amisulpride can increase serum prolactin levels and may results in short and long term side effects. Routine clinical assessment of initial and additional prolactin level and associated symptoms should be done

Effects of Manganese Exposure on the Testis Function and Serum Prolactin Concentration in Rat (망간 노출이 흰쥐의 정소기능과 혈청 프로락틴 농도에 미치는 영향)

  • This study aimed to examine the testis toxicities of metal compound, manganese (Mn), which may be generated as mist or fume in the industrial sites. As well as serum prolactin (PRL) concentration was analyzed because Mn accumulation in basal ganglia up-regulates serum PRL and hyperprolactinemia consecutively induces the testis toxicity. Male F344 rats were divided into the 4 groups (2 controls and 2 Mn treated groups, n=10) on the basis of the test condition (inhalation, Mn $1.5mg/m^3$ or not) and treatment period (for 4-weeks and 13-weeks). The treatment time was 6 hr. a day, 5 days a week for the whole body. Basic tests including changes in body weight, feed rate were observed. Blood and testis Mn concentration, and testis toxicity test such as the number and deformity test of sperm were also observed. Serum PRL level was analyzed by ELISA to certify the relationship between the Mn induced increase of the serum PRL level and sperm production. Blood and testis Mn concentrations were significantly and dose-dependently increased. Sperm count was decreased in Mn-treatment groups than control in a treatment time dependent manner. Morphological analysis of cauda epidydimal sperm showed that the frequencies of morphologically abnormal sperms such as bent tail and small head were increased in the both Mn-treatment groups than control. A significant increase in serum PRL levels was found in response to Mn treatment but it was not hyperprolactinemia range. These results suggest that treatment of Mn up-regulates the serum PRL concentration and induces the testis toxicity. The No Aversed Effect Level (NOAEL) of inhaled Mn on the male rat testis may be under the $1.5mg/m^3$.

Effects of Massage on Stress, Depression and Level of Milk Secretion Hormones in Mothers (신생아 어머니의 혈중 코티졸과 우울 및 혈중 프로락틴에 대한 신생아 마사지 적용 효과)

  • Purpose: To investigate changes in mother's stress, degree of depression and level of milk secretion hormones when the mothers provided massage to their new born babies. Method: The study was conducted with 65 women who had delivered at a convalescence institute in Incheon between Sep 25, 2003 and Feb 10, 2004. The mothers were assigned to a control group or an experimental group. Infants of mothers in the experimental group received massage from their mothers twice a day for 10 days beginning on the 4th day after delivery. The data were processed using $x^2-test$, paired t-test and repeated measures ANOVA using the SAS program. Results: The level of cortisol, a stress hormone and depression were significantly different between the two groups (p=0.01 ;p=0.02). While fur plasma prolactin level, a milk secretion hormone, there was no significant difference(p=0.14). Conclusions: When massage was provided to new-born babies the cortisol level of the mothers decreased significantly compared to the control group which did not receive the massage. As the level of depression of the control group declined as time passed while the experiment group showed no difference and there was no difference in the plasma prolactin level, further study is suggested.

Local Expression of $Mel_{la}$ and Effect of Melatonin on Expression of PLP-A Gene in the Rat Placenta (흰쥐 태반에서의 $Mel_{la}$ 유전자 발현과 멜라토닌이 PLP-A 유전자 발현에 미치는 영향)

  • Seasonal changes and circadian rhythm of plasma prolactin(PRL) concentration in mammals are mediated by melatonin. Pinealectomy or denervation of the pineal gland produces an increase in plasma PRL level. In the rat placenta several members of the PRL family gene are expressed during the late pregnancy. However, the full spectrum of their expression mechanisms and regulatory factors are not elucidated yet. Present study aimed to investigate the local expression of the melatonin receptor la(Me $l_{la}$ ) gene and the effect of melatonin on expression of prolactin-like protein A(PLP-A), a member of the PRL-family gene in the rat placenta. According to the RT-PCR, northern blot and in situ hybridization experiments, Me $l_{la}$ gene was locally expressed in the rat placenta, Me $l_{la}$ mRNA was localized mainly in the placental junctional and labyrinth zones. Interestingly, junctional zone of the placenta showed strong expression of Me $l_{la}$ at daytime(16:00) than at nighttime(22:00). Melatonin agonist, chlorornelatonin decreased the PLP-A mRNA levels in the rat placenta. These results suggest that melatonin coupled with Me $l_{la}$ , may act as a regulation factor that mediates the expression of the PLP-A gene in the rat placenta.

Prolactin Response to Antipsychotic Drug and Dopamine Transporter Gene Polymorphisms (항정신병 약물에 의한 혈중 프로락틴 변화와 도파민 전달체 유전자 다형성)

  • Object:We investigated the relationship between prolactin response to antipsychotics and clinical courses of psychotic symptoms and DAT gene polymorphisms. Method:Twenty-four acute psychotic inpatients completed the 12-week trial of risperidone. Serum prolactin, BPRS, ESRS and hyperprolactinemia-related symptoms were measured at baseline, 2, 4, 8 and 12 weeks after medication. The DAT gene polymorphisms were analyzed. Results:The serum prolactin was significantly increased over time. According to the prolactin level at 2-week, the subjects were divided into the severe group(serum prolactin>60ng/mL, N=15) and the mild group (serum prolactin<60ng/mL, N=9). The prolactin levels of the mild group didn't increase beyond 60ng/mL throughout 12 weeks. Severe group had slower decrement of BPRS scores than those of mild group. Six females in severe group complained of irregular menstruations, but no female in mild group. Most patients had 10 allele of DAT gene. Conclusion:This study suggests that the magnitude of prolactin elevation at the 2-week of risperidone medication is correlated with severity of hyperprolactinemia throughout treatments. Our results did not show the relationship between prolactin responses and DAT gene polymorphisms.

The Relationship between the Amisulpride-Induced Hyperprolactinemia and Taq1A Polymorphism of the Dopamine D2 Receptor Gene in Schizophrenia Patients (조현병 환자에서 아미설프라이드에 의한 고프로락틴혈증과 DRD2 유전자 Taq1A 다형성의 연관성)

  • Objectives This study was aimed to investigate the association between amisulpride-induced hyperprolactinemia and the Taq1A polymorphism in the D2 dopamine receptor gene (DRD2) in schizophrenic patients. Methods The plasma concentrations of prolactin were measured before and after treatment with amisulpride in one hundred and twenty-five schizophrenic patients. The effect of the Taq1A variants of the DRD2 on the risk of amisulpride-induced hyperprolactinemia was the main the outcome measure. The genotyping for Taq1A (rs1800497) polymorphism was performed using TaqMan single nucleotide polymorphism (SNP) genotyping assay. Results There was a significant difference between the prolactin level at baseline and the 6th week after treatment with amisulpride in all the subjects. However, there were no significant correlations between ΔProlactin (the difference between prolactin level at baseline and the 6th week after treatment) and the Taq1A genotypes. Conclusions This is the first study to investigate the-correlations between the Taq1A polymorphism and the amisulpride-induced hyperprolactinemia in Korean schizophrenic patients. The current results suggested the further large-scale researches on various SNPs in the DRD2 gene will establish clear goals and provide answers to the unanswered questions described in this study.

Quality of Life of Antipsychotic-Induced Hyperprolactinemia in Patients with Schizophrenia (항정신병약물로 유발된 고프로락틴혈증을 가진 조현병 환자의 삶의 질)

  • Objectives : Antipsychotic-induced hyperprolactinemia causes physical symptoms, such as amenorrhea, galactorrhea, gynecomastia, sexual dysfunction, and bone density loss, as well as psychiatric symptoms, such as depression and cognitive impairments. This study aimed to clarify the associations among hyperprolactinemia caused by antipsychotics in patients with schizophrenia, psychiatric pathology, and psychosocial factors. Methods : Ninety-nine patients with schizophrenia in the psychiatry department of a university hospital were registered between 2015 and 2017. All participants were assessed using structured questionnaires to elucidate psychopathology, social function, quality of life, and hyperprolactinemia-related side effects. The standard levels for hyperprolactinemia were 24ng/mL for women and 20ng/mL for men. Results : The average prolactin levels were $73.45{\pm}49.37ng/mL$ in patients with hyperprolactinemia and $9.16{\pm}6.42ng/mL$ in those without hyperprolactinemia. The average prolactin level in women was significantly higher than that in men(p=0.04). Risperidone was most commonly administered in patients with hyperprolactinemia(58.1%, p<0.01), while aripiprazole was most commonly administered in those without hyperprolactinemia(44.7%, p<0.01). Patients with hyperprolactinemia had significantly higher Positive and Negative Syndrome Scale(p=0.03) and Patient Health Questionnaire-9(p=0.05) scores and had significantly lower Social and Occupational Functioning Assessment Scale(p=0.04) and Strauss-Carpenter Levels of Functioning Scale(p=0.03) scores than patients without hyperprolactinemia. There were no significant differences in side effects or quality of life between the two groups. Conclusion : These findings demonstrate that hyperprolactinemia confers negative effects on depression and social function, but does not directly affect the quality of life. These results suggest that patients with schizophrenia who take antipsychotics that increase prolactin or cause side effects of hyperprolactinemia need to be assessed and receive interventions for depression.
